Dear Ms. Blalock:
ATC Associates of North Carolina, P.C. (ATC) is pleased to submit the results of an updated receptor
survey and a groundwater and water supply well sampling event conducted on December 20, 2014, at
the J.C. Woody site located at 295 Ashley Avenue in Timberlake, North Carolina. A site topographic
map is included as Figure 1. Currently on-site is a single-story building formerly used as a convenience
store and retail gasoline facility. It is currently utilized as a small engine sales and repair shop. A site
map is included as Figure 2. The sampling event was performed based on a November 13, 2014,
Task Authorization (TA-1) by the Trust Fund Branch of the Underground Storage Tank (UST)
Section, Division of Waste Management, North Carolina Department of Environment and Natural
Resources (NCDENR). The work is being performed as part of the State Lead Contract #N14004i
between NCDENR and ATC.
As authorized in Task Authorization TA-1, ATC performed an updated water supply well survey in the
surrounding vicinity of the site by conducting a visual reconnaissance. The survey was performed by
driving and/or walking throughout the surrounding area and visually assessing the individual parcels of
land for evidence of water supply wells, well houses, and/or water meters. In addition, ATC interviewed
property owners when available. The water supply well survey was performed within a radius of 1,000
feet from the source area. Following visual reconnaissance, ATC mailed water supply well survey
letters to owners of properties that either showed evidence of a water supply well or were located within
500 feet of the site. ATC also contacted Ms. Pam Daye with the City of Roxboro Utilities Department
to confirm whether or not suspect properties were connected to a municipal water supply. Water supply
well information is included as Table 1 and potential receptors are presented in Figure 3. Based on
ATC’s findings and historical reports, it appears 32 water supply wells are located within 1,000 feet of
the site, eight of which are used for drinking purposes, eight which are used for irrigation, twelve which
are inactive, and four with an unknown use. Water supply well information forms are included as
Appendix A.

According to the site topographic map, the nearest surface water body is located approximately 325
feet south southwest of the site. According to the NCDENR Division of Water Resources, it appears
the surface water body is an unnamed pond and has no classification.
On December 20, 2014, ATC mobilized to the site to conduct monitoring well sampling and water
supply well sampling. ATC collected samples from monitoring well MW-1 and MW-2 and water
supply well samples from water supply wells WSW-1A and WSW-6. ATC had costs approved to
sample up to four water supply wells; however, ATC only gained access from owners at WSW-1A
and WSW-6. Field notes from the December 20, 2014, monitoring event are included in Appendix B.
Water supply well information is included in Table 1 and water supply well locations are shown on
Figure 3. Current site photographs are included in Appendix C.
The monitoring well and water supply well samples were collected in laboratory-supplied, preserved
containers; entered into chain of custody control; placed on ice; and shipped to North Carolina-
accredited Con-Test Analytical Laboratory in East Longmeadow, Massachusetts. The samples were
analyzed by EPA Method 6200B for volatile organic compounds. The analytical report is included in
Appendix D. The analytical results were compared to the North Carolina Title 15A, NCAC,
Subchapter 2L, Part .0202 (2L Standards) for groundwater and Gross Contamination Levels (GCLs).
Based on the results, no compounds were present above the laboratory detection limits in the
monitoring wells or the potable wells. Well construction details and groundwater elevation data for
monitoring wells MW-1 and MW-2 are included as Table 2. Monitoring well analytical data is
included as Table 3. Water supply well analytical results are included as Table 4.
As part of ATC’s quality assurance and quality control (QA and QC) measures, a data review of the
laboratory reports was conducted. During the review, it was found that the samples were received
from the laboratory in acceptable condition and properly preserved. The samples were analyzed
within correct holding times and reported within the lab report, and the chain of custody was properly
completed by all parties. The laboratory QC report shows that an internal blank was analyzed for each
method that was used for the original samples. The laboratory fortified blank (LFB) and laboratory
control sample (LCS) data results were also provided for each analytical method. Refer to the
laboratory reports in Appendix C for details regarding QA results for the laboratory samples and any
noted exceptions. Overall, ATC concludes that the laboratory results are usable.
Based on historical groundwater monitoring data, C5-C8 aliphatics and C9-C22 aromatics were
detected above 2L Standards during the November 2004 sampling event. Therefore, ATC
recommends sampling the two monitoring wells for volatile petroleum hydrocarbons (VPH) by the
Massachusetts Department of Environment Protection (MADEP) method. Soil analytical results
reported in the January 10, 2005, Phase I Limited Site Assessment, did not indicate petroleum
compounds above laboratory detection limits. Therefore, if MADEP VPH values are detected below
2L Standard in MW-1 and MW-2, then site closure activities can commence.
