2.0 EXECUTIVE SUMMARY
ATC Associates of North Carolina, P.C. (ATC) has completed a Groundwater Monitoring Report
(GMR), documenting the monitoring well installation on January 9, 2023, and the groundwater
monitoring event on January 13, 2023, at the Crossroads Center-C site. The report has been
completed based on a September 12, 2022, Task Authorization (TA-02) by the Trust Fund
Branch of the UST Section, Division of Waste Management, North Carolina Department of
Environmental Quality (NCDEQ). The work was performed as part of the State Lead Contract
#N122019A between the NCDEQ and ATC.
In accordance with TA-02, two monitoring wells were installed (MW-12 and MW-13) after obtaining
access agreements with the two property owners and well permits from the NCDEQ. A
groundwater monitoring event was conducted at the site by ATC with the sampling of monitoring
wells MW-1, MW-3, MW-10, MW-12, and MW-13. Groundwater samples were analyzed for volatile
organic compounds (VOCs). Concentrations were compared to the Gross Contamination Levels
(GCLs) and the North Carolina 15A NCAC 2L .0202 groundwater quality standards (2L Standards).
The purpose of the new wells MW-12 and MW-13 was to evaluate the extent of the GCL
exceedance to the west and south of monitoring wells MW-3. The findings from the recent and
historical activities indicate the following:
There are no water supply wells within 1,000 feet of the source area or surface water
bodies within 500 feet of the source area;
Municipal water is provided to the area;
Light non-aqueous phase liquid (LNAPL) was not detected in the monitoring wells;
Benzene (6,590 micrograms per liter or µg/L) was detected in well MW-10 above the
GCL of 5,000 µg/L;
VOCs exceeded the 2L Standards in MW-1, MW-3, MW-10, MW-12, and MW-13; and
The monitoring wells are 1-inch diameter wells, excluding MW-10, MW-12, and MW-13
that are 2-inch diameter wells; and
The depth to groundwater ranged from 6.69 feet below top of casing (ft-toc) in MW-13 to
10.59 ft-toc in MW-2, and groundwater flow is indicated to be to the south-southwest.
Benzene in wells MW-3 and MW-10 and naphthalene in well MW-10 exceeded the GCLs in
October 2021. Benzene in well MW-10 was the only VOC exceeding the GCL in January 2023,
and the concentration increased by approximately 11% since October 2021. Benzene in MW-3
decreased by approximately 50% since October 2021. Naphthalene in MW-10 also decreased by
approximately 95%, however, the concentration was similar to that reported in February of 2018
and in April and June of 2019. Excluding the benzene in MW-10, concentrations of VOCs
decreased in wells MW-1, MW-3, and MW-10. The extent of the GCLs is defined with the
additional wells MW-12 and MW-13. The January 2023 VOC concentrations in the downgradient
well MW-13 were approximately 3% or less of those in MW-3, excluding tert-amyl alcohol that was
approximately 11%.
The January 13, 2023 groundwater levels were approximately 3.3 feet higher than the October
8, 2021 groundwater levels, with groundwater levels above the screen in wells MW-1, MW-3,
and MW-4. Historically, concentrations of benzene in well MW-10 have increased with rising
groundwater levels and decrease with falling groundwater levels, suggesting a possible zone of
contaminated soil near the seasonal high groundwater levels. Note that the recent increase in
benzene concentration in MW-10 appears to be less than prior increases suggesting natural
degradation is occurring.

Based on the findings, ATC recommends the following:
Conduct a combined Mobile Multi-Phase Extraction (MMPE) and Ozone Injection (OI)
event, using wells MW-1, MW-3, and MW-10 as extraction wells and VEW-1 as the
injection well. This would require an NOI to be submitted to the NCDEQ, Division of Water
Resources (DWR), Underground Injection Control (UIC) Program for the injection event;
and,
Conduct groundwater sampling following the MMPE/OI event to evaluate possible
reduction in VOC concentrations.
3.0 SITE DESCRIPTION
3.1 Site Characterization
ATC, under contract to the NCDEQ, provided environmental consulting services at the Crossroads
Center-C site, which is located at 134 Arnold Road, in Lexington, Davidson County, North Carolina.
Figure 1 depicts the site on a topographic map. The site is currently a vacant building at the
northwest intersection of Arnold Road and Old U.S. Highway 52. A railroad line is located along the
eastern site boundary (Figure 2). The topography slopes to the southwest, with elevations of
approximately 824 feet in the northeast area of the site to approximately 820 feet in the
southwestern area of the site. A generally north-south trending topographic ridge is located east of
the railroad line.
No water supply wells have been identified within 1,000 feet of the source area. The nearest
surface water body is approximately 650 feet to the southwest (Figure 3), according to the
Davidson County GIS website.
3.2 Site Background
The site’s UST system formerly consisted of five tanks, two 6,000-gallon gasoline tanks, one
5,000-gallon gasoline tank, one 3,000-gallon diesel tank, and one 2,000-gallon kerosene tank. The
tanks were reportedly in use through 2012. In April 1994, a 2,000-gallon kerosene UST and
associated piping for the UST were removed by Geo-Environmental Consultants with no evidence
of release at the time. NCDEQ issued a closure letter for the tank in July 1995.
A Phase II Environmental Site Assessment dated July 2002 performed by Environmental
Consulting Services, LTD (ECS) documented a petroleum release at the site based on
contamination present in both the soils and groundwater (Incident #24552). The source of the
release was suspected to be from the two 6,000-gallon gasoline USTs. Assessment activities
included: the installation of well MW-1 in July 2002 and wells MW-2 through MW-4 and VEW-1 in
August 2002; and soil sampling collected during well installation. Soil contamination was found to
be above soil-to-groundwater MSCCs and groundwater contaminants above NCDEQ 2L
Standards. The incident (#24552) was closed in September 2002 with a Notice of Residual
Petroleum (NORP) with a groundwater restriction for water supply wells and a soil restriction for
soil excavation, but no residential land use restriction. At the time the NORP was issued a site
check of the UST system had not been completed.
Between May 2003 and December 2021, LR&S Inc. operated the UST system, until the UST
permit to operate expired on December 31, 2012. LR&S Inc. was dissolved by the Secretary of
State on September 28, 2015.

The current owner purchased the property on January 11, 2017.
In September 2017 Progress Environmental Group, Inc. (Progress) removed the remaining four
USTs and one AST remaining at the site. Soil contamination was found to be above NCDEQ
Action Levels during the removal process and a hole was observed in one of the 6,000-gallon
gasoline USTs. Total Petroleum Hydrocarbons-Diesel Range Organics (TPH-DRO) and TPH-
Gasoline Range Organics (TPH-GRO) were found to be above their respective NCDEQ Action
Levels in various soil samples from the tank pit. No additional soils were removed from the site
during the tank removal. Incident #44919 was assigned to the reported release.
In 2018, Progress conducted a Limited Site Assessment that included the installation and sampling
of two additional monitoring wells, MW-10 and MW-11, and soil sampling. Soil contamination was
revealed above the NCDEQs Soil-to-Groundwater and Residential MSCCs and groundwater
contamination was revealed above the GCLs for benzene. One water-supply well was identified
within 500 feet of the site. A follow-up groundwater monitoring event by Progress that year found
an increase in previously identified constituent concentrations. No other remediation activities were
taken at this time.
In August 2019, Progress conducted a groundwater sampling event on wells MW-1 through MW-4,
MW-10, MW-11, and VEW-1. No LNAPL was detected during the sampling event and benzene
was detected above the GCL of 5,000 micrograms per liter (µg/L). Progress reported that the
contaminant plume appeared to be migrating to the southwest. They also reported that the rise in
the water column between 2018 and 2019 appeared to mobilize soil contaminants causing the
sudden spike in contaminant levels in the groundwater.
In May 2021 NCDEQs Corrective Action Branch performed a site inspection and found that wells
MW-3, MW-4, and MW-10 were in good condition, wells MW-1 and VEW-1 were uncovered but
may be usable, well MW-2 could not be located, and well MW-11 was reportedly destroyed. The
site was accepted to the State Lead program in May 2021.
On October 8, 2021, ATC conducted a groundwater monitoring event including wells MW-1
through MW-4, MW-10, and VEW-1 with analysis for VOCs. Benzene was detected at
concentrations higher than the GCL in wells MW-3 and MW-10, and naphthalene was detected in
well MW-10 above the GCL. Several VOCs exceeded the 2L Standards in wells MW-1, MW-3,
MW-4, MW-10, and VEW-1.
3.3 Site and Regional Geology
The Geologic Map of North Carolina produced by the North Carolina Geologic Survey (1985)
indicates that the property is located within the Charlotte Belt of the Piedmont Physiographic
Province. Rocks of the Piedmont are crystalline igneous and metamorphic rocks that were formed
during Appalachian mountain building events during the Paleozoic Era, several hundred million
years ago. The metamorphic rock type beneath the site is metagabbro, metadiorite, and mafic
plutonic-volcanic complexes. The shallow subsurface in most areas of the Piedmont contains
residual soil overburden, including structure-free residuum, saprolite, and partially weathered rock
(PWR), that derive from in-place weathering of the crystalline bedrock. Occasional areas
containing recent deposits of alluvium in the uppermost subsurface are found near streams and
rivers. Saprolite and PWR typically contain some relict structures from the original rock material.
Depth to rock ranges from ground surface at occasional outcrops to depths of up to 100 feet in

areas of easily weathered rock. Bedrock was not encountered at a depth of 45 feet below ground
surface (ft-bgs) based on the boring for monitoring well VEW-1.
3.4 Site and Regional Hydrogeology
The shallow aquifer occurrence varies in depth from ground surface at springs, creeks, and
rivers to as deep as 50 feet or more beneath upland surfaces in some parts of the Piedmont.
Groundwater in the alluvium or unconsolidated residual material, including saprolite and PWR,
usually behaves as an unconfined, or water table, aquifer and will yield water with head
elevation equivalent to the first elevation where water was encountered. Permeability varies with
lithology and is typically relatively low in residual soils, with higher permeability in saprolite or
PWR due to relict rock texture and variable susceptibility to weathering exhibited by different
minerals in the rock. Groundwater flow in residual soils or alluvium is usually in rough
concurrence with local topographic conditions and is toward local drainage features.
The bedrock fractures or other planar features generally constitute the bedrock aquifer, with the
surrounding rock material being effectively impermeable. Along with fractures, contacts between
rock bodies probably constitute zones of significant groundwater occurrence in the bedrock. The
surrounding material and overlying residuum tend to make the bedrock aquifer a semi-confined
aquifer. That is, the overlying water and soil weight normally results in pressure that causes
water in a borehole which intersects a fracture or other feature to rise above the elevation of the
fracture or feature. Such features may not occur on predicable trends, at the same elevations,
or even be present or directly connected in separate boreholes. In areas adjacent to creeks the
bedrock groundwater generally discharges to the residuum or alluvium and then into the surface
water. In upland areas away from surface water drainages, the bedrock aquifer is generally
recharged by downward infiltration of residuum or alluvial aquifer water at locations where
fractures intersect the bedrock surface.
When gradient conditions are discussed, upgradient properties are in locations which could,
based on topographic or groundwater elevation analysis, have shallow groundwater migration
from them toward the site. Crossgradient sites are nearby properties with equivalent elevation to
the site, which would be unlikely to have groundwater migration from them parallel to the site,
and downgradient sites are nearby properties, which likely receive groundwater migration from
beneath the site. Based on topography and historical groundwater level data, groundwater flow
is to the south-southwest. The available historical depth to groundwater data indicate that the
depth to groundwater has generally ranged from approximately 6 to 15 ft-bgs.
4.0 GROUNDWATER CHARACTERIZATION
4.1 Access Agreements and Well Permits
On October 4, 2022, ATC submitted access agreements to the property owners of the two
parcels where the new monitoring wells (MW-12 and MW-13) were proposed. The access
agreement for well MW-12 was signed on October 15, 2022, by Mr. Michael S. Lane for the 158
Arnold Road property. The access agreement for well MW-13 took several attempts and phone
conversations with the property owner, who had two different addresses. The agreement was
signed on November 8, 2022 by Martha Fritts Martin and received on November 15, 2022.
Copies of the access agreements are included in Appendix A.

On December 2, 2022, ATC submitted well permit applications to the NCDEQ, Division of Water
Resources. A well permit for well was issued on December 12, 2022. Copies of the well permit
applications and well permits are included in Appendix A.
4.2 Monitoring Well Installation
On January 9, 2023, ATC supervised the installation of shallow groundwater monitoring wells
MW-12 and MW-13, which are located to the west and south of MW-3, respectively. The wells
were installed to evaluate the extent of the GCL exceedances in well MW-3.
Prior to conducting any subsurface work, North Carolina 811 was notified to mark out any buried
public utilities at the site. A private utility locator from Ground Penetrating Radar Systems, LLC
(GPRS) was onsite to identify private utilities in the work area. No utilities were encountered
within 10 feet of the work area.
The monitoring wells were installed by Geologic Exploration (Geologic), a North Carolina-
registered well driller. The monitoring wells were installed using hollow stem augers. Soil
samples characterized for lithology and were screened at approximate 5-foot intervals with a
photoionization detector (PID) for relative VOC levels.
The soil from the MW-12 and MW-13 borings consisted of red-brown silty clayey silt and was
relatively consistent with depth. Elevated PID readings (416 and 401 parts per million or ppm)
were observed in soil samples from 10 to 20 ft-bgs (below the groundwater surface) from the
MW-12 boring. Elevated PID readings were not observed in the soil samples from 0 to 10 ft-bgs
from the MW-12 boring or in any of the soil samples from the MW-13 boring.
Monitoring wells MW-12 and MW-13 were installed to a depth of 20 feet below ground surface
(ft-bgs) including a 15-foot length, 0.010-inch slotted, 2-inch diameter PVC well screen
extending from 5 to 20 ft-bgs and solid PVC riser casing to near ground surface. A sand pack
filter was installed in the annular space between the well screen and the borehole from the base
of the boring to one foot above the top of the well screen, followed by one foot of bentonite seal
on top of the sand pack. A grout slurry was placed from the top of the bentonite seal to ground
surface to seal the borehole around the solid PVC well pipe riser. Soil cuttings generated from
the drilling of the monitoring wells were spread on site. The well was completed with a 2-foot by
2-foot concrete pad and manhole.
The top of casing (TOC) elevation for the wells was measured during the groundwater
monitoring event on January 13, 2023. TOC elevations were surveyed relative to an arbitrary
elevation of 100 feet and existing monitoring well MW-3. The monitoring well record and soil
boring log for monitoring wells MW-12 and MW-13 are including in Appendix B. Field notes of
the well installation are presented in Appendix C.
4.3 Groundwater Levels
On January 13, 2023, a groundwater monitoring event was conducted for monitoring wells MW-
1, MW-2, MW-3, MW-4, MW-10, MW-12, and MW-13. Wells MW-2 and MW-4 were only gauged
for depths to water and total well depths. Monitoring well MW-11 could not be located and is
presumed to have been destroyed. Figure 2 depicts the well locations on a site map. Well
construction and groundwater level data are summarized in Table 1. The calculated
groundwater elevations vary across the monitoring well network that do not allow representation
of a smooth groundwater surface. Based on the October 8, 2021 data, groundwater flow is to

the south-southwest with a hydraulic gradient of 0.025 feet/foot. The January 13, 2023
groundwater elevations are presented on Figure 4. The January 13, 2023 groundwater levels
were approximately 3.3 feet higher than the October 8, 2021 groundwater levels, with
groundwater levels above the screen in wells MW-1, MW-3, and MW-4. Field notes and
photographs from the January 13, 2023, groundwater monitoring event are presented in
Appendix C.
4.4 Groundwater Quality
On January 13, 2023, ATC collected groundwater samples from monitoring wells MW-1, MW-3,
MW-10, MW-12, and MW-13. During sample activities, a three-volume well purge was
implemented prior to groundwater sample collection to ensure a representative groundwater
sample. Field parameters are presented in Table 2. The water samples were collected in
laboratory-supplied, preserved containers; entered into chain of custody control; placed on ice;
and shipped to North Carolina accredited Pace Analytical Services, Inc. in Huntersville, North
Carolina. The groundwater samples were analyzed by Standard Method 6200B, including
ethylene dibromide (EDB) and oxygenates. The laboratory analytical report and chain-of-
custody documentation is included in Appendix D.
The recent groundwater analytical results are summarized in Table 3. The historical
groundwater analytical results are summarized in Table 4. A groundwater quality map of the
recent results with exceedances of the 2L Standards is presented as Figure 5. The
groundwater analytical results were compared to the 2L Standards and GCLs. The updated
Conceptual Site Model is presented as Table 5. Groundwater analytical results indicated the
following:
Benzene was detected above the GCL of 5,000 µg/L in MW-10 (6,590 µg/L); and
VOCs exceeded the 2L Standards in wells MW-1, MW-3, MW-10, MW-12, and MW-13.
Benzene in wells MW-3 and MW-10 and naphthalene in well MW-10 exceeded the GCLs in
October 2021. Benzene in well MW-10 was the only VOC exceeding the GCL in January 2023,
and the concentration increased by approximately 11% since October 2021. Benzene in MW-3

Historically, concentrations of benzene in well MW-10 have increased with rising groundwater
levels and decrease with falling groundwater levels, suggesting a possible zone of contaminated
soil near the seasonal high groundwater levels. Note that the recent increase in benzene
concentration in MW-10 appears to be less than prior increases suggesting natural degradation
is occurring.
As part of ATC’s quality assurance and quality control (QA and QC) measures, a data review of
the laboratory report was conducted. During the review, it was found that the samples were
received from the laboratory in acceptable condition and properly preserved. The samples were
analyzed within correct holding times and reported within the lab report, and the chain of
custody was properly completed by all parties. ATC collected one field blank during groundwater

monitoring activities with no detections. The laboratory QC report shows that laboratory reagent
blanks (LRBs) and laboratory control samples/laboratory control sample duplicates
(LCSs/LCSDs) were analyzed for each method that was used for the original samples. The LRB
indicated no detections and the LCS/LCSD results were within acceptable laboratory control
limits. Refer to the laboratory report in Appendix C for details regarding QA results for the
laboratory samples and any noted exceptions.
5.0 CONCLUSIONS AND RECOMMENDATIONS
On behalf of the Trust Fund Branch of the NCDEQ UST Section, ATC has completed
assessment activities for the Crossroads Center-C UST site (incident no 44919), located at 134
Arnold road, Lexington, Davidson County, North Carolina. The findings from the recent and
historical activities indicate the following:
There are no water supply wells within 1,000 feet of the source area or surface water
bodies within 500 feet of the source area;
Municipal water is provided to the area;
LNAPL was not detected in the monitoring wells;
Benzene (6,590 micrograms per liter or µg/L) was detected in well MW-10 above the
GCL of 5,000 µg/L;
VOCs exceeded the 2L Standards in MW-1, MW-3, MW-10, MW-12, and MW-13; and
The monitoring wells are 1-inch diameter wells, excluding MW-10, MW-12, and MW-13
that are 2-inch diameter wells; and
The depth to groundwater ranged from 6.69 ft-toc in MW-13 to 10.59 ft-toc in MW-2, and
groundwater flow is indicated to be to the south-southwest.
Benzene in wells MW-3 and MW-10 and naphthalene in well MW-10 exceeded the GCLs in
October 2021. Benzene in well MW-10 was the only VOC exceeding the GCL in January 2023,
and the concentration increased by approximately 11% since October 2021. Benzene in MW-3
decreased by approximately 50% since October 2021. Naphthalene in MW-10 also decreased by
approximately 95%, however, the concentration was similar to that reported in February of 2018
and in April and June of 2019. Excluding the benzene in MW-10, concentrations of VOCs
decreased in wells MW-1, MW-3, and MW-10. The extent of the GCLs is defined with the
additional wells MW-12 and MW-13. The January 2023 VOC concentrations in the downgradient
well MW-13 were approximately 3% or less of those in MW-3, excluding tert-amyl alcohol that was
approximately 11%.
The January 13, 2023 groundwater levels were approximately 3.3 feet higher than the October
8, 2021 groundwater levels, with groundwater levels above the screen in wells MW-1, MW-3,
and MW-4. Historically, concentrations of benzene in well MW-10 have increased with rising
groundwater levels and decrease with falling groundwater levels, suggesting a possible zone of
contaminated soil near the seasonal high groundwater levels. Note that the recent increase in
benzene concentration in MW-10 appears to be less than prior increases suggesting natural
degradation is occurring.
Based on the findings, ATC recommends the following:
Conduct a combined Mobile Multi-Phase Extraction (MMPE) and Ozone Injection (OI)
event, using wells MW-1, MW-3, and MW-10 as extraction wells and VEW-1 as the

injection well. This would require an NOI to be submitted to the NCDEQ, DWR, UIC
Program for the injection event; and,
Conduct groundwater sampling following the MMPE/OI event to evaluate possible
reduction in VOC concentrations.

I am/We are the owner(s) of a parcel of property, located at or near the incident in question,
and hereby permit the Department of Environment Quality (Department) or its contractor to enter
upon said property for the purpose of conducting an assessment and/or remediation of the
groundwater and/or soils under the authority of G.S. 143-215.94G.
I am/We are granting permission with the understanding that:
1. The investigation shall be conducted by the UST Section of the Department’s Division of
Waste Management or its contractor.
2. The costs of the investigation and/or remediation of the site and access shall be borne by
the Department or its contractor in accordance with the acceptance of the site into the
State-Lead Program. The Department or its contractor shall protect and prevent damage
to the property and surrounding lands. Any damages will be restored by the Department
or its contractor to as close to the pre-work condition as practicably possible.
3. Unless otherwise agreed, the Department or its contractor shall have access to the site
by the shortest feasible route to the nearest public road. The Department or its contractor
will notify the land owners 48 hours prior to entry and may enter upon the land at
reasonable times and have full right of access during the period of the investigation.
4. Any claims which may arise against the Department or its contractor shall be governed by
Article 31 of Chapter 143 of the North Carolina General Statutes, Tort Claims Against
State Departments and Agencies, and as otherwise provided by law.
5. The activities to be carried out by the Department or its contractor are for the primary
benefit of the Department and of the State of North Carolina. Any benefits accruing to the
owner are incidental. The Department or its contractor is not and shall not be construed
to be an agent, employee, or contractor of the landowner.

4. Each well shall have a Well Contractor Identification Plate in accordance with 15A NCAC

5. Well construction records (GW-1) for each well shall be submitted to the Division of Water
Resources’ Information Processing Unit within 30 days of the well completion.
6. When the well is discontinued or abandoned, it shall be abandoned in accordance with 15A
NCAC 02C .0113 and a well abandonment record (GW-30) shall be submitted to the
Division of Water Resources’ Information Processing Unit within 30 days of the well
abandonment.
7. The County Health Department may require a county monitoring well construction permit.
