Dear Mr. Osman:

ATC Associates of North Carolina, P.C. (ATC) submits this Well Abandonment Letter Report for the Robert
Brooks Property site located at 312 Martin Luther King Drive in Elizabethtown, Bladen County, North Carolina.
The work was performed in accordance with the December 7, 2022, Task Authorization (TA-02) authorized by
the Trust Fund Branch of the Underground Storage Tank (UST) Section, Division of Waste Management North
Carolina Department of Environmental Quality (NCDEQ). The work was performed as part of the State Lead
Contract #N122019A between the NCDEQ and ATC.

The site is currently developed with a vacant and dilapidated single-story commercial building. The site
formerly operated as a retail petroleum facility; however, details regarding operation dates and owners are
unknown. One 1,000-gallon UST of unknown contents was installed at an unknown date but was removed
from the site in June 2022. A site topographic map is included as Figure 1 and a site map depicting pertinent
site features is included as Figure 2. Two private water supply wells (WSW-1 and WSW-2) and one public
water supply well (Well #2) are located within 1,500 feet of the site; however, the wells are located beyond
1,000 feet from the source area on the site property. ATC submitted a Limited Site Assessment Report on
December 7, 2022. Based on the results of this report, ATC recommended filing a Notice of Residual
petroleum for the site restricting groundwater use, followed by site closure.

On January 28, 2023, Regional Probing Services, an NC-certified well contractor (Certification Number 3322)
abandoned monitoring well MW-1, the only well associated with the site, as part of site closure activities in
accordance with North Carolina statue 15A NCAC 02C .0113. Bentonite chips were placed into the well to
above the measured water table, and the remainder of the well casing was filled with grout. A summary of the
monitoring well information is included in Table 1. A copy of the well abandonment record is provided in
Appendix A and photographs of the pre- and post-abandonment well conditions are provided in Appendix B.
