Please refer to reports submitted for this incident for further historical information regarding the assessment and remediation activities conducted at the site. Currently, one 10,000-gallon gasoline UST, one 6,000- gallon gasoline UST, one 6,000-gallon diesel UST, and one 4,000-gallon kerosene UST operate at the site.  Latitude/Longitude: 36.316722º North / 76.987917º West EXECUTIVE SUMMARY The Green Apple (Former Red Apple Market #23) is located at 1304 US Highway 13 South in Ahoskie, Hertford County, North Carolina. A release from the onsite underground storage tank (UST) system consisting of one 10,000-gallon gasoline, one 6,000-gallon gasoline, one 6,000-gallon diesel and one 4,000-gallon kerosene USTs was discovered during February 2010 site check activities. According to historical information obtained from the project file, a previous release was discovered during the removal of two 8,000-gallon gasoline, one 4,000-gallon gasoline, one 1,000-gallon diesel and one 550- gallon kerosene underground storage tanks (USTs) in December 1992. Incident Number 11171 was issued for the previous UST system release when the site was known as the Red Apple Market #23. In March 2016, SKD Fuels, LLC agreed to take the lead in completing the required assessment and remediation at the site. The adjoining properties are commercial, residential and agricultural. Public water is available to the site and surrounding properties. The site risk classification is currently ranked as intermediate risk due to the past presence of free product. On December 5, 2016, six Type II and one Type III monitoring wells were gauged, purged and sampled. Concentrations of requested method constituents that exceeded the Maximum Allowable Concentrations specified in T15A NCAC 2L.0202 were reported in the ground water samples collected from MW-5, MW-10, MWD-12, MW-13, MW-14, GMW-1 and GMW-2. None of the reported contaminant concentrations exceeded the Gross Contamination Levels. Following the completion of the December 2016 sampling event, an absorbent boom was installed in GMW-1 in order to recover free product, should any recharge into the well between sampling events. Based on this information, Geological Resources, Inc. recommends that ground water monitoring continue at the site in order to confirm that free product does not return to GMW-1. The absorbent boom installed in GMW-1 should be removed approximately 30 days prior to the ground water monitoring event. The next semi-annual ground water monitoring event is tentatively scheduled for June 2017. A release from the onsite underground storage tank (UST) system consisting of one 10,000-gallon gasoline, one 6,000-gallon gasoline, one 6,000-gallon diesel and one 4,000-gallon kerosene USTs was discovered during February 2010 site check activities. According to historical information obtained from the project file, a previous release was discovered during the removal of two 8,000-gallon gasoline, one 4,000-gallon gasoline, one 1,000-gallon diesel and one 550-gallon kerosene USTs in December 1992. Incident Number 11171 was issued for the previous UST system release when the site was known as the Red Apple Market #23. Fourteen Type II and two Type III monitoring wells were installed at the site during past ground water assessment and remediation activities. Site activities conducted prior to the April 2008 updated receptor survey and ground water sampling activities were completed by past consultants. Although Geological Resources, Inc. (GRI) cannot verify the accuracy of information obtained from previous reports, for the purposes of this report, it is assumed to be correct. Please refer to previous submittals for further historical information regarding the site. 2.0 CURRENT SITE ASSESSMENT The purpose of this report is to present the results of the ground water sampling activities conducted on December 5, 2016 at The Green Apple site. The activities were conducted in accordance with GRI proposal number 16-456 which was submitted to NCDEQ on June 17, 2016 and approved as Task Authorization No. 11171-30 on July 15, 2016. A change order was submitted on July 19, 2016 and approved as Task Authorization No. 11171-030-1 on August 12, 2016. The purpose of the activities was to obtain current ground water quality data for the site. 3.0 GROUND WATER QUALITY Six Type II monitoring wells (MW-5, MW-10, MW-13, MW-14, GWM-1 and GMW-2) and one Type III monitoring well (MWD-12) were gauged, purged and/or sampled on December 5, 2016. The depths to 2 ground water in the Type II monitoring wells during the December 2016 sampling event ranged from 2.53 to 3.21 feet below the tops of well casings. Ground water elevations in the Type II monitoring wells relative to a temporary benchmark with an assumed datum of 100.00 feet ranged from 97.40 to 98.28 feet. Based on this information, the general ground water flow direction was generally toward the northeast with an average horizontal hydraulic gradient of approximately 0.02 feet per foot. A Water Table Surface Map for the December 2016 sampling event has been included as Figure 3. Well construction and gauging information is presented in Table 1. Laboratory analyses were performed on the ground water samples collected from the monitoring wells during the December 2016 sampling event for volatile organic compounds using EPA Method 6200B. Concentrations of individual BTEX constituents, MTBE, naphthalene, IPE, isopropylbenzene, n- propylbenzene, 1,2-dichloroethane and/or 1,2,4 trimethybenzene that exceeded the Maximum Allowable Concentrations (MACs) specified in T15A NCAC 2L.0202 were reported in the ground water samples collected from MW-5, MW-13, MW-14, GMW-1 and GMW-2. None of the reported contaminant concentrations exceeded the Gross Contamination Levels (GCLs). A Ground Water Quality Map based on the December 2016 sampling event has been included as Figure 4. A summary of ground water sample analytical results is presented in Table 2. A copy of the laboratory report has been included as Appendix A. Following the completion of the December 2016 sampling event, an absorbent boom was installed in GMW-1 in order to recover free product, should any recharge into the well between sampling events. 4.0 CONCLUSIONS AND RECOMMENDATIONS  No free product was measured in GMW-1 in December 2016.  The average depth to ground water in the Type II monitoring wells during the December 2016 sampling event was 2.90 feet. Ground water flow across the site was generally toward the northeast with an average horizontal hydraulic gradient of approximately 0.02 feet per foot.  Concentrations of individual BTEX constituents, MTBE, naphthalene, IPE, isopropylbenzene, n- propylbenzene, 1,2-dichloroethane and/or 1,2,4 trimethybenzene that exceeded the MACs were reported in the ground water samples collected from MW-5, MW-13, MW-14, GMW-1 and GMW-2. None of the reported contaminant concentrations exceeded the GCLs. 3  Based on this information, GRI recommends that ground water monitoring continue at the site in order to confirm that free product does not return to GMW-1. The absorbent boom installed in GMW-1 should be removed approximately 30 days prior to the ground water monitoring event. The next semi-annual ground water monitoring event is tentatively scheduled for June 2017. 5.0 LIMITATIONS This report has been prepared for the exclusive use of SKD Fuels, LLC for specific application to the referenced site in Hertford County, North Carolina. The assessment was conducted based on the scope of work and level of effort desired by NCDEQ and with resources adequate only for that scope of work. Our findings have been developed in accordance with generally accepted standards of environmental practices in the State of North Carolina, available information, and our professional judgment. No other warranty is expressed or implied. The data that are presented in this report are indicative of conditions that existed at the precise locations sampled and at the time the samples were collected. In addition, the data obtained from samples would be interpreted as being meaningful with respect to parameters indicated in the laboratory report. No additional information can logically be inferred from these data. Please note that field data collected prior to the April 2008 updated receptor survey and ground water sampling event was not obtained under the supervision of GRI. Although the accuracy of this information cannot be verified, for the purpose of this report GRI assumes that it is correct.
