Dear Mr. Davis:

On May 8, 2021, the Division of Waste Management (DWM) received a Groundwater Monitoring
Report, prepared on your behalf by Henry Nemargut Engineering Services. The report summarizes recent
groundwater sampling activities conducted at the subject site. The DWM thanks you for this report.

After review of this report, the DWM requests that monitoring and reporting activities be continued
on an annual basis. Additionally, a monitoring well should be installed to replace monitoring well (MW)
MW-13 and MW-16. The following wells should be sampled during the next annual sampling event: MW-
1, MW-2, MW-3, MW-10, the newly installed monitoring well and SW-3. The DWM requests that the
next Groundwater Monitoring Report be submitted to the Division no later than May 1, 2022.

Please submit a pre-approval authorization form before any work has been completed if State Trust
Fund reimbursement is anticipated. If you have any questions regarding trust fund eligibility or
