Event Description/Updates:
Local EM reported on the scene of a tractor trailer loaded with 48,000 lbs of lumber turner on the side while
pulling off of the roadway on private property. Spill of approx 60 gallons of fuel from the top of the tank.
Phillips Towing out of Wilmington responding for the tractor, trailer and the spill clean up.
NC Hwy 11 at the 4000 block is closed with the VFD detouring traffic around the scene. No State Resources
needed.

Hour Center can change the status of an EM 43 to complete and assign an incident level.
Field Descriptions
EM43 Reporting Section
Taken By: The initials of the individual completing the report form (i.e.: the EM Officer).
Date/Time Reported: This is the date and time the event was reported to the person completing the report form. Please use
military time. (mm/dd/yyyy 21:20)
Date/Time Occurred: This is the date and time the event was reported to the person completing the report form. Please use
military time. (mm/dd/yyyy 21:20).
Reported By: This is the name of the person who is reporting the event. If the caller does not want to be identified enter
Anonymous.
Agency: This is the name of the agency represented by the person reporting the event. If the caller is identified as anonymous,
then list Citizen.
Phone: This is the phone number at which the person reporting the event can be reached. Preferred Format: [phone]
County: This is the county in which the event is occurring or has occurred. If the event is occurring in more than one county,
choose Multiple.
City:This is the city in which the event is occurring or has occurred.
Event Type: This is the type of event that has occurred or is occurring.
Weather: Actual weather events or weather related events (ie watches, warnings and storm reports).
HazMat: A spill or release of a hazardous material, or other substance deemed hazardous to life or the environment.

WMD: Weapon of Mass Destruction incidents involve the threat or use of any device, material or substance used in a
manner which causes or threatens to cause death or serious injury to persons or significant damage to property
(Includes biological, chemical, nuclear and infectious disease).
FNF: A real event, exercise or communications test at a Fixed Nuclear Facility.
Non-FNF: Events involving radiological materials unrelated to an on-site fixed nuclear facility.
SAR/ELT: Search and Rescue efforts involding local, state or federal resources or activation of an Emergency Locator
Transmitter (aircraft or vessel.
Aircraft:Incidents involving any aircraft (civilian or military) within the States airspace or on the ground.
Boating: Incidents involving any vessel (civilian or military) within the inland or coastal waters.
Complaint: Calls from the public reporting illegal activity, releases of hazardous materials or other conditions which may
adversely affect the environment.
Transportation: Incidents involving methods or means of transportation (large scale traffic accidents, train derailments,
closure of Interstate routes or other major highways, bridges or evacuation routes).
Fire: Incidents involving major structures, landfill fires, tire fires, large-scale brush or forest fires in or affecting the state.
Wastewater: Release/spills of treated, untreated or industrial wastewater.
Bomb/Threat: A Bomb threat has been received or an actual or suspected device has been located and could threaten
life or property, or a device has been detonated.
Other: Non-categorized emergency or disaster events that could adversely impact the state, or relates to local, state or
federal resource requests.
Dam Failure: A potentially hazardous situation is developing at a dam structure, a failure has occurred, or is likely to
occur.
Animal Disease:Incidents involving the actual or suspected introduction of a Foreign Animal Disease (FAD) in the
States domestic or wild animal population.
Homeland Security: Potential for, or actual, civil disturbance, reported suspicious activity, reported or suspected
terrorism.
Weather Event Type: This is the type of weather event occurring. This field must be input if the Event Type is Weather.
Weather Event Name: Enter the name of the weather event (i.e. Hurricane Charlie).This field must be input if the Event Type is
Weather
HazMat Event Type: This identifies the chemical which has spilled/released. This field must be populated if the Event Type is
HazMat.
HazMat Class: This indicates the class of the HazMat incident. This field must be populated if the Event Type is HazMat.
HazMat Mode: Indicates the source of the chemical incident. This field must be populated if the Event Type is HazMat.
FNF Event: Indicates the location of the FNF incident. This field must be populated if the Event Type is FNF.
FNF Type: Indicates the type of FNF event. This field must be populated if the Event Type is FNF.
FNF Class: Indicates the nuclear incident classification. This field must be populated if the Event Type is FNF.
Event Description: Description of incident.
Attachments: Include any attachments.
