The North Carolina Underground Storage Tank Section is conducting an environmental assessment at the subject property. Based on a review of the data collected at the site to date, it is necessary to install at least one groundwater monitoring well on your property to help delineate the extent of affected groundwater in the area. Therefore, the UST Section respectfully request permission to install one Type II ground water monitoring well on your property. The well will consist of 2" PVC screen and riser, with an eight inch diameter manhole set in a 2'x2' concrete pad. Groundwater samples will be collected from the monitoring
well on a periodic basis, as determined by the UST Section. The monitoring well will be closed upon completion of the project. An access agreement for the purpose of installing the well is attached for your review. Please sign the attached site access agreement and return using the envelope provided. Your signature on the attached agreement will also serve as permission to collect a water sample from the supply well at your property. Your assistance is greatly appreciated.
