The onsite work took place on August 24 through 26, 2022.

1. SITE HISTORY AND CHARACTERIZATION
1.1. UST OWNER AND OPERATOR INFORMATION
According to Carteret County GIS information, the ±550‐gallon non‐commercial heating oil UST was located on an approximate ±0.114‐acre parcel identified by PIN 637620815732000. A Local Area map is provided as Figure 1 and a site map is provided as Figure 2. The UST was
located at the northwest corner of the residential structure and its presence was not known
by the property owner until the surficial release of petroleum occurred.
The table below summarizes the UST information. See the attached Table B‐1 for additional information regarding the UST system.

1.2. HISTORICAL & SURROUNDING UST INFORMATION
WR is not aware of additional USTs on the subject site or adjacent properties.
1.3. NON-UST INFORMATION
There was no information regarding non‐UST sources of petroleum storage (ASTs) on this property.
1.4. DESCRIPTION OF RELEASE / OFFSITE UST RELEASES
Following a rain event on or near August 5, 2022, fuel oil was observed on the ground surface
between the residential structures located at 2800 and 2802 Evans Street in Morehead City, NC. The property owner subsequently identified the presence of the heating oil UST and noted that the fill cap had been knocked off during the installation of a fence over the UST. The property owner of 2800 Evans Street was not aware that a heating oil UST was located
on the property.
WR is unaware of any nearby UST incidents.
1.5. SITE CHARACTERISTICS AND SURROUNDING PROPERTY
The site is currently owned by Mr. and Mrs. Chalk. The contiguous properties to the north, west, east, and south are residential. The subject property is located within Morehead City,
where properties are connected to municipal water and sewer. WR did not observe an
irrigation or water supply wells within 500 feet of the former UST area.

1.6. SUMMARIZE INITIAL ABATEMENT ACTIONS, ASSESSMENT
ACTIVITIES, AND CORRECTIVE ACTIONS PERFORMED TO DATE
AND LIST ALL REPORTS PREVIOUSLY SUBMITTED.
The ±550‐gallon non‐commercial UST was permanently closed in‐place on August 26, 2022. The UST was located directly under a fence and two HVAC units which prevented its removal from the subsurface. Prior to the in‐place closure, SR&R removed ±45‐gallons fluid from the UST. The UST was subsequently filled with sand and capped with concrete. The UST closure
activities are described further in Sections 2 and 3 of this report.
2. UST CLOSURE ACTIVITIES
2.1. DESCRIBE PREPARATIONS FOR CLOSURE INCLUDING
STEPS TAKEN TO NOTIFY AUTHORITIES, PERMITS OBTAINED, AND
STEPS TAKEN TO CLEAN AND PURGE THE TANKS.
±45‐gallons fluid was removed from the UST prior to closure.
NC811 and the local fire department were notified prior to closure activities.
NCDEQ – UST Section was also notified of the release and planned remedial actions.
2.2. DESCRIBE THE CLOSURE PROCEDURE, REFERENCING SITE
AND SAMPLING LOCATION MAPS. CLEARLY STATE HOW THE
INITIAL ALARM OR CAUSE FOR SUSPICION, THE TYPE OF BACKFILL,
THE DEPTH TO GROUNDWATER, AND OTHER FACTORS
APPROPRIATE FOR IDENTIFYING THE PRESENCE AND SOURCE OF
A RELEASE. IF THE PRESENCE OF GROUNDWATER OR BEDROCK
REQUIRES THE INSTALLATION AND SAMPLING OF MONITORING
WELL(S), DESCRIBE THE PROCESS.
Following the closure of the UST and removal of petroleum impacted soils, WR collected the
soil samples from the perimeter of the excavation and from the crawl space at 2802 Evans Street. The samples were provided to RedLab, LLC for analysis for TPH by UVF Technologies. The excavation encompassed an approximate of area of 200 square feet by 1 to 2 feet deep (Figure 2). Groundwater was not observed in the excavation but was determined to be
approximately 2.5 feet deep. The excavated soils were transferred to a dump trailer and taken
to SR&R’s facility in Wilmington, NC for proper disposal. See Appendix A for copies of the disposal manifests.
The excavated area was backfilled with clean fill material from an offsite location.
A completed UST‐2B form is included as Appendix C.

Soil samples collected from the extents of the over‐excavation and from within the crawl space at 2802 Evans Street did not reveal any TPH concentrations above 50 mg/kg for TPH‐GRO or 100 mg/kg for TPH‐DRO.
Soil analytical results are tabulated in Tables B‐3. Closure sample locations are illustrated on Figure 2.
3. SITE INVESTIGATION
3.1. FIELD SCREENING TECHNIQUES
Clean nitrile gloves were utilized during the collection of the sidewall soil samples. Soil samples
were collected with a decontaminated hand auger. Collected soil samples were placed in clean zip‐lock baggies and screened using the onsite MiniRae PID.
3.2. SOIL SAMPLING PROCEDURES
Following field screening activities, collected soil samples were placed in laboratory supplied bottleware and provided to the onsite lab, REDLAB.
3.3. GROUNDWATER SAMPLING PROCEDURES
Groundwater was not encountered during the UST Closure event.

3.4. QUALITY CONTROL MEASURES
Quality control measures were employed during this project and included use of clean sampling tools, clean zip‐lock baggies, calibrated field instruments, donning of clean nitrile gloves during each sample collection.
3.5 INVESTIGATIVE RESULTS – CLOSURE ASSESSMENT
The ±550‐gallon non‐commercial heating oil UST was closed in‐place on August 26, 2022.
Over‐excavation activities were undertaken to remove impacted soils, as described in Section 2.2. Following completion of over‐excavation activities, soil samples were collected from the sidewalls and from the crawl space of 8002 Evans Street. Each collected sample was placed in clean zip‐lock baggies and immediately screened using the onsite MiniRae PID. Samples
were then placed into laboratory supplied bottleware and provided to the onsite lab, REDLAB. Soil samples collected from the extents of the over‐excavation and from within the crawl space at 2802 Evans Street did not reveal any TPH concentrations above 50 mg/kg for TPH‐GRO or 100 mg/kg for TPH‐DRO.
A copy of the laboratory analytical report is included in Appendix D. Sample locations are illustrated on Figure 2 and results are provided in tabular form in Table B‐3.
4. CONCLUSIONS AND RECOMMENDATIONS
The ±550‐gallon non‐commercial heating oil UST was closed in‐place on August 26, 2022.
Approximately 5 tons of petroleum impacted soil was removed from 8000 and 8002 Evans Street. The results of the over‐excavation soil samples collected from the sidewalls of the excavation did not reveal any TPH concentrations above 50 mg/kg for TPH‐GRO or 100 mg/kg for TPH‐DRO.
Based on analytical data and the lack of nearby receptors within 150 feet, WR believes the property meets the requirements for a Low‐Risk site eligible for closure with a deed recordation.
