B. Site History 1. Provide UST Owner and Operator Information

4. Describe relation of location of current UST system to historical UST releases.
A previous release, UST incident #45093, was discovered when on-site testing of the diesel UST system found diesel product in the submersible turbine pump (STP) sump for diesel tank #3 and the sump was noted to be in need of repair or replacement. Laboratory analytical results
of a soil sample collected in proximity to the diesel tank #3 STP sump detected a C9-C22
aromatics concentration above the Soil-to-Groundwater Maximum Soil Contaminant Concentrations (STGW-MSCCs). Following assessment activities, the site was ranked as low risk and a Notice of Residual Petroleum (NORP) was recorded for the property. The incident was issued a Notice of No Further Action on June 29, 2020.
5. Provide a Brief Description of Site Characteristics The subject property consists of one convenience store building, two fast food restaurants, and petroleum fueling equipment. The subject property and surrounding area are fairly level. The
subject property is located in a light commercial area in proximity to Interstate-85/40 of Haw
River. Public water is supplied to the area. A topographic map and site map are attached as
Figures 1 and 2, respectively. C. Site Check Procedures
1. UST System Testing On July 19, 2022, a high-water alarm was actuated in the pit of the CAT scale. Upon inspection, “fresh” diesel fuel was noted in the pit and the water/fuel was pumped out for proper disposal. On July 20, 2022, ATC, on behalf of Pilot Travel Centers, submitted an
UST-61 24-Hour Release and UST Leak Reporting Form. A copy of the referenced
notification is provided in Attachment I. 2. Site Check Procedure On July 25, 2022, the North Carolina Department of Environmental Quality (NCDEQ) issued a
Notice of Regulatory Requirements requesting the performance of site check activities at the
site. On August 4, 2022, ten soil borings were advanced around the CAT scale, diesel dispensers, and diesel UST basin. Based on the results of the limited soil assessment, three monitoring wells (MW-3, MW-4, and MW-5) were installed at the facility. Following the

installation of the monitoring wells, groundwater samples were collected from the newly installed monitoring wells and existing monitoring well MW-2. Results of the soil and groundwater sampling are provided in Section D.
3. Soil Excavation Activities Soil excavation was not performed, as part of this site check. D. Site Investigation
1. Field Screening Soil samples were collected at 2.5-foot intervals from surface to boring termination depth at 15 feet below ground surface (bgs) or shallower, depending on sampling location. Each soil sample was screened with a photoionization detector (PID) during boring advancement. The respective PID readings are provided on the boring logs provided in Attachment II.
2. Soil Sampling Information On August 4, 2022, ATC supervised AmeriProbe of Huntersville, North Carolina to advance ten soil borings in proximity to the CAT scale, diesel dispensers, and diesel UST to attempt to determine the source of diesel fuel in the CAT scale sump. Soil borings SB-1 through SB-10
were advanced using macro-core samplers. The soil samples (labeled SB-1 through SB-10)
were collected at depths ranging from 2.5 to 5 feet bgs and 10 to 12.5 feet bgs. The sampling depth was determined as the sampling interval with the highest PID reading above the saturated zone. The soils encountered at the sampling depth varied. Soil boring information is provided in Table 3. The locations of the soil borings are provided on Figure 3 and boring logs are
provided in Attachment II.
The soil samples were shipped to a N.C. certified laboratory in a chilled cooler following proper chain-of-custody procedures. The soil sample was requested to be analyzed by TPH-Gasoline Range Organics (GRO) and TPH-Diesel Range Organics (DRO).
3. Groundwater and Surface Sampling Information Surface water samples were not collected during site check activities. Well Installations
On September 12, 2022, ATC supervised Carolina Soil Investigations, LLC (CSI) of Olin,
North Carolina in the installation of three monitoring wells at the site (monitoring wells MW-3, MW-4, and MW-5). Monitoring well MW-3 was installed in the location of soil boring SB-2, monitoring well MW-4 was installed in the location of soil boring SB-6, and monitoring well MW-5 was installed in proximity to the location of soil boring SB-8. It should be noted
monitoring well MW-2 was installed in 2021 during previous site activities. The locations of
the monitoring wells are depicted on Figure 2. Monitoring wells MW-3 and MW-4 were installed to a depth of 22 feet bgs and monitoring well MW-5 was installed to a depth of 20 feet bgs, using solid stem augers. Monitoring wells
MW-3, MW-4, and MW-5 were constructed with 15 feet of 2-inch diameter PVC 0.010-inch
slotted well screen. The wells were developed following well installations. The wells were installed as flush-mounted and provided with sealing and locking caps. A protective, steel,

traffic-bearing cover was placed over each well upon completion. The boring logs and well construction records are provided in Attachment II. Groundwater Elevation
On September 28, 2022, ATC collected static water level measurements from monitoring wells MW-2 through MW-5. Depths to water measurements were collected using an electronic oil/water interface probe. LNAPL was measured in monitoring well MW-3, at a thickness of 4.38 feet on this date.
The groundwater depths were converted to relative elevations using an assumed datum of 100 feet relative to an arbitrary on-site benchmark. Monitoring well construction details and current groundwater elevation data are included in Table 4. A groundwater elevation contour map, based on the September 2022 static water level data, is included as Figure 4. Based on the groundwater level measurements, groundwater appears to flow to the south across the site.
Groundwater Sampling On September 28, 2022, groundwater samples were collected from wells MW-2, MW-4, and MW-5. Monitoring well MW-3 could not be sampled, as it had 4.38 feet of LNAPL present. Prior to sample collection, the respective well volumes were calculated, and a minimum of
three well volumes were purged from each well column, to ensure replacement of stagnant
water with representative formation water. The samples were collected directly into laboratory-approved containers, packed in ice, and delivered under chain-of-custody to Pace Analytical in Mount Juliet, Tennessee, for analyses.
The groundwater samples were analyzed by Environmental Protection Agency (EPA) Method
602, EPA Method 625 plus ten highest non-target peaks, and Massachusetts Department of Environmental Protection (MADEP) Methods for volatile petroleum hydrocarbons (VPH) and extractable petroleum hydrocarbons (EPH). It should be noted the groundwater sample collected from monitoring well MW-4 could not be analyzed by MADEP method for EPH
because the well did not recharge.
4. Quality Control Measures The soil and groundwater samples were placed in laboratory provided jars and vials. The samples were immediately placed on ice and contained in a cooler during transportation. The
samples were transported by courier under a chain-of-custody to the laboratory for analysis.
5. Investigation Results Soil Laboratory analytical results of the soil samples collected detected TPH-DRO concentrations
above the established State Action Level for the following samples:
• SB-1 (2.5-5’)
• SB-2 (2.5-5’)
• SB-6 (2.5-5’)
• SB-9 (10-12.5’) A summary of the soil analytical results is provided in Table 3. Petroleum-related impacts to
soil are depicted on Figure 3. A copy of the laboratory report and chain-of-custody documents
are provided in Attachment III.

Groundwater Laboratory analytical results of the groundwater samples collected from monitoring wells MW-4 and MW-5 detected dissolved C9-C22 Aromatics concentration above the established 2L Standard. LNAPL was detected in monitoring well MW-3. Laboratory analytical results of the
groundwater sample collected from monitoring well MW-2 did not detect dissolved target compound concentrations above laboratory method detection limits (MDLs). A summary of the groundwater analytical results is provided in Table 5. Dissolved contaminants of concern are depicted on Figure 5. A copy of the laboratory report and chain-of-custody documents are provided in Attachment III.
E. Conclusion and Recommendations Petroleum-related impacts to soil and groundwater were identified during site check activities. LNAPL was detected in monitoring well MW-3, at a thickness of 4.38 feet. It appears the source of the diesel fuel is the diesel dispensers and/or product line. ATC is currently in the
process of scheduling an Aggressive Fluid Vapor Recovery (AFVR) event to address the

CASE NARRATIVE
All sample aliquots were received at the correct temperature, in the proper containers, with the
appropriate preservatives, and within method specified holding times, unless qualified or notated within
the report. Where applicable, all MDL (LOD) and RDL (LOQ) values reported for environmental samples
have been corrected for the dilution factor used in the analysis. All Method and Batch Quality Control
are within established criteria except where addressed in this case narrative, a non-conformance form
or properly qualified within the sample results. By my digital signature below, I affirm to the best of my
knowledge, all problems/anomalies observed by the laboratory as having the potential to affect the
quality of the data have been identified by the laboratory, and no information or data have been
knowingly withheld that would affect the quality of the data.

GLOSSARY OF TERMS
Guide to Reading and Understanding Your Laboratory Report
The information below is designed to better explain the various terms used in your report of analytical results from the Laboratory. This is not
intended as a comprehensive explanation, and if you have additional questions please contact your project representative.
Results Disclaimer - Information that may be provided by the customer, and contained within this report, include Permit Limits, Project Name,
Sample ID, Sample Matrix, Sample Preservation, Field Blanks, Field Spikes, Field Duplicates, On-Site Data, Sampling Collection Dates/Times, and
Sampling Location. Results relate to the accuracy of this information provided, and as the samples are received.
Abbreviations and Definitions
MDL Method Detection Limit.
ND Not detected at the Reporting Limit (or MDL where applicable).
RDL Reported Detection Limit.
Rec.Recovery.
RT Retention Time.
RPD Relative Percent Difference.
SDG Sample Delivery Group.
(S)
Surrogate (Surrogate Standard) - Analytes added to every blank, sample, Laboratory Control Sample/Duplicate and
Matrix Spike/Duplicate; used to evaluate analytical efficiency by measuring recovery. Surrogates are not expected to be
detected in all environmental media.
U Not detected at the Reporting Limit (or MDL where applicable).
Analyte The name of the particular compound or analysis performed. Some Analyses and Methods will have multiple analytes
reported.
Dilution
If the sample matrix contains an interfering material, the sample preparation volume or weight values differ from the
standard, or if concentrations of analytes in the sample are higher than the highest limit of concentration that the
laboratory can accurately report, the sample may be diluted for analysis. If a value different than 1 is used in this field, the
result reported has already been corrected for this factor.
Limits
These are the target % recovery ranges or % difference value that the laboratory has historically determined as normal
for the method and analyte being reported. Successful QC Sample analysis will target all analytes recovered or
duplicated within these ranges.
Original Sample The non-spiked sample in the prep batch used to determine the Relative Percent Difference (RPD) from a quality control
sample. The Original Sample may not be included within the reported SDG.
Qualifier
This column provides a letter and/or number designation that corresponds to additional information concerning the result
reported. If a Qualifier is present, a definition per Qualifier is provided within the Glossary and Definitions page and
potentially a discussion of possible implications of the Qualifier in the Case Narrative if applicable.
Result
The actual analytical final result (corrected for any sample specific characteristics) reported for your sample. If there was
no measurable result returned for a specific analyte, the result in this column may state “ND” (Not Detected) or “BDL”
(Below Detectable Levels). The information in the results column should always be accompanied by either an MDL
(Method Detection Limit) or RDL (Reporting Detection Limit) that defines the lowest value that the laboratory could detect
or report for this analyte.
Uncertainty
(Radiochemistry)Confidence level of 2 sigma.
Case Narrative (Cn)
A brief discussion about the included sample results, including a discussion of any non-conformances to protocol
observed either at sample receipt by the laboratory from the field or during the analytical process. If present, there will
be a section in the Case Narrative to discuss the meaning of any data qualifiers used in the report.
Quality Control
Summary (Qc)
This section of the report includes the results of the laboratory quality control analyses required by procedure or
analytical methods to assist in evaluating the validity of the results reported for your samples. These analyses are not
being performed on your samples typically, but on laboratory generated material.
Sample Chain of
Custody (Sc)
This is the document created in the field when your samples were initially collected. This is used to verify the time and
date of collection, the person collecting the samples, and the analyses that the laboratory is requested to perform. This
chain of custody also documents all persons (excluding commercial shippers) that have had control or possession of the
samples from the time of collection until delivery to the laboratory for analysis.
Sample Results (Sr)
This section of your report will provide the results of all testing performed on your samples. These results are provided
by sample ID and are separated by the analyses performed on each sample. The header line of each analysis section for
each sample will provide the name and method number for the analysis reported.
Sample Summary (Ss)This section of the Analytical Report defines the specific analyses performed for each sample ID, including the dates and
times of preparation and/or analysis.
Qualifier Description
J The identification of the analyte is acceptable; the reported value is an estimate.
J2 Surrogate recovery limits have been exceeded; values are outside lower control limits.
J3 The associated batch QC was outside the established quality control range for precision.
J6 The sample matrix interfered with the ability to make any accurate determination; spike value is low.
N The analyte is tentatively identified and the associated numerical value may not be consistent with the actual concentration present in the sample.
