Miller) 7327-G W.Friendly Avenue Greensboro, North Carolina 27410 Site Setting The approximately 0.48-acre site is located in a residential and commercial area and is currently occupied by a motorcycle repair shop in Winston-Salem, Forsyth County, North Carolina (Exhibit 1).The motorcycle repair shop formerly operated two 2,000-gallon gasoline underground storage tanks (USTs), one 550-gallon kerosene UST, one 550-gallon used motor oil UST, and one 270- gallon kerosene UST from the 1950s until July 2007.The former USTs and dispenser islands appear to have been located on the northwest portion of the site, along Old Salisbury Road. Release Summary On July 25, 2007,GeoScience Technology, P.A. supervised the removal of the five former USTs at the site.Closure soil samples collected from beneath the base of the former 270-gallon kerosene UST (UST-3), former 550-gallon kerosene UST (UST-4), and dispenser island DP-1 contained total petroleum hydrocarbons -diesel range organics (TPH-DRO) and TPH-gasoline range organics (TPH-GRO) at concentrations exceeding their respective NCDEQ Guideline Action Levels in effect at that time of 10 milligrams per kilogram (mg/kg).Subsequently,Agra Environmental, Inc. (Agra) installed two groundwater monitoring wells (MW-1 and MW-2)to depths of 38 feet below ground surface (bgs) and 40 feet bgs, respectively. Monitoring well MW- 1 was advanced through the tank pit of former UST-3 and well MW-2 was advanced through the tank pit of former UST-4 and near DP-1. Two soil samples were collected at depths of 10 feet bgs Letter Report Former Village Mart ■Winston-Salem,NC ■Incident No. 37035 October 3, 2018 ■Terracon Project No.75179046 Responsive ■ Resourceful ■ Reliable and 20 feet bgs from each monitoring well boring.The soil samples were analyzed for volatile organic compounds (VOCs) by USEPA Method 8260 including isopropyl ether (IPE) and methyl tert-butyl ether (MTBE),and semi-volatile organic compounds (SVOCs) by USEPA Method 8270. Additionally, the soil samples collected from 10 feet bgs from each boring were analyzed for volatile petroleum hydrocarbons (VPH) and extractable petroleum hydrocarbons (EPH) by the MADEP Methods.Analytical results indicated the presence of several petroleum constituents at concentrations exceeding their respective laboratory reporting limits but below their respective NCDEQ Soil-to-Groundwater Maximum Soil Contaminant Concentrations (MSCCs)in soil samples collected from both borings. Soil samples collected from the well MW-1 boring at a depth of 10 feet bgs contained C9-C22 aromatics at a concentration exceeding its Soil-to-Groundwater MSCC but below its NCDEQ Residential Cleanup Standard. Previous Investigations Agra collected groundwater samples from monitoring wells MW-1 and MW-2 on July 17, 2009 and analyzed them for VOCs by USEPA Method 8260, SVOCs by USEPA Method 8270, VPH and EPH by the MADEP Method, and lead by USEPA Method 3030C. Groundwater analytical results identified the presence of several petroleum constituents at concentrations exceeding their respective laboratory reporting limits but below their respective 15A NCAC 2L Groundwater Quality Standards (2L Standards).The groundwater samples from monitoring well MW-2 contained bromodichloromethane and C9-C22 aromatics at concentrations exceeding their respective 2L Standards but below their respective NCDEQ Gross Contamination Limits (GCLs). Lead was detected in groundwater samples from monitoring wells MW-1 and MW-2 at concentrations exceeding its 2L Standard but below its GCL. A second groundwater sampling event was conducted by Agra in November 2009. Groundwater samples collected from monitoring well MW-1 were analyzed for lead by USEPA Method 3030C and samples from well MW-2 were analyzed for VOCs by USEPA Method 6200B, SVOCs by USEPA Method 625, VPH and EPH by the MADEP Methods, and lead by USEPA Method 3030C. Analytical results identified the presence of C11-C22 aromatics and tetrachloroethylene (PCE) at concentrations exceeding their respective 2L Standards but below their respective GCLs in groundwater samples from well MW-2. Agra collected additional groundwater samples from well MW-2 on July 6, 2010 and analyzed them for VOCs by USEPA Method 6200B, SVOCs by USEPA Method 610, and VPH and EPH by the MADEP Methods. Monitoring well MW-1 was not sampled. Analytical results identified the presence of PCE and methyl tert-butyl ether (MTBE) at concentrations exceeding their laboratory reporting limits but below their respective 2L Standards. Crawford Environmental Services (Crawford) mobilized to the site on June 7, 2012 to sample monitoring wells MW-1 and MW-2 and off-site water supply well WSW-1612 for VOCs by USEPA Method 6200B. Analytical results identified the presence of several petroleum constituents at concentrations exceeding their respective laboratory reporting limits but below their respective 2L Standards in groundwater samples from monitoring wells MW-1 and MW-2. Analytical results from monitoring well MW-2 indicated the presence of benzene and PCE at concentrations exceeding their respective 2L Standards but below their respective GCLs. Groundwater samples collected from water supply well WSW-1612 did not contain petroleum constituents at concentrations exceeding their respective laboratory reporting limits. Crawford mobilized to the site on December 27, 2012 to sample monitoring wells MW-1 and MW- 2 and off-site water supply well WSW-1612 for VOCs by USEPA Method 6200B. Analytical results identified the presence of several petroleum constituents at concentrations exceeding their respective laboratory reporting limits but below their respective 2L Standards in monitoring wells MW-1 and MW-2.PCE was detected in groundwater samples from monitoring well MW-1 at concentrations exceeding its 2L standard but below its GCL. Analytical results from monitoring well MW-2 indicated the presence of benzene and PCE at concentrations exceeding their respective 2L Standards but below their respective GCLs. Groundwater samples collected from water supply well WSW-1612 did not contain petroleum constituents at concentrations exceeding their respective laboratory reporting limits. Site Reconnaissance Terracon mobilized to the site on April 20, 2018 to locate and gauge the monitoring wells and take site photographs.Monitoring well locations are shown in Exhibit 2.Liquid non-aqueous phase liquid (LNAPL or free product)was not detected in the on-site monitoring wells.The depth to water was measured at 28.33 feet below top-of-casing (btoc)in monitoring well MW-1 and 26.20 feet btoc in well MW-2.A filling station housing several petroleum USTs was observed approximately 85 feet northwest and topographically up to cross-gradient to the site, across Old Salisbury Road. Groundwater monitoring wells were not observed on the adjacent site.The potential exists that a release from the adjacent filling station’s UST system could impact the site. Water Supply Wells In an effort to update previously collected receptor survey information, Terracon completed a walking/driving reconnaissance in the site vicinity on April 20, 2018. Based on our review of the prior receptor survey and information obtained from the site vicinity reconnaissance and the Forsyth County GIS website,six water supply wells were identified within 500 feet of the site, and four wells were identified between 500 and 1,000 feet of the site.Water supply wells were not identified beyond 1,000 feet of the site.Water supply well information is included in Table 1, and a map showing water supply well locations is included as Exhibit 3. Public Water Supplies Based on information obtained from the City of Winston-Salem Water and Sewer Department, the public water sources for Forsyth County are Salem Lake,the Reddies River and the Yadkin River, all of which are located upgradient to the site.According to the Winston-Salem Water and Sewer Department, municipal water is available to the site and surrounding properties. Letter Report Former Village Mart ■Winston-Salem,NC ■Incident No. 37035 October 3, 2018 ■Terracon Project No.75179046 Responsive ■ Resourceful ■ Reliable Surface Water Based on our review of the USGS Topographic Map,the site is located in the drainage basin for Sides Branch which, at its closest point, is located approximately 730 feet east of the site.Nearby surface water bodies are shown on Exhibit 1. Sensitive Receptors First Alliance Church, Advent Moravian Church and Heritage Hill Baptist Church are located approximately 750 feet north,800 feet northeast and approximately 1,200 feet northwest of the source area of the release, respectively. All three churches are located topographically upgradient to the site. Other sensitive receptors were not identified within 1,500 feet of the source area of the release. Groundwater Sampling Activities Terracon mobilized to the site on April 20, 2018 to sample on-site monitoring wells MW-1 and MW-2. The approximate locations of the monitoring wells are shown on Exhibit 2. Prior to sampling, depth to groundwater measurements were collected from wells MW-1 and MW-2 using a decontaminated electronic water level meter.The depth to water was measured at 28.33 feet btoc in monitoring well MW-1 and 26.20 feet btoc in well MW-2. Prior to sampling, the wells were purged a minimum of three well volumes. Purge water from the monitoring wells was disposed on-site in accordance with NCDEQ guidelines. The wells were allowed to recharge a minimum of 90 percent of the original well volume before samples were collected.The groundwater samples were hand-bailed with disposable plastic bailers, placed in laboratory-supplied glassware, labeled and packed on ice in an insulated container. The samples, along with chain-of-custody documentation, were transported via courier to Prism Laboratories in Charlotte, North Carolina for analysis of VOCs by USEPA Method 6200B. Analytical Results LNAPL was not detected in the monitoring wells.The groundwater sample from well MW-1 contained several petroleum constituents at concentrations exceeding their respective laboratory reporting limits but below their respective 2L Standards.The well MW-1 sample also contained benzene (1,200 µg/L) and total xylenes (680 µg/L)at concentrations exceeding their respective 2L Standards but below their GCLs.The groundwater sample from well MW-2 contained benzene (1,500 µg/L), ethylbenzene (740 µg/L), methyl isobutyl ketone (130 µg/L), n-propylbenzene (100 µg/L), 1,2,4-trimethylbenzene (440 µg/L), toluene (2,800 µg/L), and total xylenes (4,000 µg/L) at concentrations exceeding their respective 2L Standards but below their GCLs. Petroleum concentrations generally decreased over time from removal of the on-site USTs until 2012. Since the 2012 groundwater sampling event, petroleum constituents have risen sharply in the on-site monitoring wells. Based on historical groundwater data, the potential exists that a release from the adjacent filling station’s UST system could be impacting the site.Current and Letter Report Former Village Mart ■Winston-Salem,NC ■Incident No. 37035 October 3, 2018 ■Terracon Project No.75179046 Responsive ■ Resourceful ■ Reliable historical groundwater analytical results are summarized on Table 2,and the current results are depicted on Exhibit 4. Field notes are presented in Appendix A,and a copy of the laboratory analytical report is provided in Appendix B. Conclusions and Recommendations n Six water supply wells were identified within 500 feet of the site, and four wells were identified beyond between 500 and 1,000 feet of the site.Water supply wells were not identified beyond 1,000 feet of the site. n Municipal water is available to the site and site vicinity. n The closest surface water feature is Sides Branch which, at its closest point,is located approximately 730 feet east of the site. n The groundwater sample from monitoring well MW-1 contained benzene and total xylenes at concentrations exceeding their respective 2L Standards. n The groundwater sample from monitoring well MW-2 contained benzene,ethylbenzene, methyl isobutyl ketone,n-propylbenzene,1,2,4-trimethylbenzene,toluene and total xylenes at concentrations exceeding their respective 2L Standards. n LNAPL was not detected in the monitoring wells, and petroleum constituents were not detected at concentrations exceeding their respective GCLs. n Based on historical groundwater data, the potential exists that a release from the adjacent filling station’s UST system could be impacting the site. Terracon recommends the following activities be completed at the site: n Install two additional groundwater monitoring well to establish groundwater flow direction and further investigate the possibility of a petroleum release from the adjacent filling station affecting the site. n Attempt to sample nearby water supply wells WSW-1,WSW-2, WSW-3, WSW-4, WSW- 5, and WSW-10 for VOCs.
