ADDITIONAL COMMENTS: 01 - Life of Site Permit was issued 2/20/2017 as a combined permit with facility #2805T and was reviewed along with
the operations plan and contingency plan during this inspection. Facility is permitted to receive waste from Dare Co. Passed by the Dare Co. Commissioners on August 02, 2004. Approved waste for this facility to receive: C&D, Inert Debris, Land Clearing Waste (All used as 50/50 (SM) alternative cover after mulched). and Asphalt.
02 - Facility has required signage. Locking gates and fencing for security. 03 - Interior roads are constructed of all-weather materials and well maintained. 04 - Incoming load tickets were reviewed during this inspection.
05 - Waste screening records were reviewed during this inspection and found to be up to date. 06 - Cover records were reviewed and found to be up to date. 07 - Working face is kept small (< ¼ acre) for better management and quicker covering. Looks good. 08 - Facility has a 2 large piles of Cover material stockpiled on the top of the active cell and also stockpiled at the cover material stockpile area.1 large pile of 50/50/SM is already mixed at the stockpile area. Site can cover very fast in the event of bad weather.
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

09 - John College screened one incoming load on the working face. The load had C&D waste only.
10 - One load had been screened before this inspection a 1 hot water heater was pulled out and taken to the scrap metal area.
11 - Scrap metal pile has tires pulled out to be removed to the scrap tire trailer off-site at the Dare Co. Public Works Recycling Department in Manteo.

12 - White goods containing CFC’s were upright until CFC removed and the placed onto scrap metal pile with a red X on the units. CFC removal records were reviewed during this inspection.
13 - There was more windblown trash than usual noted during this inspection due to the Compactor (using bulldozer) was broken down and very high winds. Be sure ALL windblown trash is picked up by the end of the workday. Fencing working fairly well.
14 - Land clearing waste looks good. 15 - Maintenance on side slopes on all cells looks good. 16 - Edge of waste markers present. 17 - Facility has 8 Certified Landfill Operations Specialist: Jonathan DeWeese, expires 11/16/2024, Thomas Tillett, expires 10/9/2026, Neal Dennis, expires 5/3/2024, Deborah Equils, expires 11/18/2025, Ashley Walston, expires 2/2/2026. David Wright, expires 2/2/2026, Linwood Hamilton, expires 2/2/2026 and Samuel Moore, expires 2/2/2026. 18 - Inspected ALL groundwater monitoring wells. All were locked and tagged with required information and has easy access during this inspection. 19 - Semi Annual Groundwater reports were available and reviewed. The report review were from 3/23/2022 and 11/3&6/2022.

20 - Gas monitoring reports reviewed were dated 8/24/2022 and 3/8/2023. 21 - Digital pictures were taken during this inspection with some attached to this report.
