As part of the ongoing effort to construct Cell No. 6B to permitted base grades, areas of rock have been uncovered using conventional excavation equipment. In order to meet the vertical separation requirements from rock within the Cell No. 613 area, blasting will be required. As can be seen on the attached sketches, blasting will be performed inside the boundaries of Future Cell No. 6B. The contractor is estimating that approximately 51,200 CY of material will be removed by the drilling and blasting. This material will be used in the construction of the proposed cell embankment in accordance with the project specifications and design. If the blasting required to achieve design base grades inside of Future Cell 613 does not yield enough rocky fill material for the embankment, the contractor (may) perform additional drilling and blasting activities inside the permitted limits of Borrow Area No. 1 in order to produce the remaining quantity of material needed. This area has also been included on the attached sketch. The blasting will be performed by Carolina Drilling, Inc. under the direction of Earnhardt Grading Inc., a contractor to Republic Services, Inc. A blast plan has been provided for your use. All blasting shall be performed between the hours of sunrise to sunset. During the actual blasting, monitoring will be conducted, and a formal report of this monitoring will be placed in the Facility Operating Record, and a copy will be submitted to you for your records. Proposed seismograph locations have been shown on the attached sketch, and the nearest groundwater monitoring well is MW-11. Holes will be laid on a 7'x7' to 8'x8' box pattern depending on rock depth and density. The average and most used pattern will be the 7'x7' box pattern. 2. Holes will be drilled in a 3.5" diameter maximum with an average depth of 15'. The cuts will range from 2' to 30'. 3. All explosives will be brought in daily as needed and no explosives will be stored on site. Either Quick Supply or Carolina Drilling will haul all explosives in accordance with the NCDOT and USDOT. 4. Dry holes will be loaded with a 1.25"x8" stick of Unimax dynamite and ANFO. Wet holes will be loaded with a booster and Riohit emulsion sticks. All holes will have a minimum stemming of 6'. 5. Only Non Electric blasting caps with 25/500ms factory delays will be used. 6. All holes will be loaded with non -sparking tools (depth gauges and loading poles). 7. The rock in this area is of medium thickness and will only take 3' to 11' of the boreholes to be loaded with explosives. In this case, our average stemming (78m) will be held between 6' and 10'. 8. All holes will be wired with a minimum of 8ms of delay between holes giving us one hole per delay. 9. The holes will then be walked over to make sure all connections are clean. 10. Vibratech will have all seismographs in place prior to blasting. 11. Once the area is cleared by Earnhardt and Carolina Drilling, a series of horns will be sounded. -Three short blasts for 3 minute warning -Two long blasts for 1 minute warning -One long blast for an all clear, back to work, after the shot has been inspected to ensure all holes have fired. Seismology Three Seismographs will be placed with one at the nearest liner, another at the nearest well and a third at the nearest building. Estimated PPV (Peak particle Velocity) One inch per second PPV is the specification for this project. It is extremely hard to break rock at this PPV limit if the targeted PPV reading is within 200', especially with 25' holes in areas. As many as 3 blasting caps per hole will have to be used to get the pounds per delay down to the 10 to 15 Ibs per delay needed to stay below the PPV threshold. Adjustments will be made after each monitored blast so maximum breakage can be achieved. Due to this tight specification, the contractor should expect to have to hammer and crush a good portion more of this rock. Post Blast report or Shot Report A shot report used by Carolina Drilling will be used for this project. It will show the following as required: 1. Material data information on products used. 2. Drawing of shot with hole diameter, depth, voids etc. 3. Loading diagram with weights of explosive used. 4. Amount and type of explosives. 5. Delay and initiation method. 6. Results and yardages. 7. Vibra-tech will furnish seismograph readings via email. 8. A blast damage report if necessary. Safety Plan 1. All employees will wear PPE such as high visibility vest, hard hats, safety glasses, gloves and protective footwear. 2. Blasting zone signs will be placed around the perimeter. 3. Only non -sparking brass tools will be used for pushing powder. 4. Only non -electric blasting caps will be used. 5. 78m stone will be used for stemming to prevent blowout in wet holes. 6. Once the area is cleared by Earnhardt and Carolina Drilling, a series of horns will be sounded. -Three short blasts for 3 minute warning -Two long blasts for 1 minute warning -One long blast for an all clear, back to work, after the shot has been inspected to insure all holes have fired. 7. A section pertaining to misfires will be attached. Carolina Drilling, Inc. Misfire procedure: When a misfire occurs, immediately contact Jason Tutterow at 336-607-4478. If he can't be reached, contact the office at 336- 751-2961. Take time to clear your mind and think about the condition that has been created. Document the hole or area that contains the misfire completely, while this is still fresh in your mind. Do not permit any work in the misfire area. "Danger" off the area. Notify the permittee of the misfire. Do not refire the misfire without permission from an officer of Carolina Drilling. Whenever possible, we recommend that prior to refiring a misfire, another blaster familiar with the mine/quarry is brought in to assist with the decision to fire or render inert the explosive. Experienced individuals familiar with the initiation systems and explosives used, as well as the proper techniques to handle neutralize and render safe the explosive materials should conduct proper misfire handling. Specific recommendations cannot be made concerning misfires, as every misfire is unique and very site specific. Each misfire must be handled individually. All information regarding the misfire must be analyzed completely and a plan of action established with a method to "Make Safe" the area. Specific Federal/State or Local laws may also dictate additional procedures. When a misfire occurs, the power source used to initiate the blast must be disconnected, the firing line shunted or made safe before entering the blast area to inspect the misfire. All personnel must stay out of the blast area for at least 1/2 (30 minutes) hour. Access to the blast area must remain blocked and guarded. Once the blaster in charge and another blaster make a determination familiar with the area of the stability of the area, such as adequate burdens, spacing, stemming, etc., a decision may be made to refire the misfire. Refiring a misfire is usually the safest and best way to eliminate the danger. Extra care must be taken, as the designed pattern HAS changed. Once a determination has been made to refire, the blast area must be cleared to double the initial perimeter (at a minimum). If this is not possible, alternate methods of handling should be considered. The Occupational Safety and Health Administration (OSHA) OSHA Regulations (Standards - 29 CFR) Misfires. - 1926.911 • Standard Number: 1926.911 • Standard Title: Misfires. • Subpart Number: U Subpart Title: Blasting and the Use of Explosives (a) If a misfire is found, the blaster shall provide proper safeguards for excluding all employees from the danger zone. (b) No other work shall be done except that necessary to remove the hazard of the misfire and only those employees necessary to do the work shall remain in the danger zone. (c) No attempt shall be made to extract explosives from any charged or misfired hole; a new primer shall be put in and the hole reblasted. If refiring of the misfired hole presents a hazard, the explosives may be removed by washing out with water or, where the misfire is under water, blown out with air. (d) If there are any misfires while using cap and fuse, all employees shall remain away from the charge for at least 1 hour. Misfires shall be handled under the direction of the person in charge of the blasting. All wires shall be carefully traced and a search made for unexploded charges. (e) No drilling, digging, or picking shall be permitted until all missed holes have been detonated or the authorized representative has approved that work can proceed. The Federal Mine Safety and Health Administration (MSHA) In 30 CFR (Code of Federal Regulations), Part 57.6000, defines a misfire as: The complete or partial failure of explosive material to detonate as planned. The term also is used to describe the explosive material itself that has failed to detonate. A misfire is described as the failure of an explosive charge to detonate. The best advice that can be given regarding the handling of misfires is to take every precaution to prevent their occurrence. Anytime misfired holes, portions of a misfired hole, or unexploded explosive material remains after a blast is fired, a hazardous situation is created that will exist until the proper handling of unfired explosive material. A misfire requires sound judgment and a comprehensive understanding of explosives. Most misfires occur because of improper techniques or short cuts, and sometimes because of the geological formation. It is important that any investigation into a misfire be conducted with a fair and open mind. Any preconceived idea of the cause may mask the true cause, and prevent a future occurrence. MSHA - 30 CFR. PART 57.6311 ADDRESSES THE HANDLING OF MISFIRES AS: (a) Faces and muck piles shall be examined for misfires after each blasting operation. (b) Only work necessary to remove a misfire and protect the safety of miners engaged in the removal shall be permitted in the affected area until the misfire is disposed of in a safe manner. (c) When a misfire cannot be disposed of safely, each approach to the area affected by the misfire shall be posted with a warning sign at a conspicuous location to prohibit entry, and the condition shall be reported immediately to mine management. (d) Misfires occurring during the shift shall be reported to mine management not later than the end of the shift. MSHA - 30 CFR. PART 57.6310 DEFINES THE MISFIRE WAITING PERIOD AS: When a misfire is suspected, persons shall not enter the blast area until: H1 (a) For 30 minutes if safety fuse and blasting caps are used; (b) For 15 minutes if any other type detonators are used.
