The purpose of this LSA is to further characterize the extent and magnitude of a petroleum release that was detected during the closure of three (3) 4,000 gallon petroleum underground storage tanks (USTs) in October, 2016. The LSA is being conducted to met the requirements of North Carolina Department of Environmental Quality Rule 15A NCAC 02L .0505. During the LSA, one (1) soil boring was completed, soils were screened for volatile organic vapors using a photoionization detector (PID), and three (3) soil samples were submitted for laboratory analysis. The results of our investigation revealed the following:  Groundwater was not encountered in the soil boring;  Benzene was detected in soil sample SB-1, 48-50’ at a concentration that exceeds the Soil-to- Water Maximum Contaminant Concentration (MSCC) by 0.0001 milligram per kilogram;  Soil contamination does not exceed the residential MSCC; and  The petroleum release does not pose a threat of explosion due the accumulation of vapors in a confined space nor does the release pose a risk to public health or the environment. MCE recommends a “No Further Action Required” (NFA) status for this site based on the data collected during this Limited Site Assessment. The scope of this assessment does not purport to encompass every report, record, or other form of documentation relevant to the property being evaluated. To the best of our knowledge, this subsurface investigation has been performed in compliance with ASTM Standard E1903-11 for Phase II Environmental Site Assessments. This investigation was performed at the request of Mr. Nick Wehrmann. All tasks were completed in general accordance with the American Society for Testing and Materials (ASTM) Standard Practice E 1903-11. 2.1 Purpose The purpose of this LSA is to further characterize the extent and magnitude of a petroleum release that was detected during the closure of three (3) 4,000 gallon petroleum underground storage tanks (USTs) in October, 2016. The LSA is being conducted to met the requirements of North Carolina Department of Environmental Quality (NDDEQ) Rule 15A NCAC 02L .0505. 2.2 Scope of Work The scope of this investigation was directed specifically to address the potential for soil or groundwater contamination resulting from the release identified during the October 2016 tank closure at the subject property. The following tasks were completed for this investigation:  TASK 1- One (1) soil boring was completed at the subject property to a depth of 50 feet below ground surface (BGS).  TASK 2- Headspace vapor analysis was conducted on soil samples from the borehole at five-foot intervals from 0 to 30 feet and at two-foot intervals from 30 to 50 feet to determine if volatile organic vapors were present;  TASK 3- Soil samples were collected from the soil boring and submitted for laboratory analysis; and,  TASK 4- This report was prepared to document MCE’s findings and offer recommendations for further action as needed to address site conditions. All work was completed under the direction of a North Carolina Professional Geologist. 2.3 Limitations The report and any other information which MCE prepared and submitted to Mr. Nick Wehrmann of Nick Motor Cars, (the user), in connection with this project (the “Report”), are for the sole use and benefit of the user and may not be used or relied upon by any other person or entity without the prior written consent of the user and MCE, except as provided for specifically in the agreement. The Wilkes County Tax Assessors Office identifies the subject property as Parcel ID 1505745, totaling 1.25-acres. Ground cover at the subject property is concrete, asphalt, gravel, and bare soil. The subject property is currently a Meineke auto shop. Three (3) steel 4,000 gallon petroleum USTs were removed from the subject property by Seramur & Associates between October 11 and October 13, 2016. During removal of the USTs, small holes were observed on the bottoms and sides of the tanks. Gasoline and diesel range organic compounds were detected in the soils beneath each tank. According to an Initial Abatement Action Report completed by Seramur & Associates, the USTs removed from the subject property were installed as early as the 1950s and were not registered. A Site Location Map is provided as Figure 1. A summary of the site history is presented in Table 1a and 1b. The Initial Abatement Action Report for the release is provided in Appendix A. 4 RECEPTOR INFORMATION 4.1 Public and Private Water Supply Wells MCE conducted a well survey to identify public and private water supply wells within 1,500 feet of the subject property. MCE identified one (1) private water supply well located approximately 425 feet to the northwest of the subject property. The well is located at a private residence at 2154 Congo Road. Due to the distance and topographically up-gradient position of the well from the subject property, the probability this water supply well being impacted by the release at the subject property is low. MCE also searched the United States Geological Survey National Water Information System and found no ground water wells within 1,500 feet of the subject property. All residents and businesses within the surveyed area are supplied with municipal water. Millers Creek is located approximately 1,060 feet to the northeast of the subject property and unnamed tributary to Yadkin River is located approximately 1,270 feet to the southwest of the subject property. Due to the distance from the subject property, the release is unlikely to impact these waters. A Risk Classification and Land Use Form is provided in Appendix B. 5 SITE GEOLOGY AND HYDROGEOLOGY The subject property is situated in the Piedmont Physiographic Province of North Carolina. Rock types of the Piedmont generally include gneiss, schist, granite, and amphibolite. These rocks weather to form a reddish-brown soil called residuum, which consists of various amounts of sand, clay, and silt mixtures. The upper boundary of the Piedmont groundwater system is formed by the water table. The water table is usually situated within the residuum, although it can be found within the bedrock in areas where the residuum is thin. The configuration of the water table is usually a subdued replica of the site topography. Generally, groundwater in the Piedmont flows from highlands to lowlands and discharges to streams, lakes, or wetlands. The direction of groundwater flow at the subject property is unknown, but based on the topography of the subject property the general direction of groundwater flow in the area is estimated to be to the northeast. 6 SUBSURFACE INVESTIGATION PROCEDURES MCE conducted the subsurface investigation at the subject property on January 18, 2017. The following sections include a summary of the activities completed and a discussion of the results of our investigation. 6.1 Subsurface Investigation Methodology On January 18, 2017, a Geoprobe 7822DT drill rig was used to advance a 2.25-inch diameter boring in one (1) location (SB-1) at the subject property. Continuous soil samples were collected at five-foot intervals from 0 to 30 feet with a dual-tube sampler and at two-foot intervals from 30 to 50 feet with a split spoon sampler. 6.2 Boring Location Rationale SB-1 This boring location was chosen to assess conditions in the vicinity of the former UST basin. Ground cover at this location was concrete and soils in this boring were primarily classified as clayey fill material grading to banded saprolite. Soil boring locations are shown on Figure 2. Boring logs are included in Appendix C. Limited Site Assessment Nick’s Motor Cars 3384 West US Highway 421, Wilkesboro, NC 6 P:\Nicks Motor Cars\NAC-1701-1\LSA\Report February 16, 2017 6.3 Soil Sampling Procedures A headspace vapor analysis for volatile organic vapors was conducted for each sample interval. Soil samples were collected for laboratory analysis from the sample intervals based on the highest PID readings. Based on the absence of groundwater within the boring, an additional soil sample was collected from the deepest interval within the boring. Upon completion of the boring, soil samples were placed into laboratory-provided containers, sealed, labeled, packed on ice, and delivered to Pace Analytical in Huntersville, North Carolina under standard chain of custody procedures. The soil samples collected were analyzed for extractable petroleum hydrocarbons (EPH) by Massachusetts Department of Environmental Protection (MADEP) method EPH, volatile petroleum hydrocarbons by MADEP method VPH, volatile organic compounds (VOCs) by EPA Method 8260, and semi-volatile organic compounds (SVOCs) by EPA Method 8270. A total of three (3) soil samples were submitted for analysis. 7 SUBSURFACE INVESTIGATION RESULTS 7.1 Soil Sample Analytical Results Laboratory analytical data indicated that the VOC constituent benzene was detected in soil sample SB-1, 48-50’ at a concentration of 0.0057 milligrams per kilogram (mg/kg). This concentration exceeds the NCDEQ Maximum Soil Contaminant Concentration (MSCC) levels’ Soil-to-Water Maximum Contaminant Concentration of 0.0056 mg/kg. The VOC constituents ethylbenzene, n-propylbenzene, toluene, 1,2,4-trimethylbenzene, 1,3,5-trimethylebenzene, and xylenes were detected in soil sample SB-1, 48-50’ at concentrations below their applicable MSCC level. The VOC constituents ethylbenzene, methyl-tert-butyl ether, napthalene, toluene, 1,2,4- trimethylbenzene, 1,3,5-trimethylebenzene, and xylenes were detected in soil sample SB-1, 38- 40’ at concentrations below their applicable MSCC level. The EPH constituent aliphatic (C19-C36) was detected in soil sample SB-1, 48-50’ at a concentration below the applicable MSCC level. Summaries of the VOC and EPH detections are included in Table 2 and 3. The Laboratory Analytical Report is provided in Appendix D. The three USTs (1, 2 and 3) were previously used at the property by Tommy Turner as early as the 1950’s. These were three 4,000 gallon petroleum (gasoline or diesel) USTs. A review of the UST database showed that these USTs were never registered because they were taken out of service reportedly at some time in the 1970’s according to local informants. No release incident number has been associated with these tanks. 3. There are no known spills or releases from non-UST sources at the subject property prior to the release discovered 10/11/16. 4. Provide a comprehensive description of the release, including date discovered, cause and source (including tank identification number and contents), and the relationship of historical UST releases, non-UST releases, and off-site releases (indicate incident number) to contamination from current release. The USTs were permanently closed by removal between October 11 and October 13, 2016. During removal of the tanks, it was noted that there were small holes in several spots on the tanks on the bottoms and sides. Petroleum odors and staining were observed on the sides of the tanks and under the tanks. Gasoline range organics and diesel range organics were detected under each tank. Diesel range organics were found in all three samples under the middle tank (UST 2) and it is suspected that this was the diesel storage tank. Seramur & Associates is not aware of any non-UST releases or off-site releases that could be contributing to contamination from current release. 4 5. Provide a brief description of site characteristics (including status of facility (active or inactive), land use of site and surrounding area, water supply, topography, vegetation, surface water, wells, buildings, surface cover, soil type, depth to and nature of bedrock, depth to groundwater, direction of groundwater flow, etc.) The USTs were taken out of service reportedly at some time in the 1970’s according to local informants. The property is currently used as a body shop/auto restoration facility. Surrounding land use is commercial and residential. A public water supply is available to some properties in the area and a water supply well was observed on at least one nearby property. Groundwater was not observed in the UST excavations. The property is located between two ridges and adjacent to a valley that slopes to the east (Figure 1). Miller’s Creek runs in a northwest to southeast direction in a valley east of the property. The creek is approximately 1,000 feet from the UST excavation at its closest point. There is very little vegetation on the property apart from grass near the highway and brush along the backside of the property. Roots were observed in the tank pit, some as large as tree roots. The soil consists of sandy silt in the source area. Bedrock was not encountered and depth to bedrock is unknown. Bedrock in this area is mapped as the Ashe Formation, primarily a mica schist interbedded with a gneiss (Rankin, D.W., Espenshade, G.H., and Neuman, R.B., 1972, Geologic Map of the West Half of the Winston-Salem Quadrangle, North Carolina, Virginia, and Tennessee. Misc. Geol. Invest. Map I-709-A Dept. of the Interior, U.S. Geol. Surv., Washington D.C., 1 Map.). 6. Summarize initial abatement actions, assessment activities, and corrective actions performed to date and list all reports previously submitted. The USTs were permanently closed by removal between October 11 and October 13, 2016. Each trackhoe bucket of soil removed from the two UST excavations was screened in the field for petroleum vapors with a Photocheck 100 photoionization detector. Approximately 42 cubic yards of contaminated soil was removed from around the USTs. Soil contamination appeared to be primarily limited to the area around and under the tanks. Contaminated soil was transported to Environmental Soils, Inc. in Lattimore, NC. Soil consisted of sandy silt to a depth of about 9 feet. Three soil samples were collected from below each of the three USTs (Figure 4). Another composite sample was collected from the two piles of contaminated soil. All soil samples were analyzed for gasoline range organics and diesel range organics (EPA Method 8015C, TPH GRO/TPH DRO). The only previous report prepared for NCDENR was the 24-Hour UST-61 Report. C. Site Check Report A site check report has not been completed. D. UST Closure Report The UST-2 form is included in Appendix A. 1. Describe preparations for closure including steps taken to notify authorities, permits obtained, and steps taken to clean and purge the tanks. A Utility locate request was made to NC One-Call. Seramur & Associates personnel made site visits to mark the proposed excavation area. The atmosphere in the USTs was checked for explosive vapors with an Explosimeter. The Lower Explosive Limit (LEL) readings for the gasoline USTs were below 10%. The atmosphere inside the USTs were rendered inert with carbon dioxide by placing dry ice (1.5 lbs./100 gallons) in the tanks on October 12 and 13, 2016. A UST closure permit was obtained from the County Fire Marshal. A Health and Safety Plan was prepared for the closure work and a safety meeting was conducted in the morning prior to commencing work. 5 Residual liquids (water w/some fuel) were removed from UST 3 by Petroserve, Inc. prior to the closures. 973 gallons of a gasoline/water mix were removed from the tank. UST 1 and UST 2 were empty. The tanks were excavated and removed from the excavation. Fred Reeves Construction transported the three tanks to their construction yard for cleaning and preparation for recycling. 2. Describe the closure procedure referencing site and sampling location maps and cross-sections presented in Section J of this report. Clearly state how the selection of sample types and locations was influenced by the nature of the stored substance, the type of initial alarm or cause for suspicion, the type of backfill, the depth to groundwater, and other factors appropriate for identifying the presence and source of a release. If presence of groundwater or bedrock require the installation and sampling of monitoring well(s), describe the process. Soil was excavated from the sides and one end of each UST. The USTs were then removed from the excavation using chains and the trackhoe. Soil samples were collected from below each UST (Figures 4 and 5). One additional composite soil sample was collected from the contaminated soil stockpiles. These soil samples were analyzed for gasoline range organics and diesel range organics (EPA Method 8015B, TPH GRO/TPH DRO). Soil samples from the excavation were collected using both the trackhoe bucket and decontaminated hand auger buckets. The extent of the excavation and locations of soil samples are shown on Figures 3, 4 and 5. All soil samples were collected while wearing a new pair of disposable Nitrile gloves. The soil samples were placed in containers provided by the lab, labeled and placed on ice. A chain of custody was initiated and the samples were transported to Prism Labs Inc. in Charlotte, NC for analysis. The selection of sample types and locations was not influenced by the nature of the stored substance. Depth to groundwater is not known. There was no evidence of groundwater in the excavation. The UST excavation was backfilled with clean soil from the excavation as well as clean fill obtained by Fred Reeves Construction. 3. Note the amount of residual material pumped from the tank and describe the storage, sampling and disposal of the residual material and the disposal of the tank, pumps and piping. Residual fuel was removed from UST 3 by Petroserve, Inc. prior to the closures. 973 gallons of a gasoline/water mix were removed from the tank. UST 1 and UST 2 were empty. Soil excavation was limited to the volume of soil that was necessary to remove the USTs from the ground. Three USTs were closed and removed from the site. Contaminated soil associated with the leakage from the tanks was stockpiled onsite. Approximately 42 cubic yards of contaminated soil was stockpiled onsite and later transported to Environmental Soils, Inc. in Lattimore, NC. Elevated concentrations of gasoline range organics and diesel range organics were detected in samples S-1 and S-4. Gasoline range organics were detected above the NCDENR action level in all samples collected from under the tanks except for S-8 and S-10. If petroleum vapors were detected in soil removed from the excavation, it was stockpiled onsite and later transported to a remediation facility. 6. Document site investigation. The trackhoe buckets of soil removed from the UST excavation were screened in the field for petroleum vapors with a Photocheck 100 photoionization detector. The photoionization detector is calibrated with a 100-ppm isobutylene calibration gas mixture. Soil containing petroleum vapors was stockpiled onsite. Soil consisted of sandy silt to a depth of about 9 feet. Soil sample locations are shown on Figures 4 and 5. North-South and East-West cross-sections of the UST system are shown in Figure 5. Soil samples were collected on October 12 and 13, 2016. The sample time is listed on the Chain of Custody Record. Depth to groundwater is not known. There was no evidence of groundwater in the excavation. The UST excavation was backfilled with clean fill from the tank pit and clean fill obtained by Fred Reeves Construction. The hand auger buckets were decontaminated with non-phosphate detergent, distilled water and isopropanol. Horizontal location and depth of the soil samples was measured with a fiberglass tape, estimating right angles and referencing site features. Soil samples were stored on ice and shipped to Prism Labs in Charlotte, NC via Federal Express. Elevated concentrations of gasoline range organics and diesel range organics were detected in samples S-1 and S-4. Gasoline range organics were detected above the NCDENR action level in all samples collected from under the tanks except for S-8 and S-10. Diesel range organics were detected above the NCDENR action level only in samples S-1, S-4 and S-9. 7. Present conclusions and recommendations, referencing maps The source of soil contamination was the three USTs. Petroleum constituents were detected at concentrations above the NCDENR action levels in the base of the UST excavation. Elevated concentrations of gasoline range organics and diesel range organics were detected in samples S-1 and S-4. Gasoline range organics were detected above the NCDENR action level in all samples collected from under the tanks except for S-8 and S-10. Diesel range organics were detected above the NCDENR action level only in samples S-1, S-4 and S-9. Free product has not been encountered at the site. The next assessment phase required by NCDEQ would be an LSA report. E. Free Product Investigation and Recovery Report Free product was not observed during the UST closure. F. Groundwater and Surface Water Investigation There was no groundwater or surface water investigation completed during the UST closure. G. Initial Response and Abatement Action Contaminated soil associated with the release from the USTs was stockpiled onsite. Approximately 40 cubic yards of contaminated soil was stockpiled. Nine soil samples (S-1 through S-6 and S-8 through S-10) were collected from within the excavation for TPH GRO/DRO analysis. One additional composite sample was collected from the soil stockpiles. A 24-hour report was prepared for NCDENR. 7 H. Excavation of Contaminated Soil A petroleum odor and staining was noted around the tanks for the three USTs. The Lower Explosive Limit (LEL) readings for the gasoline USTs were below 10%. The atmosphere inside the USTs were rendered inert with carbon dioxide by placing dry ice (1.5 lbs./100 gallons) in the tanks on October 12 and 13, 2016. A Health and Safety Plan was prepared for the closure work. A safety meeting was conducted prior to commencing work. Soil samples collected from the excavation were collected out of the trackhoe bucket and with decontaminated hand auger buckets. The hand auger buckets were decontaminated with non-phosphate detergent and distilled water and isopropanol rinses. Soil samples collected for laboratory analysis were placed in laboratory sample jars with Teflon-lined lids; chilled and Chain-of-Custody record was initiated. Soil samples were transported to Prism Labs Inc. in Charlotte, North Carolina for analyses. A new pair of disposable Nitrile gloves was worn while collecting each sample.
