Based on information provided to ECS, four 4,000-gallon gasoline underground storage tanks (USTs) were removed from the site in October 1992. During closure activities, free product was observed flowing into a test pit that was excavated to a depth of approximately 19 feet below the ground surface (bgs). Petroleum hydrocarbons were detected in soil samples collected from the tank basin and dispenser areas. The closure was concluded by soil excavation and remediation of approximately 1,029 tons of soil. Between 1993 and 1994, eight monitoring wells (MW-1 through MW-8) and two recovery wells (RW-5 and RW-6) were installed at the site. Monitoring well MW-1 is presumed destroyed and recovery well RW-6 is presumed paved over based on a 2008 report. An aggressive fluid vapor recovery (AFVR) event was conducted in 1996 and a mobile multi-phase extraction (MMPE) event was conducted in 2001. Free product was not observed following the MMPE event until 2008 when 0.05 feet of free product was measured in MW-2. Groundwater samples were collected from various monitoring wells on the site in 1993, 2009, 2010, 2011, and most recently by ECS in 2014. The contaminant concentrations in the impacted wells have generally decreased since the 1993 sampling event. Sixteen water-supply wells have been identified within 1,000 feet of the site. Methyl-tert-butyl-ether (MTBE) has been detected in one water supply well (WS-5) since 2009; however, the concentrations have been below the North Carolina 2L Groundwater Quality Standard (NC2LGWQS). Municipal water is not available to the site or surrounding properties. The nearest surface water body is a stream located approximately 230 feet east of the source area. The estimated quantity of the release is unknown. 2.0 GROUNDWATER ASSESSMENT 2.1 Monitoring Well Sampling On September 22, 2015, ECS personnel collected samples from four monitoring and recovery wells (MW-2/RW-2, MW-3/RW-3, MW-4/RW-4, and MW-5). The locations of the wells are shown on Figure 3. Prior to sampling, the wells were purged using a new disposable polyethylene bailer and new length of nylon cord. The wells were developed/purged by evacuating a minimum of three well volumes using the bailer and cord. The wells were developed/purged to remove sand, silt and other fine sediments and to promote communication between the surrounding formation and the sand pack surrounding the well’s screened interval. After purging was completed, the sampler wearing a new pair of disposable nitrile gloves for each sampling point poured the groundwater sample from the bailer into laboratory prepared containers. Each container was labeled and placed in a cooler containing ice to maintain the samples at approximately 4 Celsius. The sample containers were then delivered to Pace Analytical Services, Inc. (Pace) in Huntersville, North Carolina to be analyzed for volatile organic compounds (VOCs) using Standard Method 6200B. A Chain of Custody Record was maintained and is included in the Appendix A. Groundwater Monitoring Report Task Authorization Request No. 2 – Jerry’s Place 983 Highway 21 State Road, Surry County, North Carolina NCDEQ Incident No.: 9547 NCDEQ Contract No. N14007i-C ECS Project 09.26074 October 14, 2015 5 2.2 Water Supply Well Sampling On September 22, 2015 ECS collected a sample from one water-supply well (WSW-5) located in the vicinity of the site. The location of the well is shown on Figure 4. ECS personnel obtained written permission from the property owner prior to sampling each well (Appendix C). Prior to sampling, the well and household plumbing were purged by allowing the water to run for approximately 20 minutes. After purging was completed, the sampler wearing a new pair of disposable nitrile gloves, poured the water sample into laboratory prepared containers. Each container was labeled and placed in a cooler containing ice to maintain the samples at approximately 4 Celsius. The sample containers were then delivered to Pace to be analyzed for VOCs using EPA Method 6200B. 2.3 Groundwater Chemistry Prior to collecting the groundwater samples, geochemical parameters (temperature, pH, dissolved oxygen, and conductivity) were measured using a YSI Model 560 meter. The turbidity was observed during the sampling and recorded on the Groundwater Sampling Data Sheets (Appendix B). 2.4 Groundwater Flow Direction On September 22, 2015, ECS measured the static water level (SWL) in four monitoring and recovery wells using a decontaminated electronic water level meter. The static water levels and top of casing elevations were used to calculate groundwater flow direction. The static water level measurements can be found on Table 1. 3.0 RESULTS 3.1 Groundwater Flow Direction Groundwater at the site is flowing to the southwest under a gradient of 0.033ft/ft as determined between MW-3/RW-3 and MW-4 (Figure 5). 3.2 Monitoring Well Sampling Results Laboratory analysis of groundwater samples collected from monitoring wells MW-2/RW-2, MW- 3/RW-3, MW-4/RW-4, and MW-5 detected targeted compounds that exceed their respective North Carolina 2L Groundwater Quality Standard (NC2LGWQS); however, the concentrations did not exceed their respective Gross Contaminant Levels (GCLs). No free product was observed in the site monitoring wells. A summary of the current and historical groundwater analytical results are included in Table 2 and Table 3. The laboratory data sheets are included in the Appendix B. Using the laboratory analytical results of the groundwater samples collected from the groundwater monitoring wells, isoconcentration maps showing the horizontal extent for the compounds which were detected above the NC2LGWQS are included as Figure 6 through Figure 6H. Groundwater Monitoring Report Task Authorization Request No. 2 – Jerry’s Place 983 Highway 21 State Road, Surry County, North Carolina NCDEQ Incident No.: 9547 NCDEQ Contract No. N14007i-C ECS Project 09.26074 October 14, 2015 6 3.3 Water Supply Well Sampling Results Laboratory analysis of groundwater sample WSW- 5 detected methyl-tert butyl ether (MTBE) at a concentration of 2.6 parts per billion (ppb); however, the concentration does not exceed its NC2LGWQS of 20 ppb. A summary of the current and historical groundwater analytical results are included in Table 4. 3.4 Groundwater Chemistry Based on the geochemical data, the average groundwater temperature was 17.4° C and the average conductivity was 0.129mS/cm. The pH ranged from a low of 5.45 to 8.86 and the dissolved oxygen (DO) ranged from 4.30 mg/l to 8.66mg/l. The geochemical measurements are shown on Table 5. 4.0 CONCLUSIONS ECS has completed the groundwater monitoring sampling at the site. ECS sampled four Type II monitoring wells and one water-supply well. Based on on-site groundwater measurements and laboratory analytical results, ECS concludes the following:  The groundwater elevations as determined on September 22, 2015, were used to prepare a potentiometric map of the groundwater surface of the site. The groundwater flow direction is generally to the south-southeast across the site;  Laboratory analysis of groundwater samples collected from monitoring wells MW-2/RW-2, MW-3/RW-3, MW-4/RW-4, and RW-5 detected targeted compounds that exceed their respective NC2LGWQS; however, the concentrations did not exceed their respective GCLs.  No free product was detected in the monitoring wells;  The concentrations of petroleum related hydrocarbons have generally decreased in each well sampled since the 1993 sampling event.  Laboratory analysis of groundwater sample WSW- 5 detected MTBE at a concentration of 2.6 ppb; however, the concentration does not exceed its NC2LGWQS of 20 ppb. Groundwater Monitoring Report Task Authorization Request No. 2 – Jerry’s Place 983 Highway 21 State Road, Surry County, North Carolina NCDEQ Incident No.: 9547 NCDEQ Contract No. N14007i-C ECS Project 09.26074 October 14, 2015 7 5.0 RECOMMENDATIONS Based on the results of this sampling event and those performed by others, ECS has the following recommendations:  Sample the on-site water supply well (WSW-5) semi-annually;  Abandon monitoring wells MW-6, MW-7, and MW-8. Monitoring well MW-6 is the upgradient well and MW-7 and MW-8 have either been dry or when sampled, clean, since 1994. Monitoring MW-5 is positioned in a more downgradient direction and should used as the down gradient sentinel well;  Install O-SOXs in monitoring wells MW-2 and MW-3/RW-3;  Sample MW-2, MW-3/RW-3, MW-4/RW-4, and MW-5 six months following installation of the O-SOX.
