sampling water supply wells in your area due to a former petroleum release from the Steelman’s Fabric Shop located at the intersection of old Hwy 421 and Howard Steelman Dr. Your permission is needed to sample your water supply well. If you would like your water supply well to be sampled, at no cost to you, please either fill out the following information below and return the letter to me at Linda Estkowski, North Carolina Department of Environmental Quality, Winston-Salem Regional Office, 450 West Hanes Mill Road, Suite 300, Winston-Salem, NC 27105 and/or call [phone].
