Dear Mr. Phillips:
In accordance with your letter of request received June 8, 1983, we are
forwarding herewith Permit No. 7634-R2 dated June 16, 1983, to the N. C. Depart-
ment of Crime Control and Public Safety for the operation of the subject exist-
ing nondischarge type wastewater treatment and disposal facilities.
This permit shall be effective from the date of issuance until December 31,
1984 , shall be subject to the conditions and limitations as specified therein,
and shall void Permit No . 7634-R, issued November 19, 1982.
If any parts, requirements, or limitations contained in this permit are
unacceptable to you, you have the righ t to an adjudicatory hearing before a
hearing officer upon written demand to the Director within thirty (30) days
following receipt of this permit, identifying the specific issues to be
contended. Unless such demand is made, this permit shall be final and
binding.

For the Discharge of Sewage, Industrial Wastes, or Other Wastes
In accordance with the provisions of Article 21 of Chapter 143, General
Statutes of North Carolina as amended, and other applicable Laws, Rules, and
Regulations
PERMISSION IS HEREBY GRANTED TO
N. C. Department of Crime Control and Public Safety
Warren County
FOR THE
operation of 0.80 MGal. nondischarge type wastewater treatment facility
consisting of a 10 GPM influent pump with leachate collection system and
sump, a 52-square feet sandfilter , a 26-square feet activated carbon filter, a
0.80 MGal. holding pond with rainwater diversion pipe and valve, a 50 GPM port-
able effluent irrigation pump, and approximately 4.5 acres of irrigation
area t o serve the Warren County PCB Landfill with no discharge of wastewater
to the surface waters of the State,
pursuant to the letter of request received June 8, 1983, and in conformity
with the project plans, specifications, and other supporting data, subsequently
filed and approved by the Department of Natural Resources and Connnunity
Development and considered a part of this permit.
This permit shall be effective from the date of issuance until December 31,
1984, shall void Permit No. 7634-R issued November 19, 1982, and shall be
subjec t t o the following specif ied conditions and limitations :
1. This permit shall become voidable unless the facilities were constructed
in accordance with the approved plans, specifications and other supporting
data.
2. This permit is effective only with respect to the nature and volume of
wastes described in the application and other supporting data.
3. The facilities shall be properly maintained and operated at all times.

4. The wastewater collected by this system shall be adequately treated in
the Warren County PCB Landfill Wastewater Treatment Facility and analyzed
for PCB concentration prior to being discharged onto the receiving
landfill surface irrigation area.
5. This permit is not transferable.
6. This permit shall become voidable in the event of failure of the landfill
surface soil to adequately absorb the wastes, and may be reacinded unless
the facilities were installed, maintained and operated in a manner which
will protect the assigned water quality standards of the surface waters,
and prevent any contamination of the ground waters which will render them
unsatisfactory for normal use.
7. In event the facilities fail to perform satisfactorily, the Permittee shall
take such immediate corrective action as may be required by this Department.
8. Solids, sludges, or other pollutants removed or resulting from the waste-
water treatment process shall be contained and disposed of in such a manner
as to prevent any contamination of the surface or ground waters of the
State.
9. The Permittee shall employ a certified wastewater treatment plant operator
in responsible charge of the wastewater treatment facilities. Such
operator must hold a certificate of the grade at least equivalent to the
classification assigned to the wastewater treatment facilities by the
Certification Commission.
10. Diversion or bypassing of the untreated wastewater from the treatment
facilities is prohibited.
11. All of the effluent wastewater from the treatment facilities shall be
discharged onto the Warren County PCB Landfill surface irrigation area
and no wastewater shall be discharged to the surface waters of the State.
12 . The sand and carbon filter media shall be disposed of at a suitable and
approved hazardous waste disposal site. All soil in the effluent waste-
water detention pond containing PCB's at concentrations greater than or
equal to 1.0 ppm (by weight) shall also be disposed of in an approved
hazard waste disposal site .
13. Any monitoring deemed necessary by the Division of Environmental Management
to insure surface and groundwater protection will be established and an
acceptable sampling and reporting schedule shall be followed.
14. The issuance of this permit shall not relieve the N. C. Department of
Crime Control and Public Safe ty of the responsibility for damages to
surface or groundwaters resul ting from the operation of this facility.

15. The facilities shall be effectively maLntained and operated as a non-dis-
charge type system to prevent the discharge of any wastewater into the
surface waters of the State.
16. The concentration of PCB in the effluent wastewater to be land applied
by these spray irrigation facilities shall not exceed 1.0 PPB.
17. A suitable year round vegetative cover shall be maintained and sown
prior to spraying.
18. Irrigation shall not be performed during inclement weather or when the
ground is in a condition that will cause runoff such as within a 24-hour
period after a total rainfall event of 0.5 inches or greater within the
previous 24 -hour period or when the wind velocity exceeds 15 mph.
19. The application rate shall not exceed 0.25 inches per acre per week.
20. Freeboard in the storage lagoon shall not be less than 2-feet at any
time.
21. No type of wastewater other than that from the Warren County PCB Landfill
shall be treated by this facilities and spray irrigated onto the irrigation
area.
22. No root or leaf crops for public consumption shall be raised on this site.
23. The spray irrigation will only be conducted on the designated site owned
by the North Carolina Department of Crime Control and Public Safety and
shall not be conducted within 25 -feet of the property lines.
24. The spray irrigation area shall not be grazed nor shall the cover crop
be harvested or removed from the site.
25. Adequate records and an operational log shall be maintained at these
facilities detailing the following information:
a . date of irrigation and date wastewater analyzed last,
b. amount of wastewater irrigated,
c. location and amount of area irrigated, and
d. general weather conditions.
26. Within thirty (30) day s after expiration of this permit or the end of
these facilities operation, a report summarizing the total amount of
wastewater spray irrigated, analysis of the wastewa t e r, analysis of
the irrigation site soil, analysis of the irrigation site cover crop,
and groundwater monitoring wells sample analysis shall be submitted for
review by the Division.

Dear Mr. Helms:
The referenced permit for the operation of a facility for the
treatment of water potentially contaminated with PCB's expires on
July 1, 1983. I am requesting that the permit be extended to
December 31, 1984. There have been no variations in the original
operational procedures.
Permit condition number 12 specifies that in addition to the
disposing of the sand and carbon filter media, the top three inches
of soil in the effluent wastewater retention pond must be removed
to an appropriate disposal facility. I am requesting that this
condition be changed to allow the testing of the clay liner of the
effluent pond prior to disposal. All soils containing PCB's at
concentrations greater than or equal to l ppm (.0001 % by weight)
shall be removed and disposed at an appropriate facility.
