Dear Mr. Perez:
Please find enclosed a report summarizing the Underground Storage Tank (UST) closure for three (3) 6,000 gallon gasoline USTs, one (1) 4,000 gallon diesel UST, and one 550 heating oil tank formerly located at the above referenced facility. The UST closure consisted of tank removal, field sampling, and laboratory analyses of the soils in the vicinity of the underground storage tanks. A summary of these activities and our recommendations and conclusions are contained herein.
In accordance with the North Carolina Administrative Code, Title 15A, Chapter 2, Subchapter 2N, this report should be submitted to the Director of the Division of Environmental Management. This report is submitted in accordance with the outline provided in North Carolina Department of Environmental Quality (NCDEQ) form GW/UST-12.

2. Facility/UST Status: The project location was formerly operated as a filling station. The convenience store was out of operation by the time of the UST Closure. The date of last use for the USTs was November 30, 2022. 3. Surrounding Property Use: Commercial
4. Site Geology: Native soils consisted of clay and silt of varying proportions. Appendix A contains a geologic log of excavation for this project. Bedrock was not encountered, and groundwater was not observed during this project.

is included as Appendix B.
II. Closure Procedures
A. Tank Preparation
NC DEQ: Mid-State Petroleum Leasing, Inc. notified the Director of the Division of Waste Management (DWM) of their intent to permanently close by submitting a Notice of Intent: UST Permanent Closure Form GW/UST-3 on November 28, 2022. A copy of this form is included in Appendix C.
Tank Inerting: The tanks were inerted as verified by an O2 / LEL meter to ensure it was properly degassed. The fire marshal from Davie County was present on-site to ensure that the USTs were safe for removal.
B. Residuals On January 30, 2023, Zebra Environmental & Industrial Services, Inc. (Zebra) removed approximately 1,935 gallons of liquids from the five tanks. The liquids were disposed at Zebra’s facility in High Point, NC. Appendix D contains a copy of the liquids disposal manifests for this project.

C. Excavation
The UST closure project at Cohen’s Citgo (Pop Shoppe #233) was initiated on January 31, 2023. Zebra excavated the fill and vent pipes, cut and drained them to avoid release of product into the surrounding soils, then removed them from the tanks. The USTs were then removed from the ground. Figure 2 shows the site layout and the locations of the USTs removed during this closure project. Zebra and Paragon inspected the tanks for structural integrity upon removal. The gasoline tanks were in poor condition. The diesel tank and heating oil tank were in decent condition. After removal, the tanks were transported by Zebra according to API guidelines to Zebra’s facility in High Point, NC for cleaning and disposal. Appendix E contains a copy of the tank disposal manifest for the USTs removed during this closure project.
Following removal of the USTs, the excavation was visually inspected for the presence of free product and groundwater. Groundwater was not encountered in any of the tank pits, and free product was not observed during any phase of this closure project. The dimensions of the excavation created by the removal of the gasoline tanks were approximately 35 feet long by 23 feet wide by 13 feet deep. The dimensions of the excavation created by the removal of the diesel tank were approximately 27 feet long by 13 feet wide by 9 feet deep. The dimensions of the excavation created by the removal of the heating oil tank were approximately 9 feet long by 7 feet wide by 7 feet deep. D. Contaminated Soil
According to the laboratory results, contaminated soils were discovered beneath two of the gasoline tanks during the tank removal project at this facility. Following removal of the USTs and collection of the closure soil samples, the tank pits were backfilled with the overburden and additional soil as needed to fill the volume of the former tanks. III. Site Investigation
A. Soil Sampling
To confirm site conditions, Paragon collected samples from in-situ soils beneath the former USTs, product dispensers, and product piping in accordance with the current NCDEQ Guidelines for Tank Closure. Soil samples were taken from the floor of the gasoline tank excavation at depths of 13 feet below surface grade. Soil samples were taken from the floor of the diesel tank excavation at depths of 9 feet below surface grade.

The product piping samples were collected at depths of approximately 3 feet below surface grade. The dispenser samples were collected at depths of approximately 4 feet below surface grade. The samples were collected from the excavator bucket and were subsequently labeled with location and depth below surface. The soil samples were submitted to Waypoint Analytical, LLC for analyses according to Gasoline Range Organics (GRO). GRO detects Total Petroleum Hydrocarbons (TPH) from low boiling-point fuels such as gasoline, aviation fuel, and gasohol. The current action level for GRO TPH is 50 milligrams per kilogram (mg/kg). The samples from the diesel portions of the tank system were submitted to Waypoint Analytical, LLC for analyses by Diesel Range Organics (DRO). DRO detects TPH from high boiling-point hydrocarbons such as diesel, kerosene, and fuel oil. The current action level for DRO is 100 mg/kg. Figure 3 illustrates the soil sample locations for this closure project.
B. Water Sampling
No water samples were collected during the UST closure activities.
C. Quality Control Measures
The soil samples were packed into new laboratory supplied glassware. The sample containers were labeled with sample location, analyses to be performed, time, date, and the sampler's name. They were then placed in a cooler and chilled with ice to approximately 4oC in preparation for transportation to the analytical laboratory utilizing EPA approved chain-of-custody procedures. The UST closure assessment samples were collected on January 31, 2023 between 10:00 AM and 4:15 PM. The soil samples were delivered to Waypoint Analytical, LLC on February 1, 2023.
D. Investigation Results
According to the laboratory analytical results, the soil sample collected from beneath Tank #2 - East (T2-E) was reported with a concentration of 11,300 milligrams per kilogram (mg/kg) by GRO. The soil sample collected from beneath Tank #1 - East (T1-E) was reported with a concentration of 4,920 mg/kg by GRO. The soil sample labeled as Tank #2 - West (T2-W) was reported with a concentration of 666 mg/kg by GRO. The soil sample labeled as Tank #1 - West (T1-W) was reported with a concentration of 76.6 mg/kg by GRO. The in-situ soil samples obtained from beneath the Tank #3, Tank #4 (Diesel Tank), and Tank #6 (Heating Oil Tank), the dispensers, and product piping were below the NCDEQ action levels according to GRO and DRO (if applicable). Table 1 summarizes the analytical results for the soil samples collected at Cohen’s Citgo (Pop Shoppe #233). Appendix F contains the laboratory analytical report. Appendix G contains the chain-of-custody records for the soil samples.

IV. Conclusions and Recommendations
A. Conclusions
The UST Closure Report activities for four regulated USTs have been completed at Cohen’s Citgo (Pop Shoppe #233). From a review of all information gathered during this removal project, Paragon Environmental Consultants, Inc. makes the following conclusions:
o Three 6,000 gallon gasoline USTs, one 4,000 gallon diesel UST, and one 550 gallon
heating oil UST have been properly closed by removal at 1675 US Highway 601 in
Mocksville, NC by Zebra Environmental and Industrial Services, Inc.
o Analytical results for the soil samples collected beneath two of the gasoline tanks were above the current NCDEQ action levels by GRO.
o Analytical results for the soil samples collected beneath the diesel UST, heating oil UST, dispensers and product piping were all below the NCDEQ action levels by GRO and DRO (if applicable). B. Recommendations
Based upon a review of all information gathered during this UST closure project, Paragon recommends over-excavation of the contaminated soil below the gasoline tanks. A copy of this report should be forwarded to the following address:

C. Limitations
This report has been prepared for the exclusive use of Mid-State Petroleum Leasing, Inc.
for the specific application to the referenced site located in Davie County, North Carolina.
The assessment was conducted based on the scope of work and level of effort desired by
the client. Our findings have been developed in accordance with generally accepted
standards in the practice of UST Closures in the State of North Carolina, available
information, and our professional judgment. No other warranty is expressed or implied.
The data presented in this report are indicative of conditions that existed at the precise
locations sampled and at the time the samples were collected. Additionally, the data
obtained from the samples would be interpreted as meaningful with respect to the
parameters indicated in the laboratory reports. No additional information can be logically
inferred from this data.

accordance with the applicable analytical method.
The analytical data has been validated using standard quality control measures performed as required by the analytical method. Quality Assurance, method validations, instrumentation maintenance and calibration for all
parameters were performed in accordance with guidelines established by the USEPA (including 40 CFR 136 Method Update Rule May 2021) unless otherwise indicated. Certain parameters (chlorine, pH, dissolved oxygen, sulfite...) are required to be analyzed within 15 minutes of
