Section 3.2.1 Land Survey
2. This section states that representatives from the North Carolina Cooperative Extension
Service and the North Carolina Forest Reserves Office visited the site and stated that no
endangered and/or rare plant species were observed within the Shepherd Farm Subsite area.
Please provide documentation of these site visits and observations.
Section 3.2.4 Utilities Inspection
3. Please move the heading of this section from page 3-8 to page 3-9.
Section 3.2.5 Permits
4. This section states that the Asheville; North Carolina regional office of the North Carolina
Department of Environment and Natural Resources (NC DENR), Land Quality Section
approved the Soil Erosion and Sediment Control Plan. Please provide documentation of this
approval.
Section 3.3.3 Waste Characterization and Transportation
5. The third sentence of the first paragraph of this section describes the sampling of stockpiled
soils for polychlorinated biphenyl (PCB) analysis. Please provide the laboratory
documentation of the PCB analysis.
6. The last paragraph of this section describes the volume of contaminated soils that were
excavated and the volume of clean soils used as backfill. Please provide the complete truck
log documentation for the contaminated debris, contaminated soil and clean backfill soil.

Section 3.4.1 Backfill

7. The third sentence of this section describes the sampling of the backfill soils for total PCB and
pesticide analysis. Please provide the laboratory documentation of the laboratory analysis.
8. This section describes the volume of clean soils used as backfill. As stated previously, please
provide the complete truck log documentation for the contaminated debris, contaminated soil
and clean backfill soil.
Section 3.4.5 Post-Excavation
9. The last sentence of this section states that the residential survey and letter are included as
Appendix B. However, the survey and letter are included as Appendix C. Please correct this
oversight.
Section 3.5 Performance Standard Exceptions
10. The first sentence of the fifth paragraph of this section indicates that HSI Geo Trans resampled
at each of the four locations which exceeded the PCB performance criteria. Beyond
providing the validated laboratory documentation, please provide a complete description of
the resampling activities. Items to be addressed include, at a minimum, the actual sampling
location in relation to the previous sampling location, the depth of the sample collection, the
sampling methodologies, etc.
11. The sixth sentence of the fifth paragraph of this section is unclear as to the western boundary
of the proposed excavation. Please clarify this sentence.
12. The fifth paragraph of this section describes how HSI GeoTrans proposes to address four
areas in which validated sample results indicate the presence of total PCB in excess of the
cleanup criteria. HSI GeoTrans resampled those areas following completion of the
excavation activities. Laboratory analysis indicates that the total PCB concentration is
beneath the cleanup criteria except in one area. HSI GeoTrans has stated that over-
excavation of the excavation zone has removed the contaminated soil. However, Figure 3-9
and Figure 3-10 only document the initial and final elevations for the areas represented by soil
samples SF-64 and SF-80. Furthermore, Figure 3-9 and Figure 3-10 only document the initial
elevation for the areas represented by soil samples SF-90 and SF-118. HSI GeoTrans has
inadequate documentation to demonstrate that the contaminated soil was removed and

that adequate backfill was replaced in these areas. Therefore, HSI Geo Trans shail return to
these areas and complete the removal and backfill tasks. Please provide a schedule and brief
workplan (which may simply reference much of the Remedial Action Work Plan, Shepherd
Farm Surficial Soil Removal) for completing these tasks.
Section 4.1 Project QA/QC
13. The third sentence of this section indicates that the construction quality assurance officer
(CQAO) completed daily construction reports and documented each definable feature of the
work. Please include the daily construction reports and all other documentation as separate
appendices.
Section 4.2.1 Project Health and Safety
14. The last sentence of this section indicates that daily health and safety activities were
documented in the field safety briefing form. Please include this documentation as a separate
appendix.
Section 4.2.3 Air Monitoring
15. The last sentence of the first paragraph of this section indicates that the closure air monitoring
report is included as Appendix C. However, the closure air monitoring report is actually
included as Appendix D. Please correct this oversight.
Section 4.3 Project Documentation
16. This section indicates that Daily Construction Logs, Daily Field Safety Meetings,
Inspection/Acceptance Forms, and Final Inspection Forms were used to document the
activities of this project. Please include this documentation as separate appendices.

Figure 3-12 Spot Excavation to be Completed at the Shepherd Farm Subsite
24. Figure 3-12 indicates that soil sample SF-64 contained 0.0091 parts per million (ppm) total
PCBs and that soil sample SF-80 contained 1.24 ppm total PCBs. However, Table 3-5
indicates that soil sample SF-64 contained 0.009J ppm total PCBs and that soil sample SF-80
contained 2.07 ppm total PCBs. Please clarify these discrepancies.
Table 3-1 Water and Septic Incidents and Repairs
25. During the week of October 20, 1997, water was shut off to make repairs to a water line.
While the water was shut off, a Spring Haven resident's water heater burst. Please provide
the appropriate documentation of this incident, and all others, in Table 3-1.
Table 3-2 Drums Discovered During Surficial Soil Removal
26. The last item in this table indicates that only one rusted shell of a drum was found in the brick-
lined kiln. However, the third sentence of the fifth paragraph of Section 3 .3 .1 indicates that
a "couple" of drums were discovered in the kiln. Please clarify this discrepancy.
Table 3-3 Compaction Test Results
27. Table 3-3 provides the compaction results from soils placed in the dry sludge impoundment.
However, the compaction results do not meet the performance criteria for compaction as
· stated in tlie Remedial Action Work Plan, Shepherd Farm Surficial Soil Removal (HSI
Geo Trans, 1997). Please provide a justification for not meeting the compaction performance
criteria.
Appendix B Photo Documentation
28. Photograph 4 shows the decontamination of a backfill truck before leaving the excavation
zone. However, the truck is being with decontaminated with a pressure washer and the
decontamination water is being allowed to run onto the ground. In accordance with the
Remedial Action Work Plan, Shepherd Farm Surficial Soil Removal (HSI Geotrans, 1997),
decontamination water was to be contained and recycled for use in dust suppression at the
Dry Sludge Impoundment. Please provide a justification for not conducting decontamination
activities in accordance with the approved Remedial Action Work Plan, Shepherd Farm
Surficial Soil Removal.

The Remediation Contractor will be selected by September 12.
The last of the project details will be finalized during the week of'
September 15 and presented in the final RA Work Plan supporting
documents (e.g., CQAPI', HASP, and Soil and Erosion Plan). In
addition to the information provided in response to specific
comments I through I 9 (sec following pages), the following
additional information may be provided at this time for the general
comments.

Project Organization -The general project organization is
presented in Attachment 13. Specific project personnel will
be selected September 12 when the Remediation Contractor
is selected. The complete project organization will be
presented in the final RA WI'.
Health and Safety Controls -
A
secure work zone
(exclusion zone) will be set up during mobilization. The
work zone area at the Shepherd Farm Subsitc will be
similar in extent to that which was presented in the draft
RA WP. Truck traffic will be directed through the
Shepherd property. No heavy truck or equipment traffic
will be permitted in the Spring Haven Community. Roads
will be blocked at the work zone perimeter to prevent
public traffic from entering the work zone.
Truck Decon -Temporary decontamination stations wi 11 be
established within the exclusion zone at both the Shepherd
Fann Subsite and dry sludge impoumlment area. Trucks
and heavy equipment will be inspected, dry decontaminated
(using shovels and brushes) and/or pressure washed, as
necessary, to remove contaminated material.
Decontamination water will be contained and recycled for
use in dust suppression at contaminated, active construction
areas (e.g., dry sludge impoundment).

Air Sampling -
A
meteorological station equipped with
wind direction and velocity indicators will be constructed
onsitc to record the potential for off-site airborne transport
of dust generated from the site. Air particulates shall be
monitored using a Mini Real-time Aerosol Monitor (mini-
RAM) during excavation activities that potentially generate
airborne particulates. Dust levels may not exceed 10
mg/m3• Air monitoring will be performed, at a minimum,
at four corners or sides of the Shepherd Farm Subsite work
zone perimeter. Historic soil and soil gas data show that
VOC arc not a health and safety concern at the site.
Dust Control -Construction activities will be performed
with care to minimize dust levels. Engineering controls
will be utilized, as necessary, to decrease particulate levels.
The primary means of dust control will be the use of water
to wet the soil. When dust levels exceed 5 mg/m3, dust
control measures will be implemented and work will
continue. When dust levels exceed 10 mg/mJ, work
activities shall cease while dust suppression activities arc
implemented. Work activities will not begin again until
dust levels arc less than 5 mg/1113•

Based on the recent data, it has been determined that the residents
in lots 116,201,203,205, and 304 will not be required to be
relocated. Table 3.1 will be revised accordingly. Resident's
names will not be listed in the table.
The figure reference will be corrected as noted.
All grubbed material in the contaminated areas will be deposited in
the dry sludge impoundment with the excavated soil. Material that
can be chipped or mulched will be chippeu and mulched.
The ROD-selcctcu remedy slates that the surficial soil with total
PCl3s above the performance standaru will be excavated to a
prcdctcnnincd depth of 12 inches. The entire horizontal extent or
PCBs will be defined prior to the initiation of excavation. As a
result, pre-excavation, post-excavation, and post-restoration
topographic surveys will be performcu at 25-foot centers to
monitor the performance of the soil excavation and baeklill
activities.
The word "drai't" will be deleted as requested. A linal RA Work
Plan will be submitted alter your comments on these responses arc
received.
Soil in the dry sludge impoundments will be placed using the
following specifications.
A. Soil shall be placed and compacted in one foot lilts.

of moisture and density as determined for the excavated

D. As a minimum, the materials shall be compacted at a
moisture content ranging from optimum moisture content
to no greater than plus four percent of optimum moisture,
and the corresponding dry densities shall be at greater or
equal to 90 percent of the maximum dry density as
determined by the Standard Proctor Test (i\STM D698).
F One sample should be taken for each lift for moisture
content and density.
The dry sludge impoundmcnt is currently not capped. The
Sediment and Erosion Plan will address the steps to be taken to
minimize erosion at the dry sludge impoundment during
construction activities. A permeable geotcxtilc shall be placed
across the entire dry sludge irnpoundmcnt prior to placing
excavated soil rrom the Shepherd Farm Subsite. The liner serves
to control the erosion and isolate the two soil types. After the
excavated soil is placed in the dry sludge impoundmenl, the

C. 13acklill shall be placed over approved areas to a depth
surticicntly greater than required so that alicr natural
settlement and light rolling, final grade will conform to
original grade. Final grade of backfill shall be even and
uniform in appearance, even if extra soil is required to
achieve this.
As stated in response no. 11, pre-excavation, post-excavation, and
post-restoration topographic surveys will be performed al 25-foot
centers to monitor the performance of the soil excavation and
backfill activities.
A 30-mil liner could be used for accessible portions of the crawl
spaces. 1-lowcver, we recommend using a high density
polyethylene sheeting that is 8 mil thick, UV-stabilized, and
resistant to puncturing and tearing. The plastic sheeting will be
anchored securely to withstand wind, freeze/thaw conditions, and
occasional foot trartic. The sheeting recommended is very durable,
yet more flexible than a 30-mil sheeting. The more lkxible
sheeting would minimize damage to the undersides of the homes
<luring installation and could be placcc.l without retrofitting the
crawl space entrances.
The O&M plan will be submitted alier the excavation. It will
address general O&M issues at Spring Haven including any
anticipated fiiture actions to ensure a 12 inch cover.
As noted on the figures, all data arc preliminary. The data will be
checked and validated.

Draft Remedial Action Work Plan
Shepherd Farm Surficial Soil Removal Comments
GENERAL COMMENT
1. This work plan only gives a brief overview of the work to be performed and lacks
sufficient detail to perform a thorough review. For example, the following types
of tasks should be included in the plan:
• overall project organization (this is only briefly discussed in Section 3.5),
• site health and safety controls and layout of site-controlled zones,
• use of a decontamination pad and equipment decontamination (this will be
required before trucks and earthmoving equipment leave the site),
• measures for dust control,
• air sampling approach,
• confirmation sampling approach, and
• transportation plan for trucks and equipment both at the Shepherd Farm subsite
and between this site and the GE plant.
EPA realizes that some of the topics may be encompassed within the
appendices that are yet to be developed. However, the general approach
should be included within the work plan since it will be the governing document
used by field personnel during the removal activity. Sufficient detail included
now will prevent questions and possible downtime in the field.
2. Mr. Dickason, in his agreement with EPA, had specific requests for the removal
and relocation effort. While some of these provisions are in the work plan
already, they need to be put in the work plan verbatim as they appear in the EPA
agreement. They are:
restore the Respondent's property to its original condition to the extent
practicable upon completion of their work, provided however, that such
restoration does not include the replacement of non-landscaped, ordinary
vegetation cleared for access purpose;
not materially interfere with the other ongoing uses of such property, except
to the extent required to complete the work set forth in the September 29,
1995 Record of Decision for this Site;
• •
-not use the sanitary and other facilities at such property (except as
independently provided by EPA);
upon request supply data and the opportunity to take split samples from such
property without charge, and
to the extent reasonably practicable, minimize the disruptions to residents of
such property to be affected by EPA's work.
SPECIFIC COMMENTS
1. Page 4, Section 1.2.2, 2nd paragraph -In the last sentence of this paragraph,
change the word "site" to "property". Readers may improperly imply that the
entire site was deleted instead of just a portion.
2. Page 9, top of page, Section 2.2 Basis of Design -The last sentence should
read " ... total PCBs exceed the remedial goal of 1 mg/kg at Shepherd Farm."
3. Page 9, paragraph one, Section 2.2, Tables 2.2 and 2.3 -Excavation is limited
to the top foot of soil, yet the tables show PCB exceedances at other depths.
This may be confusing to the reader. Either include an explanation here of why
these soils are not being removed, or only report the surficial soil results and not
the others since these are not the basis of design.
4. Page 12, Figure 2.1 -The preliminary area of excavation is incorrectly depicted
on the western side. This area should account for the exceedances at sample
locations 51, 53, and 04.
5. Page 14, Section 2.3 -The NC Superfund Section recognizes the preliminary
surficial soil sampling activities have not been completed as of the date of this
submittal. However, care should be taken to ensure that the residential lots
mentioned in the first paragraph of this section concurs with the information
provided in Section 3.5 -Community Relations, Figure 2.3 -Extent of Soil
Removal and Layout of RA Work Area, and Table 3.1 -Residents Requiring
Relocation.
6. Page 20, 1st complete paragraph -In the first line, it states that 25 feet is the
threshold for relocation. Please delete this sentence and indicate that GE with
EPA and the State concurrence, will determine based on health and safety,
access requirements etc. the residents that will be temporarily relocated.

7. Page 20, 2nd complete paragraph -Need to input the updated dates of
relocation. Delete the sentence that refers to freezer storage and
reimbursement.
8. Page 21, Table 3.1 -Figure 2.2 does not indicate that samples were collected
within lots 116, 201, 203, 205, and 304. Why then, does this table indicate
"pe-nding results"?
Since many of the residents requested anonymity, delete the residents' names
and only include the address and reason in this table. This report will be a part
of the public record.
9. Page 22, paragraph two, Section 4.1 Surficial Soil Removal -The staging area
is depicted on Figure 2.3 and not Figure 2.2 as the text states.
10. Page 22, paragraph three, Section 4.1 Surficial Soil Removal -What will
happen to the grubbed material that is not reduced to mulch? All material (i.e.,
trees, stumps, shrubs, and other vegetation) should be stripped of contaminated
soil, to the extent possible, prior to disposal.
11. Page 22, Section 4.1 -The issue of confirmatory soil sampling has not been
addressed. Please address this issue to ensure that all contaminated surficial
soils are removed.
Please clarify in this section that all stockpiled soils will be placed on and
covered with plastic sheeting that has a minimum thickness of thirty mils.
12. Page 23, 2nd paragraph -Delete the word "draft" between "final" and "work" in
the third sentence.
13. Page 24, Section 4.2 -This section states that "The soil material placed in the
impoundment will be compacted daily in one-foot lifts." However, there is no
indication as to how compaction will be accomplished, what standards will be
used to determine adequate compaction efforts or if field density testing will be
performed to verify adequate compaction efforts. Please provide additional
information to clarify these issues.
This section does not address the placement of interim daily cover, a temporary
cover prior to the landfill cap placement, or stormwater controls. Please provide
additional details.

14. Page 24, paragraph one, Section 4.2 Soil Disposal -To the extent practicable,
the PRPs should ensure that the mulched vegetation is not contaminated (i.e.,
total PCBs above 50 ppm) before disposing offsite at a composting facility.
15. Page 25, Figure 4.1 · This figure should be titled "Truck route and entrance to
the impoundment".
The figure should show the entire route, from the Shepherd Farm property to the
GE sludge impoundment.
16. Page 26, Section 5 Site Restoration • Please provide additional details
regarding backfilling activities. This section should describe the soil to be used
as backfill, how the soil will be placed (e.g. six inch or twelve inch lifts) and any
compaction efforts to be utilized.
The plastic sheeting to be placed in accessible crawl space areas shall be of a
minimum thickness of 30 mils. Please change this section to reflect the use of
30 mil (minimum) plastic sheeting.
17. Page 27, Section 6.2 -In addition to the items mentioned, the data to be
presented to demonstrate compliance with the performance standards should
include confirmatory sampling locations, confirmatory sampling results and as-
built drawings that provide pre-excavation elevations, post-excavation elevations
and post-restoration elevations. The surveys should continue to be conducted
along a grid spacing of 25 feet.
18. Page 27, Section 6.2 -The O&M plan shall address any anticipated future
actions to ensure that a minimum of twelve inches of cover soil remains on the
properties of the affected Spring Haven residents. Additionally, the plan should
address general O&M concerns that will affect the residents of Spring Haven.
19. The total PCB concentration numbers should be double-checked. Numerous
addition errors were found in the preliminary data sheets. As this is the basis for
removal, these additions should be correct. Please note, however, that no errors
have been found that would change the status of grid. Examples are as follows:
The total PCB concentration for surficial soils sample SF-8 should be 3,210
ug/kg instead of 2,940 ug/kg;
SF-118 should be 197 ug/kg instead of 205.81 ug/kg;
SF-136 should be 6,400 ug/kg instead of 66,201 ug/kg;
SF-156 should be non-detect instead of 77.3 ug/kg;
SF-176 should be 760 ug/kg instead of 795.5 ug/kg.
