purchaser of the property from the responsible party, the Cudia family. It was discovered
that this release had not been reported during a refinancing of the property by the
current property owner and non -responsible party.

One UST, a 500-gallon capacity heating oil UST, was removed during closure activities on February 17, 2021. Post
excavation soil sampling indicated that soil concentrations above the Action Limit for TPH-DRO (1,120 mg/kg) were
detected in the base of the UST excavation. No water supply wells were identified within 150 feet of the source area,
and groundwater was not encountered within the extent of the excavation. Excavation ceased at five feet below land

1.0 SITE HISTORY AND CHARACTERIZATION
1.1 Introduction
The subject site is the location of a single-family residential home. The subject site is
located at 2114 Salome Church Road, Charlotte, Mecklenburg County, North Carolina
(Drawing 1). During field activities on February 17, 2021, HEPACO employees
uncovered and removed a 500-gallon capacity heating oil underground storage tank
(UST). The location of the former UST is shown on Drawing 2.
This report presents background information; a summary of the UST removal and closure
assessment procedures; documentation of the over -excavation of contaminated soil; the
laboratory analytical results; and our findings, conclusions and recommendations. The
opinions included herein are based on our experience and the information obtained during
the study. This report is based on limited observations made on the dates noted using the
procedures described herein. If additional information becomes available, we request the
opportunity to review the information, reassess the potential environmental concerns and
modify our conclusions and recommendations, if appropriate.

One 500-gallon capacity heating oil UST was used at the subject site to provide fuel for
the associated former residence. See Drawing 2 for the former location of the tank and
Table 1 for UST Information.
1.7 Site Characteristics
The subject site is the location of a single-family residential home. The area surrounding
the subject site is residential. Surface drainage is generally to the east. An unnamed
tributary of Mallard Creek runs approximately 1000 feet to the southeast of the subject site.
According to the North Carolina Department of Environment and Natural Resources
(NCDENR), Division of Water Resources (DWR) webpage, Mallard Creek is a Class C
waterbody. During UST closure excavation activities, stained soils exhibiting a petroleum
odor were encountered in the subsurface adjacent to the UST system. Groundwater was
not encountered during excavation activities.
1.8 Summary of Activities Performed To Date
The subject parcel has no known previous releases. The subject UST was excavated on
February 17, 2021.

2.0 UST CLOSURE PROCEDURES
2.1 Pre -Closure Procedures
Prior to removal, HEPACO personnel ensured the tank was empty and free of potentially
hazardous vapors.
2.2 Excavation Procedures
The top of the UST was located approximately 2 feet below land surface (BLS). Soil was
initially removed from the top and three sides of the UST so that it could be removed from
the excavation. Upon removal of the tank, heavy corrosion and pitting were noted along
the bottom of the UST. HEPACO removed the UST from the excavation, loaded it onto a
trailer and transported the UST to Zero Waste of Charlotte, North Carolina. A UST
Disposal certificate is included in Appendix A. A photographic log documenting
excavation activity on site is provided in Appendix B.
2.3 Excavated Soil
Tank overburden soil removed from the excavation was screened for evidence of
volatile organic vapors with a Flame Ionization/Photo Ionization Detector combination
unit (FID/PID). During removal activities, the FID/PID was used to determine the
presence of potentially contaminated soils. No potentially contaminated soils were
encountered while removing the first three feet of overburden soil from the UST. The
FID/PID combination unit recorded field screening values between 35 and 55 parts per
million (ppm) from soils removed in the four to five foot below land surface (BLS) range.
The top of the UST was located approximately one foot BLS. Excavation ceased at 5
feet BLS where rock was encountered. No contaminated soil was removed from the
site during this UST closure.
2.4 Sampling Procedures
Upon UST removal, one closure soil sample UST-1 (5) was collected according to
procedures outlined in the NCDENR's Underground Storage Tank Section Guidelines for
Site Checks, Tank Closure, and Initial Response and Abatement. The closure sample was
collected from the native soil below the former UST at an approximate depth of 5 feet BLS.
The sample was placed into pre -labeled, laboratory -supplied glassware, placed into an
ice -filled cooler and transported under chain -of -custody to Pace Analytical (Pace) in West
Columbia, South Carolina for analysis of total petroleum hydrocarbons gasoline range
organics (TPH-GRO) and diesel range organics (TPH-DRO) by EPA Methods 5035 and
3550, respectively.

The UST closure sample was collected from the track hoe bucket. New nitrile gloves were
worn during the collection and handling of the soil. Each sample was placed into pre -
labeled, laboratory -supplied glassware, placed into an ice -filled cooler and shipped under
chain -of -custody control to Pace for analysis.
2.6 Investigative Results
A laboratory analytical report for the UST closure soil sample is provided in Appendix C
and summarized in Table 2. As summarized, the closure sample UST-1 (5) exhibited
DRO concentrations exceeding the NCDENR UST Section's action level of 100 milligrams
per kilogram (mg/Kg).
3.0 REGULATORY STATUS
UST closure soil sampling indicated that a release had occurred from the UST. The UST
is a non -regulated, non-commercial tank based on the capacity being less than 1,000
gallons and contained heating oil that was oil on the premises where stored. For releases
from non -regulated non-commercial USTs, the responsible party is required to take action
to address any emergency conditions, such as vapor, fire, explosion hazards, or imminent
threats to public health, safety, welfare, or the environment. However, the responsible
party is not required to comply with the initial response and initial abatement action
requirements of 15A NCAC 2N .0702, 30703, and .0705, and 15A NCAC 2L .004(1),
unless specifically directed to by the NCDEQ based upon risk posed by the release.
Post excavation soil sampling indicates that soil concentrations above the Action Limit for
TPH-DRO were detected in the base of the UST excavation. No water supply wells were
identified within 150 feet of the source area, and groundwater was not encountered within
the extents of the excavation. As such, the subject site is considered low risk.

4.0 SUMMARY, CONCLUSIONS AND RECOMMENDATIONS
UST closure and initial abatement activities are summarized as follows:
• One UST, a 500-gallon capacity heating oil UST, was removed during closure
activities;
• No petroleum -contaminated soil was excavated or transported off -site for disposal;
• Based on the laboratory results for the post -excavation confirmatory soil sample,
the base of the former UST area contains soil above NCDEQ standards; and,
• The site is considered low risk according to the NCDEQ Guidelines for Site Checks,
Tank Closure, and Initial Response and Abatement for UST Releases.

All sample aliquots were received at the correct temperature, in the proper containers, with the FP1
appropriate preservatives, and within method specified holding times, unless qualified or notated within
the report. Where applicable, all MDL (LOD) and RDL (LOQ) values reported for environmental samples FTChave been corrected for the dilution factor used in the analysis. All Method and Batch Quality Control
are within established criteria except where addressed in this case narrative, a non-conformance form
or properly qualified within the sample results. By my digital signature below, I affirm to the best of my FSS

Guide to Reading and Understanding Your Laboratory Report
The information below is designed to better explain the various terms used in your report of analytical results from the Laboratory. This is not
intended as a comprehensive explanation, and if you have additional questions please contact your project representative.
Results Disclaimer - Information that may be provided by the customer, and contained within this report, include Permit Limits, Project Name,
Sample ID, Sample Matrix, Sample Preservation, Field Blanks, Field Spikes, Field Duplicates, On -Site Data, Sampling Collection Dates/Times, and
Sampling Location. Results relate to the accuracy of this information provided, and as the samples are received.

Analyte
The name of the particular compound or analysis performed. Some Analyses and Methods will have multiple analytes
reported.
If the sample matrix contains an interfering material, the sample preparation volume or weight values differ from the

standard, or if concentrations of analytes in the sample are higher than the highest limit of concentration that the
laboratory can accurately report, the sample may be diluted for analysis. If a value different than 1 is used in this field, the
result reported has already been corrected for this factor.
These are the target % recovery ranges or % difference value that the laboratory has historically determined as normal

for the method and analyte being reported. Successful QC Sample analysis will target all analytes recovered or
duplicated within these ranges.
Original Sample
The non -spiked sample in the prep batch used to determine the Relative Percent Difference (RPD) from a quality control
sample. The Original Sample may not be included within the reported SDG.
This column provides a letter and/or number designation that corresponds to additional information concerning the result

reported. If a Qualifier is present, a definition per Qualifier is provided within the Glossary and Definitions page and
potentially a discussion of possible implications of the Qualifier in the Case Narrative if applicable.
The actual analytical final result (corrected for any sample specific characteristics) reported for your sample. If there was
no measurable result returned for a specific analyte, the result in this column may state "ND" (Not Detected) or "BDL"

(Below Detectable Levels). The information in the results column should always be accompanied by either an MDL
(Method Detection Limit) or RDL (Reporting Detection Limit) that defines the lowest value that the laboratory could detect
or report for this analyte.

Quality Control
This section of the report includes the results of the laboratory quality control analyses required by procedure or

analytical methods to assist in evaluating the validity of the results reported for your samples. These analyses are not
being performed on your samples typically, but on laboratory generated material.
This is the document created in the field when your samples were initially collected. This is used to verify the time and

date of collection, the person collecting the samples, and the analyses that the laboratory is requested to perform. This

chain of custody also documents all persons (excluding commercial shippers) that have had control or possession of the
