Attached are the public notice receipts for the above incident. This is one of two incidents that I sent the public notice
twice because signatures were not requested the first time. The three letters to the county, city an dhealth department
were sent in the same envelopes as public notice for Incident #40972, which I will be sending after this email. Our
reference for this site is 2837.
Eight letters were sent. Letters 1,2,3,4, 7 and 8 were successfully delivered with signatures being returned. Letter 5 was
delivered successfully the first time (with no signature) but was returned to us the second time. Letter 6 was not
delivered either time.
