Permit Operating Conditions (6) This site is only permitted for the land application of domestic septage, grease trap pumpings, portable toilet waste and commercial / industrial septage. Commercial /industrial septage shall only be land applied after the waste from each source is tested and the results approved by the Solid Waste Section. (Please see item 5 of Additional Comments on page 2 of this report) The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 3 ADDITIONAL COMMENTS 1)The access roads to the disposal area were posted the required signage that was visible and legible. 2)The roads accessing the site were passable.3)Most disposal area boundaries were marked on the ground visibly and distinguishably from the surrounding landscape. Several Markers were missing or knocked down. 4)Discharges were observed outside of the permitted boundaries on the west end of field 3. (Photo set 1 below)5)No applications of septage were made during the site visit. All past applications did not appear to be made in accordance with the nutrient management plan (NMP). Material discharged on parts of field 3 and field 4 did not appear to be domestic septage, portable toilet waste, or grease trap pumpings. Off-white chalky residue having the appearance of lime sludge was observed on large parts of field 3 and 4. All fields had been cut and bailed.6)The warm season crop described by the nutrient management plan was present and was being maintained. The warm season crop on field 3 and 4 appeared to be damaged from heavy traffic on the field during wet conditions and from applications of lime sludge material. Ruts in this area were <3” in depth however, continued traffic in these damaged areas will likely result in a loss of crop coverage below 80%. In addition, the warm season crop was mostly dormant and no evidence of winter overseeding was visible and did not appear to have been completed as prescribed by the NMP. Areas of fields 2,3 and 4 had areas with no actively growing crop stand. Without overseeding as prescribed by the NMP crop coverage will likely fall below 80% with continued applications. Please note: This permit shall become void if the soils fail to adequately assimilate the septage and shall be rescinded unless the site is maintained and operated in a manner which will protect the assigned water quality standards of both surface and ground waters. 7)No septage applications were made during the site visit.8)No objectional odors where noticed.9)Required setbacks appear to be met.10)Land application records were not inspected at the time of the field inspection. 1.
