Main Street in Pilot Mountain, Surry County, North Carolina. The former Needham Service Station site was previously operated as a gas station and is currently used as an office building. Figure 1 indicates the site location and topography. Site photographs are included in Appendix III. On August 18, 1987, a release due to a leaking elbow joint in the product pipeline was reported to the Surry County Fire Marshall. On October 22, 1991 all three 4,000-gallon gasoline USTs were removed from the site. In a Groundwater Investigation and Remedial Action Plan by Engineering Tectonics (ET) dated July 22, 1988, nine groundwater monitoring wells (MW-1, 2, 3, 4, 5, 6, 7, 8 and 9) were installed at the site and northeast of the site at Hurst’s garage located across Hwy 52 at 727 East Hwy 52 bypass. Between April 2004 and January 2005, Geological Resources, Inc. (GR) sampled the site monitoring wells not containing free product and one water supply well (WSW-1). Free product was noted in MW-2 during the sampling events. Contaminant concentrations exceeded the 2L Standards in multiple wells and exceeded the Gross Contaminant Levels (GCLs) in MW-6R, MW-7 and MW-8. Monitor wells MW-1, MW-4 and MW-6 were destroyed by a snowplow and MW-6R was installed in 2004 as a replacement to MW-6. In August 2004, a mobile multiphase extraction (MMPE) event was conducted on MW-2 and MW-6R with 3,514 gallons of liquid removed. A total of 18 gallons of free product was recovered. Between March 2006 and May 21, 2007, Terraine sampled the site monitoring wells not containing free product and two water supply wells (WSW-1 and WSW-3). MW-11 was installed on May 21, 2007. Free product was noted in MW-2, MW-3 and MW-6R during the sampling event on May 24, 2007. MW-3 has had measurable product only once since its installation. Contaminant concentrations exceeded the 2L Standards in multiple wells and exceeded the Gross Contaminant Levels (GCLs) for benzene, EDB and 1, 2-DCA in MW-7. On November 20, 2008, Terraine conducted a monitoring event including ten monitor wells (MW-1R, MW-2, MW-3, MW-5, MW-6R, MW-7, MW-8, MW-9, MW-10 and MW-11) and one water supply well (WSW-1). During that event, free product was observed in MW-6R (0.14 feet). The lab results from the collected samples indicated petroleum constituents above the 2L Standard in five monitor wells (MW-1R, MW-5, MW-8, MW-9 and MW-11) with concentrations of benzene, EDB and 1, 2-DCA in MW- 2 and benzene in MW-7 exceeding the Gross Contaminant Levels (GCLs). Four water supply wells were identified within 1, 500 feet of the site (Figure 4). Water supply wells WSW-1 (approximately 1,300 feet from source) and WSW-3 (approximately 1,400 feet from source) are reported to be used for potable purposes and WSW-2 (approximately 530 feet from source) and WSW-4 (approximately 470 feet from source) are reportedly no longer in use. Both residences with WSW-2 and WSW-4 are connected to and utilize municipal water. 2 Groundwater Monitoring Report – NCDENR Incident No.3510 S&ME Project No. 1354-09-272A Needham Service Station, Pilot Mountain, Surry County NC January 18, 2011 On January 5 and 15, 2010, S&ME conducted a monitoring event including eight monitor wells (MW-1R, MW-3, MW-5, MW-7, MW-8, MW-9, MW-10 and MW-11) and one water supply well (WSW-1). During that event, free product was observed in MW-2 (0.02 feet) and in MW-6R (0.11 feet). The lab results from the collected samples indicated petroleum constituents above the 2L Standard in four monitor wells (MW-5, MW-7, MW-9, and MW-11). A water sample was collected from water supply well, WSW-1, during the sampling event on January 15, 2010. No petroleum constituents were reported above the laboratory reporting limits. Municipal water is available at the site and surrounding area. 3 FREE PRODUCT INVESTIGATION/RECOVERY S&ME, Inc. (S&ME) submits this Groundwater Monitoring Report for the above referenced site. The report was prepared in general accordance with the North Carolina Department of Environment and Natural Resources (NCDENR) Contract # 16-1713-2C for the Central Region, and S&ME’s Proposal Number 1354-22816-10 dated April 7, 2010 as approved with NCDENR’s Task Authorization #1 dated September 30, 2010. The proposed services included free product level check for MW-2 and MW-6R, sampling up to 10 monitor wells, conducting an 8-hour aggressive fluid vapor recovery (AFVR) event on wells MW-2 and MW-6R and reporting. The AFVR event was conducted on October 15, 2010 and the sampling event was conducted on October 15 and 25, 2010. During S&ME’s sampling event, free product was not present in monitor wells MW-2 and MW-6R. 3.1 Free Product Investigation S&ME, Inc. did not observe any free product in wells MW-2 and MW-6R on October 15, 2010. Historically, free product has been detected in MW-2 and MW-6R. On January 5, 2010, 0.02 and 0.11 feet of free product was measured in monitor well MW-2 and MW- 6R, respectively, before conducting two 4-hour AFVR events on MW-2, MW-6R and MW-7. A total of 1,284 gallons of total fluids was removed during both 4-hour AFVR events. On one occasion in May 2007, product was measured in MW-3. In August 2004, a MMPE event was conducted on MW-2 and MW-6R with 3,514 gallons of liquid removed. A total of 18 gallons of free product was recovered. 3.2 AFVR Event S&ME conducted one 8-hour AFVR event on October 15, 2010 using MW-2 and MW- 6R. The AFVR data is shown in Table 3. Field sampling forms and AFVR data including the manifest for disposal of liquids are provided in Appendix II. A total of 300 gallons of total fluids was removed. Air emissions from the AFVR event were measured throughout the event utilizing an organic vapor analyzer. Organic vapor measurements ranged from 1024 to 4450 parts per million (ppm) during the 8-hour event on wells MW-2 and MW-6R. Based on vapor emissions recorded during the AFVR event, approximately 472 pounds of vapor phase hydrocarbons were removed which corresponds to approximately 75 gallons of gasoline recovered. 3 Groundwater Monitoring Report – NCDENR Incident No.3510 S&ME Project No. 1354-09-272A Needham Service Station, Pilot Mountain, Surry County NC January 18, 2011 4 GROUNDWATER MONITORING 4.1 Monitor Well Sampling On October 15 and 25, 2010, S&ME was on-site to measure the depth to water and free product, and to collect groundwater samples from monitor wells without free product. The depth to water ranged from 7.17 feet below the top of casing (ft-TOC) in MW-11 to 41.14 ft-TOC in MW-8 (Table 1). Based on the top of casing elevations and depth to liquid, groundwater flow is towards the north (Figure 2). After measuring the depth-to-liquid, S&ME personnel purged each well without free product (MW-1R, MW-2, MW-3, MW-5, MW-6R, MW-7, MW-8, MW-9, MW-10 and MW-11) with a new disposable polyethylene bailer by evacuating three well volumes, or until the well was dry. After completing the well purging and allowing groundwater to recover in the monitor well, S&ME personnel collected a groundwater sample from the monitor wells. The groundwater samples were placed in laboratory-supplied containers, stored in a cooler with ice, and shipped under chain-of-custody procedures to Pace Analytical Services, Inc. (Pace), a North Carolina certified laboratory for laboratory analysis. The groundwater samples were analyzed for VOCs by EPA Method 6200B. Field data sheets are included in Appendix II. 4.2 Monitor Well Sampling Results The laboratory analytical results are summarized in Table 2 and a copy of the laboratory report and chain-of-custody form is included in Appendix I. Monitor wells MW-2, MW-5, MW-6R, MW-7, MW-8, MW-9 and MW-11 contained several contaminants at concentrations exceeding the 15A NCAC 2L .0202 groundwater quality standards (2L Standards), with the highest concentrations detected in monitor well MW-2 as summarized in the table below. Note that monitor well MW-8 contained contaminants at levels above the 2L Standards which contradict with the results for this well from the January 5, 2010 which appear to be anomalously low compared to prior sampling results. Tetrachloroethene was detected in MW-1R and MW-11 at concentrations above the 2L Standard. Benzene was detected above the gross contaminant level (GCL) in MW-2, MW6R and MW-7. Evidence of chlorinated solvents above the 2L Standard suggests a secondary source. Note that an automotive repair garage is located immediately west of the Needham Service Station building. Main Street in Pilot Mountain, Surry County, North Carolina to assess current petroleum impact to groundwater. The findings at the site are as follows: š No active drinking water supply wells are reportedly located within 1,000 feet of the source area and no active water supply wells are reportedly within 500 feet of the source area. Municipal water is available to the site and surrounding area. š On October 15, 2010, S&ME conducted one 8-hour AFVR events using MW-2 and MW-6R. The AFVR resulted in the removal of 472 gallons of total fluids and an equivalent 75 gallons of petroleum product based on vapor emissions. š Prior to the AFVR event, no free product was observed in MW-2 and MW-6R. š No free product was detected in MW-2 and MW-6R on October 25, 2010, ten days after conducting the AFVR event. š Monitor wells MW-2, MW-5, MW-6R, MW-7, MW-8, MW-9 and MW-11 each contained several contaminants at concentrations exceeding the 15A NCAC 2L .0202 groundwater quality standards (2L Standards), with the highest concentrations detected in monitor well MW-2. š Contaminant concentrations in MW-2, MW-5, MW-6R, MW-7, MW-8, MW-9 and MW-11 appear to be decreasing when compared to the November 2008 sampling event. š MW-1R and MW-11 had Tetrachloroethene concentrations above the 2L Standard. š Evidence of chlorinated solvents above the 2L Standard suggests an additional source. š The depth to water ranged from 7.17 feet below ft-TOC in MW-11 to 41.14 ft- TOC in MW-8. š Groundwater flow is to the north. š Based on site conditions, the site qualifies to be classified as an intermediate risk. 5 Groundwater Monitoring Report – NCDENR Incident No.3510 S&ME Project No. 1354-09-272A Needham Service Station, Pilot Mountain, Surry County NC January 18, 2011 6 Based on the above, S&ME recommends measuring free product levels in MW-2 and MW-6R. If free product is not present, submit an email to the incident manager recommending no further action for the site. If free product is present, then recommend the following actions on April and May 2011: 1. April 2011: š Perform an AFVR event using wells MW-2, and MW-6R. The purpose is to remove free product. 2. May 2011: š Sample up to ten monitor wells without free product: MW-1, 2, 3, 5, 6R, 7, 8, 9, 10 and 11.
