NCDEQ, Division of Waste Management, UST Section 1646 Mail Service Center Raleigh, North Carolina 27699-1646 Reference: Letter Report Jerry’s Place 983 US Highway 21 State Road, Surry County, North Carolina NCDEQ Incident No.: 9547 NCDEQ Contract No. N17006 ECS Project 49:6166-B Dear Mr. Berger: As authorized by your acceptance of ECS Southeast, LLP (ECS) Task Authorization Request (TARs) No. 3 dated April 6, 2020, ECS has completed the Letter Report for the above referenced site. Included in this report is a description of the field activities, the results obtained, and our conclusions and recommendations. PROJECT UNDERSTANDING The site is located at 983 US Highway 21, State Road, Surry County, North Carolina (Figures 1 and 2). Based on information provided to ECS, four 4,000-gallon gasoline underground storage tanks (USTs) were removed from the site in October 1992. During closure activities, free product was observed flowing into a test pit that was excavated to a depth of approximately 19 feet below the ground surface. Petroleum hydrocarbons were detected in soil samples collected from the UST basin and dispenser areas. On February 20, 2018, ECS collected groundwater samples from monitoring wells MW-2, MW-3, MW-4, and MW-5 and water supply well WSW-5. Laboratory analysis of groundwater samples collected from monitoring wells MW-2 and MW-3 detected multiple targeted compounds greater than their respective North Carolina 2L Groundwater Quality Standard (NC2LGWQS); however, the concentrations did not exceed their respective Gross Contamination Levels (GCLs). Laboratory analysis of the groundwater sample collected from monitoring well MW-4/RW-4 detected benzene greater than its NC2LGWQS, but less than its GCL. Laboratory analysis of the groundwater sample collected from monitoring well MW-5 did not detect targeted compounds greater than the laboratory detection limits. No free product was observed in the site monitoring wells. MTBE was detected in the sample collected from water supply well WSW-5 at a concentration greater than its NC2LGWQS, but less than its GCL. ECS collected groundwater samples from monitoring wells MW-2, MW-3, MW-4, and MW-5 and water supply well WSW-5 on December 12, 2019. Review of the laboratory analytical report of groundwater samples collected from monitoring wells MW-2 and MW-3 indicated multiple targeted compounds greater than the NC2LGWQS; however, the concentrations were FIGURES SOURCE: USGS TOPOGRAPHIC MAP ELKIN NORTH, NC QUADRANGLE DATED 1971, REVISED 1994 FIGURE 1 USGS TOPOGRAPHIC MAP JERRY’S PLACE 983 US HIGHWAY 21 STATE ROAD, SURRY COUNTY, NC INCIDENT #9547 ECS PROJECT NO. 49:6166-B SITE APPROXIMATE SITE BOUNDARY SCALE (IN FEET)2,000 1,000 0 2,000 SOURCE: SURRY COUNTY GIS AERIAL PHOTOGRAPH, DATED 2013 FIGURE 2AERIAL SITE MAP JERRY’S PLACE 983 US HIGHWAY 21 STATE ROAD, SURRY COUNTY, NC INCIDENT #9547 ECS PROJECT NO. 49:6166-B APPROXIMATE SITE BOUNDARY SCALE (IN FEET)100 50 0 100 SOURCE: S&ME MONITORING REPORT, DATED 2010 FIGURE 3MONITORING WELL LOCATION MAP JERRY’S PLACE 983 US HIGHWAY 21 STATE ROAD, SURRY COUNTY, NC INCIDENT #9547 ECS PROJECT NO. 49:6166-B SCALE (IN FEET)100 50 0 100 EXISTING BUILDING APPENDIX A
