All, I inspected the subject facility on March 5, 2020. I issued the owner a Notice of Deficiency today for the “new” spill, 
overfill, sump, and monthly walk through requirements. All relevant documents have been uploaded to TIMS.

After a site visit to the subject facility (conducted by CAB‐UST as part of a BR‐19 review) and concerns over 
housekeeping at this site, CAB‐UST is requesting that PIBS‐UST please complete an inspection at this location 
to include the determination of compliance with spill bucket testing requirements.

No we do not have a shell doc specifically for that. We have a basic NOV letter that depending on violations chosen, the 
violations get inserted in to the letter automatically when generated in TIMS. An example of the NOV with the spill 
bucket testing and suspected release (Requiring more than just spill testing) violations inserted is attached.

We do not have those records. The last inspection was done right before that part of the regulation went in to effect so 
we did not see the records at that inspection either.

Michael, 
I am trying to get spill bucket testing results for the subject site, Facility ID # 00‐0‐0000035021.  This facility has 
an incident associated with it from a drive‐off in 2001 and the UST owner has not been responsive to Notice of 
Regulatory requirements that were sent in 2001 and 2014.  Before I sent another NORR, I wanted to see if I 
could get the information for the spill bucket testing.
