The following represents our understanding of matters discussed and actions agreed upon. Any correction or
omission of merit should be reported promptly to the writer.
Subject: IQAT inspection/ visit
The following items were observed at the site during the IQAT inspection.
1. Clearances between the reinforcing steel and the ground are specified to be 3" in several areas they were
less than 2" of clearance.
2. Note that the pads are 3". above a 4" thick stone maintenance layer that is on top of the finished grade. This
does not appear to be the case at the pads since the top is formed by a 2 X 4 and excavated dirt. The total depth
from the top of the 2 x 4 is 24" is accordance with the plans.
3. Equipment Pads are detailed to be 11" above the main slab. Field measurements showed that the main pad is
about 5 "thick and that the total slab plus equipment pad is 9" . This must be corrected prior to pouring the slab.
4. The electrical control boxes for UD -

and UD -

have been miss labeled.
5. There has been no pad poured around LD-1 nor around the upper macon wells.
6. There is significant erosion around the Lower Dockery facility. LO 3 pad has been 50% undermined. Many of
the Valve boxes contain water.
7. The final grading of the site is going to be critical. . The covers for the valve boxes and turn around boxes are
furnished with drains. In order to work the drain line must be free running to open ground. This would indicate
that the top of these boxes must be several inches above grade. This is particularly true at the turn around box
on Lower Macon where the top of the box is 42" below original grade. In several areas silt has already covered
part of the box cover.
8. There is what appears to be a domestic water well adjacent to the revised position of the Upper Macon Source
facility. Based on the location and the service pipe it appears to have served the Macon residence.
9. The Contractor is to be reminded OSHA rules define clearly the maximum depth of trench without sloping the
sides.
· 10. The electrical underground conduits at the Upper Dockery site were not sealed nor have the pull boxes been
installed. The result is that they are probably full of silt and water. Contractor must carefully clean out the
conduits prior to installing the wire.
