On February 21, 2023 Mid -Atlantic personnel conducted a visual survey of a 1,500-foot radius of
the subject site to identify the possible presence of places of public assembly (i.e. schools,
churches, daycares, etc.), private water -supply and/or irrigation wells, basements, and surface
waters. As shown on Drawing 2, basements and residential uses were identified within the 1,000-
foot radius specified in the receptor survey form. Water -supply wells, irrigation wells, septic
systems, public water supply wells, and places of public assembly were not identified within the
search radii. Monitoring wells were not identified during the visual survey. Information available on
the Durham Maps mapping application provided locations of public storm sewer utilities as
identified on Attachment B. As shown on Drawing 3, the surrounding area is serviced by sanitary
sewer, water, electric, natural gas, and telecommunications lines.
Using information obtained from our visual survey and Durham Maps, Mid -Atlantic has completed
the Brownfields Property Receptor Survey form as provided in Attachment A.
