COM Federal Programs Corporation (COM Federal) is pleased to submit the above
referenced report for the GE/Shepherd Farm Site. This report summarizes the remedial
activities performed by HSI/Geotrans (HSI) and their contractors during the reporting
period and presents to the EPA any significant problems, issues or items of note observed
by COM Federal during the oversight activities. This oversight report is submitted in
partial fulfillment of Task 8 of COM Federal's oversight work plan for this assignment.
The activities conducted onsite during this event included excavation and confirmatory
sampling of the two onsite landfills (Landfill A and Landfill B), the backfilling of
Landfill A and Landfill B with clean fill, and the backfilling of landfill material into the
Dry Sludge Impoundment (OSI) for final disposal.. Generally, the details for the
activities performed are presented in HSI's Remedial Design Work Plan_ Attachment A
is a completed copy of the appropriate sections of the Hazardous Waste Field Overview
Checklist for the Environmental Investigations Standard Operating Procedures and
Quality Assurance Manual (EPA, 1996). A copy of the Field Oversight Logbook for the
reporting period is included in Attachment B, and photolog of the activities is presented
in Attachment C. The numbering of the photographs in Attachment C is sequential with
those submitted with the oversight report dated July 27, 1999.

SUMMARY OF OVERSIGHT ACTIVITIES
HSI/Geotrans, contractors for General Electric, were responsible for performing remedial
activities at the GE Lighting site, in support of the ROD/ESD that was issued by the EPA in
September 1995. The construction subcontractor for HSI was Earthtech.
The goal of the remedial activity was to excavate soil from two onsite landfills (Landfill A and
Landfill B) and deposit the landfill material in a newly engineered landfill (Dry Sludge
Impoundment). The requirements of the ROD and the ESD for the site state that material found
in either Landfill A or B must be excavated if it:

• Is above 10 ppm of total PCBs
• Contains visible debris
At the completion of the excavation, an engineered cap is to be installed over the DSI as a means
of preventing the migration of contaminants from the landfilled material.
The oversight activities that were observed include the excavation and partial backfilling of
Landfill A, the excavation and partial backfilling of Landfill B, the excavation and regrading of
haul road between Landfill B and the DSI, and the deposition and compaction of landfilled
materials into the DSI. CDM Federal personnel observed confirmatory sampling conducted
during excavation activities, the packaging and shipping of confirmatory samples, and the onsite
analysis of samples using an immunoassay methodology. CDM also observed the placement of
structural fill along the footprint of the DSI. The partial installation of the gas vent system and
the start of common fill placement and compaction was also observed.
PROBLEMS/ISSUES/ITEMS OF NOTE
An overriding issue during the oversight was a difference in opinion between HSI and EPA (as
well as NCDEHNR) concerning what material should be excavated from Landfill B. Initially,
HSI maintained that only landfill material that was tested to be above 10 ppm of total PCBs
should be removed, whereas the EPA and NCDEHNR wanted all visible debris removed. HSI
eventually agreed to remove all materials from the landfill that contained either PCB
concentrations above IO ppm and/or visible construction debris.
The east and north side of the excavation in Landfill B were under dispute due to the large
amount of concrete, fill with no debris, or lack of visible contamination in the excavated
material. HSI stated that the excavation of the area could be halted if sampling of the area
resulted in no PCB contamination. This was later resolved when GE decided that is would be
more advantageous to excavate the material immediately and not worry about the possibility of
having to go back and retrieve any contaminated areas.
During a site visit from both the USEPA and NCDEHNR, it was proposed that test pits be dug
along the northwest side of Landfill B to confirm that the limits of Landfill Bin that area had
been reached. GE agreed to dig two test pits 10 feet long, 10 feet deep, and 3-5 feet wide with
the use of a backhoe. Each of these test pits was located 10 feet from the edge and running
parallel to the existing excavation about 20 feet apart. Analytical results of confirmatory
samples indicate that the limit of PCB contamination has been reached and no further excavation
in that direction is required.
After the completion of excavation in Landfill B, a temporary road through the area was
constructed to allow GE employees access to the Recreation Center. Initially this relatively
narrow road was constructed on backfilled material that extended through the Landfill B
excavation. This potentially hazardous situation was remediated by the backfilling of the

excavation on both sides of the road. Other backfilling activities were haulted in order to
minimize traffic congestion. When backfilling is resumed, the temporary roadway will be
moved to prevent any interference with or from vehicles proceeding through the site to the
Recreation Center.
At the completion of excavation in Landfill B, and during the spot excavations of Landfill A,
Earthtech focused its attention on the DSI. With most of the waste in place, structural fill was
brought in to prepare for the placement of barrier soil. The gas vent system was partially
installed. The vent system will be completed when concrete is poured into the casings, they are
leveled, and the standpipes are finished off.
As noted in the previous oversight report, minor health and safety infractions occurred and were
quickly corrected. However, continuous air monitoring continued to be inconsistent and there
were several occasions when HSI did not conduct continuous air monitoring for more than an
hour. This was the case even when odors were detected in areas of Landfill A during re-
excavation. Fans were put in place to attempt to prevent accumulation of any VOC
contaminants, but continuous air monitoring was not always occurring. HSI reported that one of
the Mini Raes (photoinoization detector) was not functioning correctly. This device was
replaced with a new instrument. HSI also stated that the humidity was causing the instruments to
calibrate incorrectly, resulting in false hits. The use of a flame ionization detector to solve the
problem was recommended to HSI, and it was suggested that the practice of continuous air
monitoring should be followed.
