EXECUTIVE SUMMARY
The Flash Market #370 site is located at 1390 NW Maynard Road in Cary, Wake County, North Carolina.
The property operates as a petroleum retail facility and convenience store. The site is covered by asphalt,
concrete and grass. There is one 10,000-gallon gasoline UST and one 17,000-gallon gasoline UST currently
in use at the site with six associated dispensers.
On May 23, 2022, during triennial testing activities, one non-passing spill bucket was reported for the
17,000-gallon gasoline UST. Therefore, a site check was required by the NCDEQ. Repairs to the spill bucket
are scheduled to be conducted during the week of August 8, 2022.
On July 6, 2022, GRI conducted a site check in the area of the one 10,000-gallon gasoline and one 17,000
gasoline UST spill bucket. Three soil samples were collected during the installation of a monitoring well.
The monitoring well is located adjacent to the spill bucket of the 17,000-gallon gasoline UST. The soil
boring was advanced to 30 feet and soil samples were collected at 10 foot intervals (SS-1-10’, SS-2-20’ and
SS-3-30’). With the exception of concentrations of MTBE that exceeded the MCCs in soil sample SS-1-10’
and SS-3-30’, no other requested method constituent exceeded the MCCs.
As part of the site check activities, a ground water sample was collected from monitoring well MW-1 on
July 6, 2022. The concentrations of benzene, MTBE, naphthalene, tert-amyl alcohol, tert-butyl alcohol,
C5-C8 aliphatics and C9-C10 aromatics exceeded the MACs. The reported concentrations were below the
GCLs. No free product was noted at the site.
Based on this information, a release has occurred at the site. Please note that the presence of MTBE in
the soil and ground water samples indicate that the release pre-dates 2009, when MTBE use was phased
out. GRI recommends that a receptor survey is conducted in order to determine this risk classification of
the release.

A. SITE HISTORY AND CHARACTERIZATION
1. UST Owner and Operator Information:
In accordance with the Site Check Report guidelines, the UST owner and operator information has been
summarized in the attached Table 1.
2. UST System Information:
In accordance with the Site Check Report guidelines, the UST system information has been summarized
in the attached Table 2. There are one 10,000-gallon gasoline and one 17,000-gallon diesel USTs
currently in use at the site with six associated gasoline dispenser islands. The dispensers are located on
the southwest side of the store building. The UST information is summarized in Table 2. A copy of the
current UST Permit is shown below:

3. Non-UST Information:
There are currently no ASTs and no known ASTs formerly at the site.
4. Comprehensive Description of the Release:
Annual compliance testing was completed on May 23, 2022. Based on the testing, non-passing hydrostatic
results were reported for one 17,000-gallon regular gasoline UST spill bucket. A copy of the UST Suspected
Release 24 Hour Notice (UST-17A) form is included as Appendix A.
A site check was required according to the NCDEQ. On July 6, 2022, three soil samples (SS-1-10’, SS-1-20’
and SS-1-30’) were collected during installation of a monitoring well (MW-1) adjacent to the non-passing
spill bucket. All of the reported contaminant concentrations were below the applicable standards. A
ground water sample collected from MW-1 was analyzed. Upon receipt of the laboratory analytical results
on July 20, 2022, a release was confirmed at the site. A copy of the UST-61 Form has been included as
Appendix B.
5. Description of the Site Characteristics:
The Flash Market #370 site is located at 1390 NW Maynard Road in Cary, Wake County, North Carolina.
(Figure 1). The property operates as a petroleum retail facility and convenience store. The site is covered
by asphalt, concrete and grass. There are one 10,000-gallon gasoline and one 17,000-gallon gasoline USTs
currently in use at the site with and six gasoline dispensers. The surrounding properties are commercial
and residential. A Site Map has been included as Figure 2.
B. SITE CHECK PROCEDURES
1. Results of Tank and Line Tightness Testing:
On May 23, 2022, the spill bucket associated with the 17,000-gallon regular spill bucket failed the triennial
hydrostatic test conducted by a testing company. A crack was identified in the spill bucket. Liners are
scheduled to be installed at the earliest possible date. Testing results will be forwarded to the NCDEQ
upon receipt subsequent to spill bucket repairs.
2. Summary of Site Check Procedure:
GRI performed a site check on July 6, 2022, that included the collection of three soil samples during the
installation of a monitoring well. The monitoring well is located adjacent to the regular gasoline UST spill
bucket. The soil boring was advanced to 30 feet and soil samples were collected at 10-foot intervals (SS-
1-10’, SS-2-20’ and SS-3-30’). A Well Construction Record and soil boring log are provided as Appendix C.
3. Soil Excavation Activities:
No soil excavation activities were conducted.
C. SITE INVESTIGATION
1. Field Screening:
Soil samples (SB-1, SB-2 and SB-3) collected on July 6, 2022, were not screened with a PID for VOCs.
2. Soil Sampling Information:
Soil samples (SS-1-10’, SS-1-20’ and SS-1-30’) were collected on July 6, 2022, and submitted for laboratory
analyses of VPH by the MADEP Method. In addition, SS-1-10’ and SS-1-30’ were submitted for laboratory
analysis of BTEX by EPA Method 8260B. With the exception of concentrations of MTBE that exceeded the
MCCs in soil samples SS-1-10’ and SS-3-30’, no other requested method constituent exceeded the MCCs.

Please note that the sample collected from 30 feet BGS was below the water table. A soil quality map is
presented as Figure 3. A summary of soil sample analytical results is presented in Table 4. The laboratory
analytical report is provided as Appendix D. A summary of the soil sampling data from the July 2022 Site

3. Ground Water and Surface Water Sampling Information:
On July 6, 2022, monitoring well MW-1 was installed to 30 feet BGS. The depth to water in MW-1 was
23.71 feet BGS. A ground water elevation of 76.29 feet was measured, based on an assumed top of
casing elevation of 100.00 feet. A summarily of gauging data is provided in Table 3. The location of
MW-1 is shown in Figure 2.

A ground water sample was collected from MW-1 on July 6, 2022, and submitted for analysis of VOC by
EPA Method 6200B, EDB by EPA Method 504.1, total lead by EPA Method 6010 and VPH by the MADEP
Method. The concentrations of benzene, MTBE, naphthalene, tert-amyl alcohol, tert-butyl alcohol, C5-
C8 aliphatics and C9-C10 aromatics exceeded the MACs. The reported concentrations were below the
GCLs. A ground water quality map is presented as Figure 4. A summary of ground water sample
analytical results is presented in Table 5. The laboratory analytical report is provided as Appendix D. A
summary of the ground water sampling data from the July 6, 2022, Site Check activities is show below:

4. Quality-Control Measures:
Clean, disposable nitrile gloves were used for the collection of the soil samples. The samples were
collected in laboratory supplied containers and shipped to the laboratory in a cooler following proper
chain-of-custody procedures. A laboratory supplied trip blank was included in the sample cooler. With
the exception of acetone (common laboratory contaminant), no detectable concentrations of requested
method constituents were reported in the trip blank.
D. SUMMARY AND CONCLUSIONS
The site is commercial and surrounding properties are commercial and residential.
On May 23, 2022, non-passing hydrostatic results were reported for the 17,000-gallon regular
gasoline UST spill bucket. Repairs, including the installation of liners in both spill buckets, are
scheduled to be conducted as soon as possible. Post-repair testing results will be forwarded to
the NCDEQ upon receipt.
A Site Check was conducted on July 6, 2022, to determine if a petroleum release had occurred.
Three soil samples (SS-1-10’, SS-2-10’ and SS-3’30’) were collected during the advancement of
monitoring well (MW-1), located adjacent to the non-passing spill bucket. Concentrations of
MTBE reported in the soil samples collected from 10 feet and 30 feet, exceeded the soil to water
MSCC. All other reported contaminant concentrations were below the applicable standards.
Monitoring well MW-1 was installed and a ground water sample was collected on July 6, 2022.
The concentrations of benzene, MTBE, naphthalene, tert-amyl alcohol, tert-butyl alcohol, C5-C8
aliphatics and C9-C10 aromatics exceeded the MACs. The reported concentrations were below
the GCLs. No free product was noted at the site.
Based on this information, a petroleum release has occurred. It appears that the release pre-
dates 2009 due to the presence of MTBE. A receptor survey and a Phase I LSA should be
conducted in order to determine the risk classification of the site.

E. LIMITATIONS
This report has been prepared for the exclusive use of Transit Energy Group for the specific application to
the referenced site in Wake County, North Carolina. The assessment was conducted based on the scope
of work and level of effort specified by the NCDEQ and with resources adequate only for that scope of
work. Our findings have been developed in accordance with generally accepted standards of geology and
hydrogeology practices in the State of North Carolina, available information, and our professional
judgment. No other warranty is expressed or implied.
The data that are presented in this report are indicative of conditions that existed at the precise locations
sampled and at the time the samples were collected. In addition, the data obtained from samples would
be interpreted as being meaningful with respect to parameters indicated in the laboratory report. No
additional information can logically be inferred from these data.

UST-17A UST Suspected Release 24 Hour Notice
Do not use this form to report a 24 hour notice of a Pollution Incident. You must use form UST-61, 24 Hour Release and UST Leak
Reporting Form.
This form must be used to report to DEQ, within 24 hours of discovery, a Suspected Release from any of the following:
a) Failed or inconclusive leak detection or tightness test results. This includes positive test results, such as “Increase”, which
indicates the level in the UST system has increased more than 0.2 gph.
b) Unusual operating conditions which can include, but are not limited to: the erratic behavior of dispensing equipment; the
unexplained presence of water in the tank; the presence of fuel in containment sumps or interstitial spaces; or the degradation of
any equipment or element of an underground storage tank system to the point where that equipment or element can not reasonably
be expected to perform its intended function.
c) Internal inspection results such as, perforations, corrosion holes, weld failures, or other similar defects that indicate a release
could have occurred.
If you have more than four UST systems to report suspected releases, then attach additional sheets of the UST-17A form. You only
need to list the UST systems at your site that have suspected releases.
Within 7 days of discovery, submit a UST-17B form to document the response activities taken to investigate the suspected release.

IV. COMMENTS
The failing test was performed on 5-23-22. I received the email about the failed trienial test from Petro Test on 5-27-22. Because of the
holiday weekend I did not read it until 5-31. I have contacted Oakes Petroleum about installing Fairfield liners in the both spill buckets.
They will install as quickly as they can. I will be in Cary tomorrow. I will do my best to fill the crack to prevent leaks in the mean time. I
have also contacted Geological Resources about the required site check.
