Dear Mr. Dill:
On May 9, 2023, the Underground Storage Tank Section of the Division of Waste Management, Mooresville Regional Office, received the Limited Site Assessment Report for the above-referenced site. This report has been reviewed
and will remain in the UST Section’s files. Upon review, the Division has determined that the report is incomplete. Specifically, it does not appear that the Request for Water Supply Well Information forms were distributed to all property
occupants/managers within a 500 foot radius of the release. You must send these forms out immediately. Documentation
of this must be received by this office within 60 days of the date of this letter.
