In accordance with your application received June 24, 1997, we are forwarding herewith Well
Construction Permit No. WM0100122 dated July 7, 1997, issued to General Electric Lighting Systems
for the construction of a monitor well at the Womack property on Spartanburg Highway in Henderson
County.
If any parts, requirements, or limitations contained in this Permit are unacceptable to you, you
have the right to an adjudicatory hearing before a hearing officer upon written demand to the Director
within 30 days following receipt of this Permit, identifying the specific issues to be contended. Unless
such demand is made, this Permit shall be final and binding.
This Permit will be effective for the construction of a monitor well from the date of its issuance
until the site assessment has been completed, and shall be subject to the conditions and limitations as
specified therein. Attached is a Well Construction Record to be completed and signed by the drilling
contractor. This form should be submitted to the Department upon completion of the welL

PERMIT FOR THE
CONSTRUCTION OF A WELL
OR WELL SYSTEM
In accordance with the provisions of Article 7, Chapter 87, North
Carolina General Statutes, and other applicable Laws, Rules, and Regulations,
PERMISSION IS HEREBY GRANTED TO
GENERAL ELECTRIC LIGHTING SYSTEMS
FOR THE CONSTRUCT/ON OF a monitor well for Groundwater Incident No. 5183 in the
Residuum System of the Blue Ridge-Inner Piedmont Hydrogeologic Unit, located at the
Womack property on Spartanburg Highway in East Flat Rock, Henderson County, in
accordance with the application dated June 12, 1997, and in conformity with
specifications and supporting data, all of which are filed with the Department of
Environment, Health, and Natural Resources and are considered a part of this Permit.
This Permit is for well construction only, and does not waive any provisions or
requirements of any o~her applicable laws or regulations.
Construction of a well under this Permit shall be in compliance with the North
Carolina Well Construction Regulations and Standards, and any other State and local laws
and regulations pertaining to well construction.
This Permit will be effective for the construction of a monitor well from the date of
its issuance until the site assessment has been completed, and shall be subject to other
specified conditions, limitations, or exceptions as follows:
1. The well driller shall notify the Asheville Regional Office prior to actual
construction of the well.
2. The well shall be constructed with well casing and well screen that conform
to requirements of the "Criteria and Standards Applicable to Water Supply and
Certain Other Type Wells" (15A NCAC 2C .0100).
3. The well shall be afforded reasonable protection against damage during
construction and use.
4. Provisions shall be made on the well cover to prevent surface water with
possible contaminants from entering the well pit and contaminating the monitor
well.
5. The well shall have permanently affixed an identification plate constructed of
durable material and shall contain the following information: drilling contractor's
name and registration number; date well completed; total depth of well; a warning
that the well is not for water supply and that the groundwater may contain
hazardous materials; and depth to screen.
6. Well completion, well abandonment, and water quality reports are to be
submitted to the Division of Water'Ouality's Groundwater Section Central Office in
Raleigh.
7. When the well has served its purpose and is of no further use, it shall be
permanently abandoned by the applicant according to the requirements of Section
. 0113 of the "Criteria and Standards Applicable to Water Supply and Certain Other
Type Wells" (15A NCAC 2C.I.

PROPERTY ACCESS AGREEMENT

As you know, the United States Environmental Protection Agency (USEPA) and General Electric Company (GE) have entered Into a Consent Decree to perfonn certain work relating to the General Electric/Shepherd Farm Site (Site) In East Flat Rock, North Carolina. In order to perfonn this work, GE Is required by the Consent Decree to seek access to any property for which access may be required to complete the work. To that end, GE Is requesting that you, as Owner of the above-referenced Property, provide access to your Property to USEPA and GE, and their employees, agents and representatives, In accordance with this Agreement. Your signature at the end of this document will Indicate your agreement to provide such access under the following terms and conditions:
1. Right of Entry Owner hereby grants to USEPA, GE and their employees, agents, representatives and/or contractors a right of access and entry to the Property consistent with the tenns and conditions of this Agreement. Such access and entry shall be for the purpose of perfonning any work required under the Consent Decree, Record of Decision or Statement of Work relating to the Site and shall Include a right of ingress and egress to and from the Property with all necessary materials and equipment. Owner understands that such work may Include the Installation of wells and taking of environmental samples as required by the remedial action selected by the USEPA
2. Consideration In consideration of Owner's Agreement to provide this right of access and entry, GE shall, upon reasonable request by Owner, provide Owner with free copies of all sampling results and related Information from the Property which are submitted to USEPA. GE shall also minimize to the extent practicable its need to enter and perfonn work on the Property.
3. Reasonable Notice to Owner GE shall provide reasonable notice to Owner of any need for access or entry to the property.
4. Non-Interference GE will make reasonable efforts to avoid Interference with Owner's use of or business on the Property.
5. Term This Agreement, beginning on the date It Is signed by Owner, shall last for an Initial tenn of 5 years, or until such time as the USEPA detennines that access to the property Is no longer necessary, whichever is later. This Agreement may be extended by mutual agreement of the parties.

6. Repair GE agrees to repair, at Its own cost and expense, any damage (other than reasonable wear and tear) which may occur lo the surface of the property as a result of access obtained by GE, Its employees, agents representatives and/or contractors, except for the installation of wells (which shall not be repaired). Any such repair shall restore the surface of the Property to the condition in which GE found ii at the commencement of this Agreement.
7. Liability Owner shall not be liable for any loss, damage or injury of any kind to any person arising from any access or to or use of Property by GE, its employees, agents, consultants, or contractors, unless such loss, damage or injury is caused by the negligent or willful acts or omissions of Owner.
8. No Admissions Nothing In this Agreement is intended nor shall be construed to be an admission of any issue of law or fact in any dispute, proceeding or litigation regarding liability or responsibility for environmental conditions in any location, including locations at or near the Sile or the Property.
9. No Change in Legal Status By entering into this Agreement, neither party waives or in any way alters any rights, claims or defenses ii may have against the other under law. The parties agree that access to the Property, in and of itself, shall not form the basis for any claim against the other party for any costs, damages or liabilities (including a claim for diminution in the value of any real property) resulting from or associated in any way with the environmental conditions at the Property.
10. Enforceability If any term, condition or provision of this Agreement is held by a court of competent jurisdiction to be invalid, void or unenforceable, the remainder of the Agreement shall remain in full force and effect and shall in no way be affected, Impaired or invalidated.
11. Binding Agreement This Agreement shall be binding upon and shall inure to the benefit of the parties lo this Agreement and their successors and assigns.

This Permit will be effective for the construction of a monitor well ;rom the date of
its issuance until the site assessment has been completed, and shall be subject to other
specified conditions, limitations, or exceptions as follows:
1. The well driller shall notify the Asheville Regional Office prior to actual
construction of the well.
2. The well shall be constructed with well casing and well screen that conform
to requirements of the "Criteria and Standards Applicable to Water Supply arid
Certain Other Type Wells" (15A NCAC 2C .OiOOJ.
3. The well shall be afforded reasonable protection against damage during
construction and use.
4. Provisions shall be made ori the Well cover to prevent surface water with
possible containiriants from entering the well pit and contaminating the monitor
well.
5. The well shall have permanently affixed an identification plate constructed of
durable rriateriai and shall cciritiiiri the following information: drilling contractor's
name and registration number; date well coriiplei:ed; total rleptli of well; a warning
thai i:he well is not foi water supply and that i:he groundWiJter may contain
hazardous materials; and depth to screen.
6. Well coiripletioh, well abaridonment, and Water quality reports are to be
submitted to the Division of Water Quality's Groundwater Section Central Office in
Raleigh
7. When tlie well has served its purpose and is o; no further use, it shall be
permanently abandoned by the appiicaiit accofdirig to the requirements of Section
. 0113 of the ''Criteria and Standards Applicable to Water Supply and Certain Other
Type Wells'' (15A NCAC 2CJ. ·

In accordance with your application received June 24, 1997, we are forwarding herewith Well
Construction Permit No. WM0100116 dated July 7, 1997, issued to General Electric Lighting Systems
for the construction of a monitor well at the property of Eugene Shepherd on Roper Road in East Flat
Rock, Henderson County.
If any parts, requirements, or limitations contained .in this Permit are unacceptable to you, you
have the right to an adjudicatory hearing before a hearing officer upon written demand to the Director
within 30 days following receipt of this Permit, identifying the specific issues to be contended. Unless
such demand is made, this Permit shall be final and binding.
This Permit will be effective for the construction of a monitor well from the date of its issuance
until the site assessment has been completed, and shall be subject to the conditions and limitations as
specified therein. Attached is a Well Construction Record to be completed and signed by the drilling
contractor. This form should be submitted to the Department upon completion of the well.

In accordance with the provisions of Article 7, Chapter 87, North
Carolina General Statutes, and other applicable Laws, Rules, and Regulations,
PERMISSION IS HEREBY GRANTED TO
GENERAL ELECTRIC LIGHTING SYSTEMS
FOR THE CONSTRUCTION OF a monitor well for Groundwater Incident No. 5183 in the
Residuum System of the Blue Ridge-Inner Piedmont Hydrogeologic Unit, located at the
property of Eugene Shepherd on Roper Road in Henderson County, in accordance with the
application dated June 12, 199 7, and in conformity with specifications and supporting
data, all of which are filed with the Department of Environment, Health, and Natural
Resources and are considered a part of this Permit.
This Permit is for well construction only, and does not waive any provisions or
requirements of any other applicable laws or regulations.
Construction of a well under this Permit shall be in compliance with the North
Carolina Well Construction Regulations and Standards, and any other State and local laws
and regulations pertaining to well construction.
This Permit will be effective for the construction of a monitor-well from the date of
its issuance until the site assessment has been completed, and shall be subject to other
specified conditions, limitations, or exceptions as follows:
1. The well driller shall notify the Asheville Regional Office prior to actual
construction of the well.
2. The well shall be constructed with well casing and well screen that conform
to requirements of the "Criteria and Standards Applicable to Water Supply and
Certain Other Type Wells" (15A NCAC 2C .0100/.
3. The well shall be afforded reasonable protection against damage during
construction and use.
4. Provisions shall be made on the well cover to prevent surface water with
possible contaminants from entering the well pit and contaminating the monitor
well.
5. The well shall have permanently affixed an identification plate constructed of
durable material and shall contain the following information: drilling contractor's
name and registration number; date well completed; total depth of well; a warning
that the well is not for water supply and that the groundwater may contain
hazardous materials; and depth to screen.
6. Well completion, well abandonment, and water quality reports are to be
submitted to the Division of Water Quality's Groundwater Section Central Office in
Raleigh.
7. When the well has served its purpose and is of no further use, it shall be
permanently abandoned by the applicant according to the requirements of Section
.0113 of the "Criteria and Standards Applicable to Water Supply and Certain Other
Type Wells" (15A NCAC 2C/.

In accordance with your application received June 24, 1997, we are forwarding herewith Well
Construction Permit No. WM0100117 dated July 7, 1997, issued to General Electric Lighting Systems
for the construction of a monitor well at the property of Eugene Shepherd on Roper Road in East Flat
Rock, Henderson County.
If any parts, requirements, or limitations contained in this Permit are unacceptable to you, you
have the right to an adjudicatory hearing before a hearing officer upon written demand to the Director
within 30 days following receipt of this Permit, identifying the specific issues to be contended. Unless
·such demand is made, this Permit shall be final and binding.
This Permit will be effective for the construction of a monitor well from the date of its issuance'
until the site assessment has been completed, and shall be subject to the conditions and limitab8ns a~
specified therein. Attached is a Well Construction Record to be completed and signed by the drJ!jing ~ ;:::

In accordance with the provisions of Article 7, Chapter 87, North
Carolina General Statutes, and other applicable Laws, Rules, and Regulations,
PERMISSION IS HEREBY GRANTED TO
GENERAL ELECTRIC LIGHTING SYSTEMS
FOR THE CONSTRUCT/ON OF a monitor well for Groundwater Incident No. 5183 in the
Bedrock System of the Blue Ridge-Inner Piedmont Hydrogeologic Unit, located at the
property of Eugene Shepherd on Roper Road in Henderson County, in accordance with the
application dated June 12, 1997, and in conformity with specifications and supporting
data, all of which are filed with the Department of Environment, Health, and Natural
Resources and are considered a part of this Permit.
This Permit is for well construction only, and does not waive any provisions or
requirements of any other applicable laws or regulations.
Construction of a well under this Permit shall be in compliance with the North
Carolina Well Construction Regulations and Standards, and any other State and local laws
and regulations pertaining to well construction.
This Permit will be effective for the construction of a monitor well from the date of
its issuance until the site assessment has been completed, and shall be subject to other
specified conditions, limitations, or exceptions as follows:
1. The well driller shall notify the Asheville Regional Office prior to actual
construction of the well.
2. The well shall be constructed with well casing and well screen that conform
to requirements of the "Criteria and Standards Applicable to Water Supply and
Certain Other Type Wells" (15A NCAC 2C .0100).
3. The well shall be afforded reasonable protection against damage during
construction and use.
4. Provisions shall be made on the well cover to prevent surface water with
possible contaminants from entering the well pit and contaminating the monitor
well.
5. The well shall have permanently affixed an identification plate constructed of
durable material and shall contain the following information: drilling contractor's
name and registration number; date well completed; total depth of well; a warning
that the well is not for water supply and that the groundwater may contain
hazardous materials; and depth to screen.
6. Well completion, well abandonment, and water quality reports are to be
submitted to the Division of Water Quality's Groundwater Section Central Office in
Raleigh.
7. When the well has served its purpose and is of no further use, it shall be
permanently abandoned by the applicant according to the requirement!/ of Section
.0113 of the "Criteria and Standards Applicable to Water Supply and Certain Other
Type Wells" (15A NCAC 2C).

and Certain Other Type Wells" (15A NCAC 2C .0100).
3. All wells shall be afforded reasonable protection against damage during
construction and use.
4. Provisions shall be made on the well covers to prevent surface water with
possible contaminants from entering the well pits and contaminating the
monitor wells.
5. Each well shall have permanently affixed an identification plate
constructed of durable material and shall contain the following information: drilling
contractor's name and registration number; date well completed; total depth of well; a
warning that the well is not for water supply and that the groundwater may contain
hazardous materials; and depth to screens.
6. Well completion, well abandonment, and water quality reports are to be
submitted to the Division of Water Quality's Groundwater Section Central Office in
Raleigh.
7. When the wells have served their purpose and are of no further use, they
shall be permanently abandoned according to the requirements of Section .0113 of
the "Criteria and Standards Applicable to Water Supply and Certain Other Type Wells"
(15A NCAC 2C).

PROPERTY ACCESS AGREEMENT

' ;2.<;?'13/ As you know, the United States Environmental Protection Agency (USEPA) and General Electric Company (GE) have entered Into a Consent Decree to perform certain work relating to the General Electric/Shepherd Farm Site (Site) in East Flat Rock, North Carolina. In order to perform this work, GE is required by the Consent Decree to seek access to any property for which access may be required to complete the work. To that end, GE is requesting that you, as Owner of the above-referenced Property, provide access to your Property to USEPA and GE, and their employees, agents and representatives, In accordance with this Agreement. Your signature at the end of this document will Indicate your agreement to provide such access under the following terms and conditions:
1. Right of Entry Owner hereby grants to USEPA, GE and their employees, agents, representatives and/or contractors a right of access and entry to the Property consistent with the terms and conditions of this Agreement. Such access and entry shall be for the purpose of performing any work required under the Consent Decree, Record of Decision or Statement of Work relating to the Site and shall include a right of Ingress and egress to and from the Property with all necessary materials and equipment. Owner understands that such work may include the Installation of wells and taking of environmental samples as required by the remedial action selected by the USEPA
2. Consideration In consideration of Owner's Agreement to provide this right of access and entry, GE shall, upon reasonable request by Owner, provide Owner with free copies of all sampling results and related information from the Property which are submitted to USEPA. GE shall also minimize to the extent practicable its need to enter and perform work on the Property.
3. Reasonable Notice to Owner GE shall provide reasonable notice to Owner of any need for access or entry to the property.
4. Non-Interference GE will make reasonable efforts to avoid Interference with Owner's use of or business on the Property.
5. Term This Agreement. beginning on the date it is signed by Owner, shall last for an Initial term of 5 years, or until such time as the USEPA determines that access to the property is no longer necessary, whichever Is later. This Agreement may be extended by mutual agreement of the parties.
6. Repair GE agrees to repair, at Its own cost and expense, any damage (other than reasonable wear and tear) which may occur to the surface of the property as a result of access obtained by GE, Its employees, agents representatives and/or contractors, except tor the Installation of wells (which shall not be repaired). Any such repair shall restore the surface of the Property to the condition in which GE found it at the commencement of this Agreement.
7. Liability Owner shall not be liable for any loss, damage or injury of any kind to any person arising froni any access or to or use of Property by GE, Its employees, agents, consultants, or contractors, unless such loss, damage or injury is caused by the negligent or willful acts or omissions of Owner.
8. No Admissions Nothing in this Agreement is intended nor shall be construed to be an admission of any issue of law or fact in any dispute, proceeding or litigation regarding liability or responsibility for environmental conditions in any location, including locations at or near the Site or the Property.
9. No Change in Legal Status By entering Into this Agreement, neither party waives or in any way alters any rights, claims or defenses it may have against the other under law. The parties agree that access to the Property, in and of itself, shall not form the basis for any claim against the other party for any costs, damages or liabilities (including a claim for diminution in the value of any real property) resulting from or associated in any way with the environmental conditions at the Property.
10. Enforceability If any term, condition or provision of this Agreement is held by a court of competent jurisdiction to be invalid, void or unenforceable, the remainder of the Agreement shall remain in full force and effect and shall in no way be affected, impaired or invalidated.
11. Binding Agreement This Agreement shall be binding upon and shall inure to the benefit of the parties to this Agreement and their successors and assigns.

Carolina Well Construction Regulations and Standards, and any other State and local
laws and regulations pertaining to well construction.

subject to other specified conditions, limitations, or exceptions as follows:
1. The well driller shall notify the Asheville Regional Office prior to actual
construction of the wells.

PROPERTY ACCESS AGREEMENT

' .29'13/ As you know, the Urilted States Environmental Protection Agency (USEPA) and General Electric Company (GE) have entered Into a Consent Decree to perform certain work relatlrig to the General Electric/Shepherd Farm Site (Site) In East Flat Rock, North Carolina. In order to perform this work, GE Is required by the Consent Decree to seek access to any property for which access may be required to complete the work. To that end, GE Is requesting that you, as Owner of the above-referenced Property, provide access to your Property to USEPA and GE, and their employees, agents and representatives, In accordance with this Agreement. Your signature at the end of this document will Indicate your agreement to provide such access under the following terms and conditions:
1. Right of Entry Owner hereby grants to USEPA, GE and their employees, agents, representatives and/or contractors a right of access and entry to the Property consistent with the terms and conditions of this Agreement. Such access and entry shall be for the purpose of performing any work required under the Consent Decree, Record of Decision or Statement of Work relating to the Site and shall Include a right of Ingress and egress to and from the Property with all necessary materials and equipment. Owner understands that such work may include the Installation of wells and taking of environmental samples as required by the remedial action selected by the USEPA
2. Consideration In consideration of Owner's Agreement to provide this right of access and entry, GE shall, upon reasonable request by Owner, provide Owner with free copies of all sampling results and related Information from the Property which are submitted to USEPA. GE shall also minimize to the extent practicable Its need to enter and perform work on the Property.
3. Reasonable Notice to Owner GE shall provide reasonable notice to Owner of any need for access or entry to the property.
4. Non-Interference GE will make reasonable efforts to avoid Interference with Owner's use of or business on the Property. ·
5. Term This Agreement, beginning on the date it Is signed by Owner, shall last for an Initial term of 5 years, or until such time as the USEPA determines that access to the property Is no longer necessary, whichever Is later. This Agreement may be extended by mutual agreement of the parties.
6. Repair GE agrees to repair, at Its own cost and expense, any damage (other than reasonable wear and tear) which may occur to the surface of the property as a result of access obtained by GE, Its employees, agents representatives and/or contractors, except for the Installation of wells (which shall not be repaired). Any such repair shall restore the surface of the Property to the condition In which GE found It at the commencement of this Agreement.
7. Liability Owner shall not be liable for any loss, damage or Injury of any kind to any person arising from any access or to or use of Property by GE, Its employees, agents, consultants, or contractors, unless such loss, damage or Injury Is caused by the negligent or willful acts or omissions of Owner.
8. No Admissions Nothing In this Agreement Is Intended nor shall be construed to be an admission of any Issue of law or fact In any dispute, proceeding or litigation regarding liability or responsibility for environmental conditions In any location, Including locations at or near the Site or the Property.
9. No Change In Legal Status By entering Into this Agreement, neither party waives or In any way alters any rights, claims or defenses It may have against the other under law. The parties agree that access to the Property, In and of Itself, shall not form the basis for any claim against the other party for any costs, damages or liabilities (Including a claim for diminution in the value of any real property) resulting from or associated In any way with the environmental conditions at the Property.
10. Enforceability If any term, condition or provision of this Agreement Is held by a court of competent Jurisdiction to be Invalid, void or unenforceable, the remainder of the Agreement shall remain In full force and effect and shall in no way be affected, Impaired or Invalidated.
11. Binding Agreement This Agreement shall be binding upon and shall Inure to the benefit of the parties to this Agreement and their successors and assigns.

In response to your June 8, 1995 letter, the following are
responses to your comments on the draft Remedial Investigation
Report for the above referenced site:
1. It is possible that some of the potable wells are, or
were, in the contaminant plume. Only two of the potable
wells sampled showed voe contamination during the RI, and
these contained extremely low levels.
2. As referenced in the work plan, all drill cuttings and
purge water were discharged to the ground, onsite, within
the area of contamination. No additional precautions
were taken.
3. The detection limits and units of measure are included on.
the data summary tables and/or the analytical data sheets
in Appendix B. The analytical methods, quantitation
limits, and dilutions are specified in the Analytical
Support Branch Standard Operating Procedures which is
referenced in the work plan.

The Superfund Section has reviewed this document and forwarded it to the North
Carolina Division of Environmental Management (DEM) for their review. DEM's
comments will be conveyed to you when received. The Superfund Section offers the
following comments:
I) Page 2-34. Paragraph 3. Although, the general direction of groundwater flow at all
three subsites is towards Bat Fork Creek, in the vicinity of a pumped well installed into
the bedrock, groundwater flow is controlled by the geometry, orientation, and
interconnection of the bedrock fractures. Apparently, several pumped wells are
located in the vicinity of the subsites. Are any of these wells drawing contaminated
groundwater from these subsites?
2) Page 3-8. Paragraph 4. All drill cuttings, development water and purge water
generated while installing and sampling the wells were discharged to the ground. The
National Contingency Plan (NCP) requires that managing of investigation-derived
waste attains all applicable or relevant and appropriate requirements to the extent
practicable considering the exigencies of the situation (see Section 300.415 (i) of the
NCP). RCRA is an ARAR for CERCLA hazardous waste investigations (DERR
Directive 9345.3-02). What precautions were taken to ensure that these materials did
not contain hazardous constituents before being discharged?

3) Page 3-14. Paragraph 3. The North Carolina groundwater standards (15 NCAC 2L)
are ARARs for this site. To the extent practicable, quantitation limits for analytes in
the groundwater should be below the North Carolina groundwater standard. The text
does not indicate which analytical methods are used or the quantitation limits. EPA
analytical method 8260 has quantitation limits lower than the NC groundwater
standards for most contaminants. At a minimum, laboratory reports should include the
following parameters: method detection limits, quantitation limits, sample quantitation
limits, units of measurement, and dilutions performed.
4) Page 4-75. Paragraph 3 and Figure 3. The potentiometric map only shows the
general direction of flow. Wells on fracture zones can move groundwater in directions
not indicated by the potentiometric map.
5) Page 4-82. Paragraph 4. The elevated manganese and iron in the private wells may be
related to the degradation of the volatile organic compounds under anaerobic
conditions.
6) Page 4.6. In view of the lack of information on the movement of contaminants along
bedrock fractures, all wells used for household purposes within a one mile radius of
the site should be sampled. Depending on the contamination found, the radius of
sampling may need to be expanded.
7) Page 5-3. The most likely mechanism for exposure of the public to contaminants
migrating off-site is along fracture zones that are being pumped by wells. Migration is
not limited to the downgradient direction as indicated by potentiometric maps of the
surficial aquifer.
8) Page 6-3. In addition to the recommendations listed in the report, the following
additional recommendations are proposed:
• Additional soil samples to define the vertical and horizontal extent of PCB
contamination.
• Additional groundwater samples to define the vertical extent of groundwater
contamination. As indicated by Figures 4.10 through 4.15, the extent of
tetrachloroethene, trichloroethene, and cis-1,2-dichloroethene contamination
above the North Carolina Groundwater Standards has not been delineated. This is
particularly evident for the deep wells. Fracture studies, aquifer tests, and
additional wells are needed to define the vertical and horizontal extent of the
contamination plume.

• Periodic sampling of all wells being used for household purposes within one mile
of the sites is warranted. Based on the results of results of this sampling and full
delineation of the plumes, additional sampling may be needed.

Assessment for General Electric Company LSD/Shepherd Farm, I found the sections
regarding Private Wells Pathway in the last two paragraphs on page 28 to be
confusing and potentially misleading.
In particular, the last paragraph would seem to indicate that no.one
sampled the wells in the vicinity of the Gene Shepherd Farm, when in fact this
office did collect samples from the wells of the concerned neighbors. The
analytical results are enclosed, and the attached map shows the locations of
these wells.
The only wells known to contain volatile organic compounds were those
in! ti ally sampled on or Immediately adjacent to the Shepherd Farm I tselL No,
volatile organic compounds were detected in the wells sampled by this office.
According to General Electric Company, all residents with wells showing
contamination were offered connection to city water.
If I understand that you are saying that no recent sampling has been
performed to provide a more complete database for these residences, may I
suggest you reword the last paragraph on page 28 as follows,
" ... ATSDR knows of no continued sampling data for residents
currently using well water in the viclni ty. Thus the
exposure cannot be estimated."

I have attached the information sheet given to General Electric
Company, Henderson County, for distribution to its neighbors. Also
attached are topo maps, well location diagrams, and initial sample results
from the neighbor's water supplies.
Briefly, our involvement has been as follows:
Initial sampling in March, 1987, of a background monitor well at
General Electric Company revealed the presence of tetrachloroethylene in
concentrations from 750 to 1,000 ug/1 and trichloroethylene from 89 to 140
ug/1. A groundwater investigation was begun and more monitor wells were
installed. In October, 1988, results from the newest monitor wells
indicated levels as high as 2,500 ug/1 of tetrachloroethylene .(monitor well
11).

An upstream sample from Bat Fork Creek also showed 3.1 ug/1 of
tetrachloroethylene. Samples taken at the upstream station were split with
General Electric's Consultant, Law Engineering, on October 11, 1988, and
ranged from 1.7 to 2.7 ug/1 and downstream from 2.6 to 3.6 ug/1. On the
same day, split samples were taken from seven of the nearest neighbors
downstream from General Electric. The results.from those wells are
attached. The highest concentration was at the Harold Willis residence, at
10.0 ug/1 tetrachloroethylene.
General Electric Company has collected samples from at least 42 wells
in the neighborhood. The first seven (WW-1 through WW-7) are those split
on October 11, 1988. Analyses through WW-42 are attached. The locations
of WW-36 through WW-42 are on the attached blueprint map.
