(ATC) has completed a Monitoring Report (GMR), documenting the December 28, 2022, and January 13, 2023, sampling of three on-site monitoring wells and one water supply well at Moody’s Grocery located at 912 Old Highway 21, State Road, Wilkes County, North Carolina. The GMR is being performed based on a December 8, 2022, Task Authorization (TA-02) by the Trust Fund Branch of the Underground Storage Tank (UST) Section, Division of Waste Management, North Carolina Department of Environmental Quality (NCDEQ). The work is being performed as part of the State Lead Contract #N122019A between NCDEQ and ATC. Former petroleum USTs at the site released product to the subsurface prior to their decommissioning in 1998 and subsequent removal in 2000. During January 2019 site activities, ATC conducted soil gas sampling in MW-1, and installed Provectus® Oxygen Releasing Socks (ORS) in MW-1. The approved work included the installation of one monitoring well (MW-5); the measurement of the top of casing of MW-5; the sampling of wells MW-1 and MW-5; the measurement of water levels in wells MW-2, MW-3, and MW-4; and the sampling of five water supply wells (WSW 1, WSW-2, WSW-4, WSW-937, and WSW-955). On February 20 and 21, 2020, ATC installed groundwater monitoring well MW-5, at a location downgradient of the former UST basin. On February 21, 2020, ATC gauged and sampled groundwater monitoring wells MW-4 and MW-5. Monitoring well MW-4 was sampled instead of MW-1, and therefore, monitoring well MW-1 was sampled on April 10, 2020 along with the measurement of water levels in all monitoring wells. Site water supply well WSW-1 was also sampled. The current scope of work included obtaining groundwater samples from monitoring wells MW-1, MW-4, and MW-5, and water supply wells WSW-1 and WSW-2. The water samples were analyzed for the presence of VOCs indicative of the presence of dissolved petroleum Laboratory results from the December 28, 2022, and January 13, 2023, sampling events indicate the following:  Light non-aqueous phase liquid (LNAPL) was not found during the sampling event;  Monitoring well MW-1 contained benzene, naphthalene, and 1,2-dichloroethane (1,2- DCA) above the 15A NCAC 02L .0202 groundwater quality standards (2L Standards).  Monitoring well MW-5 contained benzene, toluene, ethylbenzene, total xylenes, 1,2- DCA, 1,2,4-trimethylbenzene, and 1,2-dibromoethane (EDB) in excess of the 2L Standards. The benzene and EDB concentrations also exceeded the respective Gross Contamination Levels (GCLs) of 5,000 micrograms per liter (µg/L) and 50 µg/L;  Volatile Organic Compounds (VOCs) were not detected in monitoring well MW-4;  Laboratory analysis of the sample obtained from WSW-1 did not identify any detectable concentrations of petroleum constituents;  Based on the January 13, 2023, groundwater elevations, groundwater flow is to the southeast; Monitoring Report February 27, 2023 Moody’s Grocery, Incident # 22446, State Road, Wilkes Co., NC ATC Project #SLC2244602 3  Based on historical data, VOC concentrations in MW-1 appear to vary seasonally. Concentrations of VOCs in MW-5 appear to be generally stable;  Based on the analytical results from monitoring well MW-5 the extent of VOCs in groundwater is not delineated to the east and southeast of MW-1 and MW-5. In addition, VOC concentrations are higher in well MW-5 than in the “source” well MW-1, suggesting that higher concentrations may be present in the groundwater west of MW-1; and  VOC concentrations in MW-5 constructed with a screened interval 40-50 feet indicate that the constituent plume has moved to the southeast of the source area at depth. Based on the well MW-5 sampling results and groundwater flow direction, ATC recommends the installation and sampling of four additional monitoring wells. These additional wells include one shallow well (MW-5S) adjacent to MW-5 to be screened across the groundwater surface to evaluate possible higher concentrations in the shallow groundwater; one shallow well (MW-6) southwest of MW-1 to evaluate possible higher concentrations in the source area. Two monitoring wells (MW-7 and MW-8) are recommended downgradient of the source area on the adjacent property Air rotary drilling will be needed for the installation of the deep monitoring well. Subsequent assessment and remediation activities would be evaluated following receipt of the groundwater quality data from the new monitoring wells. 3.0 SITE DESCRIPTION 3.1 Site Characterization The NCDEQ retained ATC to provide environmental consulting services for Moody’s Grocery, located at 912 Old Highway 21, in State Road, Wilkes County, North Carolina. Figure 1 depicts the site on a topographic map. The site currently operates as a retail convenience store that no longer has retail petroleum operations. The site is located near the intersection of Mining Church Road and Old Highway 21. 3.2 Site Background The site was developed with a single-story gasoline station in circa 1962 and is currently occupied by Moody’s Grocery. Historical records indicate that Moody’s Grocery was a retail petroleum facility until 1998, including one 6,000-gallon gasoline UST, two 3,000-gallon gasoline USTs, and one 550-gallon kerosene UST and associated piping and dispensers. The UST system ceased operation in 1998 and was removed in March 2000. During the 2000 UST removal, a 550-gallon UST of unknown content was found below the dispenser island. According to the property owner, Don Moody, the UST had been taken out of service in 1962 due to non-use. Soil analytical results indicated petroleum impact above the state action level of 10 milligrams per kilogram (mg/kg) in the area of the UST basin for the two 3,000-gallon gasoline USTs, one 550-gallon kerosene UST, and 550-gallon UST of unknown content. During a modified Limited Site Assessment (LSA) in December 2002, monitoring wells MW-1 and MW-2 were installed to a depth of 60 feet below ground surface. Monitoring well MW-1 was installed in the former gasoline UST basin and monitoring well MW-2 was installed in the former kerosene UST basin. Groundwater analytical results from both monitoring wells indicated Monitoring Report February 27, 2023 Moody’s Grocery, Incident # 22446, State Road, Wilkes Co., NC ATC Project #SLC2244602 4 target petroleum constituents exceeded the 2L Standards. In addition, a well receptor survey identified 31 potable wells within 1,500 feet of the source area, including on-site potable well WSW-1. Because municipal water was not supplied to the area and due to the groundwater impact, the site was ranked as High Risk. Between 2002 and 2005, additional assessment was performed that included monitoring of the two site monitoring wells, water supply well WSW-1, and six nearby water supply wells. Groundwater analytical results from 2005 indicated detectable concentrations of naphthalene and total xylenes in well WSW-1 that did not exceed the 2L Standard. Groundwater impact above 2L Standards for benzene and 1,2-dichloroethane were identified in potable well WSW-6, located approximately 500 feet east southeast of the source area, and 2L Standards were exceeded for benzene, methyl tertiary butyl ether (MTBE), 1,4-dicholorobenzene, and 1,2-dichloroethane for potable well WSW-2, located approximately 170 feet east of the source area. Due to the impact in the potable wells, a point of entry (POE) filtration system was installed by the NCDEQ on wells WSW-2 and WSW-6. A March 2008 NCDEQ correspondence indicated that the NCDEQ planned to remove the POE filtration system from well WSW-6 because petroleum concentrations had not been detected since February 2008. The POE system was removed on August 5, 2013. ATC performed groundwater monitoring at the site in July 2014. Groundwater impact was identified above 2L Standards in monitoring wells MW-1 and MW-2. None of the water supply wells that were sampled contained impact above 2L Standards. An updated receptor survey was completed identifying 32 water supply wells within 1,500 feet of the site, 21 of which are located within 1,000 feet of the site. Two water supply wells (WSW-4 and WSW-4616) are located downgradient and within 500 feet of the site. Five additional wells (WSW-5, WSW-250, WSW- 255, WSW-259, and WSW-4562) are also located downgradient from the site, and are within 1,000 feet of the site. In July 2015, ATC completed a groundwater monitoring event for the site. Results from the groundwater monitoring event indicated remaining impact above 2L Standards in monitoring well MW-1. Other petroleum constituents in wells MW-1 and MW-2 were below 2L Standards. No petroleum constituents were detected in wells MW-3, MW-4, or in the three water supply wells (WSW-1, WSW-3, and WSW-6) which were sampled. In November, 2017, ATC completed a groundwater monitoring event which indicated remaining VOC impact above 2L Standards in monitoring wells MW-1 and MW-3. Following the sampling event, ATC installed ORS in monitoring well MW-1. On June 14, 2018, ATC collected groundwater samples from monitoring wells MW-1, MW-2, MW-3, and MW-4. Benzene, 1,2-dichloroethane, ethylene dibromide (EDB), and isopropyl ether concentrations exceeded the 2L Standards in monitoring well MW-1. Note that the concentrations decreased by 37 percent (isopropyl benzene) to 87 percent (benzene) compared to the November 2017 sampling results, excluding EBD which was not detected in November 2017 (elevated detection limit). Monitoring Report February 27, 2023 Moody’s Grocery, Incident # 22446, State Road, Wilkes Co., NC ATC Project #SLC2244602 5 On January 8, 2019, ATC collected a soil vapor sample from monitoring well MW-1 to assess remaining soil impact in the vadose zone soils. Following collection of the soil-gas sample, ATC installed new ORS in monitoring well MW-1. Based on the January 2019 work, ATC recommended installation of one additional monitoring well (MW-5) to further characterize groundwater impact; and conducting soil vapor extraction using a new SVE well near MW-1 in an effort to remove petroleum from the subsurface and expedite subsequent remediation by natural attenuation. On February 20 and 21, 2020, ATC installed groundwater monitoring well MW-5, at a location downgradient of the former UST basin. On February 21, 2020, ATC gauged and sampled groundwater monitoring wells MW-4 and MW-5. Monitoring well MW-1 was sampled on April 10, 2020, along with the measurement of water levels in all monitoring wells. Site water supply well WSW-1 was also sampled. Groundwater monitoring wells MW-1 and MW-5 contained VOCs in excess of the 2L standard and EDB was detected in MW-5 in excess of the GCL. Laboratory analysis of the sample obtained from WSW-1 did not identify any detectable concentrations of petroleum constituents. ATC recommended the installation of downgradient wells to evaluate the horizontal extent of contaminant migration ATC obtained groundwater samples from MW-1, MW-2, MW3-, MW-4, MW-5 and WSW-1 and WSW-2. Laboratory results from February 4, 2021, sampling event indicate the following: light non-aqueous phase liquid (LNAPL) was not found in the monitoring well network. Monitoring well MW-1 contained benzene, total xylenes, diisopropyl ether (DIPE), naphthalene, 1,2- dichloroethane and (1,2-DCA) above the 15A NCAC 02L .0202 groundwater quality standards (2L Standards). Monitoring well MW-5 contained benzene, toluene, ethylbenzene, total xylenes, methyl tert butyl ether (MTBE), DIPE, naphthalene, 1,2,-DCA, ethylene dibromide (EDB), and 1,2,4-trimethylbenzene in excess of the 2L Standards. The benzene and EDB concentrations also exceeded the Gross Contamination Level (GCL). Volatile Organic Compounds (VOCs) were not detected in monitoring wells MW-2, MW-3, and MW-4. Laboratory analysis of the sample obtained from WSW-1 did not identify any detectable concentrations of petroleum constituents. Based on the February 4, 2021, groundwater elevations, groundwater flow is to the southeast. Based on historical data, ATC concluded that VOC concentrations in MW-1 appear to vary seasonally. Concentrations of VOCs in MW-5 appear to be increasing. Analytical results from monitoring well MW-5 suggested the extent of VOCs in groundwater is not delineated to the east and southeast of MW-1 and MW-5. In addition, VOC concentrations are higher in well MW-5 than in the “source” well MW-1, suggesting that higher concentrations may be present in the groundwater west of MW-1. VOC concentrations in MW-5 constructed with a screened interval 40-50 feet indicate that the constituent plume has moved to the southeast of the source area at depth. ATC recommended the installation and sampling of two additional monitoring wells. These additional wells include one shallow well (MW-5S) adjacent to MW-5 to be screened across the groundwater surface to evaluate possible higher concentrations in the shallow groundwater and Monitoring Report February 27, 2023 Moody’s Grocery, Incident # 22446, State Road, Wilkes Co., NC ATC Project #SLC2244602 6 one shallow well (MW-6) west of MW-1 to evaluate possible higher concentrations in the source area. 3.3 Site and Regional Geology According to the Geologic Map of North Carolina, dated 1985, the property lies within the Blue Ridge Physiographic Province, in the Slate of Copperhill Formation of the Ocoee Supergroup. The metamorphic rock of the Slate of Copperhill Formation is comprised of slate, phyllite, and metagraywacke that indicates graded bedding. In most areas, the upper Blue Ridge subsurface consists of unconsolidated residual soil, variously known as residuum, saprolite, or partially weathered rock (PWR) that has developed from in-place weathering of the underlying metamorphic bedrock. The residual soils vary in consistency and depth based on local conditions. Bedrock can be found at the surface as outcrops and to depths of greater than 100 feet in areas of easily weathered rock types. Sites near streams and rivers in the Blue Ridge may also contain alluvium deposited during flood events. Boring logs from the May 2010 letter report by Terraine indicated that the site subsurface consisted of brown to red clay in the upper approximately 14 feet of subsurface soils, followed by reddish brown to yellowish brown silt or clayey silt from approximately 20 ft bgs to the termination of the boring, which varied from 52 ft bgs in boring MW-3 to 45 ft bgs in boring MW-4. Bedrock was encountered at well MW-2 at a depth of 50 ft bgs. During the drilling for well MW-5, clayey silt and silt was observed to the termination depth of 50 ft bgs. Air rotary drilling was needed to drill deeper than 29 ft bgs, where auger refusal was encountered due to dense silt. Neither partially weathered rock nor bedrock were encountered, and depth to top of bedrock is unknown. 3.4 Site and Regional Hydrogeology The shallow aquifer occurrence varies in depth from ground surface at springs, creeks, and rivers to as deep as 50 feet or more beneath upland surfaces in some parts of the Blue Ridge. ATC encountered groundwater between 26.80 (MW-5) and 32.42 (MW2) feet below tops of casing (TOC) in the monitoring wells at the site during the February 4, 2021, monitoring event. Permeability varies with lithology and is typically relatively low in residual soils, with higher permeability in saprolite or PWR due to relict rock texture and variable susceptibility to weathering exhibited by different minerals in the rock. Groundwater flow in residual soils or alluvium is usually in rough concurrence with local topographic conditions and is toward local drainage features. Based on the existing site wells, the groundwater flow appears to channel to the central part of the site and flows south-southeast. The bedrock fractures or other planar features of the Blue Ridge Physiographic Province generally constitute the bedrock aquifer, with the surrounding rock material being effectively impermeable. Along with fractures, contacts between rock bodies probably constitute zones of significant groundwater occurrence in the bedrock. The surrounding material and overlying residuum tend to make the bedrock aquifer a semi-confined aquifer. That is, the overlying water and soil weight normally results in pressure that causes water in a borehole, which intersects a Monitoring Report February 27, 2023 Moody’s Grocery, Incident # 22446, State Road, Wilkes Co., NC ATC Project #SLC2244602 7 fracture or other feature, to rise above the elevation of the fracture or feature. Most modern water supply wells in the Blue Ridge consist of a casing set through the residual soils into the upper bedrock followed by an open borehole into the bedrock where water bearing fractures were encountered during the drilling. Except for attempted sampling of nearby water supply wells, evaluation of bedrock water quality or hydrology was beyond the scope of the GMR. 4.0 GROUNDWATER CHARACTERIZATION On December 28, 2022, ATC visited the site to conduct the approved sampling. ATC sampled monitoring well MW-1 and water supply well WSW-1. However due to freezing temperatures, water within the MW-4 and MW-5 well vaults had frozen solid. ATC returned to the site on January 13, 2023, and sampled wells MW-4 and MW-5, and measured the water levels in wells MW-1 and MW-3. Access to water supply well WSW-2 was not available on either date. 4.1 Water Supply Wells Twenty-one water supply wells have been identified within 1,000 feet of the site, with nine of the wells within 500 feet of the site. Groundwater flow at the site is to the southeast, with two most at-risk wells: on-site well WSW-1 and the well immediately to the east of the site, WSW-2. Seven of the water supply wells are located downgradient of the site. Figure 2 depicts the locations of the water supply wells on an aerial overlay with radial distances from the site. Table 1 summarizes water supply well and other receptor information. On December 28, 2022, ATC collected a groundwater sample from the on-site water supply well (WSW-1). Water supply well WSW-2 was not accessible during both sampling events and could not be sampled. Field notes and photographs from December 28, 2022, and January 13, 2023, sampling events are presented as Appendix A. Laboratory analysis of the water supply well sample did not identify any detectable concentrations of petroleum constituents. A summary of current and historical water supply well analytical data is included in Table 2. 4.2 Groundwater Levels On January 13, 2023, water levels were gauged in monitoring wells MW-1, MW-2, MW-3, MW- 4, and MW-5. Well construction details are presented in Table 3 and the groundwater elevation data is presented in Table 4. Figure 3 depicts a site map. Groundwater elevations were then calculated based on the measured elevations, which are referenced to an on-site benchmark of 1,330.00 feet at the well MW-1 TOC. Depth to groundwater elevation data are summarized in Table 4. Based on the results of the January 13, 2023, groundwater measurements, groundwater flow is to the southeast with a gradient of approximately 0.037 feet/foot. A groundwater elevation contour map is presented in Figure 4. 4.3 Groundwater Quality On December 28, 2022, a groundwater sample was collected from monitoring well MW-1. On January 13, 2023, groundwater samples were collected from monitoring wells MW-4 and MW-5. During sampling activities, a three-volume well purge was implemented prior to groundwater sample collection to ensure a representative groundwater sample. The samples were collected in laboratory-supplied, preserved containers; entered into chain of custody control; placed on ice; and Monitoring Report February 27, 2023 Moody’s Grocery, Incident # 22446, State Road, Wilkes Co., NC ATC Project #SLC2244602 8 shipped to North Carolina accredited Pace Analytical Services, Inc. in Huntersville, North Carolina. The water samples were analyzed by EPA Method 6200B for VOCs including petroleum constituents. The analytical reports are included in Appendix B. Groundwater field parameters including temperature, dissolved oxygen, conductivity, pH, and oxidation-reduction potential were measured during the monitoring event. Field parameter readings for each well are presented in Table 5. Groundwater analytical results from the December 28, 2022, and January 13, 2023, monitoring events are summarized in Table 6. Historical groundwater analytical results are shown in Table 7. The groundwater analytical results were compared to the 2L Standards and GCLs. Groundwater analytical results indicate the following:  Monitoring well MW-1 contained benzene, naphthalene, and 1,2-DCA above the 2L Standards.  Monitoring well MW-5 contained benzene, toluene, ethylbenzene, total xylenes, 1,2- DCA, 1,2,4-trimethylbenzene, and EDB in excess of the 2L Standards. The benzene (9,350 µg/L) and EDB (152 µg/L) concentrations also exceeded the respective GCLs of 5,000 µg/L and 50 µg/L;  VOCs were not detected in monitoring well MW-4;  Laboratory analysis of the sample obtained from WSW-1 did not identify any detectable concentrations of petroleum constituents; Figure 5 depicts a groundwater quality map with detected constituents above 2L Standards. As part of ATC’s quality assurance and quality control (QA and QC) measures, a field blank was collected, and a data review of the laboratory reports was conducted. During the review, it was found that the samples were received from the laboratory in acceptable condition and properly preserved. The samples were analyzed within correct holding times and reported within the lab report, and the chain of custody was properly completed by all parties. The laboratory QC report shows that an internal blank was analyzed for each method that was used for the original samples. The laboratory fortified blank (LFB) and laboratory control sample (LCS) data results were also provided for each analytical method. ATC collected a field blank during the December 28, 2022, groundwater monitoring activities. No concentrations of petroleum constituents were detected in the field blank. Refer to the laboratory reports in Appendix B for details regarding QA results for the laboratory samples and any noted exceptions. Overall, ATC concludes that the laboratory results are usable and any exceptions do not significantly impact the conclusions presented in this report. 5.0 CONCLUSIONS AND RECOMMENDATIONS On behalf of the Trust Fund Branch of the UST Section, ATC has completed a GMR documenting the December 28, 2022, and January 13, 2023, groundwater monitoring events for Moody’s Grocery, located at 912 Old Highway 21, State Road, Wilkes County, North Carolina. The following conclusions were derived from groundwater monitoring activities and analytical reports:  LNAPL was not found during sampling activities; Monitoring Report February 27, 2023 Moody’s Grocery, Incident # 22446, State Road, Wilkes Co., NC ATC Project #SLC2244602 9  Monitoring well MW-1 contained benzene, naphthalene, and 1,2-DCA above the 2L Standards.  Monitoring well MW-5 contained benzene, toluene, ethylbenzene, total xylenes, 1,2- DCA, 1,2,4-trimethylbenzene, and EDB in excess of the 2L Standards. The benzene and EDB concentrations also exceeded the respective GCLs of 5,000 µg/L and 50 µg/L;  VOCs were not detected in monitoring well MW-4;  Laboratory analysis of the sample obtained from WSW-1 did not identify any detectable concentrations of petroleum constituents;  Based on the January 13, 2023, groundwater elevations, groundwater flow is to the southeast;  Based on historical data, VOC concentrations in MW-1 appear to vary seasonally. Concentrations of VOCs in MW-5 appear to be generally stable;  Based on the analytical results from monitoring well MW-5 the extent of VOCs in groundwater is not delineated to the east and southeast of MW-1 and MW-5. In addition, VOC concentrations are higher in well MW-5 than in the “source” well MW-1, suggesting that higher concentrations may be present in the groundwater west of MW-1.  VOC concentrations in MW-5 constructed with a screened interval 40-50 feet indicate that the constituent plume has moved to the southeast of the source area at depth. Based on the well MW-5 sampling results and groundwater flow direction ATC recommends the installation and sampling of four additional monitoring wells. ND - Not Detected at or above adjusted reporting limit. TNTC - Too Numerous To Count J - Estimated concentration above the adjusted method detection limit and below the adjusted reporting limit. MDL - Adjusted Method Detection Limit. PQL - Practical Quantitation Limit. RL - Reporting Limit - The lowest concentration value that meets project requirements for quantitative data with known precision and bias for a specific analyte in a specific matrix. S - Surrogate 1,2-Diphenylhydrazine decomposes to and cannot be separated from Azobenzene using Method 8270. The result for each analyte is a combined concentration. Consistent with EPA guidelines, unrounded data are displayed and have been used to calculate % recovery and RPD values. LCS(D) - Laboratory Control Sample (Duplicate) MS(D) - Matrix Spike (Duplicate) DUP - Sample Duplicate RPD - Relative Percent Difference NC - Not Calculable. SG - Silica Gel - Clean-Up U - Indicates the compound was analyzed for, but not detected. Acid preservation may not be appropriate for 2 Chloroethylvinyl ether. A separate vial preserved to a pH of 4-5 is recommended in SW846 Chapter 4 for the analysis of Acrolein and Acrylonitrile by EPAMethod 8260. N-Nitrosodiphenylamine decomposes and cannot be separated from Diphenylamine using Method 8270. The result reported for each analyte is a combined concentration. Reported results are not rounded until the final step prior to reporting. Therefore, calculated parameters that are typically reported as "Total" may vary slightly from the sum of the reported component parameters. Pace Analytical is TNI accredited. Contact your Pace PM for the current list of accredited analytes. TNI - The NELAC Institute. ANALYTE QUALIFIERS The continuing calibration for this compound is outside of Pace Analytical acceptance limits. The results may be biased low.CL Sample was diluted due to the presence of high levels of non-target analytes or other matrix interference.D3 Reanalysis conducted in excess of EPA method holding time. Results confirm original analysis performed in hold time.H5 Matrix spike recovery exceeded QC limits. Batch accepted based on laboratory control sample (LCS) recovery.M1 The EPA or method required sample preservation degrades this compound, therefore acceptable recoveries may not be achieved in sample matrix spikes.P5 REPORT OF LABORATORY ANALYSIS This report shall not be reproduced, except in full, without the written consent of Pace Analytical Services, LLC.Date: 01/05/2023 02:27 PM Pace Analytical Services, LLC 9800 Kincey Ave. Suite 100 Huntersville, NC 28078 (704)875-9092 Page 25 of 29 #=CR# QUALITY CONTROL DATA CROSS REFERENCE TABLE Pace Project No.: Project: 92644184 MOODY'S GROCERY STATE ROAD, NC Lab ID Sample ID QC Batch Method QC Batch Analytical Method Analytical Batch 92644184003 746629 746747WSW#1 EPA 504.1 EPA 504.1 92644184001 747491MW#1 SM 6200B 92644184002 747036WSW#1 SM 6200B 92644184004 747036FBSM 6200B 92644184005 747036TRIP BLANK SM 6200B REPORT OF LABORATORY ANALYSIS This report shall not be reproduced, except in full, without the written consent of Pace Analytical Services, LLC.Date: 01/05/2023 02:27 PM Pace Analytical Services, LLC 9800 Kincey Ave. Suite 100 Huntersville, NC 28078 (704)875-9092 Page 26 of 29 Page 27 of 29 Page 28 of 29 Page 29 of 29
