STATUS OF PERMIT:
Permit to Operate issued December 29, 1997.
Permit Modification #1, Permit to Operate issued February 1, 2000.
Permit Amendment, Permit to Construct & Operate issued March 28, 2013.
A Life of Site Permit was issued May 8, 2019. The Permit will expire on May 8, 2069.
PURPOSE OF SITE VISIT:
Follow-up Compliance Inspection
STATUS OF PAST NOTED VIOLATIONS:
A. 15A NCAC 13B .0405 (b), "Water that comes into contact with solid waste is leachate and shall be collected
from the site for disposal to an approved facility or discharged directly from the site into a sanitary sewer
line."
RESOLVED: The Leachate Release Sampling Results Report was submitted to Ms. Elizabeth Werner,
Hydrogeologist — Solid Waste Section, on March 20, 2023. Elevated levels of Chromium were identified in all three

soil samples collected during the sampling event. The Chromium was identified in the background sample indicates
that chromium may be found in the soils onsite at concentrations occurring naturally greater than PSRG standards.
Smith Gardner concluded that additional mitigation was not warranted at the site. Ms. Werner concurred that no
additional mitigation was required on March 21, 2023.
OBSERVED VIOLATIONS:
None.
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.
ADDITIONAL COMMENTS
1. The facility is fenced and is secured by a locking gate when it is not open for operation.
2. The correct signage is at the entrance of the facility listing the facility name, permit number and emergency contact
information as well as materials accepted at the facility.
3. The facility accepts waste Monday through Saturday from 7:30 am until 4:30 pm, with the exception of Wednesday
7:30 am until 12:00 pm.
4. The transfer station walls were covered with dirt and grime and are in need of cleaning. As per 15A NCAC 13B
.0405 (11), the transfer station building shall be cleaned with a pressure washer no less than twice per year.
Ensure that the transfer station is cleaned no less than twice per year.
5. Damage to the sheet metal above the transfer trailer loading bay. Mr. Beane stated that the damaged metal would
be removed and replaced with heavy duty flexible material. The repair would be completed prior to the end of the
year.

7. The exterior leachate drains were full of waste and pooled leachate. The interior portions of the leachate drains were
also clogged with waste. Ensure that the leachate drains are cleaned as needed so that leachate can flow into
the leachate collection system.
8. During the April 4, 2023 inspection, leachate was pooling in damaged areas of the floor. During a follow-up
inspection on April 5, 2023, Ms. Thompson was able to take photographs of the damage to the tipping floor prior
to the opening of the facility. Mr. Beane has previously stated that the tipping floor would be repaired prior to the
end of the year. Continue to monitor the tipping floor to ensure that the damage to the floor has not grown
and that leachate has not come into contact with the soils below the tipping floor. The sampling of soils
beneath the tipping floor may be required to determine if leachate has come in contact with the soils.
9. At the time of this inspection, windblown waste was observed outside of the transfer station building, within the
trailer loading bay, and within the trailer staging area. Mr. Beane was reminded that windblown waste must be
picked up and returned to the transfer station prior to the close of the business day. Mr. Beane provided the Section
with photographs of the area after the area had been cleaned up. Ensure that the windblown waste is collected
and returned to the transfer station prior to close of business daily.
10. One trailer in the trailer staging area was identified to be leaking. Troughs had been placed underneath the trailer to
catch the leachate. One trailer at the eastern end of the staging area was identified to be leaking. A trough had not
been placed underneath this trailer. Mr. Beane was notified of the leak and informed that a follow-up inspection
would be completed the following morning to confirm that the leachate-stained soil had been removed from the
area. During the follow-up inspection, Ms. Thompson observed that a trough had been placed underneath the leaking
trailer. The leachate-stained soil was removed from the facility on April 5, 2023, after the trailer had been taken to
the landfill for proper disposal. Ensure that the trailers are inspected frequently to identify possible failures
where leachate can leak from the trailer. If a trailer is found to be leaking, immediately place troughs
underneath to catch the leachate. Full trailers may need to be inspected frequently prior to leaving the facility
in order to identify and contain leaks.

11. During the April 4, 2023 inspection, several areas of soil stained with hydraulic fluid were observed to the west of
the tipping floor apron. Mr. Beane stated that some of the operating equipment had been leaking hydraulic oil. It
was recommended that the equipment be parked on the tipping floor overnight so that the hydraulic oil could be
cleaned up.
12. Metals and white goods were of manageable size.
13. Digital photos were taken during the inspection.
