Figure 1 in Appendix B depicts the site location.  As directed by the NCDENR, one additional downgradient monitoring well (MW-13) and two vapor extraction wells (SVE-1 and SVE-2) were installed November 24, 2014. Replacement monitoring well MW-2R was installed January 15, 2015. The site plan depicting the monitoring well locations is presented as Figure 2, in Appendix B.  The onsite remediation system was placed into operation in January of 2015. The soil vapor extraction system is comprised of two vapor extraction wells. A summary of the field activities associated with the monthly operation and maintenance is given in subsequent sections of this report. Copies of system maintenance forms are included in Appendix D.  Operation of the remediation system has resulted in a reduction of LNAPL in MW-2 from a maximum thickness of 0.44 feet measured in April of 2015 to a current thickness of 0.05 feet during the reporting period. LNAPL in MW-4 is observed to have decreased from a maximum thickness of 0.02 feet to a not measurable thickness.  On November 17, 2015, an Excel representative collected liquid level data for all monitoring wells as part of the groundwater sampling event. Liquid level data for monitoring wells MW- 1, MW-2R, MW-3, MW-4, MW-6, MW-7, MW-8, MW-9, MW-10, MW-11, and MW-13 is included in Appendix A, Table 1. During the sampling event, free product was observed in monitoring wells MW-2R and MW-4. Based on the liquid level data collected, groundwater flow is estimated to be to the south. A groundwater contour map is included in Appendix B, Figure 3.  Groundwater quality samples were collected from monitoring wells MW-1, MW-3, MW-4, MW-6, MW-7, MW-8, MW-9, MW-10, MW-11, and MW-13 on November 17, 2015. The groundwater samples were laboratory analyzed for full range volatile organic compounds (VOCs) by EPA Method 6200B. Monitoring well MW-2R was not sampled due to the presence of free product; monitoring wells MW-5 and MW-12 could not be located. A map depicting the estimated extent of free product is provided as Figure 4.  The results of the laboratory data for the groundwater samples collected were compared against the NCAC 2L Standards and Gross Contamination Levels (GCLs). The analytical data presented for the constituents detected was determined based on the highest result from the various EPA Methods described above. A review of the laboratory analytical results indicated the following constituents were above applicable regulatory standards:  Benzene  GCL: MW-1, MW-3 and MW-7  NCAC 2L: MW-9 and MW-10  Toluene  NCAC 2L: MW-1, MW-3, MW-4, MW-6, and MW-7  Ethylbenzene  NCAC 2L: MW-3, MW-6 and MW-7 Active Remediation Monitoring Report January 7, 2016 (Former) Servco No. 01711 Incident No. 20966 Winston-Salem, North Carolina Excel No. 24090 EXCEL CIVIL & ENVIRONMENTAL ASSOCIATES, PLLC  www.excelengr.com  Total Xylenes  NCAC 2L: MW-1, MW-3, MW-4, MW-6, MW-7, and MW-9  Methyl-tert-butyl-ether (MTBE)  GCL: MW-1, MW-3, MW-6 and MW-7  NCAC 2L: MW-8 and MW-10  Diisopropyl Ether (IPE)  NCAC 2L: MW-3 and MW-7  Naphthalene  NCAC 2L: MW-9 and MW-10  1,2-dichloroethane  NCAC 2L: MW-10  1,2,4-trimethylbenzene  NCAC 2L: MW-3, MW-6, and MW-7 Current groundwater analytical data is summarized in Table 2 and a summary of historical groundwater analytical data is included in Table 3 of Appendix A. Figure 5 in Appendix B depicts select groundwater constituents. Copies of the current laboratory analytical results are included in Appendix D.  Results of past receptor surveys completed by Excel during assessment activities indicate that supply wells were not identified within 1,500 feet of the property. Additionally, according to the USGS Topographical Map no surface water bodies are located within 1,500 feet of the source area. RECOMMENDATIONS At this time and based on the results of the November 2015 sampling event, Excel recommends continued operation of the soil vapor extraction remediation system, free product recovery efforts at monitoring wells MW-2R and MW-4, and continuing semiannual groundwater sampling of select wells. Active Remediation Monitoring Report January 7, 2016 (Former) Servco No. 01711 Incident No. 20966 Winston-Salem, North Carolina Excel No. 24090 EXCEL CIVIL & ENVIRONMENTAL ASSOCIATES, PLLC  www.excelengr.com PROJECT INFORMATION PRIMARY CONSULTANT: Excel Civil & Environmental Associates, PLLC PROJECT MANAGER: Mike Stanforth SITE CONTACT: Robert Gresham SITE HISTORY: In May 1999, seven steel regulated USTs were removed from the site (Figure 1, Appendix A). Samples collected during closure activities by Wallace-Lee Consultants, LLC (WLC) revealed elevated levels of hydrocarbons. An LSA Report was completed by WLC in April 2000 with an LSA Addendum completed by WLC in December 2000, which concluded that groundwater at the site contained constituents levels exceeding both NCAC 2L standards and GCLs. During LSA activities conducted by WLC, free phase LNAPL was discovered at the site in monitoring well MW-2. Free product recovery efforts reportedly began in February 2000 by monthly site visits for liquid level gauging and hand bailing. To date, eight Aggressive Fluid Vapor Recovery (AFVR) and two 100-hour Mobile Multi-Phase Extraction (MMPE) events have previously been conducted at the site by WLC. GENERAL GEOLOGY: The subject site is located in the Milton Belts of the Inner Piedmont Geologic Physiographic Province. The 1985 USGS Geologic Map of North Carolina indicates that rock in this area is commonly characterized by inequigranular and megacrystic biotite gneiss and schist.
