Dear Mr. Berger:
As authorized by Task Authorization Request (TAR) No. 2 dated January 21, 2015, ECS
Carolinas, LLP (ECS) recently installed one additional Type II groundwater monitoring well,
surveyed the new well’s relative elevation and collected a groundwater sample from the well
located at Kodell’s Grocery in Westfield, Stokes County, North Carolina (Figures 1 and 2). This
letter, with attachments, is the report of our sampling.
FIELD ACTIVITIES
On February 9, 2015, personnel from ECS mobilized to the site to install, survey and sample
one additional Type II groundwater monitoring well (MW-5) on the north downgradient side of
the site (Figure 2). The well was installed using a Geoprobe® drill rig equipped with solid stem
augers. The drilling contractor was 3D Environmental Investigations, LLC. The boring for the
well was driilled to a depth approximately 32 feet below ground surface (bgs). The Type II well was
constructed with two-inch I.D. Schedule 40 PVC flush-threaded casing and screen. The PVC
screen and casing was lowered into the open borehole. A 15-foot length of slotted well screen with
machined 0.010-inch slot widths and a threaded bottom plug was installed at the bottom of the
borehole. The screen was installed to a depth such that the screened interval intersected the
estimated groundwater surface as encountered during drilling activities. A solid section of PVC
casing was placed above the screened interval and extended to a point just below the ground
surface. The annular space around the well was filled with a washed and graded medium sand to
approximately two feet above the top of the screen. Copies of the boring log and well construction
diagram are attached.
A minimum two-foot thick seal of bentonite pellets was placed above the sand pack. After
placement in the well, the bentonite pellets were allowed to hydrate. After hydrating the bentonite
pellets, a bentonite/neat cement mixture was poured down the borehole to fill the annular space
from the bentonite seal to the ground surface. The top of the well casing was equipped with a

lockable, water-tight well plug and a flush-mounted protective steel cover was installed over the
well.
Approximately 4 hours following installation, the well was gauged and sampled. Prior to
sampling, the depth to water was measured in the monitoring well using an electronic water
level meter. The depth to water was measured approximately 22.69 feet, below the top of
casing. Well construction information and depth to groundwater are summarized in Table 1.

The monitoring well was purged by removing five well volumes with a disposable bailer and nylon
cord. After purging was completed, a groundwater sample was collected from the well and
transferred into sample containers provided by the laboratory. The Groundwater Monitoring Well
Sampling Data Sheet is attached. The sample containers were labeled with ECS project
number, sample identification, sample date and time, and requested analytical analysis. The
containers were placed into protective packaging material and placed into a cooler with ice to
maintain the samples at approximately 4 Celsius (C). The groundwater sample was submitted
to Research and Analytical Laboratories (R&A Labs) in Kernersville, North Carolina (NC
certification #34) for chemical analysis. The groundwater sample was analyzed for volatile
organic compounds (VOCs) using Standard Method 6200B plus IPE, MTBE, and EDB.
Proper chain-of-custody (COC) procedures were followed from sample collection through the
transportation process. A copy of the COC is attached.
The relative elevation of the top of the new well casing was measured to the nearest 0.01 feet
using a level and rod. The elevations were measured relative to an arbitrary benchmark, which
was assigned an elevation of 100 feet. The calculated well elevation is summarized in Table 1.
RESULTS
Laboratory analysis detected benzene, ethylbenzene, naphthalene, styrene, toluene, 1,2,4-
trimethylbenzene, and total xylenes at concentrations exceeding their NC 2L Groundwater
Standards (NC2LGWQS) in MW-5. 1,3,5- trimethylbenzene and n-propylbenzene were
detected above the laboratory method detection limit but below their NC2LGWQSs. The
concentrations do not exceed the Gross Contamination Levels (GCLs). The analytical results
are summarized in Table 2 and the geochemical parameters are summarized in Table 3. The
laboratory report is attached.
