1.0 BACKGROUND INFORMATION
The Former Bazemore Grocery site is approximately 1.1 acres in size and is located at 730/732
Mt. Olive Road, in Republican, Bertie County, North Carolina (Figure 1). According to the Bertie
County Online GIS Database, the Parcel Identification Number (PIN) is 5875342404 (Figure 2).
The property is currently developed with an abandoned residential building. The site previously
operated two 2,000-gallon USTs, one 1,500-gallon UST, and one septic tank.
2.0 FIELD ACTIVITIES
2.1 Soil Excavation Activities
On January 6 and 7, 2020, ECS observed the excavation of approximately 171.61 tons of
petroleum-impacted soil from the site. ECS personnel field notes are included in Appendix A.
Site photographs documenting field activities are include in Appendix B. Soil excavation
activities were terminated based on encountering apparent clean soil to the south and west of
the excavation, the onsite building/subsurface utilities to the north and east of the excavation, or
the apparent water table at approximately 6 feet below ground surface (ft. bgs). Additional soils
were excavated a further 2 to 3 feet into the water table in an effort to remove any free product
alongside the petroleum-impacted soils near the former location of monitoring well MW-20.
Upon completion of soil excavation activities, the excavation measured approximately 34 feet
long by 16 feet wide by 6 feet deep. The excavated soil was transported to Greenwood Applied
Services, LLC located in Windsor, North Carolina. Non-hazardous waste manifests and weight
verification tickets for the petroleum impacted soil are included in the Appendix C.
ECS collected a total of ten (10) soil samples along the sidewalls of the soil excavation (SW-1
through SW-10). The locations of the soil samples are shown on Figure 3. The soil samples
were collected at approximately 4 to 5 feet below the ground surface (ft bgs). ECS screened the
soil collected from the excavation sidewalls using a Photo Ionization Detector (PID) to detect
volatile organic vapors. Volatile organic vapors were detected in sidewall sample SW-5 at a
depth of 4 to 5 ft bgs at 91.7 parts per million (ppm). Volatile organic vapors were not detected
in the remaining sidewall samples.
Immediately after collection, the soil samples were transferred into sample containers provided
by the laboratory. The sample containers were labeled with ECS project number, sample
identification, sample date and time, and requested analytical analysis. The containers were
placed into protective packaging material and placed into a cooler with ice to maintain the
samples at approximately 4 Celsius (C). The samples were then delivered to Rapid
Environmental Diagnostics (RedLab) in Wilmington, North Carolina to be analyzed for total
petroleum hydrocarbons (TPH) diesel range organic (DRO) and gasoline range organic (GRO)
using ultraviolet fluorescence (UVF). Chain-of-Custody (COC) procedures were followed
throughout the sampling and transportation of the samples. The Chain-of-Custody Records are
included in Appendix D.
2.2 Monitoring Well Installation Activities
On January 28, 2020, Quantex, on behalf of ECS, installed one Type II replacement
groundwater monitoring well MW-20R near the approximate location of the former abandoned

monitoring well MW-20. Well construction details and groundwater level data is summarized in
Table 2 and the location of the monitoring well is shown on Figure 3.
Monitoring well MW-20R was installed using Direct Push Technology (DPT) equipped with
hollow stem augers. Prior to advancing the boring, the down hole equipment and drill rig were
decontaminated using high pressure steam cleaner and municipal water.
The Type II monitoring well MW-20R was constructed with 2-inch schedule 40 PVC (polyvinyl
chloride plastic) flush threaded screen and casing. The PVC screen and casing was lowered
into the open borehole to a depth of approximately 15 ft. bgs, respectively. A 10-foot length of
0.01-inch slotted well screen with a threaded bottom plug was installed to a depth such that the
screened interval intersected the estimated groundwater surface, as encountered during drilling
activities for monitoring well MW-20R. The solid section of PVC casing was placed above the
screened interval and extended to a point just below the ground surface. The annular space
around the well screens was filled with washed and graded medium sand to approximately one-
foot above the top of the well screen. A minimum one-foot thick seal of bentonite pellets was
paced above the sand pack and allowed to hydrate before backfilling the remaining borehole
annulus with a bentonite/neat cement mixture. The top of the well casings were equipped with a
lockable, water tight well plug and a flush mounted protective steel cover. The NCDEQ Well
Construction Record is included in Appendix E.
Following completion of the well installation on January 28, 2020, approximately 2.7 feet (32.4
inches) of free product was measured in monitoring well MW-20R; therefore, ECS did not
collect a groundwater sample.
3.0 LABORATORY ANALYTICAL RESULTS
3.1 Soil Sampling Analytical Results
Laboratory analysis detected TPH-DRO (261 milligrams per kilograms or mg/kg) and TPH-GRO
(50.3 mg/kg) at concentrations above their respective North Carolina Department of
Environmental Quality (NCDEQ) Action Levels of 100 and 50 mg/kg, respectively in soil sample
SW-5.
Additionally, laboratory analysis detected TPH-DRO and/or TPH-GRO at concentrations above
the laboratory reporting limit but below their respective NCDEQ Action Levels in soil samples SW-
1 through SW-4 and SW-6 through SW-10.
A summary of the soil laboratory analytical results are included in Table 3. A copy of the COC and
laboratory results is included in Appendix D.
4.0 CONCLUSIONS
Based on the results of this Environmental Services Report, ECS concludes the following:
On January 6 and 7, 2020, ECS observed the removal of 171.6 tons of petroleum-impacted
soils by EVO. Soil excavation activities were terminated based on encountering apparent
clean soil to the south and west of the excavation, the onsite building/subsurface utilities to

the north and east of the excavation, or the apparent water table at approximately 6 ft. bgs.
Additional soils were excavated a further 2 to 3 feet into the water table in an effort to
remove any free product alongside the petroleum-impacted soils near the former location of
monitoring well MW-20. Upon completion of soil excavation activities, the excavation
measured approximately 34 feet long by 16 feet wide by 6 feet deep. ECS was unable to
remove any additional soils towards the northeastern extent of the excavation (SW-5) due to
encountering a marked subsurface water service line in addition to the onsite building.
Laboratory analysis detected TPH-DRO and TPH-GRO at concentrations above their
respective NCDEQ Action Levels in soil sample SW-5. Additionally, laboratory analysis
detected TPH-DRO and/or TPH-GRO at concentrations above the laboratory reporting limit
but below their respective NCDEQ Action Levels in soil samples SW-1 through SW-4 and
SW-6 through SW-10.
Based on the results and field observations of the soil samples collected, ECS concludes that
the majority of petroleum-impacted soils that potentially may have been contributing to the
continual presence of free product in the groundwater has been successfully removed and
replaced with clean fill; however, ECS believes there is likely additional petroleum-impacted
soils beneath the subsurface utilities and within the NCDOT Right-of-Way towards the
northeastern extent of the excavation as evidenced by the analytical results of soil sample
SW-5.
Upon completion of the monitoring well installation on January 28, 2020, ECS measured
approximately 2.7 feet (32.4 inches) of free product in the newly installed monitoring well
MW-20R; therefore, no groundwater sample was collected from the replacement well.
5.0 RECOMMENDATIONS
Based on the results and conclusions of this Initial Corrective Action Performance Report, ECS
recommends conducting an Aggressive Fluid Vapor Recovery (AFVR) event on monitoring well
MW-20R to remove existing free product and subsequent sampling and/or free product check
approximately two to three weeks following to determine its efficiency. Additionally, if funds are
available, ECS recommends having a discussion with NCDEQ to determine if conducting an
injection in the on-site monitoring well could help stimulate the degradation of free product and
dissolved-phase petroleum-related compounds at the site.
