This report has been prepared in general accordance with North Carolina Department of Environmental Quality, Underground Storage Tank (UST) Section Assessment Guidelines and Corrective Action Guidelines for Petroleum and Hazardous Substance UST Releases / Petroleum Non-UST Releases,dated January 19, 2021, and Task Authorization No. 03 approved on October 1, 2021; change order No. 1 authorized on November, 4 2021; change order No. 2 authorized on December 22, 2021; and change order No. 3 authorized on September 23, 2022. The Plain View Grocery site is located at 5515 Plain View Highway in Dunn, Sampson County, North Carolina and is comprised of 0.74 acres (site) (Figure 1). The site currently contains two 1,000-gallon gasoline USTs, two associated dispensers, and an approximate 500-gallon aboveground storage tank (AST) located adjacent to the northeast corner of the Plain View Grocery structure. Adjacent properties consisted of grassy vacant land, residential properties, and The Country Manor mobile home park. The USTs and AST are not currently in use. Following detections of petroleum compounds in the water supply well on the adjacent Plain View Elementary School property, four 1,000-gallon gasoline USTs were removed from the site in February 1990. Soil and groundwater samples collected on-site between April 1990 and June 2013 contained concentrations of petroleum compounds above their respective soil to groundwater maximum soil contaminant concentrations (MSCCs) and numerous petroleum constituents were detected in groundwater samples above the respective 15A NCAC 2L Groundwater Quality Plain View Grocery Dunn, North Carolina Terracon Project No. 70149610B Incident No. 3646 December 9, 2022 Standards (2L standards). Subsequent groundwater sampling was conducted by Terracon starting in 2014 and continuing through 2022. In November 2020 Terracon began the process to connect the adjacent mobile home park to the Sampson County municipal water system. Dewberry Engineers, Inc. (Dewberry) was contracted to design the water system. In March 2021 authorization to construct a Public Water System was received and a subcontractor (Burnette Utilities) was selected to construct the water line. Construction was set to begin in October 2021, however, due to supply chain issues and the inability for Burnette Utilities to acquire materials, the construction was delayed to the following year. In September 2022, construction of the water system began, and Burnette Utilities installed a water main tap under the supervision of Dewberry. Following the completion of the water main installation the connection from the tap to thirteen mobile home units, and disconnection from the onsite water supply wells, was completed on November 28, 2022, by Gary Willis Plumbing.
