CATLIN Engineers and Scientists (CATLIN) is pleased to provide you with this letter
report documenting site activities performed in general accordance with CATLIN proposal number 221185.3P, dated January 13, 2023. Services were authorized by the North Carolina Department of Environmental Quality (NCDEQ) Task Authorization (TA) #03, approved January 13, 2023, and Contract Number N122019D. The approved scope of services for this Task Authorization included:
•Review the historical files of adjacent underground storage tank (UST) incidents (#20918,#20968, and #21529);
•Coordinate the future Scope of Work with NCDEQ managers;
•Gauge the depth to water of three monitoring wells (MW-01, MW-02, and MW-03); and
•Prepare and submit a letter report summarizing the findings.
The Former Texaco Food Mart #2 site is located at 1700 Cotton Grove Road, Lexington, Davidson County, North Carolina (See Figures in Attachment A). Prior to completing any field activities,
potential hazards were evaluated and are documented on an electronic Job Hazard Analysis (JHA) form, provided in Attachment B. Site photographs are presented in Attachment C.
The file reviews were completed for the adjacent incidents and the incident manager, Mr. Ryan Bond, was interviewed. The adjacent incdients will be further discussed in the next monitoring report.

Groundwater Gauging
On February 1, 2023, CATLIN personnel measured depth to water in monitoring wells MW-01,
MW-02, and MW-03 using an electronic groundwater interface probe. Gauging data were
recorded on electronic groundwater sampling forms using a tablet device. Gauging procedures
were performed in general accordance with NCDEQ Guidelines and EPA Region 4 Operating
Procedures. The locations of the monitoring wells are illustrated on Figures 2A and 2B in
Attachment A.
The depths to water were measured at 17.76 feet below top of casing (BTOC) in MW-01, 17.03
feet BTOC in MW-02, and 18.05 feet BTOC in MW-03. Gauging results are presented in Table 1
(Attachment D). Groundwater elevations and flow direction are illustrated on Figure 3
(Attachment A). Groundwater flow during the February 2023 event was calculated to be towards
the northeast. Gauging data from the November 2022 event indicated groundwater flow to the
southeast. Historical groundwater flow reported by Shealy Engineering, Inc. (2001 to 2018) was
also to the northeast and southeast.
Recommendations
CATLIN recommends installing a Type III monitoring well adjacent to MW-03 and installing
Type II/Type III nested wells near the northwest property corner and south of MW-02 near the
southern property line.
