1.1 Background
Section 1
Introduction
Operable Unit No. 3 (OU-3), also referred to as the Former Chlorine Plant Area, is located at the Domtar
Paper Company, LLC (Domtar), Plymouth Mill in Martin County, North Carolina. The Record of
Decision (ROD) for this area was issued in September 2003. The negotiated Consent Decree [Civil
Action 4:03-CY-90(1-1)(3)] established remedial design (RD) and remedial action (RA) activities for this
area. The RA activities were completed during 2006, with the Pre-Final RA inspection accepted as the
Final RA inspection under United States Environmental Protection Agency (USEPA) approval in August
2006. The Interim RA report was completed and approved by USEPA on September 29, 2006.
The Pe,jiJrmance Standards Verification Plan (PSVP)/or Former Chlorine Plant Area (RMT, 2005)
describes procedures for the inspection, monitoring, and reporting activities required for the Former
Chlorine Plant Area. The PSVP was required by the Statement of Work (SOW) for the Former Chlorine
Plant Area in response to the US EPA Consent Order Docket No. 98-10-C issued by US EPA on March 24,
1998. According to the PSYP, semiannual monitoring was to be conducted through fall 2008, followed
by annual monitoring until the first 5-year review. USEPA issued the First Five-Year Review Report in
May 20 I 0. Recommendations presented in the First Five-Year Reviell' Report included annual sampling
of wells CP-02-1, MW-I B, CP-01-2, CP-04-1, and CP-05-1. Biennial sampling would add monitoring of
wells MW-2, CP-02-2, CP-06-1, CP-07-1, CP-08-1, CP-09-1, CP-10-1, CP-13-1, and CP-14-1. The deep
wells CP-0l-3R, CP-04-3, and CP-03-3 are to be sampled once during this next five year period.
The performance monitoring activities for the Fonner Chlorine Plant Area are being conducted according
to the PSVP and the recommendations of the First Five-Year Reviell' Report (USEPA, 2010) and as such
have been divided into two technical tasks. The objective of each task is listed below:
■ Task I, Surface Cap Containment System Monitoring
A surface cap containment system reduces the infiltration of surface water in the vicinity of the
Former Chlorine Plant Arca. The objective of the surface cap containment system is to assure that
the cover system components arc maintained, including concrete, asphalt, and stormwater system
components. The surface cap containment system is inspected during groundwater monitoring
events for surface cracks, holes, depressions, or other signs of surface integrity degradation.
Construction of the surface cap containment system was completed in August 2006.
■ Task 2. Groundwater Monitoring
The objective of the groundwater monitoring program is to assess the effectiveness of the remedy to
protect groundwater quality by reducing the migration of mercury from the Fornier Chlorine Plant
Arca. Institutional controls are applied to the Former Chlorine Plant Arca in order to prevent future
use of the groundwater as a potable water source.

1.2 Purpose
The purpose of this monitoring report is to present the results of the Fonner Chlorine Plant Area surface
cap containment system inspection (Task I) and groundwater monitoring (Task 2) events conducted
November 16, 2010. This report presents surface cap inspection results, and analytical data from
groundwater sampling, pre-and post-construction water table elevations, and provides recommendations
for continued monitoring. _Water table elevations and mercury concentrations for the semiannual
sampling events (November 2006, May 2007, November 2007, May 2008 and December 2008) and the
subsequent annual monitoring events (December 2009 and November 2010) arc compared to baseline
sampling results obtained in January 2006.

Methods
A baseline sampling event was conducted by RMT North Carolina, Inc. (RMT) during the week of
January 22, 2006. This sampling event served as the baseline for the short-and long-term perfonnance
monitoring of the Former Chlorine Plant Area. Remedial activities (target soil excavation, barrier wall
installation, and surface capping) took place between March and August 2006. Semiannual sampling
events were initiated in November 2006. The initial sampling event served to evaluate any immediate
changes in groundwater flow or groundwater mercury concentration levels due to the remedial
construction activities as well as a basis for evaluating remedy effectiveness. Subsequent semiannual
sampling events took place in May 2007, November 2007, May 2008 and December 2008, followed by
the annual monitoring in December 2009 and November 20 l 0. In accordance with the recommendations
presented in the First Five-Year Review Report (USEPA, 2010), a reduced number of wells were sampled
beginning with the November 2010 monitoring event. Groundwater monitoring well locations arc
depicted on Figure I.
Groundwater samples were collected using low flow purge methods in which a minimum of three well
Volumes of groundwater were removed before sample collection. Groundwater samples were analyzed by
Columbia Analytical Services in Jacksonville, Florida for mercury using USEPA Method 245.1. The
laboratory analytical data reports for November 2010 are included in Appendix A and the field notes arc
included in Appendix B. Purge water was contained, sampled, and properly discharged into the on-site
wastewater treatment plant (WWTP).

Results and Discussion
This section presents the results of the surface cap and groundwater monitoring activities conducted in
November 20 I 0. Where applicable, results are compared to data from previous monitoring activities.
3.1 Surface Cap Containment System Monitoring
The surface cap containment system is inspected during each groundwater monitoring sampling event as
described in the PSVP. The surface cap containment system was inspected for surface cracks, holes,
depressions, and other signs of surface integrity degradation during the November 20 IO groundwater
monitoring event. No degradation to the cap was identified. No additional maintenance of the surface
cap containment system is recommended at this time.
3.2 Groundwater Monitoring
The water table elevations measured during the baseline sampling event in January 2006 and the
subsequent semiannual and annual monitoring events are summarized in Table I. In January 2006, prior
to installation of the Waterloo® barrier wall, there was a small ridge in the water table surface extending
from a mounded area at monitoring well CP-10-1 (western end of the Former Chlorine Plant Area) to a
second, lesser mounded area at monitoring well CP-08-1 (northeastern end of the Fom1er Chlorine Plant
Area). This ridge was still present during the November 2010 sampling event. On the northern end of
this ridge, groundwater flows northward to the Roanoke River. South of the ridge, groundwater appears
to flow south and then to the northeast towards monitoring well CP-11-1 where it ultimately discharges
into the Roanoke River. The water table is at its lowest elevation at well CP-11-1.
Since November 2006, the water level within the confines of the barrier wall (CP-06-1) have fluctuated
both above and below the water elevations measured in wells surrounding the barrier. Outside of the
barrier perimeter, the configuration of the water table in November 2010 is similar to the baseline water
table map (January 2006). Both the November 20 IO (Figure 2) and the January 2006 (Figure 3) water
table surface maps are included in this report.
Table 2 presents the mercury analytical results for groundwater samples collected from the designated
wells as outlined in the Performance Standards Verification Plan (RMT 2005) and subsequently modified
by recommendations presented in the First Five-Year Review Report (USEPA 20 I 0). Field parameters
measured include pH, specific conductance (SC), dissolved oxygen (DO), turbidity, and temperature, and
are presented in Table 3.

Historically, mercury results have been below the method detection limits for most of the groundwater
samples collected. The North Carolina groundwater quality standard for mercury is 0.00 I 05 mg/L
( 15 NCAC 02L.0202, revised April 2005). Mercury concentrations detected during the baseline and
subsequent sampling events are summarized as follows:
■ Baseline Monitoring Event (January 2006)-The concentrations of mercury in monitoring wells
CP-02-1, CP-04-1, CP-05-1, and CP-08-1 were above the 2L Standard for mercury in the January
2006 monitoring event.
■ Semiannual Monitoring Event (November 2006)-The concentrations of mercury in monitoring
wells CP-01-2, CP-04-1, CP-05-1, and MW-18 were above the 2L Standard for mercury in the
November 2006 monitoring event.
■ Semiannual Monitoring Event (May 2007)-The concentrations of mercury in monitoring wells
CP-01-2, CP-04-1, CP-05-1, CP-08-1, and MW-18 were above the 2L Standard for mercury in the
May 2007 monitoring event.
■ Semiannual Monitoring Event (November 2007)-The concentrations of mercury in monitoring
wells CP-01-2, CP-01-3R, CP-04-1, CP-05-1, CP-08-1, and CP-08-3 were above the 2L Standard for
mercury in the November 2007 monitoring event
■ Semiannual Monitoring Event (May 2008)-The concentrations of mercury in monitoring wells
CP-04-1, CP-05-1, CP-08-1, and MW-18 were above the 2L Standard for mercury in the May 2008
monitoring event
■ Semiannual Monitoring Event (December 2008)-The concentrations of mercury in monitoring
wells CP-04-1, CP-05-1, MW-I B, and CP-01-2 were above the 2L Standard for mercury in the
December 2008 monitoring event
11 Annual Monitoring Event (December 2009)-The concentrations of mercury in monitoring wells
CP-04-1, CP-05-1, MW-18, CP-08-1, and CP-01-2 were above the 2L Standard for mercury in the
December 2009 monitoring event.
■ Annual Monitoring Event (November 2010)-The concentrations of mercury in monitoring wells
CP-04-1, CP-05-1, MW-IB, and CP-01-2 were above the 2L Standard for mercury in the November
20 IO monitoring event. Monitoring well CP-08-1 was not sampled in November 20 I 0.
While the concentrations in some wells remain slightly above the 2L standard for mercury, decreasing
mercury concentrations are observed in wells CP-04-1 and CP-05-1 between November 2007 and November
20 I 0. Mercury concentrations continue to fluctuate in groundwater samples from MW-I B, with detected
concentrations above the 2L standard in May 2008 through December 2009, compared to a detected
concentration slightly below the 2L standard in November 2007. Concentrations of mercury detected in
groundwater from well CP-01-2 have remained above the 2L standard since December 2008. Mercury was
not detected in groundwater sampled from well CP-02-1 in November 20 I 0, and has remained below the
2L standard since November 2006.

3.3 Recommendations
US EPA completed the first five-year review of the performance monitoring and remedy effectiveness in May
2010. Based on the monitoring results collected through December 2009, USEPA recommended a reduction
in the groundwater monitoring program. Recommended revisions included annual monitoring of wells
CP-02-1, MW-I 8, CP-01-2, CP-04-1, and CP-05-1 and biennial (every two years) monitoring of wells
MW-2, CP-02-2, CP-06-1, CP-07-1, CP-08-1, CP-09-1, CP-10-1, CP-13-1, and CP-14-1. The deep wells
CP-01-3R, CP-04-3, and CP-03-3 would be sampled once between 20 IO and 2015. Groundwater level
measurements would be collected on an annual basis. Domtar agreed with USEPAs recommendations and
implemented these changes with the November 2010 monitoring event. No additional modifications are
recommended at this time.
The surface cap containment system will continue to be inspected annually for surface cracks, holes,
depressions, or other signs of surface integrity degradation.

All analyses were performed consistent with the quality assurance program of Columbia Analytical Services, Inc.
(CAS). This repmi contains analytical results for samples designated for Tier II data deliverables, including results of
QC samples analyzt:d from this delivery group. When appropriate to the procedure, method blank results have been
reported with each analytical test. Analytical procedures performed by the lab arc validated in accordance with
NELAC standards. Parameters that arc included in the NELAC Fields of Testing but arc not included in the lab's
NELAC accreditation are identified in the discussion of each analytical procedure.
Sample Receipt
Seven water samples were received for analysis at Columbia Analytical Services on 11/l 9/l 0. The samples were
received in good condition and consistent with the accompanying chain of custody form. Samples are refrigerated at
4±2°C upon receipt at the lab except for aqueous samples designated for metals analyses, which were stored at room
temperature.

Batch QC Notes and Discussion
Quality control samples (i.e., Dup/Spike or tv1S/Drv1S samples) were performed using smnples from another sample
delivery group (SDG). The frequency requirement for quality control sample analysis was consistent with the
