C.SITE HISTORY AND CHARACTERIZATION The purpose of this report is to present the current status of the groundwater quality at the Former Village Mart site.Terracon completed this report to summarize the findings of assessment activities performed on April 20 and June 10, 2019 at the former gasoline station located at 2260 Old Salisbury Road in Winston-Salem,Forsyth County,North Carolina(Exhibit 1).This Groundwater Monitoring Report was prepared for the site in response to a request by Mr. Herb Berger,Incident Manager with the NCDEQ Division of Waste Management, State Lead Program. The Groundwater Monitoring Report was prepared in general accordance with the 15A NCAC 2L .0115 regulations and the NCDEQ UST Section Guidelines for Assessmentand CorrectiveActionfor UST Releases,dated July15,2008,revised December 2013,under the supervisionof Mr.Zachary R. Tidmore, a North Carolina registered Professional Geologist (North Carolina License No.2611). C.1 Site Setting The approximately 0.48-acre site is located in a residential and commercial area and is currently occupied by a motorcycle repair shop in Winston-Salem, Forsyth County, North Carolina. The motorcycle repair shop formerly operated two 2,000-gallon gasoline underground storage tanks (USTs), one 550-gallon kerosene UST, one 550-gallon used motor oil UST, and one 270-gallon kerosene UST from the 1950s until July 2007.The former USTs and dispenser islands appear to have been located on the northwest portion of the site, along Old Salisbury Road. C.2 Release Summary On July 25, 2007, GeoScience Technology, P.A. supervised the removal of the five former USTs at the site. Closure soil samples collected from beneath the base of the former 270-gallon kerosene UST (UST-3), former 550-gallon kerosene UST (UST-4), and dispenser island DP-1 contained total petroleum hydrocarbons -diesel range organics (TPH-DRO) and TPH-gasoline range organics (TPH-GRO) at concentrations exceeding the respective NCDEQ Guideline Action Levels in effect at Federal/State Lead Monitoring Report Former Village Mart -Incident No.37035 ■Winston-Salem,NC September 30,2019 ■Terracon Project No.75197046 Responsive ■Resourceful ■Reliable 4 that time of 10 milligrams per kilogram (mg/kg).Subsequently, Agra Environmental, Inc. (Agra) installed two groundwater monitoring wells (MW-1 and MW-2) to depths of 38 feet bgs and 40 feet bgs, respectively. Monitoring well MW-1 was advanced through the tank pit of former UST-3 and well MW-2 was advanced through the tank pit of former UST-4 and near DP-1. Two soil samples were collected at depths of 10 feet bgs and 20 feet bgs from each monitoring well boring. The soil samples were analyzed for VOCs by USEPA Method 8260 including IPE and MTBE, and semi- volatile organic compounds (SVOCs) by USEPA Method 8270. Additionally, the soil samples collected from 10 feet bgs from each boring were analyzed for volatile petroleum hydrocarbons (VPH) and extractable petroleum hydrocarbons (EPH) by the MADEP Methods. Analytical results indicated the presence of several petroleum constituents at concentrations exceeding the respective laboratory reporting limits but below the NCDEQ Soil-to-Groundwater Maximum Soil Contaminant Concentrations (MSCCs) in soil samples collected from both borings. Soil samples collected from the well MW-1 boring at a depth of 10 feet bgs contained C9-C22 aromatics at a concentration exceeding its Soil-to-Groundwater MSCC but below its NCDEQ Residential Cleanup Standard. C.3 Previous Investigations Agra collected groundwater samples from monitoring wells MW-1 and MW-2 on July 17, 2009 and analyzed them for VOCs by USEPA Method 8260, SVOCs by USEPA Method 8270, VPH and EPH by the MADEP Method, and lead by USEPA Method 3030C. Groundwater analytical results identified the presence of several petroleum constituents at concentrations exceeding the laboratory reporting limits but below the 2L Standards.The groundwater samples from monitoring well MW-2 contained bromodichloromethane and C9-C22 aromatics at concentrations exceeding the 2L Standards but below the NCDEQ Gross Contamination Limits (GCLs). Lead was detected in groundwater samples from monitoring wells MW-1 and MW-2 at concentrations exceeding the 2L Standard but below the GCL. A second groundwater sampling event was conducted by Agra in November 2009.The groundwater sample collected from monitoring well MW-1 was analyzed for lead by USEPA Method 3030C and the sample from well MW-2 was analyzed for VOCs by USEPA Method 6200B, SVOCs by USEPA Method 625, VPH and EPH by the MADEP Methods, and lead by USEPA Method 3030C. Analytical results identified the presence of C11-C22 aromatics and tetrachloroethylene (PCE) at concentrations exceeding the 2L Standards but below the GCLs in the groundwater sample collected from well MW-2. Agra collected an additional groundwater sample from well MW-2 on July 6, 2010 and analyzed it for VOCs by USEPA Method 6200B, SVOCs by USEPA Method 610, and VPH and EPH by the MADEP Methods. Monitoring well MW-1 was not sampled. Analytical results identified the presence of PCE and MTBE at concentrations exceeding the laboratory reporting limits but below the 2L Standards. Analytical results identified the presence of several petroleum constituents at concentrations exceeding the laboratory reporting limits but below the 2L Standards in groundwater samples from monitoring wells MW-1 and MW-2. PCE was detected in groundwater samples from monitoring well MW-1 at a concentration exceeding the 2L Standard but below the GCL. Analytical results from monitoring well MW-2 indicated the presence of benzene and PCE at concentrations exceeding the 2L Standards but below the GCLs.The groundwater sample collected from water supply well WSW-1612 did not contain petroleum constituents at concentrations exceeding the laboratory reporting limits. Crawford mobilized to the site on December 27, 2012 to sample monitoring wells MW-1 and MW-2 and off-site water supply well WSW-1612 for VOCs by USEPA Method 6200B. Analytical results identified the presence of several petroleum constituents at concentrations exceeding the laboratory reporting limits but below the 2L Standards in monitoring wells MW-1 and MW-2. PCE was detected in the groundwater sample from monitoring well MW-1 at a concentration exceeding the 2L standard but below the GCL. Analytical results from monitoring well MW-2 indicated the presence of benzene and PCE at concentrations exceeding the 2L Standards but below the GCLs.The groundwater sample collected from water supply well WSW-1612 did not contain petroleum constituents at concentrations exceeding the laboratory reporting limits. C.4 Water Supply Wells Terracon completed a walking/driving reconnaissance in the site vicinity on April 20, 2018. Based on our review of the prior receptor survey and information obtained from the site vicinity reconnaissance and the Forsyth County GIS website,six water supply wells were identified within 500 feet of the site and four wells were identified between 500 and 1,000 feet of the site.Water supply wells were not identified beyond 1,000 feet of the site.A water line appears to have been constructed along Zeverly Street since the prior receptor survey.Water supply well information is included in Table 1, and a map showing water supply well locations is included as Exhibit 3. D.ASSESSMENT INFORMATION D.1 Groundwater Monitoring Well Construction On April 30, 2019 Terracon mobilized to the site to install three two-inch diameter groundwater monitoring wells (MW-3, MW-4, and MW-5) on the western site corner,northern site corner, and south of the on-site building, respectively, using hollow-stem augers. Terracon encountered auger refusal at approximately 10 bgs. Terracon remobilized to the site on June 6, 2019 to continue advancement of the boreholes using air rotary drilling methods/tooling. Soil lithology was evaluated from auger soil cuttings and air rotary blow-out. Lithology generally consisted of reddish-brown silty sand and bedrock consisted of grey to black granite.Photoionization detector (PID) readings of the soils were less than 0.1 parts per million (ppm). PID readings are included on the boring logs attached in Appendix A. Federal/State Lead Monitoring Report Former Village Mart -Incident No.37035 ■Winston-Salem,NC September 30,2019 ■Terracon Project No.75197046 Responsive ■Resourceful ■Reliable 6 The permanent Type II monitoring wells were installed in the boreholes as follows: n Installation of 10 feet of two-inch diameter, 0.010-inch machine slotted PVC well screen with a threaded bottom cap; n Installation of two-inch, threaded, flush-joint PVC riser pipe to surface; n Addition of pre-sieved 20/40 grade silica sand for annular sand pack around the well screen from the bottom of the boring to approximately two feet above the top of the well screen; n Addition of approximately two feet of hydrated bentonite chips on top of the sand pack; n Grout remaining annulus above the bentonite seal to the land surface; and, n Installation of a flush mount protective cover in a traffic-rated square concrete pad. Following installation,the wells were developed, and a locking expansion plug was used to secure and seal the top of each well casing.Top-of-casing elevations for monitoring wells MW-3, MW-4, and MW-5 were measured by Terracon using a transit level. Drill cuttings were spread on-site. The monitoring well construction details are summarized on Table 2. The boring logs are presented in Appendix A, and the well construction records are appended to the boring logs. The approximate locations of the monitoring wells are depicted on Exhibit 2. D.2 Sampling Activities Terracon mobilized to the site on June 10, 2019 to collect groundwater samples from monitoring wells MW-1,MW-2,MW-3,MW-4, and MW-5 and collect water samples from water supply wells WSW-1, WSW-2, WSW-3, WSW-4, WSW-5, and WSW-10. The approximate locations of the wells are shown on the Site Map (Exhibit 2). Photographs and field notes are included in Appendix B. D.2.2 Groundwater Sampling Activities Prior to sample collection,Terracon gauged the depth to groundwater in the monitoring wells.Depth to groundwater ranged from 24.26 feet below ground surface (bgs) to 25.84 feet bgs.Current and historical groundwater elevation measurements are provided in Table 2.Based on June 2019 groundwater elevations,shallow groundwater is estimated to flow towards the south-southeast (Exhibit 4). The monitoring wells were purged with a peristaltic pump using low flow sampling techniques (i.e., <200 milliliters per minute) prior to sampling. The following parameters were measured during purging: pH, temperature, oxidation reduction potential (ORP), dissolved oxygen (DO), and conductivity. Purge water from the monitoring wells was disposed in accordance with NCDEQ guidelines.The groundwater samples were placed into laboratory-supplied glassware, labeled, and stored in an ice-packed cooler. The groundwater samples, along with chain-of-custody documentation, were delivered to Pace National Center for Testing & Innovation (Pace)in Mount Juliet, Tennessee for analysis of VOCs by USEPA Method 6200B including MTBE, IPE and EDB. Federal/State Lead Monitoring Report Former Village Mart -Incident No.37035 ■Winston-Salem,NC September 30,2019 ■Terracon Project No.75197046 Responsive ■Resourceful ■Reliable 7 D.2.3 Water Supply Well Sampling Activities Water supply wells WSW-2, WSW-3,and WSW-4 were not sampled because a response was not received from the property owners regarding permission to sample the wells.According to the owners of water supply wells WSW-9 and WSW-10, the wells are no longer in use but the property owners are not interested in abandoning the wells at this time. Samples were collected from water spigots indicated by the property owners of wells WSW-1 and WSW-5. Prior to sampling, water was allowed to flow from owner-designated spigots for approximately 15 minutes in an effort to purge the lines of stagnant water. Flow from the spigots was reduced when the samples were collected directly into laboratory supplied containers. The water supply well samples were placed in laboratory-supplied glassware, labeled and stored in an ice-packed cooler. The samples, along with chain-of-custody documentation, were delivered to Pace for analysis of VOCs by USEPA Method 6200B including MTBE, EDB, and IPE. D.2.4 Analytical Results LNAPL and petroleum constituents exceeding the GCLs were not detected in the monitoring wells. Based on the laboratory analytical reports,six petroleum constituents (benzene, ethylbenzene, methyl isobutyl ketone,1,2,4-trimethylbenzene, toluene, and total xylenes)were identified at concentrations above 2L Standards in monitoring wells MW-1,MW-2,MW-4,and/or MW-5.Non- petroleum constituents bromodichloromethane and chloroform were detected in the sample collected from water supply well WSW-1. Current and historical groundwater analytical results are summarized on Table 3,and the current results are depicted on Exhibit 5. Field notes are presented in Appendix B,and a copy of the laboratory analytical report is provided in Appendix C. E.CONCLUSIONS AND RECOMMENDATIONS Based on information obtained during the completion of these assessment activities, Terracon provides the following findings and conclusions. n LNAPL was not detected in the monitoring wells, and petroleum constituents were not detected at concentrations exceeding the GCLs. n Six petroleum constituents were identified at concentrations exceeding the 2L Standards in monitoring wells MW-1 through MW-5.Water samples collected from wells WSW-1 and WSW-5 did not contain petroleum constituents at concentrations above the laboratory detection limits.Non-petroleum constituents bromodichloromethane and chloroform were detected in the samples collected from water supply well WSW-1. Based on the data presented in this Monitoring report, Terracon recommends the following activities be completed at the site: n Groundwater monitoring wells (MW-1 through MW-5) should be sampled annually to evaluate the status of impacts by petroleum constituents. n Attempt to abandon water supply wells WSW-1 through WSW-4. n Terracon confirmed that municipal water is available along Zeverly Street. Terracon recommends attempting to abandon water supply wells WSW-6, WSW-7, and WSW-8 and connect the residences to municipal water. NATIONWIDE.GLOSSARY OF TERMS Guide to Reading and Understanding Your Laboratory Report The information below is designed to better explain the various terms used in your report of analytical results from the Laboratory. This is not intended as a comprehensive explanation, and if you have additional questions please contact your project representative. Abbreviations and Definitions MDL Method Detection Limit. ND Not detected at the Reporting Limit (or MDL where applicable). RDL Reported Detection Limit. Rec.Recovery. RPD Relative Percent Difference. SDG Sample Delivery Group. (S) Surrogate (Surrogate Standard) - Analytes added to every blank, sample, Laboratory Control Sample/Duplicate and Matrix Spike/Duplicate; used to evaluate analytical efficiency by measuring recovery. Surrogates are not expected to be detected in all environmental media. U Not detected at the Reporting Limit (or MDL where applicable). Analyte The name of the particular compound or analysis performed. Some Analyses and Methods will have multiple analytes reported. Dilution If the sample matrix contains an interfering material, the sample preparation volume or weight values differ from the standard, or if concentrations of analytes in the sample are higher than the highest limit of concentration that the laboratory can accurately report, the sample may be diluted for analysis. If a value different than 1 is used in this field, the result reported has already been corrected for this factor. Limits These are the target % recovery ranges or % difference value that the laboratory has historically determined as normal for the method and analyte being reported. Successful QC Sample analysis will target all analytes recovered or duplicated within these ranges. Qualifier This column provides a letter and/or number designation that corresponds to additional information concerning the result reported. If a Qualifier is present, a definition per Qualifier is provided within the Glossary and Definitions page and potentially a discussion of possible implications of the Qualifier in the Case Narrative if applicable. Result The actual analytical final result (corrected for any sample specific characteristics) reported for your sample. If there was no measurable result returned for a specific analyte, the result in this column may state “ND” (Not Detected) or “BDL” (Below Detectable Levels). The information in the results column should always be accompanied by either an MDL (Method Detection Limit) or RDL (Reporting Detection Limit) that defines the lowest value that the laboratory could detect or report for this analyte. Uncertainty (Radiochemistry)Confidence level of 2 sigma. Case Narrative (Cn) A brief discussion about the included sample results, including a discussion of any non-conformances to protocol observed either at sample receipt by the laboratory from the field or during the analytical process. If present, there will be a section in the Case Narrative to discuss the meaning of any data qualifiers used in the report. Quality Control Summary (Qc) This section of the report includes the results of the laboratory quality control analyses required by procedure or analytical methods to assist in evaluating the validity of the results reported for your samples. These analyses are not being performed on your samples typically, but on laboratory generated material. Sample Chain of Custody (Sc) This is the document created in the field when your samples were initially collected. This is used to verify the time and date of collection, the person collecting the samples, and the analyses that the laboratory is requested to perform. This chain of custody also documents all persons (excluding commercial shippers) that have had control or possession of the samples from the time of collection until delivery to the laboratory for analysis. Sample Results (Sr) This section of your report will provide the results of all testing performed on your samples. These results are provided by sample ID and are separated by the analyses performed on each sample. The header line of each analysis section for each sample will provide the name and method number for the analysis reported. Sample Summary (Ss)This section of the Analytical Report defines the specific analyses performed for each sample ID, including the dates and times of preparation and/or analysis. Qualifier Description J The identification of the analyte is acceptable; the reported value is an estimate.
