Ground Water Monitoring Report Prepared on: December 22, 2022 5110 Market & Cafe Project Manager: Scott Driscoll GRI Project No. 6008 i EXECUTIVE SUMMARY 5110 Market & Cafe is located at 5110 NC Highway 96 in Selma, Johnston County, North Carolina. The site is currently owned and operated by Jai Umiya, LLC as a convenience store and grill which stores and resells petroleum fuels from USTs. Currently, five underground storage tanks are operated at the site to sell unleaded gasoline, diesel fuel and dyed off-road diesel fuel. The site and surrounding area are a mixture of small business and residential properties. The site contains one building and is primarily capped with asphalt, concrete and grass. The overall topography at the site relatively flat. One previous incident (NCDEQ No. 7058) was reported at the site in September 1991, and the incident was closed under the name Stop & Shop with an NFA on March 14, 2006. The current incident (NCDEQ 48524) was a result of a Site Check investigation requested by NCDEQ in response to a failed UST compliance inspection. A Phase II LSA report was issued October 11, 2022, which detailed the results of additional soil and ground water sampling at the site. During LSA activities, twenty-eight water supply wells were identified within 1,500 feet of the site. Based on the LSA information, the site was classified as high risk and an additional sampling event was requested by NCDEQ. On November 29, 2022, five Type II monitoring wells (GMW-1 through GMW-4 and EMW-1) were gauged, purged and/or sampled in accordance with the NCDEQ guidelines. The onsite inactive water supply well (WSW-28) was also sampled. Concentrations of individual petroleum constituents were reported above the MACs in four of the five ground water monitoring wells at the site. No concentrations of detected analytes were reported above the GCLs. Based on this information, the risk classification for the site should remain high due to the the proximity of water supply wells identified during assessment activities in combination with the MAC exceedances. Ground Water Monitoring Report Prepared on: December 22, 2022 5110 Market & Cafe Project Manager: Scott Driscoll GRI Project No. 6008 1 1.0 SITE HISTORY AND CHARACTERIZATION GRI presents the results of the November 2022 ground water sampling event at the 5110 Market & Cafe site; located at 5110 NC Highway 96 in Selma, Johnston County, North Carolina (Figure 1). These activities were conducted in accordance with GRI Proposal No. 22-815, which was submitted to NCDEQ on November 11, 2022, and was approved as Task Authorization No. 48524-2 on November 15, 2022. NCDEQ correspondence has been included as Appendix A. The site is currently owned by Jai Umiya, LLC and is operated by Vivek B. Patel as a convenience store and grill which stores and sells petroleum fuels for resale. Currently, one 12,000-gallon compartmentalized gasoline (8,000/4,000-gallon split), one 4,000-gallon gasoline and one 4,000-gallon compartmentalized diesel fuel USTs (2,000-gallon split) are in service at the site. The site contains one building and is primarily capped with asphalt, concrete, and grass. The overall topography at the site relatively flat. One previous incident (NCDEQ No. 7058) was reported at the site in September 1991. A diesel fuel UST was reported to have taken on water due to a hole in a suction line. The tank was removed on October 25, 1991, and soil samples were collected during the removal. The TPH concentrations reported in the sample(s) were less than the action levels, and the incident was closed under the name Stop & Shop with an NFA on March 14, 2006. The current incident (NCDEQ 48524) was a result of a Site Check investigation requested by NCDEQ in response to a failed UST compliance inspection. The site check was conducted at the site in June 2022 and confirmed a release of petroleum to ground water was confirmed. An LSA was required to be performed by the UST owner. A Phase II LSA report was issued October 11, 2022, which detailed the results of additional soil and ground water sampling at the site. During LSA activities, twenty-eight water supply wells were identified within 1,500 feet of the site. One inactive well (WSW-28) is located onsite and one water supply well (WSW-14) is located within 250 feet of the source area. Fourteen potable wells are located between 250 feet and 1,000 feet of the site. Municipal water is available to the site and surrounding properties. There is one low-lying area and pond identified approximately 325 feet north of the site. A site vicinity map is included as Figure 2 which depicts surrounding properties and locations of the water supply wells. Receptor information gathered during LSA activities is provided in Table 3. Adjacent property owner information is presented in Table 4. Five Type II monitoring wells (GMW-1 through GMW-4, and EMW-1) have been installed at the site during previous ground water assessment activities. A Site Map showing the locations of the monitoring wells and the structures on-site has been included as Figure 3. Based on this information, the site was classified as high risk and an additional sampling event was requested by NCDEQ. 2.0 CURRENT SITE INFORMATION/ COMPARISON TO HISTORICAL ASSESSMENT INFORMATION On November 29, 2022, five Type II monitoring wells (GMW-1 through GMW-4 and EMW-1) were gauged, purged and/or sampled in accordance with the NCDEQ guidelines. The onsite inactive water supply well (WSW-28) was also sampled. Free product was not observed in any of the monitoring wells. Depths to ground water in the Type II monitoring wells ranged from 3.35 feet to 4.24 feet below the tops of well casings. Ground water elevations in the Type II monitoring wells ranged from 95.42 feet to 96.30 feet. Ground water flow was generally to the north/northeast with a horizontal gradient of less than 0.01 feet per foot. Well construction and gauging information is presented in Table 5. Concentrations of benzene, MTBE, naphthalene, tert-Amyl alcohol, tert-Butyl alcohol, isopropylbenzene, C5-C8 aliphatics, C9-12 aliphatics and/or C9-C10 aromatics that exceeded the MACs were reported in the ground water samples collected from GMW-1 through GMW-3 and EMW-1. No concentrations of detected analytes were reported above the GCLs. A summary of the analytical results for ground water samples collected on November 28, 2022, is presented in Table 6. A Ground Water Quality Map based on the October 2022 data is included as Figure 5. The complete report of laboratory analyses of the ground water samples collected from the monitoring wells is included as Appendix B. The surficial aquifer is underlain by terrace deposits and upland sediment deposits, which can be 30 to 40 feet thick in the area. The surficial aquifer is underlain by bedrock. The water table has a seasonal high depth to water of approximately two to four feet BGS. 3.0 FREE PRODUCT REMOVAL No free product has been detected in any of the onsite wells. ___________________________________________________________________________________________ Prepared by: Geological Resources, Inc. Ground Water Monitoring Report Prepared on: December 22, 2022 5110 Market & Cafe Project Manager: Scott Driscoll GRI Project No. 6008 4 4.0 NATURAL ATTENUATION / NATURAL SOURCE ZONE DEPLETION STATUS No natural attenuation monitoring plan is currently in place at the site. 5.0 CONCLUSIONS  On November 28, 2022, five Type II monitoring wells were gauged, purged and/or sampled. The onsite inactive water supply well (WSW-28) was also sampled. Free product was not found in any of the wells onsite. Ground water flow was generally to the north/northeast with a horizontal gradient of less than 0.01 feet per foot.  Concentrations of benzene, MTBE, naphthalene, tert-Amyl alcohol, tert-Butyl alcohol, isopropylbenzene, C5-C8 aliphatics, C9-12 aliphatics and/or C9-C10 aromatics that exceeded the MACs were reported in the ground water samples collected from GMW-1, GMW-2, GMW-3 and EMW-1. No concentrations of detected analytes were reported above the GCLs.  A receptor survey was conducted within 1,500 feet of the site in August 2022 during LSA activities. Public water is available to the subject site and surrounding properties through the Johnston County East Public Utilities (JCEPU); PWS ID NC4051018. Twenty-eight water supply wells were identified within 1,500 feet of the site. Two water supply wells, one confirmed to be inactive, were identified within a 250-foot radius of the site. A total of fourteen potable water supply wells were located within 1,000 feet of the site. The site is not located in a designated wellhead protection area. One unidentified pond is located 325 feet northeast of the site.  It is noted here that many water meters were witnessed at properties where water wells were identified. JCEPU, stating privacy concerns, would not allow GRI to cross reference address and ownership information to determine if any of the properties were connected to their water system. One inactive well (WSW-28), which was sampled as part of this investigation is located on the site. It is verified that the well is inactive, and the site is connected to the public water system.  Based on this information, the risk classification for the site should remain high due to the proximity of water supply wells identified during assessment activities in combination with the MAC exceedances. Further assessment should be warranted for this site. 6.0 LIMITATIONS This report has been prepared for the exclusive use of Jai Umiya, LLC for specific application to the referenced site in Johnston County, North Carolina. The assessment was conducted based on the scope of work and level of effort desired by the NCDEQ and with resources adequate only for that scope of work. Our findings have been developed in accordance with generally accepted standards of environmental practices in the State of North Carolina, available information, and our professional judgment. No other warranty is expressed or implied. The data that are presented in this report are indicative of conditions that existed at the precise locations sampled and at the time the samples were collected. In addition, the data obtained from samples would be interpreted as being meaningful with respect to parameters indicated in the laboratory report. No additional information can logically be inferred from these data.
