SECTION B. EXECUTIVE SUMMARY
1. Summary of Activities Associated with Current TA
WithersRavenel, Inc. (WR) completed a groundwater monitoring event at the JR’s Country Store located at 5492 Crabtree Road in Clyde, Haywood County, North Carolina in association with TA-
01 under North Caroline Department of Environmental Quality (NCDEQ) Contract # N70522-C. 2. Soil, Groundwater and LNAPL Assessment Information
Soils No soil samples were collected by WR in association with this task authorization. Laboratory analysis of the soil samples collected from beneath the underground storage tanks (USTs) and the pump island detected Total Petroleum Hydrocarbons – Gasoline Range Organics (TPH-GRO) concentrations ranging from 25.7 milligrams per kilogram (mg/kg) to 749 mg/kg. Approximately
147 tons of petroleum contaminated soil was excavated from beneath the USTs. Post-excavation sampling detected TPH-GRO concentrations ranging from 1.1 mg/kg to 536 mg/kg at the base of the excavation. WR is not aware of the presence of any risk-based soil data for the incident.
Groundwater WR collected groundwater samples from site monitoring wells MW-1, MW-2R, MW-5, MW-8, and MW-9 on June 29, 2022. WR was also granted permission to collect samples from water supply wells WSW-1B, WSW-2, and WSW-6. WR was unable to collect groundwater samples
from WSW-3 and WSW-7. The collected groundwater samples were submitted for laboratory analysis of volatile organic compounds (VOCs) by SM6200B. Laboratory analytical results revealed the following exceedances to the NCDEQ 15A NCAC 2L Groundwater Quality Standards (2L Standards):
• MW-1: Methyl tert-butyl ether (MTBE);
• MW-8: Di-Isopropyl Ether and MTBE.
No other compounds within the sampled monitoring wells were reported at concentrations that exceeded regulatory standards established by NCDEQ. The water supply well samples from wells WSW-2 and WSW-6 contained concentrations of one
or more of the following compounds: di-isopropyl ether, and/or MTBE at concentrations below the established 2L Standards. LNAPL
Measurable thickness of Light Non-Aqueous Phase Liquid (LNAPL) was not encountered in March
2023. WR is not aware of the historical presence of LNAPL in any of the site monitoring wells.

3. Receptor Information Fourteen (14) water supply wells and one spring have been identified within 1,000 feet of the site. Five of these water supply wells are located within 500 feet of the site (WSW-1B, WSW-2, WSW-
3, WSW-6, and WSW-7). Municipal water is reportedly unavailable to the site or surrounding properties. Well head protection areas were not identified within 1,500-ft of the subject property. 4. Remediation Activities
Based on WR’s review of historical files available for the site, the following remedial activities have been completed at the location:
• Approximately 147 tons of petroleum contaminated soil was excavated from beneath the
USTs during closure activities in January / February 1994. The excavation was reportedly advanced to a depth of 18 feet below ground surface (BGS). Based on currently available information, WR is unaware of additional remedial actions that have
been performed at the site.
5. Recommendations Based on the results of assessment and monitoring activities conducted at the site in association
with this TA, WR recommends the following:
• Continued semi-annual monitoring of site monitoring wells and water supply wells within 500 feet of the area of release.
• If funding is available, conduct a targeted injection of BOS200 near and downgradient of MW-8.

SECTION C. SITE HISTORY AND CHARACTERIZATION WR has completed a groundwater sampling event for the referenced site in association with TA-01 at the facility in March 2023. The following information summarizes the site history and
characterization. 1. UST Information The subject property is located at the 5492 Crabtree Road in Clyde, Haywood County, North
Carolina (Figures 1 and 2). According to the Haywood County GIS website, the subject property is owned by Mr. Roger Fricks. The site is currently occupied by F&F Trucking, a transport company. In January 1994, one 1,000-gallon gasoline UST and one 550-gallon gasoline UST were removed from the site. Available UST information is summarized on Table B-1. Laboratory analysis of the soil samples collected beneath the USTs and the pump island detected TPH-GRO concentrations
ranging from 25.7 mg/kg to 749 mg/kg. Approximately 147 tons of petroleum contaminated soil was excavated from beneath the USTs to a depth of 18 feet BGS. Post-excavation sampling detected TPH-GRO concentrations ranging from 1.1 mg/kg to 536 mg/kg at the floor of the excavation.
2. Description of Release/General Assessment History The following summary of the assessment history is based on WR’s review of historical files for the subject incident:
• January 1994 – July 1996 – The USTs and approximately 147 tons of petroleum impacted soils were removed from the site during January / February 1994. Subsequently, seven monitoring wells (MW-1 through MW-7) were installed on the subject property and
periodically monitored.
• November 1996 – A Comprehensive Site Assessment (CSA) was prepared.
• January 1997 through January 2021 – Groundwater sampling of site monitoring wells and some water supply wells occurred. Monitoring wells MW-6 and MW-7 have not been located
since 1996 and are presumed destroyed. At some point, on-site supply well WSW-1A was abandoned and replaced with WSW-1B.
• July 2013 – A receptor survey was completed by Crawford Environmental Services (CES).
Twenty-six (26) water supply wells and one spring were identified within 1,500 feet of the site.
Six of these water supply wells were located within 500 feet of the site (WSW-1B, WSW-2, WSW-3, WSW-6, WSW-7, and WSW-15).
• August 2015 – Monitoring well MW-8 was installed.
• January 2021 – Groundwater monitoring event identified 2L exceedances in MW-1, MW-5, and MW-8. Water supply well WSW-2 was also sampled and contained concentrations of di-isopropyl ether and MTBE at concentrations below 2L Standards.
• April 2022 – WR installed monitoring well MW-9 and replacement MW-2R at the site and
conducted a receptor survey update for the surrounding area. Groundwater sampling of select monitoring wells and water supply wells identified the MTBE, di-isopropyl ether, and/or naphthalene at concentrations exceeding 2L Standards in wells MW-1, MW-5, and MW-8.

Water supply wells WSW-2, WSW-3, and WSW-6 contained benzene, di-isopropyl ether, and/or MTBE at concentrations exceeding method detection limits but below 2L Standards.
• June 2022 – Groundwater monitoring event identified 2L exceedances in MW-1, MW-5, and
MW-8. Water supply wells WSW-2 and WSW-6 were also sampled and contained concentrations of one or more of the following compounds: di-isopropyl ether and/or MTBE at concentrations below the established 2L Standards.
3. Brief Description of Site Characteristics
The subject property is located at the 5492 Crabtree Road in Clyde, Haywood County, North Carolina (Figures 1 and 2). According to the Haywood County GIS website, the subject property is owned by Mr. Roger Fricks. The site is currently occupied by F&F Trucking, a transport company.
The subject property is located in a rural, mostly residential area.
4. Information on Receptors/Potential Receptors According to the most recent receptor survey update conducted in January 2022, fourteen (14)
water supply wells and one spring were identified to be within 1,000 feet of the site. Five of these
water supply wells are located within 500 feet of the site (WSW-1B, WSW-2, WSW-3, WSW-6, and WSW-7). According to Mr. Richard Hodge with the Town of Canton, The Town of Canton’s water line terminates near exit 31 on interstate 40 and there are no municipal water lines between exit 31 and exit 24, the Crabtree Road exit. Well head protection areas were not identified within
1,500-ft of the subject property.
The estimated locations of these water supply wells are shown on Figure 5. Information regarding the water supply sources is provided on Table B-5A.
5. Regional and Site Geology and Hydrology
As determined from the USGS Topographic Map, Clyde, North Carolina Quadrangle dated 2019
(Figure 1), the site is located at an approximate elevation of 2,550 feet above mean sea level. An
unnamed tributary to Crabtree Creek, is located approximately 1,000 feet east and south of site.
Surface runoff on the site generally flows toward the southeast.
According to the Geologic Map of North Carolina, the site is located in the Blue Ridge Belt of the
Blue Ridge Physiographic Province. The site bedrock consists of interlayered biotite-garnet gneiss
and amphibolite. The soils encountered in this area are the residual product of in-place chemical
weathering of rock presently underlying the site and/or historic depositional events. According to
the USDA Soil Survey Map, the soils at the site consists of Hayesville clay loam (HaC2). HaC2 is
found on 8 to 15 percent slopes and consists of clay loams that originated from weathered gneiss
and amphibolite. During the installation of MW-2R and MW-9, soils consisted of varying amounts
of sand and silts to depths of approximately 45 feet BGS.
Surficial unconfined groundwater movement within the overlying soils is controlled largely by
topographic gradients. However, as the groundwater percolates downward, it becomes influenced

by the subsurface geologic conditions. Therefore, the direction of groundwater movement in the
deeper aquifers may not be consistent with the surface topography. Recharge occurs primarily by
infiltration along higher elevations and typically discharges into streams or other surface water
bodies as the water flows from higher topographic elevations to lower topographic elevations.

SECTION D. GROUNDWATER SAMPLING – MARCH 2023 On March 29, 2023, WR personnel performed groundwater sampling activities at the site. The scope of work for this project included the gauging, purging, and sampling of existing monitoring
wells MW-1, MW-2R, MW-5, MW-8, and MW-9. WR was also tasked with collecting water supply wells samples from WSW-1B, WSW-2, WSW-3, WSW-6, and WSW-7. In the performance of assessment activities at the site, WR was able to collect groundwater samples from MW-1, MW-2R, MW-3, MW-4, MW-5, MW-8, and MW-9. WR also was granted
signed permission to collect a sample from WSW-1B, WSW-2, and WSW-6. The following provides information as to why wells WSW-3 and WSW-7 were not sampled during the March 2023 event:
• WSW-3 – Vacant; building under construction; and
• WSW-7 – The homeowner was not home to grant permission. The following paragraphs describe the sampling activities and associated laboratory analytical
results. Groundwater Sampling Activities Prior to the purging and sampling of the site monitoring wells on March 29, 2023, WR personnel opened and gauged those wells targeted for purging and sampling with a decontaminated
electronic water level indicator to determine the depth to groundwater at each well location. In
the performance of these activities along with the purging process, LNAPL was not measured in any of the monitoring wells gauged. The depth to groundwater measurements in the monitoring wells ranged from 31.69 feet (ft) below the top of casing (ft-BTOC) in monitoring well MW-9 to 36.55 ft-BTOC in monitoring well
MW-8. Based on data obtained during this sampling event, groundwater flow appears to trend to the east-southeast. This groundwater flow appears generally consistent with past assertions. A groundwater hydraulic gradient map based on the March 2023 measurements are shown on Figure 3. Groundwater elevation data are included in Table B-7.
Following gauging, monitoring wells were purged and sampled with separate, disposable polyethylene bailers for each well. Samples from the wells along with a field blank (FB-1) were decanted into pre-preserved laboratory provided sample containers. Samples collected from site monitoring wells were submitted to Waypoint Analytical, Inc. (Waypoint) for testing by Standard Method (SM) 6200B plus oxygenates.
Laboratory Results Analytical results for the groundwater samples collected on March 29, 2023 revealed the following exceedances to the 2L Standard.
• MW-1: MTBE (26 µg/L); and
• MW-8: Di-Isopropyl Ether (190 µg/L) and MTBE (862 µg/L)

Multiple petroleum constituents were reported in monitoring wells MW-1, MW-2R, and MW-5 at concentrations exceeding method detection limits (MDLs) but below 2L Standards. No other compounds within the sampled monitoring wells were reported at concentrations that exceeded regulatory standards established by NCDEQ.
The water supply well samples from well WSW-2 contained concentrations of di-isopropyl ether and MTBE and WSW-6 contained MTBE at concentrations below the established 2L Standards. The following table compares Total VOCs results from March 2023 to concentrations reported in
June 2022:

Total VOC concentrations in the on-site monitoring wells show general decreases since the June
2022 monitoring event. Results from water supply wells WSW-2 and WSW-6 show a decrease of 15% and 18%, respectively, when comparing total VOCs from March 2023 to results from June 2022.
Laboratory analytical results are shown on Figure 4 and included in Table B-4. Copies of the laboratory analytical report and chain-of-custody are included in Appendix A. Field notes and a photographic record are included as Appendix B. Standard Operating Procedures are included in Appendix C.
SECTION E. CONCLUSIONS AND RECOMMENDATIONS This report documents the results of groundwater sampling activities conducted at the site for TA-
01. Conclusions based on observations, analytical data and research relating to the release at the
site combined with historical information include the following:
• The site and all surrounding properties are provided potable water through private water
supply wells.
• No municipal water is supplied to the local area.
• WR is unaware of any risk-based soil samples from the former UST area.
• MTBE was reported at concentrations exceeding the 2L Standard in monitoring wells MW-1 and MW-8.
• Di-isopropyl ether was reported at a concentration exceeding 2L Standards in monitoring well
MW-8.

• The water supply well samples from WSW-2 and WSW-6 contained concentrations of di-isopropyl ether and/or MTBE at concentrations below established 2L Standards.
• No compounds were reported at concentrations that exceed their respective Gross Contamination Levels (GCLs).
• Total VOC changes over-time show significant reductions in wells MW-1, MW-5, and MW-8.
• Groundwater appears to be naturally attenuating at the site.
• Results from water supply wells WSW-2 and WSW-6 show a decrease of 15% and 18%, respectively, since June 2022.
• The concentrations in water supply well WSW-2 has remained approximately the same since January 2018 and MTBE in WSW-6 has remained at a similar concentration since it was first sampled by WR in January 2022.
• LNAPL was not observed in March 2023.
Based on the results of assessment and monitoring activities conducted at the site in association with this TA, WR recommends the following:
• Continued semi-annual monitoring of site monitoring wells and water supply wells within 500 feet of the area of release.
• If funding is available, conduct a targeted injection of BOS200 near and downgradient of MW-
8.

Soil Sampling, Soil Boring and Monitor Well Installation, Well Development Soil borings are installed utilizing a stainless-steel hand auger, direct-push hydraulic drill or auger rotary
drill using either a macro core™ sampler, hollow stem augers, or air rotary roller cone bit. Soil borings are
typically sampled at discrete intervals from the bucket of a clean stainless steel hand auger, direct push stainless steel sampler with acetate sleeve, or a split barrel sampler. When appropriate, soil samples may be collected from drill cuttings associated with the boring or well installation. Monitor wells are constructed within select soil borings of one, two, or four-inch inside diameter PVC
casing and machine slotted screen in accordance with NCAC Title 15A, Subchapter 02C (Well Construction Standards, Criteria and Standards Applicable to Water Supply and Certain Other Wells). Clean-washed “#2 Torpedo Sand” is typically utilized to fill the boring annulus surrounding the well screen, extending to a minimum of 1-foot above the well screen. A minimum 1-foot thick bentonite clay seal is emplaced above the sand pack and hydrated. Neat cement grout is placed in the remaining annulus
space from the upper portion of the bentonite seal to within 1-foot of surface grade. In coastal plain areas where shallow groundwater is encountered, a variance to construction standards may be employed to ensure the upper portion of the well screen intersects the upper aquifer surface. Wellheads may be completed with either concrete-set flush mounted traffic-bearing steel skirted manholes with bolt down covers or steel above-ground “stick-up” well protectors set in a concrete pad. The monitoring wells are
secured with locking caps. Temporary wells or piezometers may be completed with the PVC riser cut at or above grade, and are abandoned within 24-hours of their installation. Wells may be developed using a bailer, electric submersible pump, pneumatic pump, or peristaltic pump, dependent upon site conditions. When wells are developed by the driller, they may be developed with
any of the methods outlined above, in addition to air or water injection. Soil boring logs and well construction details are typically prepared for presentation in a report appendix. Borings completed in bedrock may be drilled using air rotary, coring, or sonic drilling methods. Groundwater Depth to Water Measurements
Groundwater level measurements are typically obtained using an electronic water level indicator which registers a tone and/or illuminated light when the probe contacts groundwater. Depth to groundwater measurements are obtained by lowering the probe attached to a cable usually marked in 0.01 foot intervals into the well until the probe of the meter intersects the water surface. The measurement from
the top of the riser pipe or top of casing (TOC) to the water level is recorded to the nearest 0.01 foot. The
electronic water level indicator probe and exposed probe cable is decontaminated between measurement locations.

Free Product Level Measurements Light Non-Aqueous Phase Liquid (LNAPL) or “free product” present on the water table is measured using
an electronic oil/water interface probe. The interface probe has both an audible and illuminating light
that indicate when a liquid level is encountered. The oil/water interface probe registers a solid tone and solidly lit light when LNAPL petroleum product is encountered and a beeping tone flashing light when groundwater is encountered. Depth to LNAPL measurements are obtained by lowering the probe attached to a cable marked in 0.01 foot intervals into the well until the probe of the meter intersects the product or groundwater. The measurements from the top of the riser pipe or TOC to the liquid level are
recorded to the nearest 0.01 foot. The thickness of LNAPL is calculated by subtracting the depth to LNAPL measurement from the depth to groundwater measurement. The electronic interface probe is decontaminated between measurement locations. Soil Sampling and Field Screening
Soil sampling is performed in general accordance with ASTM: 1586-84 where applicable. Sampling procedures are followed to protect the chemical integrity of the soil samples and minimize the potential for cross contamination between sample locations, including discarding of the exterior of the sample which contacts the auger bucket, acetate sleeve, or split barrel sampler, the use of stainless steel sampling
equipment, and the thorough decontamination of sampling apparatus prior to use and between sample locations. Soil samples retained as a part of excavation or other intrusive “digging” activities may be collected from the bucket of excavation equipment or using a hand shovel, if appropriate. Care to collect soil samples
from excavation equipment includes retrieving native soil from material that has not made contact with the excavating equipment. Samples are collected in sealable bags or by directly filling sample containers as described above. Sampling is performed using new, disposable nitrile gloves for each sample location to ensure sample
integrity and minimize the potential for cross contamination or field personnel exposure to contaminants.
Sampling equipment such as hand auger extensions and buckets, direct push samplers, split spoons or shovels, are decontaminated prior to their use in the collection of samples. Samples are placed within laboratory grade sample containers provided by the laboratory to which the
samples are submitted, labeled, sealed, and placed in an ice filled cooler. An aliquot of the sample is
typically placed in a clean sealable bag and allowed to reach ambient air temperature for future screening of volatile vapor content with a photoionization detector (PID), flame-ionization detector (FID) or other portable screening device.
Screening of samples with a PID, FID or other field screening device is performed in accordance with
device manufacturer specifications for screening, following calibration of the instrument based on manufacturer recommended procedures.

Groundwater Sampling Groundwater samples are generally collected using one of three standard methods – through the use of a
dedicated, disposable bailer, a peristaltic pump or submersible electric pump.
When sampling with a bailer, a minimum of three well volumes of groundwater are extracted from the well as a means of purging, unless the well is purged to a “dry” state prior to the removal of three volumes of water. Prior to collection of the groundwater sample with the bailer, the water level is allowed to recover to within 80% of the original level measured prior to purging.
When sampling with a peristaltic or submersible electric pump, polyethylene (PE) tubing (or equivalent) is emplaced into the screened section of the monitoring well, and the pump is used to extract water through the tubing. When using the either of these methods, general water quality measurements including but not limited to pH, temperature, dissolved oxygen (DO), oxygen reductive potential (ORP), specific
conductivity and turbidity may be used as methods for determining when an adequate purge of the well has been conducted. When water quality parameters are measured, the resulting data are included in well purge forms or field notes specific to each site and sampling point. Following the purging of three well volumes of groundwater (in the case of the bailed well) or stability of
monitored measurements (in the case of the peristaltic or submersible electric pump), the groundwater sample(s) are introduced directly into sample containers provided by the laboratory to which samples are ultimately submitted for analysis. Any samples requiring chemical preservation are contained in pre-preserved container(s) provided by the laboratory.
Sampling is performed using new, disposable nitrile gloves for each sample location to ensure sample integrity and minimize the potential for cross contamination of samples or field personnel exposure to contaminants. Water Supply Well Sampling
Sampling of water supply wells is accomplished through the collection of a representative sample from an in-line water tap or other point accessible for sampling. Where possible, the sample is collected from a point prior to any filtration or point of entry (POE) system which might be installed on a well. In some cases, samples will be collected from both pre and post-POE locations. Prior to the collection of samples
through an in-place sampling point, the tap or sampling port is cleaned with an Alconox® and deionized
water solution, rinsed with deionized water, and again with an isopropyl alcohol (IPA) rinse. In the case of active supply wells (e.g., a well with a useable well pump with an active power supply), the well pump is energized and allowed to run for a minimum of ten (10) minutes time, after which a sample is
contained in laboratory grade sample containers as described in the previous section. Water quality
parameters may be (but are not necessarily) measured in association with the well purging. Supply wells without active power supplies are not generally sampled, as the well seal cannot be compromised by individuals not certified as a well contractor. However, in the case of a well with no
power supply or pump installed, samples could be obtained through the use of a bailer, peristaltic pump or
electric submersible pump.

Decontamination Procedures Decontamination procedures employed in the performance of field activities will vary dependent upon
the method(s) of sampling performed at any site, but will generally include the following:
• Remove excess soil/sludge from any sampling equipment as needed, • Wash and scrub the equipment with a phosphate-free detergent such as Alconox® or Liquinox® in a contaminant-free tap water solution. Use a brush to remove particulate matter and surface films as necessary,
• Rinse thoroughly with tap water, • Rinse thoroughly with analyte-free water (deionized water),
Rinse thoroughly with isopropyl alcohol, • Allow the equipment to air dry,
• Prevent inadvertent contamination during storage and transport (accomplished through storage of equipment in foil or plastic bags/sheeting). In the case of disposable bailers and tubing utilized for the purging and sampling of wells, these items may be decontaminated prior to their disposal. In general, no decontamination of these items is necessary
prior to their use in sampling.
