(ZAVA) conducted a 96- hour hybrid Mobile Multi-Phase Extraction (MMPE)/air sparge pilot test utilizing wells MW-1, MW-3, and AS-1. A total of 2,063.58 pounds of pollutant mass of VOCs as gasoline was removed during the event. A total of 5,380 gallons of non-hazardous oily water were collected and disposed of during the event. • On March 2, 2020, S&ME collected a groundwater sample from newly-installed remediation well AS-1. On March 31, 2020, S&ME conducted groundwater monitor well sampling of monitor wells MW-1, MW-2, MW-3, and MW-4. Light non-aqueous phase liquid (LNAPL) was not observed in the monitor wells. The maximum concentrations of dissolved groundwater contaminants detected at concentrations greater than the applicable site cleanup goals during this sampling event are summarized in the table below, along with the applicable site cleanup goals, Gross Contaminant Levels (GCL), or 15A NCAC 2L .0202 groundwater quality standards (2L Standard). Exceedances of the cleanup goals are indicated in wells MW-1, MW-2, MW-3 and MW-4. Each of four monitor wells installed on the site, and the newly-installed air sparge well AS-1, are impacted by volatile organic compounds (VOCs) at levels greater than the 2L Standards. VOC levels were generally lower in source monitor well MW-1, and in cross-gradient monitor wells MW-2 and MW-4, relative to historical values. VOCs levels in downgradient monitor well MW-3 were similar to concentrations reported for March 21, 2019, with the exception of an increase in benzene to 13,000 µg/L. Based on the location of monitor wells MW-3 and MW-4 at the property boundary, the dissolved plume is migrating off-site. Based on the absence of LNAPL measured for the reporting period, relative to the detection of 0.03 feet measured in monitor well MW-1 during July 2015 and February 2016, the extent of LNAPL appears to be decreasing. 2.2 Potential Receptors Potable water supply wells PW-2, located approximately 200 feet to the northeast, and PW-3, located approximately 450 west, of the source area, have been impacted by Ethylene dibromide (EDB; 1,2-Dibromoethane) and 1,2-Dichloropropane. Groundwater impacts by EDB and 1,2-Dicloropropane have been detected in monitor wells on the subject property and on the adjoining H.A. Walker Store site (#9615). The North Carolina Department of Environmental and Quality (NCDEQ) is providing Point-of-Entry (POE) carbon-filtration systems to each of the residences served by wells PW-2 and PW-3, in response to the gasoline release at the H.A. Walker’s Grocery site located approximately 250 north of the subject site. Water supply well PW-6 is located approximately 800 feet southeast of the source area. 3.0 Scope of Services 3.1 Contract Information The scope of services for this report was performed in general accordance with S&ME’s proposal number 43- 170007093, dated February 4, 2020. The scope of services was authorized by the North Carolina Department of Environmental Quality (NCDEQ’s) Task Authorization No. 03, dated February 5, 2020, and within NCDEQ/S&ME Contract No. N17002. 3.2 Approved Scope of Services S&ME’s approved Scopes of Services for this Task Authorization included the following. • Install one 2-inch diameter air sparge well with 5 feet of screened interval to a depth of 37.5 feet below ground surface (ft-bgs). • Measure the top of casing elevation of the air sparge well relative to MW-1 and MW-3. • Provide UIC notification to perform air sparging; • Perform a hybrid MMPE/air sparge event using monitor wells MW-1, MW-3, and the air sparge well; • Collect a groundwater sample from the air sparge well for analysis by Methods 6200B and 504.1 prior to the MMPE event; • Collect groundwater samples from MW-1, MW-2, MW-3 and MW-4, after the MMPE event for analysis Methods 6200B and 504.1, and prepare one field blank sample for analysis by Method 6200B. • Preparation of this Corrective Action Performance/Monitoring report to summarize activities and findings. Corrective Action Performance/Monitoring Report Duke Foster/D&M Market, Incident 18698; UST No. WS-5420 Intersection of Nicks Road and NC Highway 119 South Mebane, Caswell County, North Carolina S&ME Project No. 4305-17-214B May 26, 2020 4 These services were completed as proposed. 4.0 Site History and Characterization The location of the UST site is depicted on Figure 1, and selected site features are shown on Figure 2. The property (1.3 acres) is developed with a one-story, masonry-framed building (1,800 square feet) used by the owner for private storage and as a shop. A concrete slab and a gravel parking area occupies the north corner of the property, with access from NC Highway 119 South and Nicks Road. The remainder of the property is a grassed yard. Water supply well PW-4 is located south of the building. The site was formerly a rural store with retail fuel sales. Photographs of the subject site are provided in Appendix III. 4.1 UST Owner and Operator Information See Table 2 for UST Owner and Operator Information. Eanes Oil Company (Eanes) owned a UST system installed in 1989 and removed in 1992. A UST system owned by Foust Oil Co. (Foust) was reportedly removed from the same tank basin before 1992. 4.2 UST Information See Table 1 for UST system information and Figure 2 for the reported location of the former UST basin. The system consisted of three 2,000-gallon gasoline USTs that operated from 1989 until 1992. Information about the previous generation of USTs has not been reviewed by S&ME. 4.3 Non-UST Information Information concerning ASTs or non-UST spills on the site is unknown. 4.4 Description of Release Eanes collected five soil samples from the base of the former Foust UST basin during July 1988. Analysis of two soil samples resulted in total petroleum hydrocarbons as gasoline at concentrations of 33 milligrams per kilogram (mg/kg) and 38 mg/kg. Eanes reportedly installed an impermeable geotextile membrane at the base of the excavation prior to installing three 2,000-gallon gasoline USTs during September 1989. A release from the Eanes UST system was not indicated by the analytical results for five soil samples collected from beneath the USTs, but above the geotextile membrane, following removal of the USTs during December 1992. Analysis of one soil sample, collected from one of two soil borings performed by Paragon Environmental Consultants, Inc., on behalf of the NCDEQ, resulted in a maximum TPH concentration of 2,460 mg/kg. Analysis of soil sample SB-2 resulted in VOCs and carbon-chain fractions at concentrations greater than the lowest Maximum Soil Contaminant Concentrations (MSCCs). On behalf of the NCDEQ, ECS Carolinas, LLP, converted a soil boring performed in the former UST basin to monitor well MW-1 on June 25, 2015. LNAPL was measured (0.03 feet) in monitor well MW-1 on June 26, 2015. ECS was unable to confirm if soil was impacted or if a Limited Site Assessment had been performed. Corrective Action Performance/Monitoring Report Duke Foster/D&M Market, Incident 18698; UST No. WS-5420 Intersection of Nicks Road and NC Highway 119 South Mebane, Caswell County, North Carolina S&ME Project No. 4305-17-214B May 26, 2020 5 During February 2016, ECS collected one soil sample each from six hydraulic-push borings at approximate depths of 20 feet below land surface (ft-bls). During My 2016, ECS collected soil samples from six hollow-stem auger borings drilled to a depth of 15 ft-bls. ECS stated that impacted soil is located between depths of 5 feet to 20 ft- bls, based on photo-ionization detector readings. Analysis of selected soil samples resulted in TPH concentrations greater than the action level in two soil samples collected from a depth of 10 ft-bls. Analysis of soil samples collected from a depth of 20 ft-bls detected compounds at concentrations greater than corresponding soil-to- groundwater and residential MSCCs. ECS installed three shallow groundwater monitor wells, MW-2 through MW-4, each to an approximate depth of 30 ft-bls. LNAPL was measured in monitor well MW-1 at a thickness of 0.03 feet on February 17, 2016. Analysis of groundwater samples collected from monitor wells MW-2, MW-3 and MW-4 resulted in VOC concentrations greater than the established 2L Standards. Benzene was detected in the MW-3 sample at a concentration greater than the Gross Contamination Level (GCL). 4.5 Site Characteristics The site is currently in use as a shop with storage, with surface cover consisting of concrete and gravel in front of the building, and grassed open areas on the remaining site. The topographic slope of the site and adjacent properties is generally to the north and northeast. The depth to groundwater in the source area typically ranges from 20 to 23 ft-bls, and the groundwater flow from the source area is generally to the north northwest. The lithology of the site was reported generally as silt, clayey silt and sandy silt (residuum) to depths of approximately 30 ft-bls. Bedrock was not encountered by soil borings performed to a depth of 30 ft-bls, and the depth to bedrock is unknown. Adjacent properties include commercial uses to the north northwest (H.A. Walker Grocery), residential uses to the north northeast and west, and undeveloped land to the south and east. 4.6 Receptor and Potential Receptor Information • Municipal water is not available to the site and surrounding area. • Active drinking water supply wells are located within 1,000 feet of the source area: PW-2, PW-3, PW-6. • Inactive water supply wells are located within 250 feet of the source area: PW-1, PW-4 (on-site). • Surface water bodies are not located within 500 feet of the source area. 5.0 Current/Historical Assessment Information 5.1 Air Sparge Well Installation On February 17, 2020, S&ME mobilized to the site with subcontractor Geologic Exploration to install air sparge well AS-1. The well was installed using a track mounted Geoprobe® drill rig using 4 ¼ - inch hollow stem augers to a depth of 37.5 ft-bgs. The well was constructed with 2-inch diameter Schedule 40 PVC riser with a 2.5-foot length of 0.010-inch slotted. 2-inch diameter PVC screen. #2 filter sand was extended to approximately 2 feet above the top of the screen, over which an approximately 3-foot later of hydrated bentonite seal was put in place. The well was then completed using grout to the surface and encased in a flush-mount vault. The Well Construction Record, boring log and well construction detail for AS-1 are provided in Appendix IV. Corrective Action Performance/Monitoring Report Duke Foster/D&M Market, Incident 18698; UST No. WS-5420 Intersection of Nicks Road and NC Highway 119 South Mebane, Caswell County, North Carolina S&ME Project No. 4305-17-214B May 26, 2020 6 During the March 31, 2020, groundwater sampling event, S&ME personnel measured the relative top of casing elevation for AS-1 utilizing a sight level and rod relative to MW-1 and MW-3. Monitor well construction details and groundwater elevation data are presented in Table 3. 5.2 Underground Injection Control-Notification of Intent On February 17, 2020, S&ME submitted the attached Notification of Intent form and supporting information to NCDEQ. S&ME received permit number WI0400541 via email on February 28, 2020 (Appendix V) 5.3 Summary of MMPE/Air Sparging Event S&ME and subcontractor ZAVA Industrial, Inc. (ZAVA) performed depth to water measurements on wells MW-1, MW-2, MW-3 and AS-1, on March 2, 2020, prior to the MMPE event. LNAPL was not measured in any of the wells. Water level measurements ranged from 18.85 feet below top of casing in monitor well MW-1 to 20.25 feet below top of casing in monitor well MW-2 (Table 3). Pre-MMPE groundwater elevations measured on March 2, 2020 are summarized in Table 3. ZAVA conducted a 96-hour MMPE event, using wells MW-1 and MW-3, from March 2 to March 6, 2020. Vacuum stinger depths were initially set and adjusted to increase recovered vapors from the extraction points. Air sparging was performed on well AS-1 from 11:15 on March 2, 2020 until 17:00 on March 5, 2020. ZAVA reported a sparge flow rate of 0.4 standard cubic feet per minute. A copy of the Mobile Multi-Phase Extraction Report provided by ZAVA is included in Appendix VI. A total of 5,380 gallons of liquid was recovered. This quantity corresponds to an approximate flow rate of 0.47 gallons per minute per well. The initial vacuum level was approximately 26.0 inches of Mercury, and levels stabilized at 24.0 inches of Mercury. Air flow rates during the MMPE event ranged from approximately 1,147 to 1,959 feet per minute. Air emissions of volatile organic vapors from the MMPE measured with a photo-ionization detector (PID) ranged from 3,800 to >15,000 parts per million (ppm). The initial discharge PID measurement was 1,180 ppm. The PID measurement increased to 11,000 ppm after starting air sparging at AS-1. During the afternoon of March 2, 2020, the compressor was turned off for approximately 1 hour, Pressure was increased from 9 pounds per square inch ( psi) to 16 psi when air sparging resumed, and an increase in air emissions of volatile organic vapors was observed, from 11,400 ppm to greater than 15,000 ppm within five minutes of resuming air sparging. Based on the recorded air flow rates and vapor emissions, 2,063.58 pounds of vapor phase hydrocarbons were removed, which corresponds to 382.85 gallons of volatile organic compounds (VOCs) as gasoline. Measurable product was not recovered as liquid. LNAPL was not detected in monitor wells MW-1 or MW-3 on March 6, 2020, after the MMPE event. Based on the subcontractor cost of the MMPE, the cost for scheduled maintenance and reporting, but not including monitoring, of $10,519, and the 382.85 gallons of vaporous hydrocarbons removed, the approximate cost of hydrocarbons removed was $27.48 per gallon. Vacuum influence was observed in MW-4 and MW-2, at 1.25 inches of water and 0.5 feet of water, respectively, on March 2, 2020. Well head PID vapor concentrations at monitor well MW-4 were less than 10 ppm. Depth to water measurements in monitor wells MW-1, MW-2 and MW-3, increased by 0.70 feet, 0.57 feet, and 0.79 feet, respectively during the MMPE event. Post-MMPE groundwater elevations measured on March 6, 2020 are summarized in Table 3. 5.4 Current Water Levels and LNAPL On March 31, 2020, S&ME was onsite to gauge liquid levels in monitor wells MW-1, MW-2, MW-3 and MW-4. LNAPL was not detected in MW-1 through MW-4. The depth-to-water ranged from 18.64 ft-BTOC in monitoring well MW-4 to 19.89 ft-BTOC in monitor well MW-2. The top of casing elevations of MW-1 through MW-4 were previously measured relative to the top of casing elevations of monitor well MW-2 and MW-9 installed for 9615 H.A. Walker’s Grocery. Relative top of casing elevations and groundwater elevations were calculated. Relative groundwater elevations ranged from 86.11 feet in MW-1 to 85.16 feet in MW-3. A summary of current and historic groundwater levels is included in Table 3. Groundwater elevations based on the March 31, 2020, measurements are depicted in Figure 3. 5.5 Groundwater Monitoring Event On March 2, 2020, after gauging the depth to water and before the MMPE event, S&ME personnel purged sparge well AS-1 with a disposable polyethylene bailer by evacuating approximately three well volumes of water. The field parameters pH, specific conductivity, and temperature were measured for each approximate casing volume and recorded on groundwater sampling forms, which are located in Appendix IV. After completing the well purging, S&ME personnel collected a groundwater sample from the sparge. The groundwater sample was placed in laboratory-supplied containers, stored in a cooler with ice, and shipped under chain-of-custody procedures to a North Carolina certified laboratory for laboratory analysis. The groundwater sample was analyzed for VOCs by Standard Method 6200B and for EDB by Method 504.1. On March 31, 2020, after measuring the depth-to-water, S&ME personnel purged monitor wells MW-1, MW-2, MW-3 and MW-4 with disposable polyethylene bailers by evacuating approximately three well volumes of water. The field parameters pH, specific conductivity, and temperature were measured for each approximate casing volume and recorded on groundwater sampling forms, which are located in Appendix IV. After completing the well purging, S&ME personnel collected a groundwater sample from each of the monitor wells. During the sampling event, field blank sample FB-1 was prepared using laboratory supplied deionized water. The field blank and groundwater samples were placed in laboratory-supplied containers, stored in a cooler with ice, and shipped under chain-of-custody procedures to a North Carolina certified laboratory for laboratory analysis. Current and historical analytical results for groundwater samples are presented in Table 4, and current results shown on Figure 4. 5.7 Regional and Site Geology and Hydrogeology 5.7.1 Soil and Bedrock Based on the USDA Web Soil Survey (http://websoilsurvey.sc.egov.usda.gov/ App/WebSoilSurvey.aspx), soil survey data is not available for Caswell County. Subsurface soil characterized during soil borings, reported by others, consisted of silt, clayey silt and sandy silt. According to the 1982 USGS Geologic Map Region G, North Carolina, the site is underlain by Intermediate Intrusive Rocks, described as well-jointed, metamorphosed, and likely diorite in composition. Bedrock was not encountered at a depth of 30 ft-bls in previous soil borings performed by others. 5.7.2 Hydrogeology Based upon measurements collected on March 31, 2020, groundwater flow conditions in the source area are to the north northwest, with an approximate gradient of 0.029 feet per foot. Figure 3 graphically depicts the approximate relative groundwater surface elevations and flow direction at the site based upon the March 31, 2020 measurements. Groundwater occurrence was approximately 1.0 to 1.25 feet lower in elevation on March 31, 2020, relative to elevations observed during March 2019. The hydraulic conductivity of soil underlying the site and the seepage velocity of groundwater at the site have not been determined. Corrective Action Performance/Monitoring Report Duke Foster/D&M Market, Incident 18698; UST No. WS-5420 Intersection of Nicks Road and NC Highway 119 South Mebane, Caswell County, North Carolina S&ME Project No. 4305-17-214B May 26, 2020 9 6.0 Evaluation of Assessment Information 6.1 Current Extent and Severity of Contamination 6.1.1 Groundwater The March 2020 constituent concentrations were generally less than the March 2019 values for monitor wells MW-1 and MW-4. Constituent concentrations in MW-2 have not shown a decreasing trend since the March 2019 sampling event. The reported March 2020 MW-3 constituent concentrations were similar to the 2019 concentrations, with the exception of a greater benzene concentration. The horizontal extent of the dissolved plume exceeding the 2L Standard is not defined. The dissolved constituent plume appears to be migrating to the intersection of Hicks Road and Highway 119. The installation of downgradient monitor wells in the intersection, and in the right-of-way, is likely not feasible. However, MW-2 and MW-9, installed for Incident #9615_HA Walkers Store, are each located in downgradient positions relative to the subject site, and may be used as downgradient monitoring points for the subject incident. Based on the presence of monitor wells MW-3 and MW-4 at or near the property boundary, the leading edge of the dissolved plume has migrated beyond the property boundary, including Benzene concentrations greater than the GCL in MW-3. Also, Benzene concentrations reported for downgradient monitor well MW-3 are greater than those in source monitor well MW-1, indicating that the dissolved constituent plume is migrating from the source area. The vertical extent of the dissolve plume exceeding the 2L Standard is not defined. However, dissolved VOC concentrations reported for air sparge well AS-1, installed between shallow monitor wells MW-1 and MW-3, and with a screened interval approximately 15 feet below the water table, were significantly less than values reported for monitor wells MW-1 and MW-3. Isoconcentration Contour Maps for Benzene and EDB are provided as Figure 5 and Figure 6, respectively. A copy of the laboratory analytical report is included in Appendix VII. 6.1.2 LNAPL LNAPL was measured at a thickness of 0.03 feet in monitor well MW-1 following installation during July 2015 and during February 2016. The UST Incident meets the following conditions for a High Risk classification: A. There are four active, potable drinking water supply wells within 1,000 feet of the source area, and two non-drinking wells located within 250 feet of the source area. B. Dissolved VOCs were detected at concentrations greater than the 2L Standards in each monitor well sampled, and Benzene was reported at concentrations greater than the GCL in monitor well MW-3. C. Soil impacts greater than the soil-to-groundwater and the residential MSCCs remain beneath the former UST basin. 2. The dissolved constituent plume appears to be unstable, based on completion of five monitoring events, and the dissolved plume appears to be migrating beyond the subject property boundary. 3. Observations of the 96-hour hybrid MMPE/air sparge pilot test suggest that soil vapor extraction with air sparging can be effective in reducing dissolved constituent concentrations. 7.2 Recommendations Based on the above, the following recommendations are presented: 1. Install soil vapor extraction wells in the source and at locations with the greatest soil impacts. 2. Continue soil vapor extraction with air sparging to reduce source area soil and dissolved impacts. PID measurement at discharge stack was 11,400 parts per million (ppm). 16:20- Turn air sparge back on. Increased pressure from 1,300 pounds per square inch (psi) to 1,800 psi. All wells located. 0950: Geologic Exploration (GEX) offloads rig and sets up at SPG-1. 1000: Begin drilling SPG-1 1050: SPG-1 drilled to 38’. Set well at 37.5’ with 2.5’ screen. See well construction form for full construction information. 1215: Well is set and grouted in. Drill crew takes lunch break. 1245: Drillers are back from lunch.
