EXECUTIVE SUMMARY The purpose of this report is to evaluate the current status of the groundwater quality at the former Johnson’s Grocery site (Incident No. 30594). Terracon completed this report to summarize the findings of assessment activities performed on May 14, 2020. The release incident is situated on the property located at 5870 NC Highway 268 in Dobson, Surry County, North Carolina in an area developed mostly as agricultural land with rural residences. The site is currently occupied by an automotive repair facility and used car sales lot. According to historical assessment information, three on-site USTs at the former Johnson’s Grocery were removed in June 2005. Soil sampling conducted during closure activities indicated a petroleum release originated from the former UST system. The release was attributed to product piping located near the northern end of the former UST basin. Additional excavation activities were completed at the site and reportedly 702.15 tons of petroleum impacted soil were removed from the site. Terracon mobilized to the site on May 14, 2020 to conduct groundwater sampling activities. Initially, Terracon gauged the groundwater elevations in monitoring wells MW-1 through MW-4, MW-7 and MW-8. Depth to groundwater in the monitoring wells ranged from approximately 15.82 feet below top of casing (btoc) in well MW-3 to 27.45 feet btoc in well MW-7. Approximately 0.01 feet of light non-aqueous phase liquid (LNAPL) was measured in well MW-1 and sheens were noted in several other on-site wells. Based on top of casing elevations, groundwater flow is to the northwest. Prior to sampling, the wells were purged a minimum of three well volumes. ORC inserts were removed from wells MW-2, MW-4, MW-7 and MW-8 and replaced with new inserts after the wells were sampled. The samples, along with chain-of-custody documentation, were delivered to Pace Analytical Services, Inc. in Huntersville, North Carolina for analysis of volatile organic compounds (VOCs) by Standard Method 6200B, including MTBE, EDB and IPE. Benzene was detected at concentrations above its Gross Contamination Level (GCL) in wells MW-1 (22,500 micrograms per liter [µg/L]) and MW-8 (8,360 µg/L). Methyl tert-butyl ether (MTBE) [31,700 µg/L] and 1,2-dibromoethane (EDB) [102J µg/L] were also detected in well MW-1 at concentrations that exceed the respective GCLs. Johnson’s Grocery Store Dobson, North Carolina Terracon Project No. 71149156A Incident No. 30594 June 25, 2020 Responsive ■ Resourceful ■ Reliable 3 The following constituents were detected in one or more on-site monitoring wells at concentrations that exceed the 15A NCAC 2L groundwater standards; benzene, 1,2-dichloroethane, diisopropyl ether (IPE), ethylbenzene, EDB, MTBE, isopropylbenzene, naphthalene, toluene, 1,2,4- trimethylbenznene and total xylenes. Various petroleum constituents were detected in the on-site wells above the laboratory method detection limits (MDLs) but below the 2L groundwater standards. Detected concentrations of benzene and toluene were also compared to the respective 15A NCAC 2B surface water standards since a drainage feature is located along the northern property boundary. Benzene and toluene exceed the respective 2B surface water standards for Class C waters in wells MW-1, MW-2, MW-7 and MW-8. Based on information obtained during the completion of our assessment activities, Terracon provides the following conclusions and findings. LNAPL was detected in well MW-1 at less than a thickness of 0.01 feet but the water level in the well during this sampling event was above the screened interval. Sheens were observed in several other wells at the site. Benzene was detected at concentrations that exceed its GCL in wells MW-1 and MW-8. MTBE and EDB were also detected above the respective GCLs in well MW-1. Benzene, 1,2-dichloroethane, IPE, ethylbenzene, EDB, isopropylbenzene, MTBE, naphthalene, toluene, 1,2,4-trimethylbenznene and total xylenes were detected in one or more monitoring wells above the respective 2L standards. Benzene and toluene were detected in wells MW-1, MW-2, MW-7 and MW-8 above the respective 2B standards. Various petroleum constituents were also detected in the on-site wells at concentrations below the 2L groundwater standards. Based on water level measurements, groundwater flow is towards the northwest. Five private water-supply wells are reportedly located within 1,000 feet of the source area that are currently utilized as sources of potable water. Municipal water is not available to the site or the immediately surrounding properties. Previous sampling events have shown no contamination within the supply wells. Constituent concentrations trends are generally decreasing at the site. Based on the data presented in this monitoring report, Terracon recommends the following activities be completed at the site. Terracon recommends replacing the O-sox inserts in the on-site wells while gauging LNAPL in well MW-1 in August 2020 Based on the analytical results, Terracon also recommends conducting surface water sampling on the on-site tributary of Little Beaver Creek in August 2020. Johnson’s Grocery Store Dobson, North Carolina Terracon Project No. 71149156A Incident No. 30594 June 25, 2020 Responsive ■ Resourceful ■ Reliable 4 Groundwater monitoring wells and the on-site water supply well should be sampled semi- annually to evaluate the status of impact by petroleum constituents. Terracon believes that the use of injectants (potassium persulfate activated by ferric iron) is a viable approach based on the results of the test injection previously conducted at well MW-3. Terracon recommends contacting remedial vendors to evaluate the data and discuss an injection event in wells MW-1, MW-7 and MW-8. C. SITE HISTORY AND CHARACTERIZATION The purpose of this report is to evaluate the current status of the groundwater quality at the former Johnson’s Grocery site. Terracon completed this report to summarize the findings of assessment activities performed on May 14, 2020 at the former Johnson’s Grocery located at 5870 NC Highway 268 in Dobson, Surry County, North Carolina (Figure 1). This Subsequent Monitoring Report was prepared for the site in response to a request by the Incident Manager, Mr. Herb Berger with the NCDEQ Division of Waste Management, Trust Fund Branch. The subsequent monitoring report was prepared in general accordance with 15A NCAC 2L .0115 regulations and the NCDEQ Underground Storage Tank (UST) Section Guidelines for Assessment and Corrective Action for UST Releases, dated July 15, 2008, revised December 2013, under the supervision of Mr. Christopher L. Corbitt, a North Carolina registered licensed geologist (North Carolina License No. 1015). C.1 Site Setting The Johnson’s Grocery site located at 5870 NC Highway 268 in Dobson, North Carolina is comprised of 2.919 acres. The property contains an approximate 8,550 square foot commercial building occupied by The Car & Truck Hut an automotive maintenance shop). A stand-alone garage structure is also located on the site and is utilized by the automotive repair shop and used car sales. The site is depicted in Figure 2. The site is bound to the north by a chicken farm, wooded acreage, and a surface water drainage feature. NC Highway 268 bounds the site to the south. A large area of farm land is located farther to the south, across NC Highway 268. Properties to the east and west, along NC Highway 268, are developed as rural residential. C.2 Release Summary A petroleum release was discovered following the removal of the former on-site UST systems in August 2005. Soil sampling conducted during closure activities indicated the petroleum release was attributed to product piping located near the northern end of the former UST basin. A chronological summary of the UST removal activities and subsequent groundwater assessment activities at the Johnson’s Grocery Store Dobson, North Carolina Terracon Project No. 71149156A Incident No. 30594 June 25, 2020 Responsive ■ Resourceful ■ Reliable 5 site is discussed in the following section of the report. C.3 Previous Investigations In June 2005, three former on-site USTs were reportedly removed from the site. Soil sampling conducted during closure activities indicated a petroleum release had occurred from the former UST system. The release was attributed to product piping located near the northern end of the UST basin. Additional excavation activities were completed at the site, removing 702.15 tons of petroleum-impacted soil. Following over excavation activities, four soil samples were collected from the bottom of the excavation, the risk-based analytical results indicated the presence of various petroleum constituents at concentrations that exceeded the respective Soil-to-Water Maximum Soil Contaminant Concentrations (MSCCs). Benzene, naphthalene, n-propylbenzene, 1,3,5- trimethylbenzene, C5-C8 aliphatics, and C9-C22 aromatics were also detected at concentrations exceeding the respective Residential MSCCs. A Phase II LSA Report was completed for the site by GeoScience & Technology, P.A. (GeoScience) in February 2006 to further assess the on-site petroleum release. GeoScience personnel visited the site to install two soil borings/monitoring wells (B1/MW-1 and B2/MW-2) in previously identified source areas from August 2005. Soil samples collected from the two borings identified multiple petroleum (gasoline range) constituents. GeoScience personnel returned to the site in January 2006 to install three additional monitoring wells (MW-3, MW-4 and MW-5). The on-site wells were sampled in January 2006 with the exception of well MW-1 which contained approximately 2.74 feet of product. Sampling of the wells indicated multiple petroleum constituents were detected in the groundwater above the 2L groundwater standards. Terracon mobilized to the site on July 24, 2014 to conduct groundwater sampling activities. Terracon was unable to locate well MW-5 with a metal detector due to dense vegetation to the west of the vacant grocery store building. During the sampling event, depth to groundwater in the monitoring wells ranged from approximately 17.9 to 24.4 feet bgs. Groundwater flow was determined to be towards the northwest. Terracon also sampled two nearby water supply wells (WSW-1 and WSW-3) to evaluate potential impacts to off-site receptors. Terracon attempted to sample wells WSW-2 and WSW-4 but did not receive a response from the well owners. Laboratory results indicated various petroleum constituents were detected in the monitoring wells at the site. The following constituents were detected at concentrations above the respective 2L groundwater standards: benzene in wells MW-2 (67.4 µg/L) and MW-4 (46.4 µg/L); naphthalene in wells MW-2 (19.1 µg/L) and MW-4 (27.8 µg/L); n-propylbenzene in well MW-4 (115 µg/L); and 1,2,4- trimethylbenzene in well MW-4 (621 µg/L). No petroleum constituents were detected in the two water supply wells sampled during this event. Terracon personnel returned to the site on January 12, 2015 to conduct a 96-hour Mobile Multi- Phase Extraction (MMPE) event at monitoring wells MW-1 and MW-5 which contained the highest levels of petroleum constituents or LNAPL based on recent and historic groundwater sampling Johnson’s Grocery Store Dobson, North Carolina Terracon Project No. 71149156A Incident No. 30594 June 25, 2020 Responsive ■ Resourceful ■ Reliable 6 events conducted on the site. Terracon was unable to locate well MW-5 and subsequently MMPE activities were only conducted utilizing well MW-1. The MMPE event was conducted by ZAVA Industrial Inc. from January 12, 2015 to January 16, 2015. According to ZAVA’s field report dated January 16, 2015, a total of 1,784 gallons of water was recovered from the well and mass balance calculations indicated total VOC emissions removed from the well equated to 24.52 gallons. In addition to the MMPE event, two soil borings (GP-6 and GP-7) were advanced by Probe Technology, Inc. to approximate depths of 30 feet bgs. Soils collected from the borings were screened for organic vapors using a photo-ionization detector (PID). Soil Samples GP-6(20-25) and GP-7(20-25) were submitted to Pace Analytical in Huntersville, North Carolina for analysis of risk- based parameters. According to the laboratory analytical report, benzene was detected in sample GP-7(20-25) at 9.6 micrograms per kilogram (µg/kg) which exceeded the Soil-to-Water MSCC of 5.6 µg/kg. Additional analytes were detected at concentrations below the respective Soil-to-Water MSCCs. The two borings (GP-6 and GP-7) were converted to monitoring wells to assess the lateral extent of petroleum constituents in the groundwater. In August 18, 2015, Terracon attempted to sample the seven on-site monitoring wells. Terracon was unable to locate wells MW-5 and MW-6 with a metal detector due to dense vegetation along the northern property boundary. Approximately 3 feet of LNAPL was measured in well MW-1. Benzene was detected in well MW-7 above the established GCL. Various other petroleum constituents were detected in wells MW-2, MW-4 and MW-7 above the respective 2L groundwater standards. Terracon mobilized to the site in March 2018 to sample wells MW-1, MW-2 through MW-4 and MW- 7. Approximately 1.58 feet of LNAPL was observed in well MW-1. Terracon hand bailed approximately 0.5 gallons of LNAPL into an on-site drum and installed a NewPig Absorbent sock into the well after hand bailing was completed. Based on the analytical results, multiple constituents were detected above the established 2L standards in wells MW-2, MW-4 and MW-7; however, benzene concentrations in well MW-7 decreased from 9,300 µg/L (2015) to 3,330 µg/L during the March 2018 sampling event. In October 2018, Terracon collected groundwater samples from the on-site monitoring wells and collected a surface water sample (SW-01) from the unnamed tributary that borders the site to the north and east. Approximately 0.04 feet of LNAPL was measured in well MW-1; therefore, a water sample was not collected. Multiple target constituents were detected above the established 2L standards in wells MW-2, MW-4 and MW-7. Benzene was also detected above the established 15A NCAC 2B surface water standards in sample SW-01. In February 2019, Terracon mobilized to the site to provide oversight and document an injection pilot study conducted at the site. Terracon observed the installation of four injection points using direct push technology (DPT) to the target injection depth of 35 ft bgs. The solution used at the site was designed to destroy organic compounds dissolved in the groundwater and adsorbed to the aquifer materials. The solution combined the use of sodium persulfate, potassium persulfate and ferric oxide to produce long-term oxidation within the subsurface and provide the appropriate components for a longer lasting biological attenuation process following the initial oxidation event. Terracon also installed well MW-8 down- gradient of the source area and up-gradient of well MW-7 to further delineate the contaminant plume. In September 2019, Terracon mobilized to collect groundwater samples from six of the on-site monitoring wells. Prior to sample collection, Terracon gauged the groundwater levels in monitoring wells MW-1 through MW-4, MW-7 and MW-8. Depth to groundwater in the monitoring wells ranged from approximately 16.33 feet (MW-3) below ground surface (bgs) to 25.57 feet (MW-7) bgs. Approximately 0.01 feet of LNAPL was measured in well MW-1. Benzene was detected at a concentration above the established GCL in wells MW-1, MW-7 and MW-8. MTBE and EDB were also detected in well MW-1 at concentrations that exceed the GCLs. The following constituents were detected at concentrations that exceed the 2L standards in various on-site wells: 1,2-dichloroethane, IPE, ethylbenzene, EDB, isopropylbenzene, MTBE, naphthalene, toluene, 1,2,4-trimethylbenznene and total xylenes. Various petroleum constituents were detected in wells MW-2 and MW-4 below the 2L groundwater standards. Target analytes were not detected above the MDLs in monitoring well MW-3. In January 2020, Terracon and Advanced Environmental Services (AES) conducted a 96-hour MMPE event at monitoring well MW-1. According to the AES field report dated February 6, 2020, a total of 2,357 gallons of a product/water mixture was recovered from the well and mass balance calculations indicated total VOC emissions removed from the well equated to 3.77 gallons. C.4 Water Supply Wells The nearest water-supply well (WSW-1), located on the site within 100 feet of the release area, is currently utilized by the on-site auto repair shop. Well WSW-3 is located approximately 850 feet west and down-gradient of the release area. The well is utilized as a source of potable water by an on-site residence. Well WSW-2 is located on an up-gradient residential property to the east of the release area and up-gradient well WSW-4 is associated with a large farm located to the south across NC Highway 268. Well WSW-5 was observed on the northwestern adjacent property, currently developed as a chicken farm, during our site visit in March 2018. Results of the water- supply well inventory are presented in Table 1 and well locations are depicted on Figure 6. C.5 Surface Water According to the USGS topographic map, the site is situated on the northwestern flank of a northwest-southeast trending topographic ridge system that generally parallels NC Highway 268. The site is located within the drainage basin for an unnamed tributary of Little Beaver Creek, which is located along the northern and eastern property boundaries. According to the NC Surface Water Classifications online database, Little Beaver Creek is classified as a Class C surface water feature. Surface waters are depicted on Figure 1. NC Highway 268 bounds the site to the south, with a large farm located farther south. Properties to the east and west along NC Highway 268 are developed as residential. D. ASSESSMENT INFORMATION D.1 Site Geology and Hydrogeology The following resources were reviewed to evaluate site-specific geological and hydrogeological characteristics. Copeland Quadrangle, North Carolina, USGS 7.5-minute series topographic map, dated 1994 (Figure 1). Geologic Map of North Carolina, dated 1985. The Geology of the Carolinas, Carolina Geological Society Fiftieth Anniversary Volume, edited by J. Wright Horton, Jr., and Victor A. Zullo, The University of Tennessee Press, dated 1991. Soil Survey of Surry County, North Carolina, US Department of Agriculture, Natural Resources Conservation Service, Web Soil Survey via the Internet, http://websoilsurvey.nrcs.usda.gov/app/ . The site is located within the Sauratown Mountains Anticlinorium of the Piedmont Physiographic Province. The Piedmont Province generally consists of low to medium elevation, rounded hills and long rolling northeast to southwest trending ridges with incised creeks and river channels. Underlying rock in the site area is described as biotite gneiss, amphibolite and kyanite schist. In-place chemical and physical weathering of parent rocks typically form residual soils in the Piedmont region. Weathering is facilitated by the presence of fractures, joints, and less resistant minerals in the rock. In areas not altered by erosion, alluvial deposition or the activities of man, the typical residual soil profile consists of clay-rich unconsolidated materials at the surface, transitioning to sandy silts and silty sands at depth. Typically, the boundary between soil and rock is not sharply defined. This transitional zone is termed “partially weathered rock” or saprolite. Information obtained from the Soil Survey for Surry County, depicts soils on the site as Fairview sandy clay loam (FeC2). In the Piedmont, groundwater generally occurs in pore spaces within soils and in the structural features present in the underlying rock (i.e. joints, fractures, and faults). Recharge to the water table is primarily by precipitation infiltrating the surficial soils and percolating downward under the influence of gravity to the water table (vadose zone). Typically, the water table is not a level surface, but a subdued reflection of the land surface. The depth to the water table is not consistent and is Johnson’s Grocery Store Dobson, North Carolina Terracon Project No. 71149156A Incident No. 30594 June 25, 2020 Responsive ■ Resourceful ■ Reliable 9 dependent upon several factors that include the amount of rainfall, permeability of the soil, the extent of fracturing in the underlying rock and the influences of groundwater pumping. Groundwater typically flows in directions parallel to the ground surface and under the influence of gravity, migrates to discharge points such as surface water features, the toes of slopes or natural springs. Fluctuations in the depth of the water table can be expected depending on variations in precipitation, surface water run-off and other factors not evident at the time of our subsurface exploration. Normally, the highest levels of the water table occur in late winter and spring and the lowest levels occur in the late summer and fall. Groundwater has been interpreted to flow to the northwest from the site. D.2 Sampling Activities D.2.1 LNAPL Evaluation Approximately 0.01 feet of LNAPL was measured in monitoring well MW-1 on May 14, 2020. Due to the decrease in LNAPL thickness, Terracon collected a groundwater sample from well MW-1. The absorbent sock was not replaced during this sampling event to allow monitoring of the LNAPL thickness during the next sampling event. During the May 2020 sampling event, the screened interval in well MW-1 was submerged, which could attribute to a decrease in LNAPL during this sampling event. It could also be attributed to movement of the contaminant plume towards down- gradient areas. D.2.2 Groundwater Sampling Activities Terracon mobilized to the site on May 14, 2020 to conduct groundwater sampling activities. Initially, Terracon gauged the groundwater elevations in monitoring wells MW-1 through MW-4, MW-7 and MW-8. Depth to groundwater in the monitoring wells ranged from approximately 15.82 feet btoc in well MW-3 to 27.45 feet btoc in well MW-7. Approximately 0.01 feet of LNAPL was measured in well MW-1 and sheens were noted in several other on-site wells. Based on top of casing elevations, groundwater flow is to the northwest. Prior to sampling, the wells were purged a minimum of three well volumes. ORC inserts were removed from wells MW-2, MW-4, MW-7 and MW-8 and replaced with new inserts after the wells were sampled. The approximate locations of the wells are shown on the Site Plan (Figure 2). A groundwater elevation map depicting groundwater flow contours and direction is shown on Figure 3. Current and historical groundwater elevation measurements are provided in Table 3. The groundwater samples and an equipment blank were placed in laboratory-supplied glassware, labeled and stored in an ice-packed cooler. The samples, along with chain-of-custody documentation, were delivered to Pace Analytical Services, Inc. in Huntersville, North Carolina for analysis of VOCs by Standard Method 6200B, including MTBE, EDB and IPE. Johnson’s Grocery Store Dobson, North Carolina Terracon Project No. 71149156A Incident No. 30594 June 25, 2020 Responsive ■ Resourceful ■ Reliable 10 D.2.3 Analytical Results Based on the laboratory analytical results, various petroleum constituents were identified at concentrations above the laboratory reporting limits in monitoring wells MW-1 through MW-4, MW-7 and MW-8. Benzene was detected at concentrations above its GCL in wells MW-1 (22,500 µg/L) and MW-8 (8,360 µg/L). MTBE (31,700 µg/L) and EDB (102J µg/L) were also detected in well MW-1 at concentrations that exceed the respective GCLs. The following constituents were detected in one or more on-site monitoring wells at concentrations that exceed the 15A NCAC 2L groundwater standards; benzene, 1,2-dichloroethane, diisopropyl ether (IPE), ethylbenzene, EDB, MTBE, isopropylbenzene, naphthalene, toluene, 1,2,4- trimethylbenznene and total xylenes. Various petroleum constituents were detected in the on-site wells above the laboratory MDLs but below the 2L groundwater standards. Detected concentrations of benzene and toluene were also compared to the respective 15A NCAC 2B surface water standards since a drainage feature is located along the northern property boundary. Benzene and toluene exceed the respective 2B surface water standards for Class C waters in wells MW-1, MW-2, MW-7 and MW-8. The groundwater analytical results from the monitoring wells are summarized in Table 2. The analytical results are also depicted on Figure 4. A benzene isoconcentration map is provided as Figure 5. Copies of the laboratory analytical reports are provided in Appendix A. E. CONCLUSIONS AND RECOMMENDATIONS Based on information obtained during the completion of our assessment activities, Terracon provides the following conclusions and findings. LNAPL was detected in well MW-1 at less than a thickness of 0.01 feet but the water level in the well during this sampling event was above the screened interval. Sheens were observed in several other wells at the site. Benzene was detected at concentrations that exceed its GCL in wells MW-1 and MW-8. MTBE and EDB were also detected above the respective GCLs in well MW-1. Benzene, 1,2-dichloroethane, IPE, ethylbenzene, EDB, isopropylbenzene, MTBE, naphthalene, toluene, 1,2,4-trimethylbenznene and total xylenes were detected in one or more monitoring wells above the respective 2L standards. Benzene and toluene were detected in wells MW-1, MW-2, MW-7 and MW-8 above the respective 2B standards. Based on water level measurements, groundwater flow is towards the northwest. Five private water-supply wells are reportedly located within 1,000 feet of the source area that are currently utilized as sources of potable water. Municipal water is not available to the site or the immediately surrounding properties. Previous sampling events have shown no contamination within the supply wells. Constituent concentrations trends are generally decreasing at the site. Based on the data presented in this monitoring report, Terracon recommends the following activities be completed at the site. Terracon recommends replacing the O-sox inserts in the on-site wells while gauging LNAPL in well MW-1 in August 2020 Based on the analytical results, Terracon also recommends conducting surface water sampling on the on-site tributary of Little Beaver Creek in August 2020. Groundwater monitoring wells and the on-site water supply well should be sampled semi- annually to evaluate the status of impact by petroleum constituents. Terracon believes that the use of injectants (potassium persulfate activated by ferric iron) is a viable approach based on the results of the test injection previously conducted at well MW-3. Terracon recommends contacting remedial vendors to evaluate the data and discuss an injection event in wells MW-1, MW-7 and MW-8.
