GRI Project No. 2512 ii EXECUTIVE SUMMARY Al Simmons Store is located at US Highway 13 and NCSR 1409 (Hall Siding Road) in Ahoskie, Hertford County, North Carolina. The site is currently owned by Deborah Pope Simmons and is currently vacant. The adjoining properties are residential. Municipal water is available to the site and surrounding properties. The site risk classification is currently ranked as high risk due to the presence of three water supply wells within 250 feet of the source area. In September 1993, three 1,000-gallon gasoline underground storage tanks (USTs) were removed from the site. A total of twenty Type II monitoring wells (MW-1, MW-2, MW-3, MW-5, MW-6, MW-7 and MW- 9 through MW-22), two Type III monitoring wells (MWD-4 and MW-8) and five recovery wells (RW-1 through RW-5) were installed during previous ground water assessment and corrective action activities. A Corrective Action Plan, proposing the installation of a ground water pump and treat system, was submitted for the site on March 4, 1996, and was approved on June 27, 1996. The corrective action system (CAS), along with five associated recovery wells (RW-1 through RW-5), were installed at the site, and the system was activated in March 1997. The CAS was ran until September 2005, when it was deactivated due to lack of funding. The CAS was restarted in August 2008 and ran until June 2010, when it was deactivated at the direction of NCDEQ. A total of 2,759.56 tons of contaminated soil was excavated from the site in June and July 2011. During the June and July 2011 soil excavation activities, MW-1, MW-2, MW-3, MWD-4, MW-8, RW-1 and RW-2 were destroyed. Replacement wells MW-1R through MW-4R were installed at the site subsequent to the soil excavation activities. On February 18, 2020, four monitoring wells and one recovery well were gauged, purged and sampled. No free product was observed. The Maximum Allowable Concentrations were below the concentrations of benzene, MTBE, naphthalene, IPE and/or n-propylbenzene reported in the ground water samples collected from MW-3R, MW-4R and MW-12. All of the reported contaminant concentrations were below the Gross Contamination Levels. Based on this information, the risk classification for the site should remain high risk. The on-site water supply well (WSW-1) should be abandoned, and the site should be connected to municipal water. Semi- annual ground water monitoring should continue at the site. GRI Project No. 2512 1 1.0 SITE HISTORY AND CHARACTERIZATION Geological Resources, Inc. (GRI) presents the results of the February 2020 ground water sampling event at the Al Simmons Store site; located at US Highway 13 and NCSR 1409 (Hall Siding Road) in Ahoskie, Hertford County, North Carolina (Figure 1). These activities were conducted in accordance with GRI Proposal No. 20-007, which was submitted to the North Carolina Department of Environmental Quality (NCDEQ) on January 10, 2020, and was approved as Task Authorization No. 11114-63 on January 14, 2020. State correspondence has been included as Appendix A. The site is currently owned by Deborah Pope Simmons and is currently vacant. On September 17, 1993, three 1,000-gallon gasoline USTs were removed from the site. UST owner/operator information and UST system information are included as Tables 1 and 2, respectively. The Registered Tank Report is shown below: The surrounding properties are residential. The site contains seven buildings and is primarily capped with gravel and grass. The overall topography at the site is relatively flat. No surface water bodies were identified within a 500-foot radius of the site. Twenty Type II monitoring wells (MW-1, MW-2, MW-3, MW-5, MW-6, MW-7 and MW-9 through MW-22), two Type III monitoring wells (MWD-4 and MW-8) and five recovery wells (RW-1 through RW-5) were installed at the site during past assessment activities. Based on the historical presence of free product and high contaminant concentrations, a soil excavation was conducted in the vicinity of MW-1 through MW-4 in June and July 2011. Monitoring wells MW-1 through MW-4, MW-8 and RW-1 were destroyed during the soil excavation activities. Replacement wells MW-1R through MW-4R were installed upon the completion of the excavation. The site has been given a high risk classification by NCDEQ due to the presence of potable water supply wells within a 1,000-foot radius of the site. GRI Project No. 2512 2 The site is a former convenience store and petroleum retail facility. According to available information, a Corrective Action Plan proposing the installation of a ground water pump and treat Corrective Action System (CAS) was submitted to NCDEQ on March 4, 1996. The CAP was approved by NCDEQ on June 27, 1996. The CAS operated from March 1997 to September 2005, when it was shut down due to a lack of funding. The CAS was restarted in August 2008 and ran until it was shut down at the direction of NCDEQ in June 2010. 2.0 PRESENTATION OF CURRENT SITE ASSESSMENT INFORMATION/ COMPARISON TO HISTORICAL ASSESSMENT INFORMATION On February 18, 2020, four Type II monitoring wells (MW-2R, MW-3R, MW-4R and MW-12) and one recovery well (RW-4) were gauged, purged and sampled. Free product was not observed. Depths to ground water in the Type II monitoring wells ranged from 1.33 feet to 12.96 feet. Ground water elevations in the Type II monitoring wells ranged from 33.14 feet to 49.09 feet. Ground water flow was generally to the north with a horizontal gradient of approximately 0.05 feet per foot. Well construction and gauging information is presented in Table 5. The Maximum Allowable Concentrations (MACs) as specified in T15A NCAC 2L.0202 were below the concentrations of benzene, MTBE, naphthalene, IPE and/or n-propylbenzene reported in the ground water samples collected from MW-3R, MW-4R and MW-12. All of the reported contaminant concentrations were below the Gross Contamination Levels (GCLs). A summary of the analytical results for ground water samples collected on February 18, 2020, is presented in Table 6. A Ground Water Quality Map based on the February 2020 data is included as Figure 5. A complete report of laboratory analyses of the ground water samples collected from the monitoring wells during the February 2020 sampling event has been included as Appendix C. The June 2010 ground water sampling event was the final sampling event prior to the June- July 2011 soil excavation. The contaminant plume was divided into three areas (Areas 1 through 3) based on the benzene concentrations during the June 2010 ground water sampling event. The contaminant mass has been reduced by 99%, 100% and 98.7% in Areas 1, 2 and 3, respectively. Contaminant mass calculations have been included as Appendix C. A map of the contaminant mass areas has been included as Figure 6. A chart depicting the overall contaminant mass trend is shown below: The site is located in the Coastal Plain Physiographic Province of North Carolina. GRI Project No. 2512 4 4.0 NATURAL ATTENUATION / NATURAL SOURCE ZONE DEPLETION STATUS No natural attenuation monitoring plan is currently in place at the site. 5.0 CONCLUSIONS  On February 18, 2020, four monitoring wells and one recovery well were gauged, purged and sample. Ground water flow was generally to the north with a horizontal gradient of approximately 0.05 feet per foot.  The MACs were below the concentrations of benzene, MTBE, naphthalene, IPE, n-propylbenzene and/or 1,2,4-trimethylbenzene reported in the ground water samples collected from MW-3R, MW-4R, and MW-12. All of the reported contaminant concentrations were below the GCLs.  The contaminant mass has been reduced 99%, 100% and 98.7% in Areas 1, 2 and 3, respectively.  Based on this information, the risk classification for the site should remain high risk. The on-site water supply well (WSW-1) should be abandoned, and the site should be connected to municipal water. Semi-annual ground water monitoring should continue at the site. The next sampling event is tentatively scheduled for August 2020. 6.0 LIMITATIONS This report has been prepared for the exclusive use of North East Oil Company, Inc. for specific application to the referenced site in Hertford County, North Carolina. The assessment was conducted based on the scope of work and level of effort desired by NCDEQ and with resources adequate only for that scope of work. Our findings have been developed in accordance with generally accepted standards of environmental practices in the State of North Carolina, available information, and our professional judgment. No other warranty is expressed or implied. The data that are presented in this report are indicative of conditions that existed at the precise locations sampled and at the time the samples were collected. In addition, the data obtained from samples would be interpreted as being meaningful with respect to parameters indicated in the laboratory report.
