Currently, a used car lot and a gym operate at the site. The site is surrounded predominately by residential properties with Crestwood Cemetery to the east, across Highway 21. Four 4,000-gallon gasoline tanks were closed and removed from the site in October 1992. During the closure activities, free product was observed flowing to a test pit that was excavated to a depth of 19 feet below land surface. Petroleum hydrocarbons were detected in the soil samples collected from the tank basin and dispenser areas. The closure was concluded by soil excavation and remediation of approximately 1,029 cubic yards of soil. Between 1993 and 1994, eight monitoring wells (MW-1 through MW-8) and two recovery wells (RW-5 and RW-6) were installed at the site. MW-1 is presumed destroyed and RW-6 is presumed to be paved over based on a December 2008 Terraine, Inc. Assessment Report. ATEC conducted an eight-hour aggressive fluid vapor recovery event in July 1996 and a five-day mobile multi-phase extraction (MMPE) event in December 2001. Free product was not observed following the MMPE event until September 24, 2008 when 0.05 feet of free product was measured in MW-2. During S&ME’s October 2009 sampling event, free product was not observed in MW-2. Sixteen water supply wells have been identified within 1,000 feet of the site. The water supply wells are illustrated in Figure 4. Municipal water is not present in the vicinity of the subject site. Well Use for these properties is listed in Table 3. The nearest surface water body to the site is an intermittent stream approximately 230 feet east of the source area. The site and vicinity are shown in Figure 1, and the site and pertinent features are shown on Figure 2. Photographs of the site are included in Appendix III. 3 GROUNDWATER MONITORING S&ME, Inc. (S&ME) submits this Monitoring Report for the above referenced site. The report was prepared in accordance with Contract # 16-1713-2C for the Central Region, and S&ME’s Proposal Number 1354-22118-09 dated September 25, 2009, as approved with NCDENR’s Task Authorization (TA) #01. The scope of the approved TA included in part, the sampling and analysis of groundwater from eight monitor wells (MW-2 through MW-8, and RW-5) and one water supply well (WSW-5), checking the free product level in MW-2, the installation of five well booms (MW-2, MW-3, MW-4, MW- 5 and RW-5), updating receptor survey and reporting of the results. 3.1 Monitor Well Sampling On October 7, 2009, S&ME was on-site to gauge depth-to-water and collect groundwater samples from eight monitor wells (MW-2 through MW-8, and RW-5). 2 Monitoring Report – NCDENR Incident No.9547 S&ME Project No. 1354-09-220 Jerry’s Place, State Road, Surry County NC November 4, 2009 The depth-to-water measurements were gauged from the top-of-casing. The depth to the groundwater surface ranged from 5.88 (MW-5) to 24.43 (MW-7) feet below top of casing. A summary of groundwater levels and elevations is included in Table 1. The groundwater elevations with equipotentials are depicted on Figure 2. The data indicate groundwater flow to be to the south-southeast. After measuring the depth-to-water, S&ME personnel purged the monitor wells with a dedicated disposable polyethylene bailer by evacuating three well volumes, or until the well was dry. After completing the well purging and allowing groundwater to recover in the monitor wells, S&ME personnel collected a groundwater sample from each of the monitor wells. The groundwater samples were placed in laboratory-supplied containers, stored in a cooler with ice, and shipped under chain-of-custody procedures to Prism Laboratories, Inc. (Prism) a North Carolina certified laboratory for laboratory analysis. The groundwater samples were analyzed for VOCs by EPA Method 6200B. 3.2 Enhanced Bioremediation Following sample collection, S&ME installed new 48-inch long well booms (Universal Remediation Inc., Petroleum Remediation Product) in monitor wells MW-2, MW-3, MW-4, MW-5 and RW-5. 3.3 Monitor Well Sampling Results The laboratory analytical results indicated that groundwater from MW-2, MW-3, MW-4, MW-5, RW-5 and WSW-5 contains petroleum constituents at concentrations above 2L Standards. The highest concentrations were detected in monitor wells MW-2 and MW-3. Concentrations do not exceed the Gross Contaminant Levels. A summary of groundwater analytical results and a comparison to NCDENR’s regulatory standards is included on Table 2. A copy of the laboratory analytical results and chain- of-custody form is included in Appendix I. Well Sampling Forms are included in Appendix II. 3.4 Water Supply Well Survey Update Results Mr. Herb Berger, with the NCDENR-Division of Waste Management-UST Section, requested on June 17, 2009, that S&ME update the previously completed water supply survey. As part of the assessment, S&ME personnel searched within a 1,000-foot radius of the Jerry Place site for supply wells and water meters on October 7, 2009. Municipal water is not available along Highway 21 in the vicinity of the subject site. S&ME’s reconnaissance of the area identified the presence of sixteen water supply wells within a 1,000-foot radius of the incident. Six wells are located within 500 feet and ten wells are located between 500 and 1,000 feet of the source area. WSW-5 is located on the subject site. Figure 4 illustrates the locations of the WSWs. Refer to Table 3 for a complete list of water supply wells identified during the survey update. 3 Monitoring Report – NCDENR Incident No.9547 S&ME Project No. 1354-09-220 Jerry’s Place, State Road, Surry County NC November 4, 2009 4 4 CONCLUSIONS AND RECOMMENDATIONS A groundwater monitoring event was conducted at the Jerry’s Place property located at Lindell and Pope Farm Road, Surry County North Carolina to assess current petroleum impact to groundwater. The findings at the site are as follows: • Sixteen water supply wells are located within 1000 feet of the site including one inactive/not in service well (WSW-15), two non-drinking water wells (WSW-5 and WSW-16), five active drinking water supply wells (WSW-1 through WSW-4 and WSW-6) and eight wells with unknown well use status. WSW-5 is used for flushing toilets only. None of these properties are connected to municipal water. • Contaminant concentrations from the October 7, 2009 sampling event exceed the 15A NCAC 2L groundwater quality standards in MW-2, MW-3, MW-4, MW-5, RW-5 and WSW-5. Free product was not observed in MW-2 (0.05 feet in September 2008). Contaminant concentrations generally increased in all wells from the September 2008 sampling event, with the exception of downgradient well MW-5 where concentrations decreased slightly. • An intermittent stream is located approximately 230 feet east of the source area. • The depth to the groundwater surface ranged from approximately 5.88 (MW-5) to 24.43 (MW-7) feet below top of casing across the site. Groundwater flow is indicated to be to the south-southeast, which is consistent with Terraine’s December, 2008 report. • MW-7 and MW-8 were dry and therefore not sampled. Based on the findings presented in this report, S&ME recommends the following for the next quarter: 1. Sample and analyze groundwater from the eight monitor wells MW-2, MW-3, MW-4, MW-5, MW-6, MW-7 MW-8 and RW-5 using EPA Method 6200B on a semi-annual basis with the next event to occur in April 2010. 2. Sample WSW-5 on a semi-annual basis. Sample WSW-1 through WSW-4 and WSW-6 on an annual basis, beginning with the next quarter. 3. Remove and replace the well booms in MW-2, MW-3, MW-4, MW-5 and RW-5 following the semi-annual monitoring events.
