Submitted via: Herbert.Berger@ncdenr.gov Reference: Report of Mobile Multi-Phase Extraction Needham Service Station/Incident #: 3510; UST #: 2467 719 Main Street, Pilot Mountain, Surry County, North Carolina S&ME Project No. 4305-17-229A Dear Mr. Berger: S&ME, Inc., (S&ME) presents this report in accordance with the North Carolina Department of Environmental Quality/S&ME Contract No. N17002 and S&ME Proposal No. 43-1700706A, dated January 23, 2019, and as authorized by Task Authorization No. 02, dated February 12, 2019. The scope of services included the following. Provide UIC Notification of Intent for proposed air sparge activities. Provide professional time for site activities. Perform Mobile Multi-phase Extraction (MMPE) on monitor wells MW- 6R, MW-13 and MW-14, and perform periodic air sparging on monitor well MW-2 (Figure 1). Summarize MMPE activities in a Letter Report. The services were conducted as proposed. Services were delayed until receipt of a signed Site Access Agreement by the NCDEQ on March 7, 2019.  Underground Injection Control-Notification of Intent On March 14, 2019, S&ME submitted the attached Notification of Intent form and supporting information to the NCDEQ. S&ME received permit number WI0400520 via email on March 27, 2019 (Attachment I).  Summary of MMPE Event S&ME performed depth to water measurements on selected monitor wells, on April 22, 2019, and prior to the MMPE event. Light non-aqueous phase liquid (LNAPL) was measured in monitor well MW-13 at a thickness of 1.00 foot. Measurements in monitor wells not exhibiting LNAPL ranged from 33.83 feet below top of casing in monitor well MW-12 to 37.44 feet below top of casing in monitor well MW-6R (Table 1). Pre-MMPE groundwater elevations are shown on Figure 2. Groundwater flow conditions were similar to observations recorded on November 13, 2018; however, groundwater occurrence ranged from approximately 3 feet to approximately 3.5 feet higher in elevation. Environmental Field Reports dated April 22 and April 26, 2019, are provided in Attachment II. Advanced Environmental Services, Inc., (Advanced) conducted a 96-hour MMPE event using monitor wells MW-6R, MW-13 and MW-14, from April 22 to April 26, 2019. I arrived on site and opened all wells to let levels equilibrate. I had to walk across the street to Pilot Heating and Air and get an employee to move his car before AES arrived to set up equipment. AES arrived on site at approximately 1030 hours. The MMPE started at approximately 1205 hours. They will monitor the equipment several times a day and get water levels in MW-12. They set up the vacuum on wells MW-6R, MW-13 and MW-14. Sparging will start tomorrow at MW-2. The total amount of liquid recovered was 4560 gallons. The amount of free product was 75 gallons. Zebra Environmental was on site to pump out and haul off the recovered liquids. Note: While on site today, I used the metal detector to locate MW-1. The MMPE system was active for 96.0 hours. During the event vacuum at the recovery pump ranged from 11.0 to 13.0 inches of mercury (hg) and stabilized at approximately 11.5 inches of mercury. Airflow rates ranged from 3,900 to 4,300 feet per minute and stabilized at approximately 4,200 feet per minute. System temperature ranged from 90 – 108 degrees F and humidity maintained 99 percent. Field gauged volatile organic compounds (VOC’s) decreased from 34,000 to 14,000 parts per-million (PPM) throughout the event. Groundwater / Product Mix Recovered: During the event a total of 4,560 gallons of product/water mix were removed from the site and properly disposed at a certified North Carolina disposal facility. Approximately 92 gallons of free product was recovered as liquid. Total fluids recovered, was measured based on stick readings conducted during and following the event. Based on 96.0 hours of operation and a total of 4,560 gallons recovered, the average groundwater recovery rate was calculated to be approximately 47.50 gallons per hour. Events Results: Based on mass-removal calculations 428.27 pounds or 69.61 gallons of VOC’s were removed as vapor. Using Dwyer Magnahelic Gauges vacuum influence was measured at MW-12.
