STATUS OF PERMIT:
4302-LCID-1998 Permit to Operate issued May 26, 2015. Permit expired on May 26, 2020. Permit renewal paperwork
was submitted May 22, 2020.
Life of Site Permits issued for 4302-CDLF-1998 and 4302-CDLF-2018 on July 23, 2020. The Life of Site Permits will
expire on March 2, 2058.
PURPOSE OF SITE VISIT:
Phase 2A Pre -Operation Meeting and Partial Compliance Inspection

During this inspection, leachate was observed to have been pooling at the base of the western side slope. The leachate
had also flowed along the roadway to a drainage ditch. The Section did not follow the leachate flow along the drainage
ditch. The LCID must be covered and sloped to prevent ponding water and the formation of leachate.
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.
ADDITIONAL COMMENTS
Phase 2A Pre -Operation Meeting
1. This meeting was held to satisfy the preconstruction meeting requirement as identified in Attachment 2, Part 1 (5).
The Section Permitting Engineer and Section Hydrogeologist for the facility were also in attendance.
2. Some windblown waste was observed within the new cell, Phase 2A.
3. Mr. Beane stated that the southern portion of Phase 2A, did not meet the CQA requirements and had been staked
off until requirements could be met.
4. A drainage ditch had been dug along the western edge of Phase 2A and the new borrow area to the south of Phase
2A. Erosion was observed at the southern end of the drainage pipe underneath the access roadway between Phase
2A and the borrow area. The drainage ditch had not been seeded nor had riprap been placed in the drainage ditch.
During the meeting, it was recommended that riprap be placed at the southern end of the drainage pipe beneath the
roadway and the drainage ditch be seeded and have areas of riprap to help prevent erosion of the channel. The
drainage ditch did not continue around the borrow area and into the sediment basin for Phase 2A. During the
meeting, it was recommended that Mr. Beane contact DEMLR to ensure that the erosion controls have been
constructed properly.
5. An area of erosion was also observed near the southeastern corner of the natural drainage of the borrow area. This
erosion indicated that the drainage of the borrow area was not connected to the stormwater basin. During the
meeting, it was recommended that Mr. Beane contact DEMLR to ensure that the erosion controls have been
constructed properly.
6. An area of charred vegetative debris was identified within the sediment basin adjacent to the borrow area. Mr. Beane
stated that they had obtained a burn permit from Forestry and had burned the clearing waste from the borrow area
several months prior. The Section stated that Mr. Beane would need to contact Permitting and obtain permission to
burn waste at the landfill.
7. Windblown waste was observed around the edges of Phase 2A and the borrow area. Several pieces of metal and
plastic waste were observed within Phase 2A. Mr. Berube picked up the metal waste during the inspection. Ensure
that windblown waste is collected and returned to the working face prior to the end of the operating day.
Facility Inspection
8. The facility is fenced and secured by a locking gate when it is not open for operation.
9. The correct signage is at the entrance of the facility listing the facility name, permit number and emergency contact
information as well as materials accepted at the facility.
10. The facility accepts waste Monday through Saturday from 7:30 am until 4:30 pm, with the exception of Wednesday
7:30 am until 12:00 pm.
11. Paperwork was not reviewed during this inspection.
12. Edge of waste markers were observed around the CDLF Area 2 and Phase 2A, LCID, and asbestos area.
13. The CDLF Area 1, Asbestos Disposal Area, and Closed MSWLF were observed to have overgrown grassy and
woody vegetation. Ensure that the landfill areas are mown as often as necessary and the woody vegetation
removed to allow for proper inspection.

14. Two monitoring wells were identified to be unlocked during this inspection. It was recommended that the facility
check and confirm that the monitoring wells are locked after sampling events. Ensure that the monitoring wells
are locked and secured.
CDLF Area 2
15. The working face was approaching the %2 acre size limit. Six (6) inches of earthen material should be properly
applied once weekly or when the working face reaches 1/2 acre. Ensure that six inches of cover is applied once
weekly or when the working face reaches '/2 acre in size, whichever occurs first.
16. Little vegetation and numerous erosion rills were identified on the side slopes of CDLF Area 2. Some exposed
waste was also observed near the erosion in the side slopes. Mr. Beane stated that wood waste from the LCID
area would be ground and used to help stabilize and facilitate vegetation growth on the side slopes of CDLF Area
2. Monitor the CDLF Area 2 for erosion and exposed waste and repair any problem areas as they are
observed.
17. No unacceptable wastes were observed within the waste stream.
LCID
18. The top of the LCID was being used to store uncovered vegetative waste. Mr. Beane stated that the land clearing
waste on the LCID would be ground and used for stabilization on CDLF Area 2.
19. The side slopes of the LCID were largely uncovered. Ensure that the LCID is covered with six inches of soil
monthly or when the working face reaches one acre in size, whichever occurs first.
20. Some unacceptable wastes, such as plastic piping and landscaping fabric, were observed within the waste stream.
Ensure that unacceptable wastes are removed from the waste stream and properly disposed of.
21. In accordance with 15A NCAC 13B .0564 (7), the facility must maintain an access roadway around the LCID.
