STATUS OF PERMIT: Active EXPIRES: 612712025 PURPOSE OF SITE VISIT: Routine Inspection STATUS OF PAST NOTED VIOLATIONS: Past observed violations have been resolved. OBSERVED VIOLATIONS: N/A E:-83.3402 The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. Page 1 of 2 Oepartmenl o� Envimnmental0uali� ADDITIONAL COMMENTS FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section 1) This facility consists of one 4,500-gallon, three 12,000-gallon, one 13,700-gallon, one 24,000-gallon, and one 24,500-gallon tanks (including two grease dump tanks, two septage dump tanks, and a dewatering box). The seven storage tanks have a total capacity of 102,700-gallons for the detention of untreated and treated septage. 2) The access road is well maintained, meeting the requirements of 15A NCAC 13B .0838(b). 3) No holes were noted which may allow the escape of septage on tanks currently in use, meeting the requirements of 15A NCAC 13B .0838(c). 4) No vectors were noted at the time of inspection, meeting the requirements of 15A NCAC 13B .0838(e)(2). 5) Some odors were detected around the facility, but none were noted off site, meeting the requirements of 15A NCAC 13B .0838(g). 6) Vegetation which would prevent access for inspection was not noted within 5 feet of the tank per requirements in 15A NCAC 13B .0838(i). 7) Setback requirements appear to have been met as required per 15A NCAC 13B .0838(m).
