Each of four monitor wells installed on the site are impacted by volatile organic compounds (VOCs) at levels greater than the 2L Standards. Based on the location of two monitor wells at the property boundary, the dissolved plume is migrating off-site. Based on the absence of LNAPL measured for the reporting period, relative to the detection of 0.03 feet measured in monitor well MW-1 during July 2015 and February 2016, the extent of LNAPL appears to be decreasing. 2.4 Remedial Actions No remedial action plan is currently in use or was previously in use to address the subject release incident. 2.5 Potential Receptors Potable water supply wells PW-2, located approximately 200 feet to the northeast, and PW-3, located approximately 450 west, of the source area, have been impacted by Ethylene dibromide (EDB; 1,2-Dibromoethane and 1,2-Dichloropropane. Groundwater impacts by EDB and 1,2-Dicloropropane have been detected in monitor wells on the subject property and on the adjoining H.A. Walker Store site (#9615). The North Carolina Department of Environmental and Quality (NCDEQ) is providing Point-of-Entry (POE) carbon-filtration systems to each of the residences served by wells PW-2 and PW-3, in response to the gasoline release at the H.A. Walker’s Grocery site located approximately 250 north of the subject site. Water supply well PW-6 is located approximately 800 feet southeast of the source area. 3.0 Scope of Services 3.1 Contract Information The scope of services for this report was performed in general accordance with S&ME’s proposal number 43- 17000709B, dated February 28, 2019. The scope of services was authorized by the North Carolina Department of Environmental Quality (NCDEQ’s) Task Authorization No. 18698-02, dated March 6, 2019, and within NCDEQ/S&ME Contract No. N17002. 3.2 Approved Scope of Services S&ME’s approved Scopes of Services for this Task Authorization included the following.  Measure LNAPL occurrence in monitor well MW-1. Collect groundwater samples from monitor wells MW- 1 (if LNAPL is not detected), MW-2, MW-3 and MW-4.  Preparation and submittal of this report. These services were completed as proposed. State-Lead Monitoring Report Duke Foster/D&M Market, Incident 18698 Intersection of Nicks Road and NC Highway 119 South Mebane, Caswell County, North Carolina S&ME Project No. 4305-17-214A May 24, 2019 4 4.0 Site History and Characterization The location of the UST site is depicted on Figure 1. Selected site features are shown on Figure 2, and an aerial photograph of the site is depicted on Figure 5. The property (1.3 acres) is developed with a one-story, masonry- framed building (1,800 square feet) used by the owner for private storage and as a shop. A concrete slab and a gravel parking area occupies the north corner of the property, with access from NC Highway 119 South and Nicks Road. The remainder of the property is a grassed yard. Water supply well PW-4 is located south of the building. The site was formerly a rural store with retail fuel sales. Photographs of the subject site are provided in Appendix III. 4.1 UST Owner and Operator Information See Table 2 for UST Owner and Operator Information. Eanes Oil Company (Eanes) owned a UST system installed in 1989 and removed in 1992. A UST system owned by Foust Oil Co. (Foust) was reportedly removed from the same tank basin before 1992. 4.2 UST Information See Table 1 for UST system information and Figure 2 for the reported location of the former UST basin. The system consisted of three 2,000-gallon gasoline USTs that operated from 1989 until 1992. Information about the previous generation of USTs has not been reviewed by S&ME. 4.3 Non-UST Information Information concerning ASTs or non-UST spills on the site is unknown. 4.4 Description of Release Eanes collected five soil samples from the base of the former Foust UST basin during July 1988. Analysis of two soil samples resulted in total petroleum hydrocarbons as gasoline at concentrations of 33 milligrams per kilogram (mg/kg) and 38 mg/kg. Eanes reportedly installed an impermeable geotextile membrane at the base of the excavation prior to installing three 2,000-gallon gasoline USTs during September 1989. A release from the Eanes UST system was not indicated by the analytical results for five soil samples collected from beneath the USTs, but above the geotextile membrane, following removal of the USTs during December 1992. Analysis of one soil sample, collected from one of two soil borings performed by Paragon Environmental Consultants, Inc., on behalf of the NCDEQ, resulted in a maximum TPH concentration of 2,460 mg/kg. Analysis of soil sample SB-2 resulted in VOCs and carbon-chain fractions at concentrations greater than the lowest Maximum Soil Contaminant Concentrations (MSCCs). On behalf of the NCDEQ, ECS Carolinas, LLP, converted a soil boring performed in the former UST basin to monitor well MW-1 on June 25, 2015. LNAPL was measured (0.03 feet) in monitor well MW-1 on June 26, 2015. ECS was unable to confirm if soil was impacted or if a Limited Site Assessment had been performed. State-Lead Monitoring Report Duke Foster/D&M Market, Incident 18698 Intersection of Nicks Road and NC Highway 119 South Mebane, Caswell County, North Carolina S&ME Project No. 4305-17-214A May 24, 2019 5 During February 2016, ECS collected one soil sample each from six hydraulic-push borings at approximate depths of 20 feet below land surface (ft-bls). During My 2016, ECS collected soil samples from six hollow-stem auger borings drilled to a depth of 15 ft-bls. ECS stated that impacted soil is located between depths of 5 feet to 20 ft- bls, based on photo-ionization detector readings. Analysis of selected soil samples resulted in TPH concentrations greater than the action level in two soil samples collected from a depth of 10 ft-bls. Analysis of soil samples collected from a depth of 20 ft-bls detected compounds at concentrations greater than corresponding soil-to- groundwater and residential MSCCs. ECS installed three shallow groundwater monitor wells, MW-2 through MW-4, each to an approximate depth of 30 ft-bls. LNAPL was measured in monitor well MW-1 at a thickness of 0.03 feet on February 17, 2016. Analysis of groundwater samples collected from monitor wells MW-2, MW-3 and MW-4 resulted in VOC concentrations greater than the established 2L Standards. Benzene was detected in the MW-3 sample at a concentration greater than the Gross Contamination Level (GCL). 4.5 Site Characteristics The site is currently in use as a shop with storage, with surface cover consisting of concrete and gravel in front of the building, and grassed open areas on the remaining site. The topographic slope of the site and adjacent properties is generally to the north and northeast. The depth to groundwater in the source area typically ranges from 20 to 23 ft-bls, and the groundwater flow from the source area is generally to the north northwest. The lithology of the site was reported generally as silt, clayey silt and sandy silt (residuum) to depths of approximately 30 ft-bls. Bedrock was not encountered by soil borings performed to a depth of 30 ft-bls, and the depth to bedrock is unknown. Adjacent properties include commercial uses to the north northwest (H.A. Walker Grocery), residential uses to the north northeast and west, and undeveloped land to the south and east. 4.6 Adjoining Property Information Refer to Table 6 for property information of properties within or adjacent to area containing contamination or the area where contamination is expected to migrate. Refer to Figure 5 for the location of these properties relative to the site. 4.7 Receptor and Potential Receptor Information Refer to Figure 5 for the location of the potential receptors.  Municipal water is not available to the site and surrounding area.  Active drinking water supply wells are located within 1,000 feet of the source area: PW-2, PW-3, PW-6.  Inactive water supply wells are located within 250 feet of the source area: PW-1, PW-4 (on-site).  Surface water bodies are not located within 500 feet of the source area. State-Lead Monitoring Report Duke Foster/D&M Market, Incident 18698 Intersection of Nicks Road and NC Highway 119 South Mebane, Caswell County, North Carolina S&ME Project No. 4305-17-214A May 24, 2019 6 5.0 Current/Historical Assessment Information 5.1 Current Water Levels and LNAPL On March 21, 2019, S&ME was onsite to gauge liquid levels in monitor wells MW-1, MW-2, MW-3, and MW-4. LNAPL was not detected in monitor well MW-1. The depth-to-water ranged from 17.38 ft-BTOC in monitoring well MW-4 to 19.93 ft-BTOC in monitor well MW-2. The top of casing elevations of MW-1 through MW-4 were previously measured relative to the top of casing elevations of monitor well MW-2 and MW-9 installed for 9615 H.A. Walker’s Grocery. Relative top of casing elevations and groundwater elevations were calculated. Relative groundwater elevations ranged from 87.17 feet in MW-1 to 85.76 feet in MW-2. A summary of current and historic groundwater levels is included in Table 3. Groundwater elevations based on the March 21, 2019, measurements are depicted in Figure 3, with groundwater elevations for the 9615 H.A. Walker’s Grocery site. 5.2 Monitor Well Installations / Soil Borings No monitor wells or soil borings were installed or conducted during this monitoring event. 5.3 Groundwater Monitoring Event After measuring the depth-to-water, S&ME personnel purged monitor wells MW-1, MW-2, MW-3 and MW-4 with disposable polyethylene bailers by evacuating approximately three well volumes of water. The field parameters pH, specific conductivity, and temperature were measured for each approximate casing volume and recorded on groundwater sampling forms, which are located in Appendix IV. After completing the well purging, S&ME personnel collected a groundwater sample from each of the monitor wells. During the sampling event, field blank sample FB-1 was prepared using laboratory supplied deionized water. The field blank and groundwater samples were placed in laboratory-supplied containers, stored in a cooler with ice, and shipped under chain-of-custody procedures to a North Carolina certified laboratory for laboratory analysis. The groundwater samples were analyzed for VOCs by Standard Method 6200B and for EDB by Method 504.1. 5.4 Surface Water Monitoring Event No surface water sampling was conducted during this monitoring event. 5.5 Soil Monitoring Event No soil sampling was conducted during this monitoring event. State-Lead Monitoring Report Duke Foster/D&M Market, Incident 18698 Intersection of Nicks Road and NC Highway 119 South Mebane, Caswell County, North Carolina S&ME Project No. 4305-17-214A May 24, 2019 7 5.6 Regional and Site Geology and Hydrogeology 5.6.1 Soil and Bedrock Based on the USDA Web Soil Survey (http://websoilsurvey.sc.egov.usda.gov/ App/WebSoilSurvey.aspx), soil survey data is not available for Caswell County. Subsurface soil characterized during soil borings, reported by others, consisted of silt, clayey silt and sandy silt. According to the 1982 USGS Geologic Map Region G, North Carolina, the site is underlain by Intermediate Intrusive Rocks, described as well-jointed, metamorphosed, and likely diorite in composition. Bedrock was not encountered at a depth of 30 ft-bls in previous soil borings performed by others. 5.6.2 Hydrogeology Based upon measurements collected on March 21, 2019, groundwater flow conditions in the source area are divergent to the northwest and west northwest, with an approximate gradient of 0.050 feet per foot. Groundwater flow downgradient of the source area is generally to the west northwest, with an approximate gradient of 0.031 feet per foot Figure 3 graphically depicts the approximate groundwater surface contours and flow direction at the site based upon the March 21, 2019, measurements, in conjunction with contours representing the 9615 H.A. Walker’s Grocery site. Groundwater occurrence ranged from approximately 2.5 feet to approximately 4.5 feet higher in elevation on March 21, 2019, relative to elevations observed during October 2017. The hydraulic conductivity of soil underlying the site and the seepage velocity of groundwater at the site have not been determined. 6.0 Evaluation of Assessment Information 6.1 Current Extent and Severity of Contamination 6.1.1 Soil Refer to Table 4, Summary of Soil Sampling Results. Analysis of six soil samples SB-1 through SB-6, each collected from a depth of 20 ft-bls from within the former UST basin by ECS during February 2016, resulted in VOC or carbon-chain fraction concentrations greater than the corresponding soil-to-groundwater MSCCs. The analytical results for two soil samples, SB-3 and SB-6, were greater than the residential MSCCs for carbon-chain fractions. Based on the presence of impacted groundwater and depth to water measurements of approximately 18 to 24 ft-bls, soil impacts beneath the former UST basin extend to the saturated zone. However, the soil samples were each collected from a depth deeper than March 2019 groundwater occurrence. The horizontal extent of soil impacts exceeding the MSCCs has not been determined. State-Lead Monitoring Report Duke Foster/D&M Market, Incident 18698 Intersection of Nicks Road and NC Highway 119 South Mebane, Caswell County, North Carolina S&ME Project No. 4305-17-214A May 24, 2019 8 6.1.2 Groundwater Refer to Table 5, Summary of Groundwater Sampling Results, and Figure 4, Groundwater Constituent Map. Following the detection of LNAPL in source monitor well MW-1 during June 2015 and February 2016, the highest dissolved VOC concentrations were reported in the sample collected from MW-1 on October 4, 2017, including Benzene and EDB concentrations greater than the GCL. Elevated VOC concentrations were also reported for the sample collected from MW-3, located downgradient of the source area, including Benzene at a concentration greater than the GCL, but less than the MW-1 value. The March 2019 constituent concentrations were less than the October 2017 values for monitor wells MW-1 and MW-2. The March 21, 2019, MW-3 constituent concentrations were greater than the 2016 and 2017 concentrations for monitor well MW-3. The horizontal extent of the dissolved plume exceeding the 2L Standard is not defined. Based on the presence of monitor wells MW-3 and MW-4 at or near the property boundary, the leading edge of the dissolved plume has migrated beyond the property boundary, including Benzene concentrations greater than the GCL. Also, Benzene concentrations reported for downgradient monitor well MW-3 are now greater than those in source monitor well MW-1, indicating that the dissolved constituent plume is migrating from the source area. It is possible that the elevated top of groundwater elevations observed influenced the lower VOC concentrations reported for monitor wells MW-1 and MW-2. Due to typical VOC specific gravities being less than 1, impacts are typically greater at the water table than at depth, and the greater water column heights available to sampling in the monitor wells may allow for mixing of groundwater with lower concentrations. However, groundwater occurrence was also higher in monitor well MW-3, but dissolved concentrations increased instead of decreasing. Isoconcentration Contour Maps for Benzene and EDB are provided as Figure 6 and Figure 7, respectively, in conjunction with data from the adjacent 9615 H.A. Walker’s Grocery site. A copy of the groundwater analytical report is included in Appendix V. 6.1.3 LNAPL LNAPL was measured at a thickness of 0.03 feet in monitor well MW-1 following installation during July 2015 and during February 2016. LNAPL was not detected in MW-1 during October 2017, and has not been observed in monitor wells MW-2 through MW-4. 6.2 Cleanup Levels Achieved  Soil (soil-to-groundwater maximum soil contaminant concentrations) No  Groundwater (2L standard concentrations) No  LNAPL (0.01’) Yes State-Lead Monitoring Report Duke Foster/D&M Market, Incident 18698 Intersection of Nicks Road and NC Highway 119 South Mebane, Caswell County, North Carolina S&ME Project No. 4305-17-214A May 24, 2019 9 7.0 Conclusions and Recommendations 7.1 Conclusions 1. The UST Incident meets the following conditions for a High Risk classification: A. There are four active, potable drinking water supply wells within 1,000 feet of the source area, and two non-drinking wells located within 250 feet of the source area. B. Dissolved VOCs were detected at concentrations greater than the 2L Standards in each monitor well sampled, and Benzene was reported at concentrations greater than the GCL in monitor well MW-3. C. Soil impacts greater than the soil-to-groundwater and the residential MSCCs remain beneath the former UST basin. 2. No remedial activities have been performed. 3. The dissolved constituent plume appears to be unstable, based on completion of three monitoring events, and the dissolved plume appears to be migrating beyond the subject property boundary. 7.2 Recommendations Based on the above, the following recommendations are presented: 1. Consider the feasibility of installing a monitor well on the west adjacent property to contribute to delineation of the horizontal extent of the dissolved plume. 2. Gauge for the presence of LNAPL and perform a groundwater monitoring event at a time with anticipated lower groundwater occurrence, to evaluate plume stability. 3. Collect additional soil samples from adjacent to the former UST basin, to assess soil quality by the risk- based methods at depths less than 18 feet below land surface. Provide horizontal delineation of soil impacts exceeding the soil-to-groundwater and residential MSCCs, in order to evaluate the feasibility of performing soil excavation to remove secondary source soil. Analysis of five soil samples collected from over a geotechnical liner did not report TPH. The report referenced a past release from the same tank basin in July 1988, prior to installation of the USTs listed above, and included an analytical report for five soil samples analyzed on 7/28/1988, with three sample results <10 milligram per kilogram ( mg/kg), and two soil samples at 33 mg/kg and 38 mg/kg petroleum hydrocarbons by GC. Paragon Environmental Consultants reported analytical results from one soil sample collected from a soil boring of 2,460 parts per million TPH and risk-based compounds concentrations greater than the lowest MSCC (soil boring location and soil sample depth not reported). Wide recovery limits established for difficult compound. Wide RPD limits established for difficult compound. #Data exceeded client recommended or regulatory level Percent recoveries and relative percent differences (RPDs) are determined by the software using values in the calculation which have not been rounded. No results have been blank subtracted unless specified in the case narrative section. Detected but below the Reporting Limit (lowest calibration standard); therefore, result is an estimated concentration (CLP J-Flag). J Laboratory fortified blank/laboratory control sample recovery and duplicate recoveries outside of control limits. Data validation is not affected since all results are "not detected" for associated samples in this batch and bias is on the high side. L-02 Laboratory fortified blank/laboratory control sample recovery and duplicate recovery are outside of control limits. Reported value for this compound is likely to be biased on the low side. L-04 Laboratory fortified blank/laboratory control sample recovery and duplicate recovery are outside of control limits. Reported value for this compound is likely to be biased on the high side. L-06 Either laboratory fortified blank/laboratory control sample or duplicate recovery is outside of control limits, but the other is within limits. RPD between the two LFB/LCS results is within method specified criteria. L-07 Laboratory fortified blank duplicate RPD is outside of control limits. Reduced precision is anticipated for any reported value for this compound. R-05 Elevated reporting limit due to high concentration of target compounds.RL-11 [TOC_1]Flag/Qualifier Summary[TOC] Page 44 of 49 Table of Contents 39 Spruce Street * East Longmeadow, MA 01028 * FAX 413/525-6405 * TEL. 413/525-2332 CertificationsAnalyte CERTIFICATIONS Certified Analyses included in this Report SM21-22 6200B in Water NCAcetone NCBenzene NCBromobenzene NCBromochloromethane NCBromodichloromethane NCBromoform NCBromomethane NC2-Butanone (MEK) NCn-Butylbenzene NCsec-Butylbenzene NCtert-Butylbenzene NCCarbon Tetrachloride NCChlorobenzene NCEthanol NCChlorodibromomethane NCChloroethane NCChloroform NCChloromethane NC2-Chlorotoluene NC4-Chlorotoluene NC1,2-Dibromoethane (EDB) NC1,2-Dichlorobenzene NC1,3-Dichlorobenzene NC1,4-Dichlorobenzene NCDichlorodifluoromethane (Freon 12) NC1,1-Dichloroethane NC1,2-Dichloroethane NC1,1-Dichloroethylene NCcis-1,2-Dichloroethylene NCtrans-1,2-Dichloroethylene NC1,2-Dichloropropane NC1,3-Dichloropropane NC2,2-Dichloropropane NC1,1-Dichloropropene NCcis-1,3-Dichloropropene NCtrans-1,3-Dichloropropene NCDiisopropyl Ether (DIPE) NCEthylbenzene NC2-Hexanone (MBK) NCIsopropylbenzene (Cumene) NCp-Isopropyltoluene (p-Cymene) NCMethyl tert-Butyl Ether (MTBE) NCMethylene Chloride NC4-Methyl-2-pentanone (MIBK) NCNaphthalene NCn-Propylbenzene NCStyrene [TOC_1]Certifications[TOC] Page 45 of 49 Table of Contents 39 Spruce Street * East Longmeadow, MA 01028 * FAX 413/525-6405 * TEL. 413/525-2332 CertificationsAnalyte CERTIFICATIONS Certified Analyses included in this Report SM21-22 6200B in Water NC1,1,2,2-Tetrachloroethane NCTetrachloroethylene NCToluene NC1,2,3-Trichlorobenzene NC1,2,4-Trichlorobenzene NC1,1,1-Trichloroethane NC1,1,2-Trichloroethane NCTrichloroethylene NCTrichlorofluoromethane (Freon 11) NC1,2,3-Trichloropropane NC1,2,4-Trimethylbenzene NC1,3,5-Trimethylbenzene NCVinyl Acetate NCVinyl Chloride NCm+p Xylene NCo-Xylene The CON-TEST Environmental Laboratory operates under the following certifications and accreditations: Code Description Number Expires 100033AIHA-LAP, LLC - ISO17025:2005AIHA 03/1/2020 M-MA100Massachusetts DEPMA 06/30/2019 PH-0567Connecticut Department of Publilc HealthCT 09/30/2019 10899 NELAPNew York State Department of HealthNY 04/1/2020 2516 NELAPNew Hampshire Environmental LabNH-S 02/5/2020 LAO00112Rhode Island Department of HealthRI 12/30/2019 652North Carolina Div. of Water QualityNC 12/31/2019 MA007 NELAPNew Jersey DEPNJ 06/30/2019 E871027 NELAPFlorida Department of HealthFL 06/30/2019 LL015036Vermont Department of Health Lead LaboratoryVT 07/30/2019 2011028State of MaineME 06/9/2019 460217Commonwealth of VirginiaVA 12/14/2019 2557 NELAPNew Hampshire Environmental LabNH-P 09/6/2019 VT-255716Vermont Department of Health Drinking WaterVT-DW 06/12/2019 25703North Carolina Department of HealthNC-DW 07/31/2019 Page 46 of 49 Table of Contents Run FB-1 for 6200 per Lyndal B. -KKM 3/25Page 47 of 49 Table of Contents Page 48 of 49 Table of Contents Page 49 of 49 Table of Contents
