Summary of Activities Associated with Current TA WithersRavenel, Inc. (WR) conducted a groundwater monitoring event at the Needham Service Station incident located at 719 East Main Street in Pilot Mountain, Surry County, North Carolina. These activities were completed in association with TA-01, under NCDEQ Contract #N122019B. The scope of work for this project included the gauging and sampling of MW-2, MW-5, MW-6R, MW-7, MW-9, MW-11, MW-12, MW-13, and MW-14. WR was unable to locate MW-9 but did locate MW-8 and collected a groundwater sample from that well, I lieu of MW-9. LNAPL was identified in MW-13 which was, therefore, not sampled. 2. Soil, Groundwater and LNAPL Assessment Information Soils The UST system was removed in October 1991. Soil borings advanced in the vicinity of the USTs during 1987 identified elevated TPH concentrations in soil. Soil assessment activities conducted during the installation of MW-12 and MW-14 identified several petroleum compounds between 5 and 10 feet below ground surface at concentrations that exceeded the Soil-to-Groundwater Maximum Soil Contaminant Concentrations (MSCCs). C9-C18 Aliphatic hydrocarbons were reported in MW-12 (10’) at a concentration that exceeded the Residential MSCC. TPH-GRO concentrations above the NCDEQ standard of 50 mg/kg extended to 20 feet bgs in the MW-12 boring. Groundwater Laboratory results for the groundwater samples obtained from MW-6R and MW-14 on April 23, 2021 revealed benzene at concentrations that exceeded the Gross Contamination Level (GCL). EDB was reported in MW-14 at a concentration that exceeded the GCL for the compound. Several additional compounds were reported at concentrations that exceeded their respective NCAC 2L Standards in monitoring wells MW-2, MW-5, MW-6R, MW-7, MW-8, MW-12, and MW-14. LNAPL/Free Product LNAPL/Free Product was measured in MW-13 at a thickness of 1.18 feet in April 2021. LNAPL has been consistently measured in MW-13 since the well was installed in March 2015. LNAPL has historically been identified in MW-2 and MW-6R but not since January 2010. 3. Receptor Information According to historical reporting, municipal water is available to the local area. WR did not complete a receptor survey during the April 2021 monitoring event. Based on historical receptor information (December 2008), WSW-4, an inactive supply well, is located within 500 feet of the subject property; WSW-2, an inactive supply well, is located between 500 feet and 1,000 feet of the subject property; while WSW-1 and WSW-2 are active water supply wells located between 1,000 feet and 1,500 feet of the subject property. The locations of both WSW-2 and WSW-4 are reportedly connected to the municipal water system. WithersRavenel Our People. Your Success. Needham Service Station WR Project # 02172214.50 Annual Monitoring Report June 2021 4 One intermittent blue line tributary of Toms Creek was identified approximately 720-feet east northeast of the subject property. According to the SWAP Mapping Application, no wellhead protection areas are located within 1,500-feet of the subject property. According to the SWAP Mapping Application the subject property is located within the Stream Zone Inside Protection Area for the Town of Pilot Mountain (PWS ID: NC0286025). 4. Remediation Activities Along with the closure of the UST system, the following remedial activities have occurred: • AFVR Events: October 2011, May 2011, and January 2012 recovered vapor as gasoline equivalent to 75 gallons, 188 gallons, and 3 gallons, respectively. An MMPE event performed during August 2012 using monitoring wells MW-2 and MW-6R recovered 87 gallons of gasoline as vapor. • A hybrid air sparge/MMPE event performed during April 2019 using monitoring wells MW-2 (air sparge), and MW-6R, MW-13 and MW-14 (MMPE), recovered approximately 70 gallons of gasoline as vapor and 4,560 gallons of product/water mix, including approximately 92 gallons of gasoline as liquid. 5. Recommendations Based on the results of assessment and monitoring activities conducted at the site in association with this TA, WR recommends the following if funding levels are sufficient: • Targeted excavation to remove petroleum impacted soils. • Surfactant Enhanced MMPE on MW-13 to abate LNAPL. • Following abatement of LNAPL, emplacement of ISCO agent to reduce dissolved phase petroleum impacts to concentrations less than GCLs. • Continue semi-annual monitoring of site monitoring wells. SITE HISTORY & SOURCE CHARACTERISTIZATION WR has completed assessment and sampling activities at the referenced site in association with TA-01. The following information summarizes the site history and characterization. 1. UST Information The site is located at 719 East Main Street in Pilot Mountain, Surry County, NC (Figure 1). Three 4,000-gallon gasoline USTs were removed from the subject property in October 1991 (Figure 2). UST information is provided in Table B-1. 2. Description of Release/General Assessment History The following summary of the assessment history is based on WR’s review of historical files for the subject incident: • August 18, 1987 - a release due to a leaking elbow joint in the product pipeline was reported to the Surry County Fire Marshall. On October 22, 1991, three 4,000-gallon gasoline USTs were removed from the site. Based on a Groundwater Investigation and Remedial Action Plan by Engineering Tectonics (ET), dated July 22, 1988, nine groundwater monitoring wells (MW-1, 2, 3, 4, 5, 6, 7, 8, 9) were installed at the site and northeast of the site at 723 (Hurst Garage) and 733 52 Bypass located across Highway 52 Bypass from the subject site. • April 2004 to January 2005 - Geological Resources, Inc. (GRI) sampled the site monitoring wells not containing LNAPL and one water supply well, WSW-1. LNAPL was noted in monitoring well MW-2 during the sampling events. Petroleum constituent concentrations greater than the 2L Standards were detected in samples collected from multiple monitoring wells. Concentrations greater than the GCLs were reported in monitoring wells MW-6R, MW-7 and MW-8. Monitoring wells MW-1, MW-4 and MW-6 were destroyed by a snowplow. MW-6R was installed in 2004 as a replacement to MW-6. In August 2004. An MMPE event was conducted on monitor wells MW-2 and MW-6R with 3,514 gallons of liquid removed. A total of 18 gallons of LNAPL was recovered. • March 2006 to May 21, 2007 - Terraine sampled the site monitoring wells not containing LNAPL and two water supply wells (WSW-1 and WSW-3). MW-11 was installed on May 21, 2007. LNAPL was noted in MW-2, MW-3 and MW-6R during the sampling event on May 24, 2007. MW-3 had measurable LNAPL only once since its installation. Constituent concentrations greater than the 2L Standards were reported in multiple monitoring wells and GCL exceedances were reported for benzene, ethylene dibromide (EDB) and 1, 2-dichloroethane (1,2-DCA) in monitoring well MW-7. • November 20, 2008 - Terraine conducted a monitoring event including 10 monitoring wells (MW-1R, MW-2, MW-3, MW-5, MW-6R, MW-7, MW-8, MW-9, MW-10 and MW-11) and one water supply well (WSW-1). LNAPL was observed in MW-6R at a thickness of 0.14 feet. The lab results from the collected samples indicated petroleum constituent concentrations greater than the 2L Standards in five monitoring wells (MW-1R, MW-5, WithersRavenel Our People. Your Success. Needham Service Station WR Project # 02172214.50 Annual Monitoring Report June 2021 6 MW-8, MW-9, MW-11) with concentrations of benzene, EDB and 1, 2-DCA in MW-2, and benzene in MW-7, at levels greater than their corresponding GCLs. • January 5 and 15, 2010 - S&ME conducted a monitoring event including eight monitoring wells (MW-1R, MW-3, MW-5, MW-7, MW-8, MW-9, MW-10, MW-11) and one water supply well (WSW-1). LNAPL was observed in MW-2 (0.02 feet) and in MW-6R (0.11 feet). The lab results from the collected samples indicated petroleum constituent concentrations greater than the 2L Standards in four monitoring wells (MW-5, MW-7, MW-9, and MW- 11). A water sample was collected from water supply well WSW-1 on January 15, 2010. No petroleum constituents were reported above the laboratory reporting limits. • October 15, 2010 - S&ME conducted an 8-hour AFVR event on MW-2 and MW-6R. During the AFVR event, approximately 472 pounds of vapor phase hydrocarbons were removed, which corresponds to approximately 75 gallons of gasoline recovered. • October 15 and 25, 2010 - S&ME conducted a monitor well sampling event on monitoring wells MW-2, MW-5, MW-6R, MW-7, MW-8, MW-9 and MW-11. Each sample contained several constituent concentrations greater than the 2L Standards, with the highest concentrations detected in monitor well MW-2. Concentrations in monitoring wells MW- 2, MW-5, MW-6R, MW-7, MW-8, MW-9 and MW-11 appeared to be decreasing relative to the November 2008 sampling event. • May 11, 2011 - S&ME conducted an 8-hour AFVR event using monitoring wells MW-2 and MW-6R. Prior to the AFVR event, no LNAPL was measured in MW-2 or MW-6R. A total of 1,197 gallons of total fluids was removed, and an estimated total of 1,177 pounds of vapor phase hydrocarbons (which corresponds to approximately 188 gallons of gasoline) was recovered from MW-2 and MW-6R. • May 11 and 24, 2011 - S&ME collected groundwater samples from monitoring wells MW- 1R, MW-2, MW-5, MW-6R, MW-7, MW-8, MW-9 and MW-11. All sampled wells contained several constituent concentrations greater than the corresponding 2L Standards, with the highest concentrations detected in monitor well MW-6R. Benzene was detected above the GCL in monitoring wells MW-2, MW-6R, and MW-7. EDB was detected above the GCL in monitoring well MW-6R. • January 4, 2012 - S&ME conducted an 8-hour AFVR event using monitoring wells MW-2 and MW-6R. Prior to the AFVR event, no LNAPL was measured in MW-2 or MW-6R. A total of 855 gallons of total fluids was removed, and an estimated total of 16 pounds of vapor phase hydrocarbons (which corresponds to approximately 3 gallons of gasoline) was recovered from MW-2 and MW-6R. • February 22 and 23, 2012 - S&ME was on-site to measure the depth to water and LNAPL, and to collect groundwater samples from monitoring wells without LNAPL. Monitoring wells MW-2, MW-5, MW-6R, MW-7, MW-8, MW-9 and MW-11 contained several constituent concentrations greater than the corresponding 2L Standards, with the highest concentrations detected in monitor well MW-6R. Benzene was detected above the GCL in monitoring wells MW-2, MW-6R, and MW-7. EDB exceeded the GCL in monitoring well MW-6R. • September 19, 2012 - S&ME collected groundwater samples from monitoring wells without LNAPL. Monitoring wells MW-2, MW-5, MW-6R, MW-7, MW-8, MW-9 and MW-11 contained several constituent concentrations greater than the corresponding 2L Standards, with the highest concentrations detected in monitor well MW-6R. EDB exceeded the GCL in monitoring well MW-6R. • April 2014 - S&ME completed a groundwater sampling event (Groundwater Monitoring Report dated May 16, 2014). Dissolved petroleum concentrations were reported in the site groundwater at concentrations greater than GCLs. • March 12, 2015 - S&ME installed soil borings MW-12, MW-13 and MW-14. MW-12 was installed near the reported, former line leak location, MW-13 was installed in the apparent downgradient position to the former UST basin, and MW-14 was installed at the former dispenser. Each soil boring was extended to approximately 44 feet to 46 feet below the ground surface. Soil samples were collected at periodic intervals during the performance of each soil boring and screened in the field with a photoionization meter (PID). PID readings at each soil boring location measured greater than 15,000 parts per million (ppm) from near the ground surface to the termination depth. Soil samples at soil boring MW-12 were selected from the 10 feet, 20 feet and 30 feet depth intervals for laboratory analysis. Soil samples at soil boring MW-14 were selected from the 5 feet, 15 feet and 25 feet depth intervals for laboratory analysis. No soil samples were selected from soil boring MW-13 for laboratory analysis. TPH-GRO was reported in samples collected from the 10- and 20-feet depth intervals at MW-12, and from the 5- and 15-feet depth interval at MW-14. TPH-GRO was not reported in the sample collected from 30 feet at MW-12, or from 25 feet at MW-14. Multiple petroleum constituents were reported in samples MW-12-10 and MW-14-5 at concentrations greater than the soil-to-groundwater MSCCs. Only C9-C18 Aliphatics were reported at a concentration greater than the Residential MSCC. No target analytes were reported at concentrations greater than the Industrial/Commercial MSCCs. Each well was constructed of Schedule 40 PVC with a 15-foot length of well screen set at the bottom of the borehole. • March 13, 2015 - S&ME was on-site to gauge depth-to-water and collect groundwater samples from monitoring wells MW-1R, MW-2, MW-3, MW-6R, MW-7, MW-11, MW-12, MW-13 and MW-14. Multiple compounds were reported in the groundwater samples at concentrations exceeding the 2L Standards. Benzene was reported in monitor wells MW-7 and MW-14 at concentrations exceeding the GCL for the compound. EDB was reported in sample MW-14 at a concentration greater than the GCL. LNAPL was measured at a thickness of 0.64 foot in newly installed monitor well MW-13. • August 20, 2020 – S&ME collected groundwater samples from MW-2, MW-3, MW-6R, MW-7, MW-9, MW-12 and MW-14. Benzene was reported in monitoring wells 6R and MW-14 at concentrations greater than the GCL. EDB was reported in monitoring well MW-14 at a concentration greater than the GCL. Several petroleum related constituents in monitoring wells MW-2, MW-6R, MW-7, MW-9, MW-12 and MW-14, were reported at concentrations greater than their corresponding 2L Standards. LNAPL was measured at a thickness of 1.00 foot in MW-13. 3. Brief Description of Site Characteristics WithersRavenel Our People. Your Success. Needham Service Station WR Project # 02172214.50 Annual Monitoring Report June 2021 8 The subject property is located at 709 East Main Street in Pilot Mountain, Surry County, NC. The property includes one story brick office building operated as J-N-R Auto, LLC (used car dealership). Surface drainage through the area generally follows topography or is channeled through sub-surface stormwater structures or wet-weather ditches running parallel to primary and secondary roadways. 4. Information on Receptors/Potential Receptors According to historical reporting, municipal water is available to the local area. WR did not complete a receptor survey during the April 2021 monitoring event. Based on historical receptor information (December 2008), WSW-4, an inactive supply well, is located within 500 feet of the subject property; WSW-2, an inactive supply well, is located between 500 feet and 1,000 feet of the subject property; and WSW-1 and WSW-2 are active water supply wells located between 1,000 feet and 1,500 feet of the subject property. The locations of WSW-2 and WSW-4 are reportedly connected to the municipal water system. SITE GEOLOGY AND HYDROGEOLOGY According to the Geologic Map of North Carolina, dated 1985, the site is underlain by Proterozoic to Cambrian metamorphosed granitic rocks. Bedrock was not encountered within 30.4 feet of the ground surface during installation of monitoring well MW-13. During prior assessment activities performed by others, bedrock was encountered by soil borings performed to install monitoring wells MW-2 and MW-3 at depths of 55 feet and 65 feet below ground surface, respectively. Monitoring well MW-2 was installed in unconsolidated material to the top of bedrock, but MW-3 was installed approximately 10 feet into bedrock [Cross-section A-A’, Groundwater Investigation, Engineering Tectonics, P.A. (July 1988)]. Based on the Soil Survey of Surry County, North Carolina, soils on the property are classified as Urban land (Ur). Urban land is described as altered by cutting, filling, grading and shaping so that the original topography and drainage have been changed. Subsurface soil characterized during installation of monitoring wells MW-2 through MW-9 by others consisted of silt, sandy silt and silty fine sand overlying weathered rock. Subsurface soil characterized during installation of monitoring well MW-13 consisted of silty clay and clayey silt, underlain by silt. Groundwater has historically been measured to be approximately 32 to 38 feet below ground surface on the subject property. WithersRavenel Our People. Your Success. GROUNDWATER INVESTIGATION – APRIL 2021 On April 21, 2021, WR personnel mobilized to the subject property to perform groundwater sampling activities at the facility in association with TA-01 under NCDEQ Contract # N122019B. The following paragraphs describe the sampling activities and associated laboratory analytical results. E.1. MONITORING WELL GAUGING AND SAMPLING WR collected a groundwater samples from MW-2, MW-5, MW-6R, MW-7, MW-8, MW-11, MW-12, and MW-14 following purging. WR was unable to locate MW-9 but did locate MW-8 and collected a groundwater sample from the well, in lieu of MW-9. LNAPL was identified in MW-13 at a thickness of 1.18 feet and the well was, therefore, not sampled. Groundwater elevations ranged from 33.05 feet below top of casing (BTOC) in MW-12 to 38.36 feet BTOC in monitoring well MW-7. Based on these elevations, groundwater flow direction appears to be towards the east-northeast. This groundwater flow direction corresponds with historical reporting. Monitoring well information is included in Table B-9 and a groundwater contour map is included as Figure 3. E.2. LABORATORY RESULTS Analytical results for the groundwater samples revealed the following: • One or more VOC compounds were reported in each of the sampled monitoring wells at concentrations that exceeded their respective NCAC 2L Standards. o Tetrachloroethene (PCE), a non-petroleum related compound, was reported in MW-8 and MW-12 at a concentration that exceeded the NCAC 2L Standard. • Benzene was reported in MW-6R and MW-14 at concentrations that exceeded the GCL. • EDB was identified in MW-14 at a concentration that exceeded the GCL. CONCLUSIONS & RECOMMENDATIONS This report documents the results of field activities conducted at the site for TA-01, which included the sampling of site monitoring wells. Conclusions based on observations, analytical data and research relating to the release at the site combined with historical information include the following: • Municipal water is available to the local area. • Two active water supply wells have historically been identified between 1,000-feet and 1,500-feet from the subject property. • No active water supply wells are reported to be located within 1,000-feet of the subject property. • One intermittent tributary is located within 1,000-feet of the subject property. • No wellhead protection areas are located within 1,500 ft of the site. • LNAPL was measured at a thickness of 1.18 feet in MW-13. • GCL exceedances were reported in MW-6R and MW-14. • NCAC 2L exceedances were reported in all eight sampled monitoring wells. • Based current and historical total VOC concentrations, dissolved phase impacts appear to be increasing or remaining steady in the vicinity of MW-6R and MW-14 and decreasing in downgradient offsite monitoring wells MW-7 and MW-8. Based on the results of assessment and monitoring activities conducted at the site in association with this TA, WR recommends the following if funding levels are sufficient: • Targeted excavation to remove petroleum impacted soils. • Surfactant Enhanced MMPE on MW-13 to abate LNAPL. • Following abatement of LNAPL, emplacement of ISCO agent to reduce dissolved phase petroleum impacts to concentrations less than GCLs. • Continue semi-annual monitoring of site monitoring wells.
